1.Main factors of hypokelemia after PCI for acute myocardial infarction

Objective To explore the main factors of hypokalemia in the patients with acute myocardial infarction after PCI.Method The clinical data of ninety-six patients with myocardial infarction after PCI were analyzed to look into the reasons for hypokalemia.Results Fifty-seven patients contracted hypokalemia,with a rate of 59.4%.The complication was cured by dietetic supplementation of potassium,oral supplementation of potassium,and potassium supplementation by atomization inhalation and intravenous infusion.The reasons for hypokelemia included inadequate input,over-ejectment,abnormal distribution of potassium and use of contrast agent.Conclusions Patients with myocardial infarction after PCI are more susceptible to hypokalemia.Therefore,it is a need to correct hypokelemia by means of potassium supplementation.

3.Tensile mechanical properties of lateral collateral ligaments in animal models of osteoarthritis

BACKGROUND:It is very nscessary for the prevention and treatment of ostaoarthritis(OA)to understand the tensile mechanical properties of lateral collateral ligaments(LCLs)of knee joints with osteoarthritis,while reports on it are still few.OBJECTIVE:To compare the LCL tensile mechanical property indexes of knee joints between normal animals and OA medels established for this purpose,and to provide qualitative and quantitative referential data about effects of OA on the LCL tensile mechanical properties of animal knee joints.DESlGN,TIME AND SETTING:A randomized controlled trial was finished at the Mechanical Expedmental Center of Jilin University from December 20th to 30th in 2008.MATERIALS:A total of 20 male SD rats of 6 months old were divided into a normal control group and a model group,with 10ones in each group.METHOOS:Rats in the model group were copied into OA models.after which test on tensile was performed to each of the ten rats in the two groups at the speed of 5mm/min on Universal Testing Machine,Shimadzu.Data obtained from the test were processed in polynomial though least square method.MAIN OUTCOME MEASURES:Tensile maximum load.maximum displacement,maximum stress,maximum strain and stress-strain curves.RESULTS:The tensile maximum load,maximum stress,maximum displacement and maximum strain in the normal control group and the model group were(12.754±2.795)N,(27.681±5.832)MPa,(2.754±0.707)mm,(11.679±2.373)%and (7.183±1.817)N,(16.162±3.403)MPa,(1.827±0.768)mm,(8.019±2.811)% respectively,from which we could see that each index in the normal control group was greater than that in the model group obviously (P<0.05).CONCLUSION:The tensile maximum load,maximum stress,maximum displacement and maximum strain all decreased after OA,which indicates that osteoarthritis has certain effects on tensile mechanical properties of LCLs of knee joints as well as stress relaxation.
4.The validity of applying questionnaires for screening mental disorder in the army

Objective To explore the most convenient and valid tool for screening mental disorders among servicemen. Methods Soldiers and officers stationed at Guilin were enrolled for the study. They were first examined with the Symptom Check-list 90 (SCL-90), Cornell Medicine Index (M-R)[CMI(M-R)], University Student Personality Inventory (UPI) and WHO neurosis screening schedule respectively. Then they were subjected clinical interview and examination individually, and clinical diagnosis of mental disorder was made according to CCMD-3 standard. The validity of each questionnaire was evaluated by using Criterion-related Validity. Results The sensitivity of WHO neurosis screening schedule was found to be 100%, and the specificity was 92.1%. SCL-90 and CMI (M-R) yielded similar results, their sensitivity was found to be 46.9% and 43.4%, and the specificity to be 86.1% and 78.9%, respectively. The sensitivity of UPI was found to be 83.1% and the specificity 46.1%. Conclusion The WHO neurosis screening schedule is the best in both sensitivity and specificity of validity, but its practice is time-consuming and laborious, which limits its application. SCL-90 and CMI (M-R) are imperfect in sensitivity of validity, and they cannot screen out the mental disorder in more than a half of the subjects. In a group test, they can hardly serve as a screening tool. UPI can screen out most of the mental disorder, and its sensitivity of validity reaches 83.1%. Additionally, the UPI questionnaire contains fake-test items, which could retest those who were missed due to untruly or not conscientiously answer the questionnaires. An acceptable way to obtain a true screening test is to give the individuals a propaganda of mental health and explain the questionnaires for 30 minutes before the test, which helps the examinee to understand the procedure and accept the questionnaires, so that the results of the test could be more reliable. In conclusion, UPI is a valuable screening tool to be recommended.

