1.Correlation analysis of the treatment of the time of maternal pulmonary tuberculosis and the prognosis of their children

Objective To evaluate the correlation of the diagnostic chance of maternal pulmonary tuberculosis and the prognosis of their children. Methods One hundred and fifty-six patients with maternal pulmonary tuberculosis were analyzed,which were assigned to two groups based on the diagnostic chance: pregnant group ( 60 cases) and postpartum group ( 96 cases). Living quality of their children was counted, and viewed in parallel with children (98 cases,control group) of healthy mother at the same time. Results The morbidity in postpartum group was 69.79% (67/96),significantly higher than that in pregnant group [l3.33%(8/60)](P<0.05) and control group [4.08%(4/98)](P<0.05). The mortality in postpartum group was 8.33% (8/96), significantly higher than that in pregnant group (0)(P< 0.05) and control group (0)(P< 0.05).The morbidity and mortality in pregnant group were not significant difference compared with control group(P > 0.05 ). Conclusion Diagnosis and treatment before the parturition of maternal pulmonary tuberculosis can reduce the morbidity and mortality of the children and improve their life quality.
2.Analysis on Application of TCM Decoction Pieces for Blood-activating and Stasis-removing in TCM Hospital of Jiangsu Province During 2011-2015

Objective To provide references for rational clinical usage and purchase of TCM decoction pieces for blood-activating and stasis-removing. Methods Excel 2013 software was used to conduct statistical analysis on categories, consumption quantity, consumption sum and annual average growth rate of TCM decoction pieces for blood-activating and stasis-removing during 2011-2015 in TCM Hospital of Jiangsu Province (hereinafter refered to as our hospital). Results The consumption quantity and consumption sum of TCM decoction pieces for blood-activating and stasis-removing in our hospital increased year by year. Among all types of TCM decoction pieces for blood-activating and stasis-removing, TCM decoction pieces for activating blood and adjusting maridians occupied the largest amount, followed by TCM decoction pieces for activating blood and relieving pains, breaking up the static blood and eliminating masses, and blood-activating and wound - curing. The TCM decoction pieces with the biggest annual consumption quantity were Salviae Miltiorrhizae Radix et Rhizoma, Chuanxiong Rhizoma, and Curcumae Radix, and the TCM decoction pieces with the biggest annual consumption sum were Notoginseng Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma, and Carthami Flos. Conclusion Both the consumption quantity and the consumption sum of the TCM decoction pieces for blood-activating and stasis-removing in our hospital have been increasing year by year. However, the proportion consumption quantity and the consumption sum of TCM decoction pieces and medicine utilization structure have kept stable, and in basically rational use.
3.Motor coordination function of attention deficit hyperactivity disorder(review)

Attention deficit hyperactivity disorder(ADHD) and developmental coordination disorder(DCD) are both childhood disorders identified in the DSM-Ⅳ.Studies investigating ADHD have found that around half of ADHD cases also have motor problems severe enough to be diagnosed as DCD.Further,children initially diagnosed as DCD have also been found to meet moderate to severe diagnosis for ADHD.Both disorders have been linked to a lot of psychosocial problems.Furthermore,when ADHD and DCD are co-morbid,the outcome tends to be more severe than when either disorder occurs alone.In the past decade,many studies identified shown that ADHD children experienced difficulties with both fine motor movements and gross motor movements.Notably,most of these foregoing studies found balance functions of ADHD were impaired.In order to maintain balance under a verity of environmental conditions,sensory information from somatosensory,visual,and vestibular origins must be integrated by the central nervous system.But there are rich evidences suggesting that children with ADHD can not organize the motor information effectively.The maintenance and control of posture and balance,whether in static or dynamic conditions,are essential requirements for daily activity.The balance function has been closely associated not only with both gross motor movements,such as sitting,standing,walking and fine motor movements,but also with human behaviors.There are increasing evidences suggesting that balance deficit correlates with symptoms of ADHD.Therefore,motor coordination difficulties co-occurring with ADHD should be given emphasis in clinical practice;the training of balance may be one of the possible methods to improve the motor coordination function in ADHD children.
4.A pilot study on effect of methylphenidate on balance function of children with attention deficit hyperactivity disorder

