Objective To investigate the incidence of insomnia in patients with nasopharyngeal carcinoma during radiotherapy and analyze the influencing factors.Method A sleep quality questionnaire for patients with nasopharyngeal carcinoma during radiotherapy was designed and used for the investigation of the incidence of insomnia as well as the influencing factors related to insomnia.Results The incidence of insomnia was 42.2%.Univariate analysis revealed that radioactive xerostomia,nausea,hospital environmental noise,radioactive oral mucositis,worry about job,worry about prognosis,burp,vomiting,worry about family influence were related to insomnia in nasopharyngeal carcinoma patients during radiotherapy;multivariate analysis showed that radioactive oral mucositis,environmental noise and job stress were the independent factors leading to insomnia(All P<0.05).Conclusions There is a high incidence of insomnia in patients with nasopharyngeal carcinoma during radiotherapy.Radioactive oral mucositis,environmental noise and job stress might be the independent factors relative with insomnia during radiotherapy.

BACKGROUND: Ginkgetin has been widely acknowledged as having multiple pharmaceutical values in domestic and abroad. In many western countries, ginaton is imported in large amount. Domestic production of ginkgetin is great, however, seldom applied and there is no ginaton agent for injection.OBJECTIVE: To observe the effects of the increased contents of total flavonoid glycoside and quercetin glycoside of ginkgetin injection on memory function of mice, superoxide dismutase (SOD) activity in myocardial tissues and hemorrheological indexes of rabbits with ischemia-reperfusion injury.DESIGN: Randomized control animal experiment.SETTING: Hebei Medical College of Employees.MATERIALS: Ninety Kunming mice (3-4 months old), weight (25±1) g and thirty-two Japanese rabbits (3-4 months old) were selected. Self-made high-purity ginkgetin injection [5 mL containing 17.5 mg ginaton, of which there were 8.7 mg ginkgo flavonoid glycoside (49.8%) and 4.61% lactone];Ginkgetin injection made in German (Jinnaduo): Manufactured by Schwabe Germany [5 mL containing 17.5 mg ginaton, of which there were 4.2 mg ginkgo flavonoid glycoside (24%)].METHODS: The experiment was conducted in the Central Department of Experimental Animal, Hebei Medical College of Employees from September to November 2002. ①Ninety mice were randomly divided into 6 groups:1, 2 mL/kg self-made ginkgetin injection group, 1, 2 mL/kg German ginkgetin injection group, model group and control group with 15 mice in each group. Mice in the model group and control group were intraperitoneally injected with normal saline, mice in each group were intraperitoneally injected respectively with testing medicine for 10 continuous days,One hour after the 10th day of administration, mice were intraperitoneally injected with 2 mL/kg scopolamine hydrobromide and dysmnesy models were duplicated. Ten minutes after that, mice were put in the step-down instrument for 36V-voltage-stimulus after accommodation. Measurements were re-performed respectively at 5 minutes and 24 hours after training.Latency and times of electric shock within 5 minutes were recorded.②Thirty-two Japanese rabbits were selected and randomly divided into normal control group, German ginkgetin injection group, 1 mL/kg, 0.5 mL/kg self-made ginkgetin injection group with 8 rabbits in each group. Medicineswere continuously injected into aural veins. Three days after administration, blood was collected to detect the hemorheological indexes. ③Thirtytwo rabbits were randomized into 4 groups: 1 mL/kg, 2 mL/kg self-made ginkgetin injection group, 2 mL/kg German ginkgetin injection group and normal saline group with 8 ones in each group. Rabbit models with ischemia-reperfusion injury in myocardial tissues were established, rabbits were ligated for 30 minutes and then unclamped to get ischemic reperfusion injury in myocardial tissues, testing drugs were injected via carotid artery at the moment of reperfusion according to different groups. Before reperfusion and 30, 60 minutes after reperfusion, blood was drawn from the arteria femoralis, activity of SOD in plasma was measured. Animals were executed to obtain myocardial tissues so as to measure SOD activity.MAIN OUTCOME MEASURES: ①Latency and times of shock within 5 minutes in the experiment were recorded. ②Hemorheological indexes and determination of SOD activity in myocardial tissues.RESULTS: All experimental animals were involved in the analysis of results and no one died. ①Test for memory: Latency and times of errors in step down test in the injection group were better than those in the control group, and differences were significant (P ＜ 0.01 or 0.05). Times of errors within 5 minutes in 1 mL/kg self-made ginkgetin irjection group was less than that in the German ginkgetin injection group and the differences were obvious.②Hemorheological indexes: Whole-blood viscosity low shear value,rigid index of red cells and gathering index of red cells etc. in injection groups decreased. ③SOD activity: Compared with control group, that was significantly increased in 1, 2 mL/kg self-made ginkgetin injection group and were in a dose-dependent manner. Those were obviously increased in the 1 mL/kg German ginkgetin injection group. Increase in SOD activity of ischemic myocardial tissues in the 1 mL/kg self-made ginkgetin injection group was more significant than that in the 2 mL/kg German ginkgetin injection group.CONCLUSION: Both self-made and German ginkgetin injections can enhance the ability of memory; While at the basis of same dose, self-made high-purity ginkgetin injection is superior to German ginkgetin injection.

