1.Experimental study about the effect of different scan parameters and post-processing methods of multi-detector CT on metallic artifacts

Objective To explore the effect of multi-detector computed tomography (MDCT)scan parameters and imaging post-processing methods on metallic artifacts,so as to achieve the perfection in reduction of artifacts caused by metallic implants.Methods Water phantom with titanium alloy steel plate was scanned under MDCT with alternating scan parameters including tube voltage, tube current,pitch and focal spot.The areas of image artifacts were measured and correlated with the scan parameters.Another 1 5 postoperative patients with vertebral implantation scaned with the conventional scan parameters were also included in the study.Ima-ging post-processing including multi-planar reconstruction (MPR),window width and level adjustment,and volume rendering tech-nique (VRT)was performed by using optimized and conventional methods,respectively,and then the image quality was evaluated. Results The areas of image artifacts under the differnt tube voltage,pitch,and focal spot were statistically different.With the in-crease of the tube voltage or the decrease of the pitch,the area of image artifacts decreased gradually.The area of image artifacts scanned with the small focal spot was smaller than the large one.There was no significant difference for the area of image artifacts scanned with the different tube currents.The metallic artifacts decreased by using the soft tissue algorithm (Kernel B40f)in the bone window reconstruction,wide windows (2 000 HU window width,600 HU window level)in observing,and VRT.Conclusion Selection of appropriate MDCT scan parameters and imaging post-processing methods can reduce the metal artifacts,improve the image quality and diagnosis.
2.Treatment of 54 Cases of Lactation Insufficiency with Acupuncture

Fifty-four cases of woman with lactation insufficiency were treated with acupuncture at acupoints Shaoze (SI 1), Danzhong (CV 17), Rugen (ST 18), Pishu (BL 20), Zusanli (ST 36) and Sanyinjiao (SP 6), and 42 cases got significant effect that the milk could fully meet baby's nutritional requirement, 9 cases got improvement, 3 cases discontinued the treatment.
3.The expression of Caspase-8 in gastric carcinoma tissues and its clinical significance

ObjectiveTo investigate the expression characteristics of Caspase-8 in gastric carcinoma tissues and its correlation with clinical pathological features.Methods The mRNA and protein of Caspase-8 in gastric carcinoma and its surrounding tissues were detected by reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry,respectively.ResultsThe Caspase-8 mRNA in gastric carcinoma tissues was obviously lower than that in its surrounding ones (0.154±0.065 vs 0.394±0.107,t =14.04,P <0.01),which was correlated with lymph node metastasis,tumor stage,differentiation grade (P<0.01).The Caspase-8 protein in gastric carcinoma tissues was also significantly lower than that in its surrounding ones (31.48% vs 85.19%,x2 =32.04,P < 0.01),which was correlated with tumor stage,differentiation grade (P<0.05).ConclusionsCaspase-8 is low-expressed in gastric carcinoma,which may play an important role in the occurrence and progression of gastric carcinoma.
4.Application value of MRI and H1-MRS for the neonates with hypoxic-ischemic encephalopathy

Objective To study the value of MRI and proton magnetic resonance spectroscopy(H 1-MRS)for neonatal hypoxic-is-chemic encephalopathy(HIE).Methods Magnetic resonance imaging (MRI)and proton magnetic resonance spectroscopy (H 1-MRS)were performed in 30 cases of full-term neonates with HIE,and 10 infant control group without evidence of birth asphyxia. Cerebral MRI and H 1-MRS were performed within 1 5 days after birth.The results of H 1-MRS such as subwave crest values of me-tabolites in lesion areas were recorded.The data were analyzed statistically.Results (1)MRI showed abnormal fetures of HIE such as diffuse cerebral edema,loss of hyperintensity in the posterior limb of the internal capsule on T1 WI,gyrus sign,diffuse parenchy-mal hemorrhage,which could predict the severity of brain damage.(2)On H 1-MRS,the ratio of Lac/Cr in HIE group was much higer than that in control group,which was statistically significant (P <0.05).The ratio of Lac/Cr showed a rising trend with clini-cal grading of HIE.The ratio of NAA/Cr and NAA/Cho were lower in HIE group than that in control group (P <0.05),which showed a trend of gradually reduce with clinical grading of HIE.The difference between ratio of Glx-α/Cr in HIE group and control group was also significantly,the moderate-severe group was much higher than the mild group and control group.There was no sig-nificant difference in the ratio of Cho/Cr between the 4 groups.Conclusion The combination of MRI and H 1-MRS can objectively re-flect brain morphology and metabolic changes of HIE,and evaluate the severity of the brain injury,and provide an effective evidence for clinical diagnosis and treatment.
5.Investigation of Perioperative Prophylactic Utilization of Antibiotics in Cataract Patients and Its Rationality Evaluation