Objective To evaluate the effect of combination of growth hormone(GH),somatostatin(ST) with hypocaloric parenteral nutrition(HPN) on SAP patients with MOD. Methods forty one cases of SAP complicated with MOD were randomedly divided into control group (20 cases) and experiment group (21 cases). In experiment group ,total calories and nitrogen were given 62.9~83.9kJ/(kg?d) and 0.10~ 0.12g/(kg?d). lipid calorie was supplied in less than 40 percent of the total energy.GH was percutaneously injected for 7d and ST was intravenously injected for 7~14d. The stage of impaired glucose regulation is a necessary stage in the development of diabetes.Nearly a quarter of patients without diabetes intervention develop into diabetes within four to seven years without intervention.Therefore,the intervention of pre-diabetes treatment in the prevention and control of diabetes is of great significance.This study was to investigate the effect of Tangnaikang granule at the early stage of type 2 diabetes mellitus.The age,course of disease,sex,symptom score of TCM were recorded,so were the blood lipid,blood glucose,liver and kidney function,electrocardiogram were recorded.Under the condition of low sugar and low salt diet and healthy exercise,the patients were divided into the control group (n--31) and the treatment group (n=43).The patients in the double-blind control group were treated with Kangnaikang granule and Tangnaikang granule to treat 74 patients with type 2 diabetes mellitus three months.Three months after the detection of fasting and postprandial blood glucose,glycosylated hemoglobin,glucose tolerance,insulin tolerance and other indicators to examine the efficacy of both.Tangnaikang granules were superior to placebo in terms of effective rate of treatment,TCM symptom score and blood sugar recovery rate,and taking Tangnaikang granule without liver and kidney function and other injuries.Tangnaikang is safe and effective for pre-diabetes patients.

8.STUDIES ON XYLANASE PRODUCTION BY ASPERGILLUS CLAVATUS 22

A high xylanase producing strain 22 of Aspergillus clavatus was screened from 105 strains of molds and yeasts. The suitable medium consisted of (g/L): bagasse hemicellulose 30, NH_4NO_3 5, yeast extract 5, wheat bran 10, Tween 80 1 and a small quantity of other minerals; initial pH 5.5. Theoptimalsporeinoculumwas4.9X10~6spores/ml (final concentration). Theactivity of xylanase was as high as 335.9 U/ml in shake-flask experiment at 28℃ for 72 h. The optimal temperature and pH for xylanase reaction were 50℃ and pH 4.8. 72.6% of its original activity was remained after incubation at 50℃ for 1 h, and 90% of the enzym activity was observed upon storage at 8℃ for 9 days . Sugars. Na~+. Ca~(2+). and Zn~(2+) increased its activity wherease Co~(2+)and Cu~(2+) inhibited it.
9.Effects of acid sensing ion channels inhibitor amiloride on pain behavior in a rat model of post-incision pain

ObjectiveTo explore the effects and underlying mechanisms of acid sensing ion channels (ASICs) on pain behavior in a rat model of post-incision pain.MethodsFifty-eight adult male Sprague Dawley rats were used in this study,four rats were used for immunofluorescence test,thirty rats were employed for pain behavior test,and twenty-four rats were used for Western blot.Rats used for pain behavior test and Western blot were randomly divided into 3 groups:control group ( C group),incision pain model group ( I group) and amiloride group (A group).Plantar skin of rats in A group were infiltrated with 20 μl(200 μg)amiloride solution.Paw withdrawal mechanical threshold(PWMT) and paw withdrawal thermal latency(PWTL) of all rats in pain behavior test was tested at 24 h preoperative,2 h,4 h,8 h,12 h,24 h postoperative.Western blot was tested at 4 h postoperative.ResultsImmunofluorescence test displayed ASIC3 was expressed in plantar skin of all rats.The basal level of PWMT and PWTL of all rats in three groups was C group( (23.15 ± 5.10) g,( 11.32 ± 1.21 ) s),I group ( (23.26 ± 5.69) g,( 11.75 ± 2.01 ) s),A group ( (23.63 ± 4.96 ) g,( 11.47 ± 1.96) s) respectively,which was no significantly difference (P > 0.05 ).PWMT and PWTL of I group and A group was significantly lower than that of C group at all time points postoperative (P < 0.05) ; PWMT and PWTL of A group was at 2 h( ( 13.75 ±3.25)g,(9.96±1.32)s),4h((14.05±3.75)g,(9.17±2.11)s),8 h((9.75 ±2.74)g,(8.11 ±1.22)s)postoperative,which was significantly higher than that of I group (P < 0.05 ).Compared with that of C group,the level of pERK1/2 expression was significantly increased in I group at 4 h postoperative (P < 0.05 ),which could be inhibited by amiloride local infiltration (P < 0.05 ).ConclusionASIC3 can mediate incision pain in a rat model of post-incision pain,through pERK1/2 signaling pathway,which can be inhibited by amiloride.
10.Effects of preoperative intranasal dexmedetomidine in children inguinal hernia operation

Objective To observe the clinical effect of preoperative intranasal dexmedetomidine on sedation and preventing postoperative agitation in children inguinal hernia operation .Methods Forty children patients ,aged 1-s5 years old ,ASA Ⅰ ,were selected and equally randomized into 2 groups ,receiving intranasally dexmedetomidine 1 μg/kg(group Dex) and the same volume of normal saline (group NS) at preoperative 30 min respectively .The sedation score ,emotion score when separating from their par‐ents ,mask inhalation induction resistance score were evaluated at 30 min after medication ,furthermore the agitation scores were ob‐served and recorded at postoperative 30 min ,1 ,2 h .Results The sedation score ,emotion score when separating from their parents and mask inhalation induction resistance score in the Dex group were superior to the NS group ,moreover the agitation score at each points were lower than those in the NS group(P<0 .05) .Conclusion Preoperative intranasal dexmedetomidine can be safely used in children inguinal hernia operation ,has better sedative effect ,meanw hile reduces the postoperative agitation occurrence .