Objective:To explore the effect of methylphenidate on the balance function of children with attention deficit hyperactivity disorder,and the correlation between the variations of balance function and subjective clinical assessment before and after treatment.Methods:From July to December 2006,23 children(19 boys and 4 girls) who met DSM-Ⅳ criteria for ADHD participated in an open study conducted at Institute of Mental Health,Peking University.The variation of their balance functions before and after methylphenidate extended-release tablets(OROS methylphenidate) treatment were evaluated,and the differences of balance functions between 19 ADHD boys pre-and post-treatment and 38 age-matched typically developing control boys were compared.Balance functions were assessed with sensory organization test(SOT) provided by SMART EquiTest system.Subjective clinical assessments were measured with ADHD rating scale-Ⅳ and IOWA Conners rating scale.Results:The equilibrium score(ES) in SOT5,SOT6,and composite score (61.84?14.19,65.88?15.41,and 75.70?10.30,respectively),and the strategy score(SS) in SOT4 and SOT5(89.20?3.17 and 83.22?6.56),and the sensory analysis of vestibular ratio(67.64?14.43) improved significantly after OROS methylphenidate treatment compared to those before treatment(49.81?16.26,57.61?16.72,68.70?11.96,86.84?4.49,78.55?6.55,and 54.46?17.27,respectively,P0.05). Conclusion:This study suggests that methylphenidate can improve the balance function of children with ADHD through direct effect on the balance system.
5.Contribution of disruptive behavior disorder to balance function of children with attention deficit hyperactivity disorder

Objective:To investigate whether the likelihood of balance impairment in children with attention deficit hyperactivity disorder(ADHD) increases with the presence of disruptive behavior disorder(DBD),and whether the likelihood of co-occurring diagnosis of DBD increases in ADHD children with severer balance deficit.Methods:Three groups of 20 children each,representing pure ADHD,ADHD+DBD,and typically developing control children,were included and group matched by age and gender.Balance functions were assessed with the Balance Master in different conditions.We compared the percentages of children in each group who swayed faster than 85% of the typical control children in all conditions.Results:In the conditions of firm surface with eyes open(FI/EO) and eyes closed(FI/EC),and foam surface with eyes open(FO/EO) and eyes closed(FO/EC),the sway velocities of ADHD only group(0.65?0.26 ?/s,0.65?0.30 ?/s,0.95?0.29 ?/s,and 1.70?0.49?/s,respectively) were significantly higher than those of the typical control group(0.47?0.18 ?/s,0.43?0.19 ?/s,0.76?0.21 ?/s,and 1.27?0.38 ?/s,respectively)(P

Mesenchymal stem cells (MSCs) have a strong self-renewal capacity and multi-differentiation potential,which make them become ideal seed cells of tissue repair and target cells of gene therapy.However,the labeling and tracing method of MSCs transplanted in vivo is the hot spot and difficult problem of the current research.At present,the commonly used tracer methods of MSCs include fluorescence dye labeling,molecular marker,imaging marker technology.This paper summarized the advantages and disadvantages of these tracer methods.
7.Epidemiology of hepatitis C virus infection and its genotype analysis in Yancheng area

Objective To investigate the characteristics of hepatitis C viurs (HCV)infection and its genotypes in Yancheng area . Methods A total of 20 185 cases of subjects receiving healthy examination were collected ,and fasting blood levels of serum anti‐HCV were detected .Clinical data of patients with HCV infection were statistically analysed .HCV genotypes and levels of HCV RNA were detected ,and their clinical prognosis was judged by type‐B ultrasonic .Results The total infection rate of HCV was 1 .22% .The infection rate of male was higher than that of female and the infection rate was increased with the elevation of age .The genotype 1b was accounted for 73 .17% .The results of type‐B ultrasonic shown that all patients infected with genotype 6 and 1b/2a HCV only had liver damage .80 .77% of patients infected with genotype 2a HCV had liver damage ,which was higher than that of patients infected with 16 and 3a+3b genotypes .Conclusion Most of HCV infected patients are male ,and the infection rate might be increased with the elevation of age .The prognosis is in various different genotypes of HCV ,which indicates that the prognosis could be evaluated by genotyping .
8.Intraductal ultrasonography in diagnosis of bile duct stones

Objective To investigate the diagnostic value of intraductal ultrasonography (IDUS) for bile duct stones by comparing between magnetic resonance cholangiopancreatography ( MRCP), endoscopic retrograde cholangiography (ERC) and IDUS. Methods ERC was performed in 30 patients with suspected extrahepatic cholelithiotic after MRCP. IDUS was applied through guide wire after ERC, and stones were removed if necessary. Results MRCP detected bile duct stones in 22 cases, misdiagnosed bile duct floc as stones in 2 and missed stones in 2 others. The accuracy, sensitivity and specificity of MRCP in diagnosis of bile duct stone were 86. 7% (26/30), 91. 7% (22/24), and 66. 7% (4/6), respectively. ERC detected bile duct stones in 23 cases, misdiagnosed bile duct air bubble as stones in 2 and missed stones in 1. The accuracy , sensitivity and specificity of MRCP in diagnosis of bile duct stone were 90% (27/30), 92% (23/25) and 66.7% (4/6) , respectively. IDUS detected 24 cases of bile duct stones, 4 bile duct floc and 2 air bubble without any misdiagnosis. The accuracy, sensitivity and specificity of IDUS in diagnosis of bile duct stone were 100%. Conclusion IDUS, safe and reliable, is superior to ERC and MRCP in diagnosis of bile duct stone.
9.Influence of Propofol and Etomidate on Cognitive Dysfunction of Elderly Patients after Laparoscopic Chole-cystectomy

