1.Development and Practice of Electronic Prescription Evaluation System for Outpatient Department

OBJECTIVE:To promote the working efficiency of hospital prescription evaluation and improve pharmacists profes-sional quality through developmenting electronic prescription evaluation system (EPES) for outpatient department used by taking full advantage of large amount of information of electronic prescription. METHODS:By using system structure of Client/Server, Web Service middleware,programming tool of PowerBuilder and database technique of Oracle and sharing Hospital information sys-tem data,EPES was designed and applied,which could collect prescription information and generate summary form automatically after artificial prescription evaluation. RESULTS & CONCLUSIONS:Integration of outpatient electronic medical records,historical prescriptions and medical test results query system make the EPES carry comprehensive information which could make up for disad-vantages of paper prescription form in the aspect of obtaining outpatient medical records,historical prescriptions or medical test re-sults. EPES can accomplish general and special prescription evaluation and also pledges the equity and justness of process by ran-dom sampling. The operation of the system could improve the efficiency from taking 24-36 h in paper prescription form to 4-6 h to complete general prescription evaluation,which also improves professional quality of pharmacists by intensive training.
2.Carry Out Pharmaceutical Care Surrounding Pharmacists' Visiting Card and Double - efficiency Elevation

OBJECTIVE:To discuss approaches of developing pharmaceutical care in the primary level hospitals.METHODS: The establishment of pharmacists' visiting card system was introduced and the outcome of carrying out pharmaceutical care by establishing pharmacists' visiting card system was summarized. RESULTS:The practice of pharmaceutical care by enforcing pharmacists' visiting card system enriched the connotation of clinical pharmacy work, raised pharmacists' position both in the minds of patients and in the clinics,enriched the contents of hospital dispensing and preparations and meanwhile increased economic returns.CONCLUSION:Being an initiative attempt,pharmacists visiting cards system is a passive pharmaceutical care mode.To do a better job in the pharmaceutical care, pharmacists must improve their service level by taking into account of clinical needs.
3.Video-assisted Thoracoscopic Cardiac Surgery for Congenital Heart Diseases

Objective To probe the feasibility of video-assisted thoracoscopic operation for congenital heart diseases.Methods From October 2004 to August 2008,156 patients with congenital heart disease,including 79 cases of atrial septal defect(ASD),65 cases of ventricular septal defect(VSD),1 case of aortic aneurysm rupture combined with VSD,3 cases of double chamber right ventricles(DCRV) combined with VSD,and 8 cases of partial anomalous pulmonary venous drainage,were treated totally under thoracoscope.Surgical procedures were performed through 3 trocars inserted at the right chest wall,and catheters were placed in the right femoral artery and vein to set up extracorporeal circulation.The ascending aorta was cross-clamped with long tailor-made forceps and the myocardium was protected by coronary perfusion with cold crystalloid cardioplegia. Results All the thoracoscopic operations were completed successfully.The mean extracorporeal circulation and cross-clamping time were(74?28) min and(29?13) min respectively.Postoperative ventilation was withdrawn in(3.2?1.1) h,and the patients were discharged from the hospital in(6.1?1.2) d.Thirteen of the patients had postoperative complications,including 7 cases of right pneumothorax(healed by thoracentesis),and 6 cases of fat liquefaction of the incision at the right axillary(delayed healing).No severe complications occurred in this series.UCG performed 5-7 days after the operation revealed no residual shunt of ASD or VSD;in the patients who had aortic aneurysm rupture combined with VSD,slight aortic valve regurgitation was detected after the neoplasty;and in the 3 patients with DCRV combined with VSD,the right ventricular outflow tract was patent.ECG showed sinus rhythm without AVB in the patients.Follow-up up to 3 months to 3 years were available in 132 cases(over 12 months in 95 cases).During the period,no patient had complaints;and the heart function was confirmed as level Ⅰ.Conclusions Video-assisted thoracoscopic cardiac surgery is feasible,safe,and minimal invasive for patients with VSD or ASD,resulting in quick recovery and good cosmetic outcomes.
4.Plasma kisspeptin levels in normal female pubertal stages and in girls with idiopathic central precocious puberty

