1.Clinical study on the treatment of 29 cases of HIV/AIDS with the traditional Chinese medicine combined with HAART

Objective To explore a reasonable therapeutic plan for treating HIV/AIDS with traditional Chinese medicine plus western medicine. Methods Twenty-nine patients with HIV/AIDS were selected and recruited into a HIV group and a AIDS group according to the clinical diagnosis. Each group was further divided into comparison stage and treatment stage. In HIV group, observations and statistics of observation indexes were performed during the comparison stage; while TCM treatment was applied in the treatment stage. In AIDS group, HAART was applied in the comparison stage, and HAART combined with TCM therapy were used in the treatment stage. Both comparison stage and the treatment stage lasted 12 months. Results In HIV group, self control results showed that immune and virus indexes did not have significant changes, but also with no side or toxic effects, In the AIDS group, contrasting between the comparison stage and the treatment stage showed that there were significant improvement of symptoms (χ2=4.9231, 2.5000, P<0.05) , anti-toxic effects (χ2=9.333、 16.4091、10.2273, P<0.01) and immune indexed(t=3.1990,P<0.01) after treated additionally with traditional Chinese medicine. Conclusion Traditional Chinese medicine could improve the immunity function and clinical symptoms, reduce side effect of HAART medicine and stabilize CD4+ T cells of patient with HIV/AIDS.
2.Effect of γ-rays combined with blocking VEGF expression on esophageal cancer xenografts

Objective To investigate the effect of blocking VEGF expression on the radiation sensitivity of esophageal cancer cell line TE-1 in vivo. Methods 32 male Balb/c/nu nude mice were randomly divided into four groups, including control group, radiation group, anti-VEGF group, and anti-VEGF + radiation group. The anti-VEGFcDNA cells were subcutaneously injected into the paw pats of mice (2 × 106/100 μl). The subcutaneous tumors were irradiated with 18 Gy of 60Co y-rays when the diameter of tumors varied from 0. 8 to 1.0 cm. The volume of the tumors was measured before and after irradiation, respectively. The expression level of VEGF mRNA and protein were examined by RT-PCR and Western blotting, respectively. Apoptotic cells were detected by electron micrographs. Results Latent period of the tumor formation of anti-VEGF group was lengthened compared with other groups(t = 13. 898,P <0.01 ). The volumes of tumor in anti-VEGF group [ ( 1207. 50 ± 97.07 ) mm3 ] and anti-VEGF +radiation group [ ( 1057. 5 ± 91.50 ) mm3 ] were not statistically different post-irradiation ( t = 1. 124, P >0.05 ) , but smaller than those in control group [ ( 5442. 50 ± 185.08 ) mm3 ] and radiation group [ (2922. 50 ± 152. 773)mm3 ] with statistical differences( t = 9. 475-21. 238, P < 0. 01 ). The expression level of endogenous VEGFmRNA and protein in anti-VEGF group and anti-VEGF + radiation group were statistically different from control group and radiation group (F = 387.394, 13.519, P < 0.01 ).Conclusions Antisense VEGF could inhibit the proliferation of esophageal cancer cell in the nude mice.Effect of blocking VEGF expression before irradiation on esophageal cancer xenografts might be limited.

Objective To observe the preventive and therapeutic effect of Yangyin Shengxue Mixture (Herbal decoction for nourishing yin and blood) on radiation-induced oral mucosal injury.Methods Sixty nasopharyngeal carcinoma patients were randomized into two groups: treatment group (30 cases) receiving Yangyin Shengxue Mixture three days before till one week after radiotherapy, and control group (30 patients) using conventional Tinidazole Gargle for prevention of oral-mucositis. Before and after treatment, the body weight, hemoglobin and state of infection of patients were examined. The time of occurrence of symptoms, degree of acute radiation mucositis and degree of pain were observed to evaluate the damage of oral mucosa according to RTOG standard for radiation injury of oral mucosa and Numeric Rating Scales (NRS). 4.Relationships between posttraumatic growth and psychological distress among patients with prostate cancer

Objective To investigate the level and relationships of posttraumatic growth (PTG) and psychological distress among patients with prostate cancer. Method Totally 116 patients with prostate cancer involved in the investigation by a self-designed demographic questionnaire, posttraumatic growth inventory (PTGI) and distress thermometer (DT). Results The total score of PTGI was (53.12 ± 13.51), at a low level, and the score of DT was (4.32 ± 2.59), at a medium level. The score of DT was negatively correlated to the scores of PTG and its dimensions (all P<0.05). Conclusions Patients with prostate cancer show a low level of PTG and a medium level of psychological distress and they are negatively related . Therefore , nurses should take measures to reduce the patients psychological distress and then improve their PTG level.
5.Detection of Th17 cell in patients with bronchial asthma and itssignificance