OBJECTIVE:To discuss the effect of etomidate and propofol on early postoperative cognitive dysfunction (POCD)of elderly patients after laparoscopic cholecystectomy(LC)and significance of serum protein S100β to the occurrence of early POCD in total intravenous anesthesia. METHODS:60 patients aged 65 years old above undergoing LC in total LMA intrave-nous anesthesia were selected and randomly divided into etomidate group(group E)and propofol group(group P),with 30 cases in each group. Anesthesia was induced by etomidate 0.3 mg/kg (group E) or propofol 1.5 mg/kg (group P),and additionally in-duced by sufentanil 0.4 μg/kg and vecuronium 0.12 mg/kg. Anesthesia was maintained with intravenous pump of remifentanil 0.15 μg/(kg·min),continuous target controlled infusion of etomidate(target concentration 1.0-1.5 μg/ml)(group E)or propofol(target concentration 3.0-4.0 μg/ml)(group P);the dual brain index(BIS)values were maintained between 40 and 50 throμgh adjusting target concentration of etomidate or propofol. The blood samples were collected 1 h before operation(T0),2 h(T1),24 h(T2), 48 h(T3)after operation,and the content of S100βprotein was detected and mini-mental state examination(MMSE)score were re-corded. Meanwhile,recovery time,laryngeal mask removal time,intraoperative dosage and the occurrence of intraoperative aware-ness were observed and recorded in 2 groups. RESULTS:There was no statistically significant difference in MMSE score between 2 groups at different time points(P>0.05);MMSE score of 2 groups at T1 and T2 was significantly lower than at T0,with statisti-cal significance(P<0.05). There was no statistically significant difference in the content of S100β protein between 2 groups at dif-ferent time points(P>0.05);The contents of S100β protein in 2 groups at T1 and T2 were significantly higher than at T0,with sta-tistical significance(P<0.05). The recovery time and larynge-al mask removal time were both short in 2 groups,with statis-tical significance (P>0.05). The amount of ephedrine in group P was significantly higher than in group E,with statisti-cal significance (P<0.05). No intraoperative awareness oc-curred in 2 groups throμgh postoperative follow-up. CONCLUSIONS:Etomidate and propofol total intravenous anesthesia can be safely used in elderly patients with LC,and they can cause short-term POCD at different degrees. The amount of S100β protein has some relevance with the occurrence of early POCD .
10.Expressions and significance of Vimentin and E-cadherin during epithelial-mesenchymal transition of hepatocellular carcinoma tissue

Objective To investigate the molecular expression and clinical significance of Vimentin and E-cadherin during epithelial-mesenchymal transformation of hepatocellular carcinoma (HCC) tissues.Methods Hepatocellular carcinoma and paracancerous tissue were collected from 55 patients,while nonneoplastic nonnal liver tissue were collected from 20 patients without cancerous diseases as control group.Expressions of Vimentin and E-cadherin were examined by immunohistochemistry staining.Clinicopathological features of the relative expression of Vimentin and E-cadherin,and its influence on prognosis were discussed as well.Results Vimentin was positively expressed in 69.1% cancer tissue,and was significantly higher than paracancerous tissues and normal liver tissues(P < 0.05).E-cadherin was positively expressed in 27.3% cancer tissue and was significantly lower compared with paracancerous tissues and normal liver tissues(P <0.05).The expression of Vimentin and E-cadherin was negatively correlated(r =-0.653,P < 0.05).Vimentin expressions in subgroups of TNM stage Ⅲ-Ⅳ,poor differentiation,multiple tumor nodules,metastasis and recurrence were all significantly higher than the respective subgroups as TNM stage Ⅰ-Ⅱ] (x2 =7.267,P < 0.05),moderate to well differentiation (x2 =4.045,P < 0.05),single tumor node (x2=12.143,P <0.05),without metastasis or recurrence (x2 =7.267,P < 0.05).On the contrary,Ecadherin showed higher expression in the subgroup of single tumor node compared with multiple tumor nodules(x2 =6.878,P < 0.05).The follow-up of patients further demonstrated that patients with positive Vimentin expression had a significantly lower disease-free survival compared with the Vimentin negative group (P <0.05).Conclusions Vimentin and E-cadherin both participated in epithelial-mesenchymal transition process of hepatocellular carcinoma.High expression of Vimentin and low expression of E-cadherin were closely correlated with occurrence and development of HCC.Vimentin and E-cadherin may serve as impottant biomarkers in clinical diagnosis and prognosis prediction of HCC.