Objective To investigate the pattern of plasma kisspeptin levels in normal female during various pubertal Tanner stages and the girls with idiopathic central precocious puberty(ICPP) or with premature thelarche(PT), and to evaluate the significance of detecting plasma kisspeptin levels as a new criterion for early differentiation between ICPP and PT.Methods Each study group of normal pubertal females with Tanner stage Ⅰ to Ⅴ comprised 16 to 19 individuals.The levels of plasma kisspeptin were also detected in girls with ICPP(n= 10)or PT(n = 12).The plasma kisspeptin levels were detected by enzyme-linked immunosorbent assay (ELISA).Results The level of kisspeptin was significantly higher in ICPP group than in that of PT group [(1.73±0.23 vs1.43±0.29) ng/ml, P<0.05].Among the normal pubertal females, the level of kisspeptin decreased gradually from Tanner stage Ⅱ to Tanner stage Ⅴ, being highest in Tanner stage Ⅱ [(1.73±0.22) ag/ml] ,lower in stage Ⅳ and Ⅴ than in stage Ⅰ and Ⅲ (P<0.01).Conclusions Plasma kisspeptin level was the highest during Tanner stage Ⅱ in normal female pubertal development.It is significant to detect plasma kisspeptin level for the differential diagnosis of ICPP and PT.
6.Evaluation of subclinical left ventricular systolic dysfunction in patients with severe aortic stenosis by three dimensional speckle tracking echocardiography

Objective To evaluate the role of three dimensional (3D) speckle tracking echocardiography (STE) in detection of subclinical left ventricular (LV) systolic dysfunction in patients with aortic stenosis (AS).Methods The study included 47 patients with AS and 50 age-matched healthy individuals.Conventional echocardiographic parameters were measures,and 3D Speckle tracking imaging of the peak systolic strain was recorded.Results Three-dimensional global longitudinal strain (3D-GLS) were significantly reduced in patients with AS compared to controls (P < 0.01).Three-dimensional global circumferential strain (3D-GCS) were significantly increased (P < 0.05).Three-dimensional global area strain (3D-GAS) and three-dimensional global radial strain (3D-GRS) were similar between two groups (P >0.05).Correlation analysis showed that |3D-GLS| was negatively correlated with left ventricular mass index (LVMI) (r =-0.59,P <0.05) irrespective of |3D-GAS|,|3D-GCS|,3D-GRS,and left ventricular ejection fraction (LVEF).Conclusions Patients with AS have evidence of subclinical LV systolic dysfunction despite preserved EF%.3D speckle tracking appears is useful in detection of subclinical LV dysfunction in patients with AS.
7.New development in the research on FoxO3a and urologic neoplasm

The forkhead box O (FoxO) transcription factor family plays an important role in cell functions, including metabo-lism, apoptosis, cellular proliferation, stress reactions, DNA repair, and immune response. As a member of this family, forkhead box O3a (FoxO3a) regulates its target genes by modulating histone modifications, including phosphorylation, acetylation, and methylation. FoxO3a expression is abnormally downregulated in urologic neoplasm. Protein modifications and FoxO3a activity are mainly con-trolled by PI3K/Akt signal pathway and other signaling pathways. FoxO3a is also involved in the initiation, progression, and prognosis of urologic neoplasm. This review focuses on the function of FoxO3a in urologic neoplasm and elucidates the regulatory mechanisms involved. This article will provide novel strategies to clinical diagnosis and drug therapy of urologic neoplasms.
8.The learning curve of thoracocopic technology for atrial septal defect and ventrical septal defect repair

