3.Effect of CBP on the level of blood lactic acid and 6 h lactic clearance rate in the patients with severe sepsis

Objective To investigate the effect of continuous blood purification ( CBP) on the level of blood lactic acid and 6 h lactic clearance in the patients with severe sepsis. Methods 48 patients with severe sepsis were cho-sen in ICU, and they were randomly divided into two groups: control group ( n=20 ) and CBP group ( n=28 ) . Their arterial blood lactic acid level was tested on admission and was retested 6, 24, 48 and 72 h after treatment. The patients’6 h blood lactic clearance rate and the corresponding APACHEIIscore were calculated. The level of arterial blood lactic acid and the score of APACHE IIwere compared at different time. 6 h blood lactic clearance, the period of stay in ICU and the 28-days mortality were compared too. Results The level of lactic acid and the score of APACHE II after 24 h were lower in CBP group, and the period of stay in ICU was shorter in CBP group than that in control group at different time ( P<0.05 ) . 6 h blood lactic clearance was higher than that in control group (P<0.05). Moreover, the 28-days mortality had no significant difference in CBP group. Conclusion The CBP plays an important role in clearing the blood lactic acid and it can reduce the severity of the sepsis as well as shorten the period of stay in ICU.
4.Clinical application of devascularization of venous traffic branches by endoscope.

Objective To discuss the technique,advantages and follow-up results of subfascial endoscopic perforator surgery.Methods Subfascial Endoscopic Perforator Surgery was performed for 23 patients wth chronic venous insufficiency.Over two years postoperative follow-up was done.Results 21 in 23 cases with ulcers healed.Only one case had wound infection.Conclusion Subfascial Endoscopic Perforator Surgery is a safe and effective procedure in treating venous insufficiency.
5.Laparoscopic splenectomy plus esophagogastric devascularization for the treatment of portal hypertension

Objective To evaluate the safety and efficacy of laparoscopic splenectomy plus esophagogastric devascularization for the treatment of portal hypertensive variceal bleeding and secondary hypersplenism. Methods Laparoscopic splenectomy and devascularization of the lower esophagus and upper stomach were performed in ten cirrhotic patients between January 2000 and July 2005. Esophageal transection and reanastomosis performed by a stapler at the lower end of the esophagus was added to two patients through a small accessory incision. Results The procedure was successful in all cases without conversion to open surgery. The operation time ranged from 4.5 to 5.5 hours and the blood loss was 100 ~ 400 ml. The postoperative hospital stay was 8-15 days. Conclusion Laparoscopic splenectomy and portaazygous disconnection is a feasible, effective and safe procedure as well as minimally invasive hence is applicable for patients with portal hypertension and hypersplenism.

7.Laparoscopic splenectomy performed in 41 cases

Objective To evaluate the feasibility and efficacy of laparoscopic splenectomy(LS). Methods In this study 41 cases were enrolled including 10 cirrhotic splenomegaly cases and 17 idiopathic thrombocytopenic purpura cases. The clinical data of 41 cases undergoing LS was retrospectively reviewed. KG2Result Two cases were shifted to open surgery, LS was successful in 39 cases with average operating time of 238 min, the average spleen weight was 389 g, blood loss 318 ml, postoperative GI movement started from 12 to 24 hrs. The average postoperative hospital stay was 4 days. Postoperative complications occurred in 3 cases including intraabdominal bleeding, subcutaneous emphysema, and stabbing-caused abdominal wall bleeding in one each. Conclusion LS is safe and less traumatic in selected patients indicated for splenectomy, especially for those suffering from haemotologic diseases.
8.Dynamic analysis of the preparation of Huangqi injection

