1.Investigation of nosocomial infection of 6395 hospitalized neonates

Objective To identify risk factors, incidence and trends of nosocomial infections in the neonate ward. Methods Total 6395 hospitalized neonates were sub-grouped by state of maturity, attending to neonate intensive care unit (NICU), length of stay, underlying diseases, use of antibacterial agents and mechanical ventilation. Clinical data were retrospectively analyzed in each subgroup. Results Among 6395 hospitalized neonates, 168 (2.63%) had nosocomial infections, and the incidence rates were of statistical difference among groups (P<0.01), Lower respiratory tract was the most frequent infection site accounting for 80.9% of all infections. 61.3% eases were caused by gram negative bacteria, in which 72.06% were enterobacteriaceae. Conclusions To prevent nosecomial infection among neonates the following measures should taken: improve care for NICU and premature babies, reduce length of stay, avoid invasive procedures, isolate infected neonates and use antibacterial agents rationally.
2.Effect of heat preservation on the recovery of BIS and the concentration of propofol in the recovery of propofol in elderly patients

Objective To analyze and discuss the effect of heat preservation on the recovery time of BIS and the concentration of propofol in the recovery period of the elderly patients with general anesthesia in order to provide guidance for clinical treatment.Methods 78 elderly patients with general anesthesia were selected as the subjects, and the patients were divided into the control group and the study group with 39 cases in each group.The patients were divided into the control group and the study group.Control group, only to be liquid input and covered by drapes, research group take liquid input and peritoneal flushing fluid heating and heating blanket coverage of insulation measures, were recorded and compared between the two groups from anesthesia began to operation to complete the different time points of esophageal temperature and MAP, and to observe the recovery time of patients and effect compartment concentration differences.Results Two groups at T0, T1 esophageal temperature no significant difference, study group T2-T6 esophageal temperatures were significantly higher than those of the control group (P<0.05), maps of the other two groups at different time points had no significant difference;when two groups of patients with propofol withdrawal BIS values had no significant difference, and in the time of discontinuation BIS is more than or equal to 80 recovery time and effect compartment concentration have significant difference (P<0.05).Conclusion The surgical treatment of elderly patients with general anesthesia can help maintain the body temperature, shorten the recovery time of BIS and reduce the concentration of propofol in the recovery period.

4.Robots for the rehabilitation of upper limb motor function after stroke

Objective To observe the effect of robot-assisted upper limb training on the recovery of upper limb motor function after stroke.Methods One hundred patients within one month of suffering a stroke were randomly divided into a control group (50 cases) and a therapy group (50 cases).Both groups were given conventional drug therapy and rehabilitation training,while the therapy group also received robot-assisted training once a day,5 days per week for 12 weeks.The Fugl-Meyer assessment for the upper extremities (FMA-UE),the modified Ashworth scale (MAS) and the modified Barthel index (MBI) were used to evaluate the subjects before the experiment and after 4,8 and 12 weeks of treatment.Results During the treatment,the average FMA-UE scores and the average MBI of both groups increased gradually.The treatment group's averages were significantly better than those of the control group at each time point,except that there was no significant difference in elbow and wrist MAS scores.Conclusion Upper limb robots can be used with acute stroke patients in a clinical setting,and they may be beneficial for improving upper limb function and ability in the activities of daily living.
5.Effects of acupoints on pericardium meridian of hand-Jueyin with acupuncture on cell apoptosis induced by myocardial ischemic reperfused injury