Its character and classification are introduced, especially basic prineiple, development and application. The technology of water-jet will paly an important role in medicine.

Objective To explore the effects of H 2 S on neuronal injuries induced by oxygen glucose deprivation /reoxygenation ( OGD/R) in cortical neurons .Methods For OGD, the primary cultured cortical neurons were incubated with glucose-free EBSS media for 4h in N2/CO2/O2 (93%/5%/2%) atmosphere.Thereafter, the media were replaced by Neurobasal/B27 culture media and the neurons were incubated for 12 h in a 5%CO2 incubator at 37℃.NaHS was used as a H2S donor and cell survival rate was determined by cell counting kit 8(CCk-8).[Ca2+]i was determined using fura-2/AM and fluorescence microscopic imaging systems .The release rate of lactate dehydrogenase ( LDH) was determined by lactate dehydrogenase assay kit , and cell damage was analyzed by staining of propidium iodide ( PI ) .Results After pretreated with 200, 300 and 600μmol/L sodium hydrosulfide ( NaHS) for 30min before OGD/R, the cell survival rate of neurons significantly increased (n=4).[Ca2+]I(n=5), LDH release rate (n=4) and cell damage percentage (n=6) in the neuron pretreated with 300 μM NaHS were significantly lower than those in ODG/R cells.Treatment with 10μmol/L calcium chelator BAPTA also reduced the LDH release rate and cell damage percentage induced by ODG /R in neurons . Conclusion The results indicate that H 2 S may inhibit the OGD/R induced damage in cortical neurons via reducing calcium overload of neurons .

Objective To study the pathologic feature and rational diagnosis and treatment of Mirizzi Syndrome. Method The clinical data of 43 cases treated by surgery were retrospectively analysed. Results All the 43 cases underwent operation, including partial cholecystecomy in 8 cases, cholecystectomy in 16cases , cholecystectomy plus common bile duct exploration with T tube drainage in 9 cases, choledochojejunostomy in 10 cases. Of the 43 cases, 36 cases were followed up for 1～5 years. Of them, 29 cases were in excellent, 6 cases in good and 1 case in poor. Conclusions The pathologic type of Mirizzi Syndrome is variant. It is difficult to make a definite diagnoses before operation. So vary imaginal technique should be adopted. Different operative procedures should be used according to patients' pathologic type.

Objective To assess the long-term surgical outcomes,patient's satisfaction and functional results of correction for patients with penile curvature by 16-dot minimal tension plication.Methods Between August 2004 and December 2010,92 patients with penile curvature underwent surgical correction of penile curvature by 16-dot minimal tension plication.Of them,86 patients (53 patients with congenital penile curvature and 33 patients with Peyronie's disease (PD)) were reached for long-term follow-up.Indications for operation included patients with 30 to 90 degrees of penile curvature,penile length greater than 10 cm and patients who had one or more symptoms,such as difficulty or inability to vaginal penetration and partner discomfort during sexual intercourse.Patients were excluded from study if they had severe penile curvature (greater than 90 degrees),hourglass deformities or large plaques.Preoperative evaluation included international index of erectile function 5 (IIEF-5) questionnaire score,pain on erection and physical examination.Patients were recommended daily low dose phosphodiesterase type 5 (PDE-5) inhibitors 2 weeks after surgery and maintained for 2 to 3 months.At follow-up,recurrence rate,complications,erectile function,patients and partner's satisfaction with the surgery were assessed.Results The mean time of follow-up was 79 months (range,35-107 months).Of 86 patients who had been reached for long-term follow up,82 patients (95.3％) and 77 partners (89.5％) reported overall satisfaction after the surgery.Complete penile straightening was achieved in 79 patients (91.8％).Slight residual curvatures were observed in 7 patients,but did not affect their sexual intercourse.Postoperative penile length shortening (median penile length loss 1.3 cm) was noted in 35 patients (40.7％),which did not affect their sexual intercourse.In addition,no de novo erectile dysfunction was observed,and sexual function was significantly improved evidenced by the IIEF-5 scores,especially in the patients who had received PDE-5 inhibitors for 2 to 3 months.There were no surgery-related complications,such as hematoma,urethral injury,or infection.Conclusion The longterm results demonstrate that correction of penile curvature using 16-dot plication is a simple and safe method to achieve cosmetic and functional satisfaction in patients with mild to moderate penile curvature.

