1.The research about emotional approach coping and PTSD after Wenchuan-earthquake in unaffected region

Objective To investigate the post traumatic stress disorder scores of undergraduate and the correlation between emotional approach coping(EAC) in unaffected region after Wenchuan-earthquake.Methods PTSD Check list-Civilian Version (PCL-C) and emotional approach coping scale(EACS) were used to assessed 880 undergraduates in Nanjing. Results (1)The average score of PCL-C was 30.52±9.64, and the prevalence rates of PTSD was found 5.3%.(2)Gender difference were found in social function(male:1.66±0.80,female:1.52±0.63, t =2.664,P =0.005) and positive scores of PTSD(male:31.32±10.81,female:29.81±8.41,t =2.263,P =0.022).(3)No significant difference was found in different specialty between PTSD,but in emotional procession and emotional expression.(4)Emotional-approach coping and expose time in front of media came into the regression equation of PTSD symptoms.Conclusion The score of PCL-C of undergraduate in unaffected region are infected by how long he spend in front of media and how he use emotion-approach coping strategy to cope with this shock bringing by Wenchuan-Earthquake
2.Reliability and validity of Connor-Davidson resilience scale in Chinese college students

Objective To examine the reliability and validity of Chinese revision of connor-davidson resilience scale(CD-RISC) in Chinese college students.Methods 1610 college students were tested with CD-RISC.783 of them took a more test by BFI and SCL-90 at the same time.After three weeks,83 students were retested by CD-RISC.Results (1) The results of confirmatory factor analysis (x2/df =6.632,RMSEA =0.059,GFI =0.934,AGFI =0.915,CFI =0.927,NFI =0.915) indicated that the three-factor model reasonably fitted Chinese college students.(2) The Cronabach α coefficients of the CD-RISC and three factors called adaptability,tenacity and autonomy were 0.914,0.865,0.784,0.767 respectively; the mean inter-item correlation coefficients of them were 0.364,0.417,0.425,0.398 respectively; the Guttman split-half coefficients were 0.888,0.843,0.707,0.650 respectively; the retest-reliability coefficients were 0.856,0.742,0.777,0.747.(3) The scores of the total scale of CDRISC and three factors were significantly correlated to BFI and SCL-90(P<0.01).There were significant differences between the low-resilient group and the high-resilient group in every index of BFI and SCL-90.(4) There were significant differences between male and female students in CD-RISC and factor tenacity and autonomy(male:2.77±0.60,2.70-±0.74,2.65±0.68;female:Z70±0.53,2.62±0.67,2.54±0.62; P<0.05).Conclusion The Chinese version of CD-RISC is a reliable and valid method for assessing resilience in Chinese college students.
3.Application of ventricular septal defect occluders in infants and young children with large patent ductus arteriosus

Objective To observe the availability and safety of ventricular septal defect (VSD) occluder in infants and young children with large patent ductus arteriosus (PDA) associated with severe pulmonary hypertension.Methods Five patients (1 male and 4 fomale) of large PDA aged 5 months to 3 years,weighted from 5.1 to 15 kg,body surface area (BSA) 0.37-0.58 m2 underwent transcathter intervention with concentric VSD occluders from June 2008 to May 2009.Arterial ducta were tube-like and their diameters were 5.7 to 8.5 mm,with ulmonary vascular resistance from 4.8 to 5.7 Wood Unit,Qp/Qs 3.4-4.6.Three patients were given Bosentan after intervention.Results The large PDAs were successfully closed with VSD occluders,including 1 concentric perimembranous VSD occluder and 4 muscular VSD occluders.They all discharged 4 to 5 days with hidrosis and weight improved.Echocardiogram indicated VSD occluder was stable,no residue shunt and no stricture of left pulmonary artery and descending aorta were found.According to tricuspid and pulmonary regurgitation,pulmonary arterial pressure decreased differently and returned to normal after 6 months follow-up.Conclusion VSD occluder is available and effective to close large PDA associated with severe pulmonary hypertension in inrants and young children,but more cases and long-term follow-up are necessary.
4.Cognitive behavioral therapy in China: reflections on current issues and the future development

