Journal of Integrative Medicine 2005;3(5):366-9
OBJECTIVE: To observe the effects of Huobahuagen Tablets combined with irbesartan on the risk factors of IgA nephropathy. METHODS: Sixty-two patients diagnosed as IgA nephropathy were randomly divided into control group and treatment group. Thirty patients in the control group were treated with Huobahuagen Tablets (5 tablets po t.i.d.), and 32 patients in the treatment group were treated with irbesartan (150 to 300 mg po q.d.), besides the same treatment as the control group. After 3 months of treatment, the levels of blood pressure (BP), 24 h urine protein (Upr), urinary red blood cells (URBC), blood triglycerides (TG), total cholesterol (TC), albumin (Alb), alanine transaminase (ALT), white blood cells (WBC) and serum creatinine (Scr) were observed. RESULTS: After treatment, the levels of Upr, URBC and Scr in both groups were decreased, as compared with those before treatment (P<0.05 or P<0.01), and the levels of BP, Upr, URBC and Scr in the treatment group were lower than those in the control group (P<0.01). The levels of Alb in both groups were increased, as compared with those before treatment (P<0.05 or P<0.01), and the level of Alb in the treatment group was higher than that in the control group (P<0.05). CONCLUSION: Huobahuagen Tablets, when used together with irbesartan, may improve the renal function of the patients with IgA nephropathy and slow the deterioration of the disease by reducing BP, Upr, URBC and Scr.

Objective To investigate the effect of cerebral ischemic preconditioning (IP) on the expressions of angiopoietin-1 (Ang-1) and its receptor Tie-2 mRNA in cerebral ischemia in rats. Methods Ninety-nine Wistar rats were randomly assigned to three groups:sham operation (n =9),non-ischemic preconditioning (NIP) (n =45),and IP (n =45).The latter two groups were redivided into 5 subgroups:ischemia-reperfusion 1,3,7,14,and 21 days (n =9 in each group).A model of transient middle cerebral artery occlusion (MCAO) was induced by the intraluminal suture method for focal IP (ischemia for 10 minutes and restoring perfusion).Infarct volume was determined by 2,3,5-triphenyltetrazolium staining. The expression levels of Ang-1/Tie-2 mRNA were detected by in situ hybridization. Results The infarct volumes in the 1 -,3-,and 7-day subgroups of the IP group were significantly smaller than those in the relative subgroups of the NIP group (all P< 0.05).The expression of Ang-1 mRNA in the 3- and 7-day subgroups of the IP group and the expression of Tie-2 mRNA in the 1-,3-,and 7-day subgroups of the NIP group were upregulated significantly (all P < 0.05).The infarct volume in the 3-day subgroup of the IP group was reduced most significantly (P < 0.05).The expression of Ang-1 mRNA in the 7-day subgroup was upregulated significantly, and the peak expression of its receptor Tie-2 mRNA appeared at day 3 after IP and continued to day 7.Pearson correlation analysis showed that the expression levels of Ang-1/Tie-2 mRNA were significantly negatively correlated with infarct volume (P <0.01).Conclusions The expression of Ang-1/Tie-2 mRNA in the IP group was upregulated within the time window of ischemic tolerance (1 - 7 days after preconditioning),in which Ang-1 may mainly act on the later stage of the cerebral ischemic tolerance.

Objective To study the pulmonary surfactant (PS) on prevention of neonatal respiratory distress syndrome (NRDS) in neonates delivered via caesarean section. Methods From selective cesarean section infants (gestational age 34-38+6 W), 80 cases whose test tube oscillation tests were negative and amniotic fluid pulmonary surfactant associated protein A (SP-A) concentrations were lower than <10μg/L, and were randomly divided into PS prevention group and control group, with 40 cases in each group. PS prevention group within 1 h of birth were administrated poractant alfa injection by endotracheal tube (dose 100 mg/kg), but the control group was not given special treatment, leaving only the observation. The incidence of NRDS, treatment status and clinical progression were compared between two groups. Results The incidence of NRDS in control group was 82.5%(33/40), in PS prevention group was 37.5%(15/40), and there was significant difference (P<0.05). The degree of NRDS in control group was more severe. The incidence rate of persistent pulmonary hypertension of the new-born (PPHN), pulmonary air leak, patent ductus arteriosus and oxygenation index above 25 mmHg (1 mmHg=0.133 kPa) in control group were significantly higher than those in PS prevention group (P<0.05). The time of mechanical ventilation, the time of oxygen inhalation, ratio of arterial partial pressure of oxygen (PaO2) before mechanical ventilation to fraction of inspired oxygen (FiO2), and costs of hospitalization in control group were significantly higher than those in PS prevention group (P<0.05). Conclusions PS prevention can reduce the incidence of NRDS of neonates delivered by elective caesarean section, can alleviate the symptoms of NRDS, shorten length of stay and reduce the cost of hospitalization.