Objective To dynamically the preparation process of Huangqi injection and to verify the rationality and existing problems of the process. Methods The preparation was made by the current standard (WS3-B-3335-98) issued by Ministry of Health, and the solid amount of the key processes were measured. The HPLC separation was performed on a Agilent Zorbax Bio-C18 reversed-phase column (250 mm × 4.6 mm, 5μm) in gradient mode of acetonitrile-water with UV detection at 254 nm. The column temperature was kept at 25 ℃, and the flow rate of mobile phase was 1.0 ml/min. Results Solid amounts of different operations of Huangqi injection were measured accurately. The content of the third water extracts was only 6.1%, and the changes of HPLC pointed the content of the 12 peaks of the second peaks decreased obviously. Conclusion The technological rationality of Huangqi injection need to be verified and optimized, the dynamic analysis of HPLC describes the changes of chemical constituents of Huangqi injection qualitatively, which also provides a reference value for establishing its fingerprint.
9.Study on hepatitis C virus genotyping in guizhou area

Objective To study the distribution of genotypes of hepatitis C virus in Guizhou and its relationship between infec-tious route of genotype and age ,gender was analyzed .Methods Serum specimens in this study were obtained from 198 patients , whose anti-HCV and HCV RNA were positive .A reverse transcriptase PCR(RT nested-PCR)assay using conserved primers de-duced from the core-envelope 1(C-E1)region of the hepatitis C virus(HCV)genome was employed to amplify a 474-nucleotide-long fragment .Phylogenetic analysis of the C-E1 sequences was conducted by direct sequencing of the RT-PCR products and alignment with published HCV subtypes in GenBank .Subtypes of the samples were determined by nucleotide sequencing followed by composi-tion of a phylogenetic tree .Results Among the 198 patients surveyed ,genotype 1a was detected in 4 cases(2 .0% ) ,genotype 1b in 71 cases(35 .9% ) ,genotype 2a in 9 cases(4 .6% ) ,genotype 3a in 29 cases(14 .7% ) ,genotype 3b in 47 cases(23 .7% ) ,genotype 6 a in 37 cases(18 .7% )and genotype 6d in 1 cases(0 .5% ) .Genotype distribution on gender had no statistical significance(P>0 .05) , and its distribution on people with different ages had statistical significance(P<0 .05) ,and its distribution on patients with different infectious routes was significantly different(P<0 .05) .Conclusion The major genotypes of HCV are 1b ,3b ,6a and 3a in Guizhou , and genotype 1a is predominant .Genotypes 1a ,2a and 6d exist too .Genotypes of patients infected with HCV are related to their in-fectious routes ,and the HCV genotypes are in a great variety .
10.Nanocomoposite probes composed of fluorescent magnetic nanoparticles and PSA ScFv antibody for targeted imaging and therapy of prostate cancer

Objective:To investigate the feasibility of targeted imaging and therapy of prostate cancer using nanocomposite probes composed of fluorescent magnetic nanoparticles(FMCNPs) and single chain Fv(ScFv) antibody specific for gama-seminoprotein.Methods:The nanocomposite probes(FMCNPs-ScFv) were prepared by conjugating fluorescent magnetic nanoparticles with singlegama-chain Fv antibody specific gama-seminoprotein,and were characterized by high resolution transmission electron microscopy,fluorescent spectrum and magnetic spectrum.Nanocomposite probes were incubated with prostate cancer LNCaP cells,and the targeting results of nanocomposite probes were observed by fluorescent microscopy.The cytotoxicity effect of the nanocomposite probes was measured by MTT.Nude mice models of prostate cancer were established and identified by immunohistochemistry method.The nanocomposite probes were injected into nude mice via tail vein.The distribution of nanocomposite probes in the nude mice was observed by Micro-animal imaging system,targeted imaging of the prostate cancer was observed by MR instrument.The nude mice with prostate cancer were irradiated with 100 W magnetic field for 30 min,and the changes of tumor sizes were observed.Results:The FMCNPs-ScFv nanocomposite probes were successfully prepared.Nanocomposite probes entered into the cytoplasm of cancer cells and exhibited low cytotoxicity effect.Nude mice model with prostate cancer were successfully fabricated;the nanocomposite probes distributed quickly in the main organs of mice,and gradually concentrated on the tumor tissues within 24 h.MR images showed that the tumor images were gradually enhanced from 6 h to 24 h after injection of the nanocomposite probe.Four days after magnetic irradiation,the tumors in the nude mice grew slower compared with the control nude mice(P
