1.Transumbilical single site versus conventional three-port appendicectomy with a systematic review and Meta analysis

Objective This study aimed at evaluating the safety and feasibility of transumbilical single site laparoscopic appendectomy (SSLA) by systematic review and Meta analysis of literature comparing with conventional tree-port laparoscopic appendectomy (CLA).Methods The articles published from January 2000 to December 2012 were searched from electronic databases of PubMed,Embase,and the Cochrane Library.The literature with comparative studies of SSLA with CLA for adult patients was selected.And,the systematic review and Meta analysis carried out using Stata software.Results Eleven articles with 998 cases receiving operation during March 2008 to October 2011 were enrolled into study.There were 435 cases with SSLA and 563 cases with CLA.Compared the cases with SSLA and cases with CLA,there was no difference significantly in operative time (z =1.48,P =0.140),pain score in the 24 hours after surgery (z =0.83,P =0.409),diet start(z =0.38,P =0.707),postoperative complications (z =0.46,P =0.647),hospital stay (z =0.36,P =0.722).Conclusions SSLA are not different in operative time,postoperative pain,diet start,postoperative complications,hospital time comparing with that in CLA.
2.THE EXPRESSION OF nNOS IN THE CHRONIC SPINAL CORD COMPRESSIVE INJURIES

Objective To study the immunohistochemistry change of neuronal nitric oxide synthase(nNOS) during the chronic spinal cord compression. Methods 18 healthy rabbits were randomly divided into normal,control and compressed groups.Membranous sac filled with cardiografin was applied to produce an animal model of chronic spinal cord compression.The sac was gradually enlarged resulting in chronic spinal cord compression in compressed groups for 12 weeks.Nissl's staining was applied to observe histopathological change and immunohistochemistry to nNOS change. Results The damage of motorneurons in the compressed block of compressed group was observed.No histopathological change was observed in normal and control groups.The number of nNOS positive motor neurons in the compressed block of compressed group was higher than that in the blocks of other groups.Conclusion\ The NO synthesis increased in chronic spinal cord injury.\;[
3.Comparison of two treatment reconstruction modalities for hilar bile duct stricture

Objective To explore an effective treatment for inflammatory hilar bile duct stricture. Methods The clinical data with patients of intra hepatic cholelithiasis complicated with hilar bile duct stricture that received plastic repair of hilar bile duct stricture(PHBDS) with pedicled cholecystic graft or Roux en Y cholangio jejunostomy (RYCJ) in our hospital during recent 10 years were retrospectively analyzed. The peri operative conditions, postoperative morbidity of cholangitis and recurrence of hepaticolithiasis were observed . Results The peri operative morbidity of biliary fistula and inflammatory ileus were not significantly different between the two groups. The postoperative morbidity rate of cholangitis was 5.66% and 21.88%,(P=0.010) and recurrence rate of hepaticolithiasis was 3.77% and 16.67%,(P=0.021) in PHBDS group and RYCJ group respectively . Conclusions PHBDS is an easy and safe operation. RHBDS can preserve the physiologic function of Oddi′s spincter, so the reflux of intestinal contents to bile duct is avoided , and the results are better than the RVCJ.