This paper summarizes the current situation of cognitive behavioral therapy (CBT) in China,and focuses on the issues existing in the current CBT application,including oversimplified and one-sided understanding of CBT,insufficient and inadequacy trainning,insufficient combination with Chinese Culture,reflection of the vocation being seen more important than back-feeding society,the lack of standardized practitioners management,and so on.Therefore,it is critical to strengthen the training of psychiatrists,establish a complete CBT training system,improve the training of practitioners,establish a CBT training system based on high education gradually,promote mutual understanding and cooperation of psychotherapists and psychiatrists,promote the research onto nativization of CBT,establish normalized certification system,promote the quality of CBT practitioners overall,strengthen service consciousness of awareness,and take advantage of CBT.
5. A case of Aspergillus salwaensis-induced spinal infection

Abstract: To report a case of Aspergillus salwaensis-induced spinal infection and its laboratory detection. The inflammatory granulation and necrotic tissue samples of a patient with spinal infection were collected from, the Affiliated Hospital of Chengde Medical College on June 17, 2020 for direct smear microscopy and culture, and the isolated strain was identified by microscopy by smear staining, matrix-assisted laser desorption ionization-time-of-flight mass spectrometry (MALDI-TOF-MS), molecular identification and in vitro antifungal susceptibility test. The patient was 62 years old female and presented with recurrent chest and back pain with no obvious cause. The initial diagnosis was spinal infection, after 7 days of treatment with levofloxacin, the effect was not good. Surgery was then performed remove the lesion via posterior thoracic debridement, and fungal hypha was observed under microscope in tissue specimens. The isolated strains had no typical structure, MALDI-TOF-MS was used for identification for many times, but there was no identification result. After 7 days of fluconazole treatment, the patient's condition improved, and her chest and back pain were alleviated compared to before surgery. The patient was discharged and followed up in the outpatient department, the fungus was later identified as Aspergillus salwaensis by sequence analysis of the internal transcribed spacer (ITS) gene sequencing, and the patient's antifungal medication was changed to voriconazole after with the attending physician. The patient consciously recovered well with no pain in the operative area and normal spinal activity at 1 year follow-up. The possibility of spinal fungal infection should be considered in patients with back pain without a clear cause and poor response to routine antibiotic treatment. Direct smear report of microscopic results are very important for guiding clinical antibiotic selection for rare filament fungi with atypical colony and microscopic morphology and unsuccessful MALDI-TOF-MS identification, molecular biological methods such as ITS sequence analysis can be helpful for early identification of the fungal species, improving identification speed.
6.Study on quality evaluation of Buddleja officinalis from different habitats based on HPLC fingerprint and content determination

Objective:To establish UPLC fingerprint method and 2 contents determination methods of Buddleja officinalis; To provide a reference for improving the quality control standard and evaluation of Buddleja officinalis from different habitats.Methods:UPLC method was used to establish the fingerprints of 17 batches of Buddleja officinalis. The similarity evaluation, clustering analysis, principal component analysis and orthogonal partial least squares discriminant analysis were used to compare the quality differences of Buddleja officinalis from different habitats. The contents of acteoside and linarin in Buddleja officinalis were determined.Results:There were 12 common peaks in UPLC fingerprints of Buddleja officinalis, six of which were identified as echinacoside, acteoside, cynaroside, isoacteoside, linarin, and apigenin. The fingerprint similarity of 17 batches of Buddleja officinalis was more than 0.9; Buddleja officinalis from different habitats were classified into 2 groups. Five differential markers were determined by OPLS-DA analysis. The order of significance was acteoside > peak 3 > echinacoside > isoacteoside > linarin. Edgeworthia chrysantha was identified by the method of fingerprint as counterfeit. The results of content determination showed that the content of Buddleja officinalis in Hubei and Sichuan was the high and stable.Conclusion:The method can effectively analyze the differences of Buddleja officinalis from different habitats, and provide reference for the quality control of Buddleja officinalis.
7.Early diagnosis, prevention and treatment for calcific uremic arteriolopathy.