Objective To evaluate the cost-effectiveness of various immuno-diagnostic systems for trichinosis and to find out the best testing system with low cost and high effectiveness.Methods The basic methods for the research are to difine and classify the direct and indirect cost for each testing system, and to identify a cost-effective testing system which would be practical and workable in case finding and/or mass survey. The data collected were analysed with parameters which are as follows: (1) the parameter of cost effectiveness expressed by cost/one sample in a given test that is unit cost. (2) the parameters of cost effectiveness: (a) for finding patients: cost/positive rate; (b) for examination carried out in normal individuals: cost/(100%-positive rate); (c) for the result of cross reaction: cost/(100%-cross reaction rate).Results Among the costs for detecting antibodies,the total costs vary from 98.06 yuan to 193.15 yuan. F-ELISA is the cheapest test. The unit cost for the five studied testing systems is ranging from 3.98 yuan to 4.22 yuan. According to the cost, the recommended order would be: F-ELISA, SPA-ELISA, ELISA, Western blot and IIP.Using the value of cost/effectiveness in analysis of the cost and comparing the results of detecting the positive cases ,the normal persons and cross reaction. The final results show that the methods for detecting antibodies are not only cheap but also practical and the preference for selection would be F-ELISA, ELISA and SPA-ELISA. The best method for detecting antigens is F-PcAb.Conclusions When these methods are used in practice, we suggest that F-ELISA should be the best method for choice in screening, when the result is positive, F-PcAb may be used for confirmation and can obtain a correct diagnosis.

Objective To study the the causes, management and preventive measures for complications of holmium laser ureterolithotripsy. Methods Three hundred and seven cases of uteteral calculi from Jan. 2006 to Jan. 2010 were reported (203 men and 104 women, age ranged from 18-83 years,mean age 51.6 years). One hundred thirty-three patients had calculi in the lower ureter,94 in the middle part and 80in the upper ureter. Results The overall comminution rate was 89.9% (276/307 cases). sixteen cases (5.2%) were followed by ESWL due to the shift of the stones to the renal pelvis. The operation failed in 9 cases(2.9%), 3 cases(1%) had perforation,and recoveried after open operation. Deep infection was found in 3(1%)cases,which recoveried after anti-infective therapy. Conclusions Holmium laser ureterolithotripsy is an effective method for treating ureteral calculi, with advantage of high efficiacy,safety and minimal invasion. To master the operating induction, to raise operating technique and to take correct measure could reduce complications effectually.

Objective: To investigate the effect of the frenulum identification positioning method with a disposable suture device in circumcision for the prevention of postoperative penile frenulum malposition. METHODS: Totally 212 patients with phimosis or redundant prepuce underwent circumcision from March 2015 to September 2016, including 109 cases of conventional circumcision (the control group) and 103 cases treated by frenulum identification positioning with a disposable suture device (the observation group). We observed the postoperative position of the penile frenulum and median raphe and compared the deviation angles of the frenulum between the two groups of patients. RESULTS: The median of penile frenulum deviation angle (interquartile range) was 0 (3.56) in the observation group, significantly smaller than 12.41 (19.59) in the control (P <0.001, P = 0.000). And the rate of frenulum deviation was remarkably lower in the former (8.74% ［9/103］) than in the latter group (66.06% ［72/109］) (P <0.01). CONCLUSIONS Circumcision using the frenulum identification positioning method with a disposable suture device can effectively avoid postoperative penile frenulum malposition. With the advantages of safety and easy operation, it deserves clinical application and popularization.

OBJECTIVETo explore the possible relationship between erectile dysfunction (ED) and benign prostate hyperplasia (BPH) in men, and to assess the positive effect of Sildenafil on the lower urinary tract symptoms (LUTS) from BPH while treating ED.

METHODSThirty-two patients with ED and BPH were offered oral Sildenafil and reviewed before and six months after the administration of Sildenafil by the International Index of Erectile Function-5 (IIEF-5) and the International Prostate Symptom Score (IPSS) questionnaires. Scores were tested by chi-square.

RESULTSIIEF-5 scores were increased by 42.36% and IPSS scores declined by 20.14%, with statistical significance (P < 0.01).

CONCLUSIONTreatment of ED with Sildenafil appears to improve urinary symptom scores. A lower IPSS at baseline seems to predict a better response to Sildenafil therapy for ED.
