1.Cost-minimization Analysis of Xiangdan Injection and Danshen Chuanxiongqin Injection in the Treatment of Unstable Angina Pectoris of Coronary Heart Disease

OBJECTIVE:To compare the cost of Xiangdan injection and Danshen chuanxiongqin injection in the treatment of unstable angina pectoris of coronary heart disease,and to provide reference for rational and economic drug selection. METHODS:42 inpatients with unstable angina pectoris of coronary heart disease were divided into Xiangdan injection group(group A,20 cas-es) and Danshen chuanxiongqin injection group (group B,22 cases). Two groups were given conventional symptomatic treatment according to the specific clinical situation;on this basis,group A was given Xiangdan injection 20 ml intravenously,qd;group B was given Danshen chuanxiongqin injection 10 ml intravenously,qd,for 14 d. Clinical efficacy and the occurrence of ADR were compared between 2 groups,and cost-minimization analysis was conducted. RESULTS:After treatment,total effective rate of group A and B was 95.00% and 95.45%,without statistical significance(P>0.05);there was also no statistical significance in the incidence of ADR (P>0.05). The costs of 2 groups were equal in addition to the cost of drugs. According to cost-minimization analysis,the drug cost of group A was 77.28 yuan and total cost was 705.88 yuan;the drug cost of group B was 1 310.40 yuan and total cost was 1 939.00 yuan;the drug cost and total cost of group A were far more less than that of group B,with statistical significance(P<0.05). The results of sensitivity analysis were consistent with it. CONCLUSIONS:The cost of Xiangdan injection is lower and more economical than that of Danshen chuanxiongqin injection in the treatment of unstable angina pectoris of coronary heart disease.
2.Change rule of coagulation indicators and platelet parameters in progression of liver diseases

Objective To investigate the change rule of common coagulation indicators and platelet parameters in the patients with chronic viral hepatitis,liver cirrhosis and liver failure.Methods One hundrend and twenty patients with liver diseases in our hospital from January 2015 to June 2016 were selected and divided into chronic viral hepatitis group (46 cases),liver cirrhosis group (Child-Pugh class A,B,C,44 cases) and liver failure group (30 cases).Contemporaneous 30 persons undergoing healthy physical examination served as the healthy control group.The coagulation indicators (PT,PTA,APTT,TT, Fg,D-D) and platelet parameters (PLT,MPV,PDW, PCT) were detected in all subjects.Results Except TT,Fg and D-D had no statistical difference between the healthy control group and chronic viral hepatitis group (P>0.05),PT,APTT and TT in the chronic viral hepatitis group,liver cirrhosis group (Child-Pugh class A,B,C) and liver failure group were gradually extended,PTA and Fg were decreased and D-D value was increased,the difference in pairwise comparison was statistically significant (P<0.05).The PLT value in the chronic viral hepatitis group,liver cirrhosis group (Child-Pugh class A,B,C) and liver failure group was decreased gradually,while the MPV and PDW values were gradually increased,showing significantlystatistically significant difference (P<0.05),the PCT value in the liver cirrhosis group and liver failure group was lower than that in the chronic viral hepatitis group(P<0.05).Conclusion The regularity change of coagulation indicators and platelet parameters occurs in different periods of chronic liver disease progression,which provides some reference for clinical diagnosis,treatment and prognosis judgment.
3.Clinical and imaging characteristics of reversible posterior encephalopathy syndrome in 8 patients

A retrospective study was conducted in 8 patients with reversible posterior encephalopathy syndrome to analyze the clinical and MRI findings and follow-up outcomes of the disease.The participants clinically presented with elevated blood pressure(n=8),blurred vision(n=6),severe headache(n=4),dysphofia(n=4),impaired shon-term memory(n=2),and seizure(n=6).The most important imaging finding Was vasogenic edema,which showed better prognosis.

5.Complications related to transcatheter occlusion of atrial septal defect with Amplatzer septal occluder in children

Objective To probe into the causes,therapeutic and preventive methods of complications associated with transcatheter occlusion of atrial septal defect using the Amplatzer septal occluder in children. Methods Complications occurred in 9 of 211 cases. The complications in procedure of occlusion were those in which there appeared systemic circulatory systemic air embolism in 2,pulmonary air embolism in 1,pericardial tamponade in 1,ASO malposition requiring emergency surgical removal in 1,transient atrial extrasystoles in 1 and sizing balloon rupture in 1 case. Two cases with postoperative complications were found in the follow-up studies. These included perforation of mitral valve and ASO partially dislodged. In this study,5 children with intra-procedure complication received emergency therapy including surgical intervention,and others were needed only in the follow-up,as the complications were transient or asymptomatic. Results There were no children′s deaths in this study. The 5 cases that received treatment completely healed,and the others with intra-procedure complications were also had no sequela. Cases with mitral valve perforation and ASO partially dislodged were still in our follow-up studies,as the 2 patients had no symptoms. Conclusion Air embolisms occurred easily in atrial septal defect cases that received ASO therapy. Complications were mainly caused by inappropriate operative procedure and some complications needed emergency treatments. Follow-up studies were important to cases with transcatheter occlusion therapy.
6.Video colposcopy in diagnosis of cervical squamous intraepithelial lesions

Objective To study the accuracy and efficiency of video colposcopy in diagnosis of cervical squamous intraepithelial lesions and clinical significance of seven colpocopic signs. Methods Two handred and senventy five patients with an abnormal cervical smear or vulva condyloma acuminata were re evaluated by video colposcopy. 265 cervical biopsies performed under the guidance of video colposcopy for pathological study, as well 8 endocervical curettage (ECC) and 237 loop electrosurgical excision procedure (LEEP). Results The sensitivity, specificity and positive rate of video colposcopy in evaluation of cervical squamous intraepithelial lesions were 95 9%, 77 7%, and 84 5%. Video colposcopy diagnostic accuracy was 93 8% tally with the pathological results in observing normal transformation zone. False negative rate was 6 1%. 76 2% of cases with subclinical papillomavirus infection by acetowhite epitheliun, while false negative rate was 23 7%. Twin signs, triple signs and cervical carcinoma were 100 0% tallying with the diagnosis of cervical lesions. Conclusions Video colposcopy can be a very accurate diagnostic method in clinical practice. It is quite sensitive in diagnosis of severe cervical lesions. However, there was a relative high false negative rate in diagnosing mild cervical lesions.

This article expounded that developing health service is the realistic demand for deepening TCM education reform from the direction, number and quality of talent cultivation, and proposed the measures of deepening TCM education to train TCM health services: establish mechanism for co-ordination of supply and demand of TCM talents under the direction of market and guidance of government; form and optimize multilevel, multi-standard and multi-grade talent cultivation system; build talent evaluation mechanism and cultivation goals focusing on post competency.

10.Serum cardiac troponin Ⅰ and lactic acid in neonatal patients with hypoxic-ischemic encephalopathy

Serum Cardiac troponin Ⅰ (cTn Ⅰ) and lactic acid were measured in 128 neonatal patients with hypoxic-ischemic encephalopathy and 38 healthy neonates (control group). Serum cTn Ⅰ and lactic acid levels were higher in neonates with hypoxic-ischemic encephalopathy than those in healthy neonates (P < 0. 05 ); and the differences of these values were also statistically significant between the serious patients and the mild patients (P<0.05). Serum cTn Ⅰ and lactic acid are two sensitive markers for degree of myocardial injury and hypoxia in neonatal patients with hypoxic-ischemic encephalopathy.