ObjectiveTo investigate the learning curve of total thoracoscopicy cardiac surgery.MethodsClinical data of a succession of 125 patients undering total thoracoscopicy ASD and VSD repair between October 2004 to January 2010 were collected and reviewed.The procedure was perfomed by the same surgeon.The patients were divided equally into 5 groups (groupA,B,C,D and E,n =25 in each group ) according to the sequence of the operation.The operative time,extracorporeal circulation time,aortic cross-clamped time,the rate of conversion rate to thoracotomy and postoperative complications were compared between the 5 groups.ResultsThere were no statistically significant differences between the 5 groups with respect to age,gender,weight,dieases and surgical approach(P > 0.05).The operative time,extracorporeal circulation time and aortic clump time in group A and group B significant longer then that in group C,group D and group E(P <0.05).Group A and group B was no statistically significant difference each other( P > 0.05 ).Group C,D and E have no statistically significant differences between(P >0.05 ).The rate of conversion rate to thoracotomy and postoperative complications in 5 groups have no statistically significant differences comparative( P > 0.05 ).ConclusionThe learning curve of total thoracoscopicy surgery is approximalely 50 cases.
9.Protective Effect of Erqi Decoction on Intestinal Tract of Acute Radiation Intestinal Injury Rats and Its Mechanism

Objective To investigate the protective effects of Erqi Decoction(EQD; mainly composed of Radix Aristolochiae Kaempferi, Radix Rhizoma Seu Flos Cypripedii, Cortex Fraxini, Cortex Phellodendri, Radix et Rhizoma Rhei) on the intestinal tract in rats with acute radiation intestinal injury and its mechanism. Methods Sixty SD rats were randomly divided into normal group, model group, EQD group and Baitouweng Decoction group (BD group), 15 rats in each group. The acute radiation enteritis model was established by exposing the whole abdomen to a total dose of 10 Gy of 6 MV higher-energy X-rays. EQD group and BD group were given intragastrical administration with corresponding medicine of EQD at the dose of 8.85 g·kg-1·d-1, BD at the dose of 4.69 g·kg-1·d-1 respectively, and the normal group and the model group were given intragastrical administration with the same volume of normal saline. The treatment lasted for 7 continuous days. After modeling, the morphological change of the proximal ileum tissue was observed under light microscope. Villus height, crypt depth, and thickness of the ileal mucosa and entire wall were measured by image analysis system. The myeloperoxidase (MPO) content in ileum tissue was determined by spectrophotometer, and the expression levels of caspase -3 and proliferating cell nuclear antigen (PCNA) in ileum tissue were determined by immunohistochemistry. Results EQD group and BD group had milder injuries of the ileal structure, and had higher villus height, crypt depth, and thickness of mucosa and entire wall than those in the model group (P <0.05), but there were no differences between the two medication groups(P > 0.05). MPO content in EQD group and BD group was decreased(P<0.05 compared with that in the model group), and MPO content in EQD group was lower than that in BD group. The expression levels of caspase-3 and PCNA were increased in EQD group and BD group(P < 0.05 compared with those in the model group), but there were no statistical differences between the two medication groups (P>0.05). Conclusion EQD has certain protective effects against radiation-induced intestinal damage, which mechanism is probably associated with relieving the local intestinal inflammatory reaction, accelerating intestinal epithelial cell proliferation, and inhibiting intestinal epithelial cell apoptosis.
10.Effect of chitosan on PKC activity and DAG concentration in murine peritoneal macrophages

ABSTRACT AIM To investigate the effect of ch-itosan on protein kinase C (PKC) activitives and diacyl glycerol(DAG)concentration in murine peritoneal macrophages. METHODS A new ion-pair reversed-phase high liquid chromatography and ra-dioimmunoassay were used to determinate the activity of PKC and the concentration of DAG respectively. RESULTS ( 1) Chitosan induced activation and translocation of PKC in murine peritoneal macrophages. The peak time was 25 min and the activity of PKC came back the basic level at 1. 5 h. (2)Chitosan increased the production of DAG in murine peritoneal macrophages. The peak time was 30 s and the concentration of DAG came back the basic level at 3 min. CONCLUSION The im-munopoteniating effect of chitosan may be associated with the channel of DAG /PKC.