Calcific uremic arteriopathy (CUA), termed calciphylaxis, is a rare but highly fatal clinical syndrome. There is no clearly laboratory diagnostic criteria for CUA. The medium and small arterial calcification and microthrombosis discovered by skin biopsy, radiologic imaging,bone scan and the evidence of activation of the bone morphogenetic protein signal (BMPs) transduction pathway are useful for early diagnosis of this disease. The common therapies (including intravenous sodium thiosulfate (STS) and bisphosphonates, hyperbaric oxygen therapy and other symptomatic supports) are used for the management of wounds, pain, nutrition, dialysis and so on. Controlling the chronic kidney disease-mineral and bone disorder (CKD-MBD) and some complications of dialysis and drugs (such as warfarin, active vitamin D) can prevent CUA. However, CUA patients still have poor prognosis and high mortality. Since some patients progress rapidly, it is of great importance to make early diagnosis and provide effective treatments with multidisciplinary management.

8.Meta-analysis and Trial Sequential Analysis of Therapeutic Efficacy of Modified Cangfu Daotan Decoction Combined with Chemical Medicine versus Chemical Medicine Alone in the Treatment of Polycystic Ovarian Syndrome

OBJECTIVE: To systematically evaluate therapeutic efficacy of modified Cangfu daotan decoction (MCDD) combined with chemical medicine versus chemical medicine alone in the treatment of polycystic ovarian syndrome (PCOS), and to provide evidence-based reference for clinical decision. METHODS: Retrieved from PubMed, Embase, Cochrane Library, CJFD, Wanfang database, VIP and CBM, randomized controlled trials (RCTs) about MCDD combined with chemical medicine [ethynestradiol cycloprogesterone (Diane-35), clomiphene, metformin] (trial group) versus chemical medicine alone (control group) in the treatment of PCOS were collected. After data extraction and quality evaluation with Cochrane 5.1.0 bias risk evaluation tool and Jadad scale, Meta-analysis was conducted for total response rate, serum hormone level (FSH, LH, LH/FSH, testosterone), BMI, ovulation rate and physical signs (hirsutism, acne) by using Stata 14.0 software. Trial sequential analysis(TSA)was conducted by using TSA 0.9 software. RESULTS: A total of 20 RCTs were included, involving 1 484 patients. Results of Meta analysis showed that total response rate [RR=1.13,95%CI (1.02,1.24),P<0.05], serum hormone level {FSH [WMD=-0.59,95%CI(-0.98,-0.20),P<0.05],LH [WMD=-0.95,95%CI(-1.41, -0.52),P<0.05],LH/FSH [WMD=-1.04,95%CI(-1.78,-0.33),P<0.05],testosterone [WMD=-0.93,95%CI(-1.38,-0.28),P<0.05]}, BMI [SMD=-1.01,95%CI (-1.76,-0.27),P<0.05], ovulation rate [RR=1.17,95%CI(1.02,1.34),P<0.05] and physical signs {hirsutism [WMD=-0.48,95%CI(-0.86, -0.10),P<0.05], acne [WMD=-1.16,95%CI(-1.56,-0.75),P<0.05]} of trial group were all better than those of control group, with statistical significance. TSA showed that there are reliable evidences for MCDD combined with chemical medicine in the treatment of PCOS. CONCLUSIONS: Versus chemical medicine alone in the treatment of PCOS, MCDD combined with chemical medicine can improve total response rate and ovulation rate, reduce serum hormone levels, BMI and physical signs.
9.Efficacy analysis of radiotherapy combined with surgery for locally advanced rectal mucinous adenocarcinoma: a retrospective study based on data of Surveillance, Epidemiology, and End results population.