OBJECTIVE:To investigate the perioperative prophylactic utilization of antibiotics in cataract patients in ophthalmology department of our hospital.METHODS:100 cases of cataract surgery in 2009 were randomly collected from our hospital.Category of antibiotics,administration time point,medication time,wound healing and drug costs in hospitalization period were analyzed statistically and the rationality of above indexes was evaluated.RESULTS:100 cases all received prophylactic antimicrobials by local and systematic administration in the perioperative period(100%).Drugs for systematic administration contained Levofloxacin(n=67,67%),Cefuroxime sodium for injection(n=25,25%),Clindamycin phosphate injection(n=2,2%) and Ceftazidime for injection(n=6,6%).63 cases were only given drugs 30 min before operation(63%);12 cases were given medicine 30 min before operation and within 24 h after operation(12%);above situation accounted for 75%.25 cases were given medicine before operation and within 24 h after operation(25%),in which the average medication duration was 2.5 days and the longest was 4 days.CONCLUSION:The irrational use of drugs in our hospital manifests inappropriate selection and too long use of antibiotics after operation.The interference of perioperative prophylactic utilization of antibiotics should be reinforced and it is urgent to intensify consciousness of physicians about rational use of antibiotics in perioperative period.
6.Effects of polysaccharide from Sargassum fusiforme on blood-lipids and antioxidation ability in hyperlipemia rats

Objective To investigate the effects of polysaccharide from Sargassum fusiforme on blood-lipid and antioxidation ability in hyperlipemia rats.Methods The hyperlipemia rats as the experimental animal were established by feeding with fat-rich forage,meanwhile the rats were ig SFPS at doses of 0.4、0.8、1.6g?kg-1for 4wk.The levels of blood-lipid and the indices of antioxidation ability which include the total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),lipid peroxidation(LPO),malonaldehyde(MDA),superoxide dismutase(SOD) and glutathion peroxidase(GSH-Px) were determined.Results and Conclusion SFPS could significantly reduce the contents of TC,TG,LDL-C,LPO and MDA.In the mean time,SFPS increased the content of HDL-C and enhanced the activities of SOD and GSH-Px.
7.Effect of Multichannel Functional Electrical Stimulation on Motor and Balance Function of Lower Extremity in Adolescents with Cerebral Palsy

Objective To evaluate multichannel functional electrical stimulation (FES) on motor and balance function of lower limbs in adolescent with cerebral palsy. Methods From January, 2014 to June, 2015, 20 adolescents with cerebral palsy were randomly divided into FES group (n=10) and control group (n=10). Both groups received routine exercise therapy. FES group received active and passive FES treadmill based on normal walking pattern. The Fugl-Meyer Assessment (FMA) was used to evaluate the motor function of lower limbs;the Berg Balance Scale (BBS) was used to assess the balance function, and the Gross Motor Function Measure (GMFM) was used to test the gross motor function. Results The scores of FMA, BBS and GMFM significantly improved 30 days after treatment (F>223.749, P<0.001), and the scores were higher in the FES group than in the control group (t>2.706, P<0.05). Conclusion FES treatment can improve the motor and the balance function of lower limbs in adolescents with cerebral palsy.
8.Research on classification of prescriptions in Treatment Record of Hundreds of Selected Ancient Prescriptions based on hierarchical clustering