Objective To detect the expression of Th17 levels about peripheral blood in patients with bron-chial asthma in bronchoalveolar lavage fluid,and to investigate its effect in pathogenesis of bronchial asthmaand. Methods Patients with acute bronchial asthma from 2014 April to 2014 November were collected in the experiment. There are 52 cases as observation group (group of patients with asthma),and the patients were then divided according the severity (mild,moderate,severe);in addition,select 50 healthy people in the same period in our hospital as con-trol group healthy control group.Collect peripheral blood and bronchoalveolar lavage fluid,the expression of specific transcription factors ROR gamma T and cytokine IL -17 in the peripheral blood and bronchoalveolar lavage fluid in Th17 cells levels were detected by real -time fluorescent quantitative PCR.Results The expression of IL -17,ROR gamma T in peripheral blood and bronchoalveolar lavage fluid of the observation group were higher than the control group;the expression of IL -17,ROR gamma T in alveolar lavage fluid quantity and peripheral blood with severe patients in the observation group were higher than that in the mild patients,differences were statistically significant (P <0.05).Conclusion There are 17 auxiliary T lymphocytes in patients with bronchial asthma in vivo (Th17) advantage of expression,Th17 may play an important role in the pathogenesis of bronchial asthma.Bronchial asthma patients body may have advantage to heip expression of T lymphocytes 17 (Th17).Th17 may play an important role in the pathogenesis of bronchial asthma.
6.Research progress in contrast-induced nephropathy

With the wide application of contrast media in modem medicine,contrast-induced nephropathy (CIN) has attracted more clinical attention.Renal ischemia and renal tubular toxicity have been considered to be the pathogenesis of CIN.The most promising biomarkers,except for serum creatinine,include neutrophil gelatinase associated lipocalin (NGAL),cystatin C (Cys C),kidney injury molecule-1 (KIM-1),urine N-acetyl beta-D amino glucosidase (NAG) and micro molecular RNA (microRNA).Before use of contrast media for angiography,both the patient's own risk factors and the contrast-associated risk factors should be carefully evaluated.The patient's own risk factors include basic renal function,diabetes,anemia,homocysteine,etc.The contrast-associated risk factors include the osmotic pressure,viscosity,dosage,application frequency of the used contrast agent,etc.At present,hydration therapy is still the main method for CIN,and other therapeutic methods include medication,such as statins,vasodilators,antioxidants,traditional Chinese medicine,etc.,and blood purification therapy.This paper aims to make a brief summary about the research progress in CIN,focusing on its diagnosis,pathogenesis,risk factors and preventive measures.
7.Application of speech audiometry in hearing aid selection

BACKGROUND: Speech audiometry is decisive in selecting hearing aid,but it is seldom used in clinic in China.OBJECTIVE: To understand the differences in the speech resolution with speech audiometry between in-the-ear (ITE) and behind-the-ear (BTE)hearing aids.DESIGN: Paired controlled study.SETTING: Auditory Center of the 414 Hospital of Chinese PLA, Auditory Rehabilitation Center of Jiangsu Province.PARTICIPANTS: A total of 62 patients with auditory disability were selected from the Auditory Center of the 414 Hospital of Chinese PLA and Auditory Rehabilitation Center of Jiangsu Province. Among 32 ITE hearing aid users, average hearing loss at 500, 1 000, 2 000 and 4 000 Hz, was less than 85 dB, and the patients were aged from 16 to 60 years. Among 30 BTE hearing aid users, average hearing loss at 500, 1 000, 2 000 and 4 000 Hz, was also less than 85 dB, and the patients were aged from 16 to 60 years. All patients were consent.METHODS: Open and close methods were adopted for speech audiometry among ITE and BTE hearing aids users. Monosyllabic and disyllabic words were chosen as the type of speech and 20 words were given to one ear, respectively.MAIN OUTCOME MEASURES: Comparison of speech resolution with monosyllabic and disyllabic words test between ITE and BTE hearing aid.RESULTS: In aspect of the effect of ITE and BTE hearing aids, there were no significant differences of both monosyllabic and disyllabic words no matter through open or close method (P > 0.05). At the opening state, there were significant differences in resolution of monosyllabic and disyllabic words (P < 0.05); while, there were no significant differences (P > 0.05) at the closing state.CONCLUSION: Carrying out speech audiometry in the selection hearing aid is influenced by many factors.
8.Study of biofilm formation by Trichosporon asahii