OBJECTIVE:
To explore the efficacy of radiotherapy combined with surgery for locally advanced rectal mucinous adenocarcinoma.
METHODS:
Clinical data of patients with locally advanced rectal mucinous adenocarcinoma (T3-4 and/or N+) diagnosed by postoperative pathology from 1992 to 2013 were retrieved from the US Surveillance, Epidemiology, and End Results (SEER) database. Patients with local excision only, tumor biopsy or combined organ excision and incomplete follow-up information were excluded. All the enrolled patients were divided into three groups according to different treatments, including surgery alone (SA) group, preoperative radiotherapy combined with surgery (RT+S) group and surgery combined with postoperative radiotherapy (S+RT) group. The extracted data included basic data of patients and tumor, treatment status, and follow-up results. The χ² test was used to compare the count data. Kaplan-Meier method was used to draw the survival curve and calculate the survival rate. The survival was analyzed and compared by Log-rank test. The R language 2.8.1 was used to match the patients as 1:1 pairing through the propensity score matching (PSM). The matching variables included gender, age at diagnosis, year at diagnosis, ethnicity, degree of tissue differentiation, TNM stage, depth of invasion, making the baseline data of subgroups comparable. The Cox proportional hazard model was used for multivariate analysis of prognostic factors.
RESULTS:
A total of 2 149 patients with locally advanced rectal mucinous adenocarcinoma were enrolled in the study, including 1 255 males (58.4%) and 894 females (41.6%). There were 706 patients (32.9%) in the SA group, 772 patients (35.9%) in the RT+S group and 671 patients (31.2%) in the S+RT group. In SA, RT+S and S+RT groups, the median overall survival time was 39, 85, and 74 months respectively; the 5-year overall survival (OS) rate was 38.7%, 56.5%, and 55.2% respectively; the median cancer-specific survival (CSS) time was 86, 127, and 111 months respectively, and the 5-year CSS rate was 53.7%, 62.2% and 60.7% respectively. In comparison among the 3 groups, the 5-year OS rate and CSS rate in the SA group were significantly lower than those in the RT+S group and S+RT group (all P<0.001); the 5-year OS rate and CSS rate between RT+S group and S+RT group were not significantly different (P=0.166 and 0.392,respectively). After the baseline data of subgroups were corrected through PSM, the 5-year OS rate and CSS rate in the SA group (n=375) were significantly lower than those in the RT+S group (n=375)(OS:40.1% vs. 54.5%, P<0.001; CSS:54.3% vs. 63.3%, P=0.023). The 5-year OS rate and CSS rate in the SA group (n=403) were also lower than those in the S+RT group (n=403) (OS:37.4% vs. 54.7%,P<0.001;CSS:51.6% vs. 61.0%,P=0.031). The 5-year OS rate and CSS rate between RT+S group (n=363) and S+RT group (n=363) were not significantly different (OS:51.7% vs. 55.5%, P=0.789; CSS:57.7% vs. 60.5%, P=0.484). Cox multivariate analysis showed that radiotherapy (HR=0.845, 95%CI: 0.790 to 0.903, P=0.001) was an independent prognostic factor for OS of locally advanced rectal mucinous adenocarcinoma; radiotherapy (HR=0.907, 95% CI: 0.835 to 0.985, P=0.021) was also an independent prognostic factor affecting CSS in patients with locally advanced rectal mucinous adenocarcinoma.
CONCLUSION
As compared with surgery alone, surgery combined with preoperative or postoperative radiotherapy is beneficial to the long-term survival of patients with locally advanced rectal mucinous adenocarcinoma.

10.Comparison of Chemical Constituents of Sinapis alba before and after Stir-frying Based on UPLC-MS and Chemometrics Methods

OBJECTIVE:To compare the chemical components in Sinapis alba before and after stir-frying. METHODS : UPLC-Q-Exactive Obitrap MS was adopted to analyze chemical constituents of S. alba before and after stir-frying. The determination was performed on Waters CORTECS T 3 column with mobile phase consisted of methanol- 0.1% formic acid solution (gradient elution )at the flow rate of 0.25 mL/min. The column temperature was 30 ℃ and the sample size was 2 μL. High resolution MS adopted heating electrospray electron source ,positive ion scanning mode ,scanning range m/z 120-1 000. The chemical constituents of S. alba before and after stir-frying were identified by Compound Discover 3.2 software combined with relevant database ,and the content changes of chemical constituents were analyzed by using peak area. Chemometrics analysis was performed for the content changes of chemical constituents using peak area as variable. RESULTS :A total of 54 chemical components were identified in S. alba ,mainly fatty acids (represented by erucic acid ),alkaloids(represented by sinapine ), flavonoids. After stir-frying ,the contents of 19 chemical components changed significantly ,of which the contents of 10 components decreased significantly and those of 9 components increased significantly (P<0.05). Principal component analysis and orthogonal partial least squares discriminant analysis could clearly distinguish S. alba from stir-fried S. alba . CONCLUSIONS :The contents of some chemical components of S. alba change significantly after stir-frying ,which may be one of the important reasons for the change of efficacy after stir-frying.