BACKGROUND: Apoptosis of cardiomyocytes is speeded up during ischemic reperfused injury. Acupuncture on the points of hand- Jueyin meridian can inhibit the progression of reperfused injury and reduce apoptosis of cardiomyocytes so as to provide the protection on the heart.OBJECTIVE: To observe the effects of acupuncture on the points of hand-Jueyin meridian on apoptosis of cardiomyocytes during ischemic reperfused injury.DESIGN: Randomized controlled experiment based on the experimental animals.SETTING: Department of acupuncture and massage in a university of traditional Chinese medicine.MATERIALS: The experiment was performed in Central Laboratory of Shandong University of Traditional Chinese Medicine from April to December 2001. Forty Wistar male rats were employed, and were randomized into 5 groups, named as blank group, model group, "Neiguan(PC 6)" group,"Ximen(PC 4)" group and "Zhigou(TE 6)" group.METHODS: After electric acupuncture on the points of pericardium meridian for 20 minutes, the ligature was done on the anterior-descending branch of the left coronary artery for 40 minutes. Electrocardiogram (ECG) monitoring was followed. The ligature was released after electric acupuncture on the points for another 20 minutes and perfusion was restored for 60 minutes. Afterward, the heart was extracted, the cellular suspension was prepared and the determination with flow-type cell apparatus was applied.MAIN OUTCOME MEASURES: Effects of acupuncture on the changes in apoptosis rate of cardiomyocytes.RESULTS: In the model group, the typical apoptosis peak presented, the apoptosis rate was remarkably increased. The comparison between the points of pericardium meridian groups and the model group indicated very significant difference(P < 0.01) .The apoptosis rates in "Neiguan(PC 6)" group, "Ximen(PC 4)" group and "Zhigou(TE 6)" group as well as the model groupwere(4.29±0.76)%, (5. 17±0.88)%, (13.96 ±2.77)% and (16.01 ± 1.04)% respectively.CONCLUSION: Acupuncture on the points of hand-Jueyin meridian inhibits myocardial apoptosis effectively so as to reduce myocardial injury caused by cell apoptosis.
6.Nursing of the morbidly obese patients treated with laparoscopic vertical banded gastroplasty

Objective: To study the nursing of patients with morbid obesity treated with laparoscopic vertical banded gastroplasty (LVBG). Methods: Before operation, obese degree, obesity-related conditions and mental states were examined routinely. Monitoring of respiratory tract, observing operative complications and instructing of diets were done after operation. Results: Among 6 patients, 5 were at the third degree of obese, one was at second. In obesity-related conditions, 4 patients had hypertension and acantha derma, 1 had arthritis, and all had respiratory sleeping syndrome. The operations were all successful. The food amount food and body weight both decreased significantly 1 month after operation. The common operative complications were mild bleeding (1 case), shoulder-back pain (1 case), nausea and vomiting (5 cases). Diet principle was high protein, low energy, liquid food was the first choice. Conclusion: Observing and preventing respiratory sleeping syndrome are the main points of postoperative cares. Instructing patients to establish correct diet habit is the key to reach the best efficacy of LVBG.
7.The analysis of risk factors for mortality in multiple trauma patients with acute respiratory distress syndrome

Objective To retrospectively demonstrate risk factors for mortality in multiple trauma patients with acute respiratory distress syndrome(ARDS).Method This wes a retrospective cohort stuay regarding multi-ple trauma as a single cause for intensive care unit admission.Patients identified multiple trauma with ARDS en-rolled in prospectively maintained database between May 2003 and April 2008 were observed,and 23 items of po-tential risk factors of impacting mortality were calculated by univariate and multivariate logistic analyses in order to find distinctive items in these multiple trauma patients.Information on patients demographics characteristics,treat-ment procedures and injury severity were collected at the time of EICU admission.The criteria used for ARDS met definition of the guideline(2006)of Chinese medical association.The commonly accepted definition of multiple injuries was consistent with both several injury sites(generated from two or more than two anatomic sites)and in-jury in one anatomic site at least threatening life.Severity of injury was quantified by injury severity seore and the simplified acute physiology score and chronic health evaluation score (APACHE Ⅱ)in EICU admission.We in-cluded adult patients(age≥18 years),those with an EICU length of stay longer than 48 hours,and those accept-ing mechanical ventilation more than 24 hours.Patients who were readmitted to EICU by virtue of non-traffic injury or transferred to EICU from other hospitals after long-term treatment were excluded.Mortality was assessed at the 28th clay after trauma.Results There were 269 multiple trauma patients with posttranmatic ARDS admitted to ICU during the study period,the unadjusted odds ratio(OR)and 95% confidence intervals(CI)of mortality were associated with six risk factors(APACHE Ⅱ score,duration of tratuna factor,pulmonary contusion,aspiration of gastric contents,sepsis and duration of mechanical ventilation)out of 23 items.The adjusted Odds Ratios(ORs) with 95% CI were denoted with respect to surviving beyond 96 hours ICU admission(APACHE Ⅱ score,duration of trauma factor,aspiration of gastric contents),APACHE Ⅱ score beyond 20 ICU admission(duration of trauma factor,scpsis,duration of mechanical ventilation)and mechanical ventilation beyond 7 days ICU admission(dura-tion of trauma factor and sepsis).Conclusions Impact of pulmonary contusion and APACHE Ⅱ score contribut-ing to prediction of mortality may exist in prophase after multiple trauma.Sepsis is still a vital risk factor referring to systemic inflammatory response syndrome,infection,and secondary multiple organs dysthnetion.Aspiration of gastric contents could lead to incremental mortality due to scvere ventilation associated pneumonia.Duration of trauma factor determined degree of injury and outcomes,longer duration generally manifested higher mortality.Long-standing mechanical ventilation should be constrained on account of occurring severe refractory complications.
8.Effects of Fresh Slices on Content of Ferulic Acid in Angelica