Objective To observe morphological characteristics and activity distribution of T. asahii biofilm. Methods The morphological characteristics of T. asahii biofilm were observed under an inverted microscope and scanning electron microscope, and activity was measured and quantitatively analyzed by 2,3-bis (2-methoxy-4-nitro-5-sulfophenyl)-5-[(phenylamino) carbonyl]-2H-tetrazo-lium hydroxide (XTT) assay and viable count, respectively. Spatial distribution of dead/vital cells, activity and thickness of biofilm at different layers were assessed under a confocal laser scanning microscope (CLSM) following double staining with FDA/PI. Results T. asahii formed a biofilm in vitro on the surface of polystyrene materials. Under a scanning microscope, the biofilm displayed a complex three-dimensional structure which composed of spores, pseudohy-pha and true hypha. As time prolonged, the activity and quantity of biofilm increased. The results of XTT assay were correlated with those of viable count (r = 0.94, P < 0.01). The activity was of no obvious difference between different layers of the biofilm. The thickness of biofilm varied from 14.3 μm to 31 μm. Conclusions The structure of T. asahii biofilm in vitro is more complex than that of planktonic T. asahii. The activity is of no significant difference between different layers of T. asahii biofilm.
9.Effect of remote post-ischemic conditioning on the prognosis of elderly patients with acute cerebral infarction

Objective To observe the effect of remote post-ischemic conditioning(RPIOC)on the cerebral blood flow,neural function and prognosis of patients with acute cerebral infarction and the risk factors for short-term prognosis.Methods 133 patients with acute cerebral infarction in the Second Hospital of Beijing from January 2016 to December 2017 were selected,and randomly divided into the RIPOC group(66 cases,with RPIOC)and the control group(67 cases,without RIPOC).In the first day after hospital,patients in the RIPOC group were given RIPOC,which was tightening the left aim with a tonometer bandage for 5 minutes per time and 2 times a day at an interval of 5 minutes.All patients were provided routine treatment.All patients' cerebral blood flow,neural function and survival data were recorded.Recovery was assessed by modified Rankin Scale(mRS)180 d after stroke.Logistic regression was used to evaluate the risk factors for prognosis.Results Of the 133 patients enrolled,there were 67 males.The mean age was (73.1 ± 10.1)year.Basic clinical characteristics,neural function and cerebral blood flow were similar between groups(P>0.05).After 10 d treatment,cerebral blood flow and neural function was significantly increased (P<0.05)in the RIPOC group.After 180 d follow up,the RIPOC group had significantly higher rate of adverse cerebrovascular events(P<0.05).Logistic regression analyses demonstrated that advanced age(P =0.003),hypertension(P =0.03)and high NIHSS score(P =0.005)were all risk factors for prognosis.Conclusions RIPOC can enhance the cerebral blood flow,activities of daily living,limb function and prognosis.However,it does not reduce the risk of mortality.Advanced age,hypertension and high NIHSS score are risk factors for short-term prognosis.

Objective:To analyze the fail mode of neoadjuvant therapy combined with surgery for locally advanced esophageal squamous cell carcinoma (ESCC) after long-term follow-up.Methods:Clinical data of consecutive 238 patients with locally advanced resectable ESCC who underwent neoadjuvant therapy combined with surgery in Zhejiang Cancer Hospital from September 2012 to October 2019 were retrospectively analyzed. The failure mode in the whole cohort was analyzed after long-term follow-up. The overall survival (OS) and disease free survival (DFS) rates were analyzed by Kaplan-Meier method. Survival differences were determined by log-rank test.Results:The pathological complete response (pCR) rate was 42.0% in 238 patients. After a median follow-up of 46.1 months, tumor progression occurred in 96 patients (40.3%), including 25 patients (10.5%) with local recurrence, 61 patients (25.6%) with distant metastases, and 10 patients (4.2%) with simultaneous local recurrence and distant metastases. The median OS and DFS were 64.7 months and 49.9 months. And the 3-, 5-, and 7-year OS and DFS rates were 70.0%, 52.8%, 36.4% and 63.5%, 42.5%, and 30.0%, respectively. The 3-, 5-, and 7-year locoregional recurrence-free survival rates and distant metastasis-free survival rates were 86.0%, 71.4%, 61.2% and 70.6%, 55.9%, 43.0%. Compared with non-pCR patients, the overall progression rate and distant metastasis rate of pCR patients were lower (26.0% vs. 50.7%, 16.0% vs. 32.6%, both P<0.05). And the 3-, 5-, and 7-year OS (83.0% vs. 60.2%, 69.7% vs. 41.7%, 50.4% vs. 27.7%, all P<0.001) and DFS rates (80.4% vs. 51.4%, 63.9% vs. 31.2%, 45.9% vs. 20.3%, all P<0.001) were significantly better in pCR patients. Conclusions:Distant metastasis is the main failure mode of patients with locally advanced ESCC after neoadjuvant therapy. Patients with postoperative pCR can achieve better long-term survival.
