The surgical intervention and criteria of severe acute pancreatitis has significantly changed in the last several years with the advent of enhanced imaging techniques and minimally invasive surgery.Many kinds of minimally invasive technologies which take laparoscopy as a representative are constantly applied to the treatment of severe acute pancreatitis and make its mortality and postoperative complication rate decrease obviously.However,there are many different ways using laparoscopic techniques to treat severe acute pancreatitis.In this review,the author will discuss the current status of the different approaches.

Objective To study the clinical characteristics of Henoch-Sch(o)nlein purpura (HSP) in children.Methods We retrospectively analyzed the clinical data of 124 children with HSP.Results Among these 124 children,54.84% of the children presented upper respiratory tract infection before the onset of HSP.The purpura recurred in 30 of 47 children with Henoch-Sch(o)nlein purpura nephritis and in 22 of 77 children without Henoch-Schonlein purpura nephritis,respectively (P＜0.05).The incidence of Henoch-Sch(o)nlein purpura nephritis was significantly higher in children who had purpura,gastrointestinal symptoms,and joint symptoms simultaneously than in those only with purpura (P＜0.05).The elevated levels of IgA and C reactive protein were found in 67.35% and 39.39% of the children,respectively.Conclusion The most important predisposing factor of HSP is upper respiratory tract infection.The children with recurrence of purpura or those who have purpura,gastrointestinal symptoms,and joint symptoms simultaneously are more likely to present renal damage.

Takayasu arteritis (TA) is a devastating vasculitis of the aorta and its major branches,coronary and pulmonary arteries.The clinical manifestations in children are less specific than in adnlts:the disease in children presents with fever,arthralgias,vomiting,weight loss and hypertension.Conventional angiography,which is recognized as the golden standard in evaluating vascular lesions in TA,combined with computer tomography angiography (CTA),magnetic resonance angiography (MRA),ultrasonography,could not only provide important information for early diagnosis,but also detect disease activity.New immunosuppressive agents and biological therapies,such as TNF-a blocking agents,have been verified to be effective although corticosteroids and conventional immunosuppressive agents are still basic treatment.

Breast cancer is a group of heterogeneous diseases.Molecular portraits provide a new insight for personalized cancer management in breast cancer.According to the molecular pathology,molecular biology and system biology,breast cancer goes through different typing methods,including four subclasses,geneexpression signature and integrated genomic classification.These major subtypes of breast cancer may provide guidance for precise therapeutics.

Rational microenvironment contributes actively to cancer development through multiple mechanisms including triggering genomics instability,providing shield,promoting immune escape,inducing differentiation and permissive nich.Tumorigenesis reversely promotes the form of tumor microenvironment composed of hypoxia,decreasing pH,angiogenesis and nutrition deficiency.Tumor microenvironment not only is the reasons but also results in cancer development and progression.A better understanding of how the tumor environment affects cancer progression would provide the new strategy for cancer therapy.

As semi-autonomous organelles,mitochondria are easily damaged by various factors.Mitochondrial dysfunction such as abnormal oxidative phosphorylation,metabolism alterations,apoptosis prevention,damaged mitophagy,immune escape and deregulated signaling pathway have been hypothesized to be involved in tumorigenesis.Modulated mitochondria may prevent the occurrence of tumors.

Objective To investigate the distribution and the characteristics of HBV genotype in Kunming City,China.Methods The HBV genotypes and HBV-DNA levels of 360 patients in Kunming were detected by real-time quantitative PCR using type-specific primers,the serum-types were also detected in ELISA at the same time.Results Of the 360 specimens,346(96.11%) could be detected directly,among them 196 were genotype C(56.65%),91 were genotype B(26.30%),91 were genotype B(26.30%),37 were in B-C mix-type (10.69%),21 were genotype D(6.07%),1 were genotype A(0.29%).Among them,the genotype C was higer than the others (P<0.01);HBV-DNA levels of the patients with genotype C and B-C mix-type were higer than the genotype B and the others(P<0.01,P<0.05) (beside the positive proportions of genotype B was lower in the serum with"HB-sAg( + ),HBeAg( + ),anti-HBc( + )"(P<0.01),the other genotypes were no relation with their serum-types (P>0.05);the 360 specimens,14(3.89%)could not be detected directly,the average level of them was 4.136X 103IU/ml,it was lower than that of others(P<0.05).Conclusion The HBV-genotypes in Kunming area are abundant,including genotype C,B,D,A and B/C mix-type.The dominant genotype of HBV are C and B,and the HBV-DNA level of genotypes B/C mix-type and C were singnificantly higher than that of others ;The negative relation existed between the genotype B's frequency and the positive proportions of HBeAg;The genotype D frequency of this region is higher than that of lower-altitude inland areas and lower than that of plateau areas around Lahsa,which probably stands for its epidemiologic feature.Meanwhile,those samples without direct identification contain relatively lower level of HBV-DNA,which probably suggests that HBV genotype should be conducted on a basis of a certain HBV-DNA quantity.

Cancer precision therapeutics is defined as a therapy strategy based on molecular profiles and classifiers generated from high-throughput molecular assays.Precision therapeutics in oncology is becoming reality thanks to the next-generation sequencing and cancer targeted agents.Selecting optimal targets,identif-ying appropriate combinations of therapies and real-time monitoring of tumor evolution may be critical for practi-cing precision therapeutics.
