Objective By analyzing the urinary calculi ingredient and 24-hour urinary stone risk factors of urolithiasis patients in Hengyang area,to investigate the effects of diet intervention on recurrence rate in urolithiasis patients, and provide the measures for prevention and treatment. Methods Prospectively collected 692 patients that permanent residents in Hengyang area from September 2008 to September 2012,who had implementation of minimally invasive operation and taken stone specimens to analyze composition, and also collected 24 hours urine to analyze the urinary stone risk factors. They were divided into test group and control group by random number table method,346 cases in each, control group without diet intervention, and test group was given diet intervention according to the stone composition and urinary stonerisk factors. All patients were followed up for 1 year, the urinary stone recurrence rate in Hengyang area was observed. Results Among 692 urolithiasis patients,663 patients completed the study (test group of 341 cases and control group of 322 cases),the expulsion rate was 4.19%(29/692).The 24-hour urinary stone risk factors in control group before and after diet intervention had no significant difference(P > 0.05).In test group after diet intervention, the excretion of ingredients in urine such as dietary calcium (t =3.412,P < 0.05),oxalate(t =3.018,P < 0.05) and uric acid(t =1.990,P < 0.05) was obviously decreased, and urinary citrate (t =3.174,P < 0.05) was increased, but the excretion of ingredients such as magnesium and phosphorus had no significant difference (P > 0.05).After 1 year after diet intervention, the recurrence rate in test group was lower than that in control group [0.88% (3/341) vs.7.76% (25/322)],there was significant difference (P < 0.01).Conclusion Diet intervention can effectively reduce the risk of urinary stone according to the stone composition and the 24 hours urine stone risk factors, plays an important role on reducing urinary stone recurrence, which is worth clinical promotion.

Objective To observe the curative effect of noninvasive mechanical ventilation in the treatment of patients with Respiratory failure. Methods 90 cases of patients with Respiratory failure from emergency care unit in November 2012 to November 2014 were randomly divided into observation group and control group, each group included 45 cases. Observation group was provided noninvasive ventilator therapy based on the traditional drug treatment, while the con-trol group was given conventional treatment and oxygen. The heart rate, breathing rate, arterial blood gas index and the clinical curative effect were compared after 5 treatment hours within two groups. Results The breathing rate, heart rate, oxygen partial pressure, blood oxygen saturation had significant differences between before and after treatment in both groups(P<0.05). The treatment effect of observation group was significantly better than the control group, and the difference was statistically significant (P<0.05).The difference of total effective rate was statistically significant be-tween two groups(P<0.05). Conclusion Noninvasive mechanical ventilation treatment can rapidly improve the symp-toms and rectify hypoxemia of patients with acute left heart failure, and the curative effect is remarkable.

OBJECTIVE:
To clarify the association between regulatory region of HLA-DPB1 (3'UTR) with Naso pharyngeal carcinoma in Guangdong Province Hans.
METHOD:
The allelic types of HLA-DPB1-3'UTR were detected by sequence specific primer (SSP) in 104 patients with NPC and 105 control individuals from Guangdong Province Hans.
RESULT:
The frequencies of allelic types B/B, haplotype B were higher in patients with NPC than those of the control individuals.
CONCLUSION
Positive association may exist between certain HLA-DPB1 alleles and NPC in Guangdong Province Hans.

Objective To investigate the pattern of nodal recurrence after curative resection in adenocarcinoma of the gastroesophageal junction ( AGE ) , and to provide a basis for delineation of the radiation range in the high-risk lymphatic drainage area. Methods A retrospective analysis was performed in 78 patients with locally advanced AGE who were newly treated in our hospital from January 2009 to December 2013 and had complete clinical data. All patients received curative resection and were pathologically diagnosed with stage T3/T4 or N (+) AGE.Those patients were also diagnosed with SiewertⅡor Ⅲ AGE by endoscopy, upper gastroenterography, macroscopic examination during operation, and pathological specimens.None of the patients received preoperative or postoperative radiotherapy. All patients were diagnosed by imaging with postoperative nodal recurrence. The computed tomography images of those
patients were accessible and had all the recurrence sites clearly and fully displayed. Results The median time to recurrence was 10 months ( 1-48 months) , and 90%of the recurrence occurred within 2 years after surgery. The lymph nodes with the highest risk of recurrence were No.16b1( 39%) , No.16a2( 37%) , No.9 (30%), and No.11p (26%), respectively. There was no significant difference in the recurrence rate within each lymphatic drainage area between patients with SiewertⅡandⅢAGE ( P=0.090-1.000) .The lymph nodes with the most frequent recurrence were No.16b1, No.16a2, No.9, No.16b2, No.11p, and No.7 in patients with stage N3 AGE and No.11p, No.16b1, No.16a2, No.9, No.8, and No.7 in patients with stage non-N3 AGE.Patients with stage N3 AGE had a significantly higher recurrence rate in the para-aortic regions (No.16a2-b2) than those with stage non-N3 AGE (67%vs.33%, P=0.004, OR=4.00, 95% CI=1.54-10.37) .Conclusions The lymph nodes with the highest risk of recurrence are located in the celiac artery, proximal splenic artery, and retroperitoneal areas ( No.16a2 and No.16b1) in patients with SiewertⅡorⅢlocally advanced AEG.Moreover, patients with stage N3 AGE have a higher risk of retroperitoneal recurrence. The above areas should be involved in target volume delineation for postoperative radiotherapy.