Objective:Based on systematic idea of clustering methods, a novel approach for classifying prescriptions was explored in the light of the method of classifying analogous prescriptions.Methods:A total of 581 ancient prescriptions were selected from Treatment Record of Hundreds of Selected Ancient Prescriptions, and the standardized names of Traditional Chinese Medicines in these prescriptions were entered in Microsoft Excel 2007, then imported to SPSS 24.0 to generate dendrograms using the hierarchical clustering function. The classification of 581 selected ancient prescriptions was analyzed. Results:All 581 prescriptions could be classified into 86 categories through repeated clustering, the largest group has 29 prescriptions, the smallest group has 2 prescriptions, and the average group has about 6.75 prescriptions. In general, the later the intercepted group, the lower the similarity of its internal prescriptions.Conclusion:This method could realize the classification of prescriptions in the light of the method of classifying analogous prescriptions, which may help to break the original thinking bondage and further deepen the understanding of compatibility rules of prescriptions. However, its disadvantage lied in that the theoretical clues would be reduced when analyzing the compatibility rules of prescriptions and the issues of drug dosage, nature, taste and meridian categories were not specially considered.
9.Analysis of the Long-term Curative Effects of Radical Nephrectomy

Objective:To analyze the factors that affect patient prognosis after radical nephrectomy.Meth-ods:A total of 389 cases of renal cell carcinoma treated with radical nephrectomy between January 1 993 and December 2006 were reviewed.All the data were encoded.inserted into an Excel database and then ana-lyzed by SPSS 1 3.0 software.The cumulative survival rates were calculated by life-table method.We as-sessed the impact of multiple covariates on survival time with the Cox Regression model.Results:The patho-logical results showed that 307 cases were clear call carcinoma,51 cases were papillary renal cell carcinoma,21 cases were chromophobic renal cell carcinoma,2 cases were collecting duct carcinoma.and 8 cases were unclassified.One hundred and ninety-eight cases were of T1N0M0, 113 cases were of T2N0M0, 3 cases were of T1N1M0,10 cases were of T2N1M0, 51 cases were of T3N0M0, and 14 cases were of T3N1M0, Two hundred and sixty-eight cases were followed up.The 1-year survival rate was 96.5%,the 3-year survival rate was 90.7%.the 5-year survival rate was 75.7%.and the 10-year survival rate was 65.8%.Multivariable analysis revealed that significant prognostic factors included TNM stage,Robson stage.vena cava and supplementary treat-ment(X2=22.50.P=0.001).The most important prognostic factor was pathological stage(TNM and Robson).The regression coefficients were 0.533 and 0.674,and the relative risk was 1.941 and 2.01 1(P=0.004 and p=0.002).Conclusion:Radical nephrectomy is safe and effective.TNM stage.Robson stage and vena cava are prognostic factors.Supplementary treatment is a protective factor.
10.Therapeutic effectiveness of thalidomide to multiple myeloma and its mechanism.

OBJECTIVETo observe the effective mechanism and side effects of thalidomide to multiple myeloma (MM).
METHODSTen cases of MM were studied, of which 3 were previously untreated and 7 refractory or relapsed. Bone marrow microvascular density (MVD) was detected by factor-VIII related antigen and CD(34) immunohistological staining and serum concentration of vascular endothelial growth factor (VEGF) before and after treatment was determined by ELISA. The initial dosage of thalidomide was 100 approximately 200 mg/d with a weekly escalation of 50 mg/d to 450 approximately 650 mg/d. The therapeutic effectiveness is classified into partial remission, improvement and uneffective according to the decrease of serum M protein and bone marrow myeloma cells. Anemia, renal function and blood electrolytes were also observed.
RESULTSBefore treatment, MVD was 73.32 +/- 28.80 and 32.30 +/- 12.50 in MM and control group, respectively, (P < 0.01). MVD in MM group decreased to 56.12 +/- 19.34 after treatment, and was of significant difference (P < 0.05) as compared to the pretreatment value. However, there was still a significant difference as compared to control (56.12 +/- 19.34 vs 32.30 +/- 12.50, P < 0.01). The concentration of VEGF significantly decreased after treatment [from (178.23 +/- 26.56) ng/L to (78.48 +/- 19.98) ng/L, P < 0.01)]. The total effective rate was 70%. There were no serious side effects.
CONCLUSIONMVD and VEGF concentration were decreased obviously by thalidomide treatment. The dosage of 450 approximately 650 mg/d might be effective in refractory or initial MM.