Objective To explore effects of fresh slices on quality of angelica;To provide a reliable method for scientific evaluation and the effective control of quality of angelica;To compare the quality of fresh slices and pieces of angelica.Methods Extraction method and the chromatographic conditions were optimized on the basis of 2010 edition of Chinese Pharmacopoeia. Ferulic acid content was determined by HPLC, with a Kromasil C18 column (4.6 mm×250 mm, 5μm) and mobile phase of acetonitrile-0.05% phosphoric acid-water gradient elution at flow rate of 1.0 mL/min, detection wavelength of 316 nm, and the column temperature was 28℃.Results The content of ferulic acid in the fresh slices of angelica was higher than that in angelica pieces. When other external conditions were the same, the content of ferulic acid in the fresh sliced angelica was the highest with 70% methanol extracted for 30 min.Conclusion The method for determination of ferulic acid in angelica is simple, reliable, reproducible, and can be used as an effective way to control quality of angelica.
9.Condition and Related Factors of Anxiety and Depression in Caregivers of Patients in Persist Vegetative State or Minimally Conscious State

Objective To explore the anxiety and depression in caregivers of patients in persist vegetative state and minimally conscious state, and discuss the related factors. Methods From January, 2010 to December, 2014, 60 caregivers of 60 patients in persist vegetative state and minimally conscious state were assessed with Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). The sex, ed-ucation level, relation to the patients and family income of the caregivers, and the age of the patients and number of the families of the pa-tients were recorded, and analyzed with Logistic regression. Results 55%caregivers were found in anxiety, and 48.4%in depression. The fe-male caregivers (OR>6.870, P<0.05), the number of the family of the patients (OR<0.233, P<0.05) and age of the patients (OR<0.294, P<0.05) were independent factors associated with anxiety and depression. Conclusion The caregivers of patients in persist vegetative state and minimally conscious state are troubled in anxiety and depression, especially the female, while age and the number of the families of patient may be important factors for it.

AIDS antiviral therapy (ART) has achieved great success.Originaly,AIDS had been regarded as a fatal disease,but it has become a kind of infectious disease that could be cured and administrated.Global HIV / AIDS cases were still up to about 38 million,but more than half have been effectively treated.In addition to drug treatment,at present,some new technologies and new methods,such as genome editing,have also been involved in the treatment of AIDS,and in the humanized animal experiment has shown very good results.There is no doubt that AIDS will eventually be stopped its epidemic.However,with the continuous development of AIDS antiviral treatment,the most fundamental problem is that HIV latent library has become increasingly prominent one,whether molecular therapy and hybrid cure have being developed for AIDS treatment,there are still such problem existence.Great efforts shoud be made to continuously search for new markers of latent viral cells and to reduce the latent pool.In addition,despite the prevention and treatment of AIDS has made great achievements,but the world still produces nearly 6000 cases of HIV/AIDS every day.Therefore,the development of safe and effective vaccine,whether in the field of prevention,or in clinical treatment,has its positive significance.
