Diabetic retinopathy (DR) is one of the most significant microvascular complications of diabetes,and its incidence increases with the prolongation of DM;meanwhile,it can lead to severe vision loss.Recently,the pathogenesis of DR has been one of the focuses both at home and abroad,but the basic mechanisms are not fully elucidated.More and more scholars believe that DR may be a chronic and low degree of inflammatory response.Moreover,it is well established that microglia is a monocyte-macrophage,with antigen-presenting function.Especially in the retina,as microglia is activated,followed by the release of a large number of inflammatory factors,microglia involves in DR inflammatory response.Understanding the activation mechanism of microglia can provide a new perspective for the prevention and treatment of DR.Therefore,recent advances in activation of microglia in diabetic retinopathy and its intervention mechanism will be reviewed in the article.

TORCH infection during pregnancy is a group of infectious diseases resulting from viruses and other microorganisms. According to the past over 20-year experiences in serum screening in China, several infections which should be performed in the first trimester, or even much earlier in pregnant period, are hepatitis B, syphilis, HIV, and these three infections are confirmed to be harmful to the fetus or infants and also diagnostic methods and treatment are available. Screening for rubella should be better performed before conception and no specific management is required with IgM+, while immunization is recommended before pregnancy with IgM-. Screenings for cytomegalovirus, toxoplasmosis and herpes virus are not recommended. Scnography is warranted if fetal abnormalities were resulted from the above infections and amniocentesis or cordocentesis might be required for further investigations.

Objective To put forward suggestions for improving the writing standards of package inserts of Chinese patent drug in China. Methods Totally 84 package inserts of Chinese patent drug were chosen randomly. The labeled items in them would be investigated and analyzed according to legal documents in China. Results The rate of complete labeled items was 0%. Meanwhile, many problems existed in the items which affected medication safety seriously. In addition, there were defects in expression of the content, revision of package inserts of Chinese patent drug and so on. Conclusion It is urgent to improve the writing standards and the completeness of the labeled items as well as the scientificalness on the expressions of package inserts of Chinese patent drug.

Objective To explore the change of occludin mRNA in the lung tissue under hyperoxia induced lung injury condition and their regulation of platelet-derived growth factor B(PDGFB).Methods Three hundred and twenty newborn rats were divided into 4 groups accor-ding to different oxygen concentrations(FiO2):experimental group 1(FiO2=800 mL/L),experimental group 2 (FiO2=600 mL/L),experimental group 3(FiO2=400 mL/L) and room-air control group(FiO2=210 mL/L).Rats were killed at 1st,3rd,5th,7th and 14th day respectively during the experiment,the expression of occludin was examined by reverse transcription polymerase chain reaction(RT-PCR) method.The expression of PDGFB in the lung tissue was also observed by immunohistochemistry.SPSS 11.5 software was used to analyze the data.Results The expressions of PDGFB in the lung tissue in experimental group 1 and experimental group 2 were lower than those of the control group at 1th day(Pa

As one of the serious complications of diabetes, diabetic retinopathy( DR) has become a main eye disease which causes blindness. The occurrence and development of DR is related to many factors. The pathogenesis is complicated, and the mechanism has not been clear. Early data suggest that the occurrence and development of DR has relations with many factors such as blood sugar level, diabetes duration and the environment. Among the factors, mitochondrial dysfunction and oxidative stress is the important mechanisms of DR and has become research focus in recent years. Consequences of mitochondrial dysfunction within cells include elevation of the rate of reactive oxygen species( ROS) production due to damage of electron transport chain proteins, mitochondrial DNA ( mtDNA ) damage, and loss of metabolic capacity. Clear understanding on the mechanism of mitochondrial functional change under high sugar level and oxidative stress response in the occurrence and development of DR is of great significance on prevention and cure of DR. ln this article, the development of mitochondrial metabolism and oxidative stress of DR is reviewed.

Objective:To evaluate the impact of laparotomy pancreatoduodenectomy (OPD), laparoscopic pancreatoduodenectomy (LPD) and Da Vinci robot assisted pancreatoduodenectomy (RPD) on the risk of pancreatic fistula in postoperative patients.Methods:The key words were laparoscopic, pancreaticoduodenectomy, robot, DaVinci, Whipple, complication, fistula, pancreaticoduodenectomy, laparoscopy, robot, postoperative complications and pancreatic fistula, and the databases of CNKI, Wanfang, VIP, CBM, Cochrane Library, PubMed, EMBASE and web of science were searched until August 1, 2020 for retrospective cohort studies and randomized controlled trials comparing the efficacy of OPD, LPD and RPD. The articles were screened according to the pre-set inclusion and exclusion criteria, and the quality of the articles was evaluated and the data were extracted. Based on Bayesian framework, R4.0.2, Revman 5.3 and Stata 16.0 software were used to analyze the extracted data for mesh meta analysis.Results:Thirty articles involving 3 428 patients were included, including 26 retrospective cohort studies and 4 randomized controlled studies. Mesh meta-analysis showed that in the incidence of pancreatic fistula, there was no significant difference between LPD and OPD, between LPD and RPD ( OR=0.93, 95% CI 0.63-1.4; OR=1.4, 95% CI 0.87-2.2, P<0.05), but the incidence of postoperative pancreatic fistula was significantly lower than that of OPD ( OR=1.5, 95% CI 1.1-2.1, P<0.05), 95% CI 1.12.1; P>0.05). The risk probability of pancreatic fistula was ranked as RPD (0.00), LPD(0.34) and OPD(0.65), that is, for the risk of postoperative pancreatic fistula, the advantages and disadvantages of the three surgical methods were ranked as RPD, LPD and OPD. Conclusions:Compared with OPD, RPD can significantly reduce the risk of pancreatic fistula after PD and improve the quality of operation; there was no significant difference between LPD and RPD in the incidence of postoperative pancreatic fistula, and both of them were safe and feasible.

Objective To investigate the changes of tumor necrosis factor- ?(TNF- ?) and souble intercellular adhesion molecule - 1 (sICAM - 1) in serum and cerebrospinal fluid (CSF) in children with viral encephalitis (VE) before and after treatment,and to explore the pathogenesis of the cytokins in VE. Methods The levels of TNF - ? and sICAM - 1 in serum and CSF were determined before and after treatment using EL1SA in 38 children who were admitted with VE, and 20 children as normal controls. AIM: To establish the quality control for Lidanzhitong Tablet (Radix Paeoniae Rubra,Fructus Aurantii,Rhizoma Corydalis,etc.). METHODS:Radix Paeoniae Rubra, Fructus Aurantii and Rhizoma Corydalis were identified by TLC, and the content of paeoniflorin in Radix Paeoniae Rubra was determined by HPLC. RESULTS: Paeoniflorin showed a good linear relationship at a range of 0.597-4.997 ?g, r=0.999 9. The average recovery was 99.06%, and RSD was 3.4%. CONCLUSION: The method is accurate and reliable, and can be used to control the quality of this preparation effectively。

Objective To investigate the effects of duration of active phase on delivery mode and pregnancy outcome.Methods Data of 417 women with abnormal active phase identified from 1744 term-birth,singleton,cephalic presentation primiparas who had tried vaginal delivery in Peking University First Hospital from January 1,2009 to December 31,2009 were retrospectively studied.Effects of different durations of protracted active phase on pregnancy outcomes were compared between women with vaginal delivery or cesarean section by Chi square test.Results The incidence of protracted active phase was 23.9％ (417/1744); and the incidence of prolonged active phase was 2.9％ (50/1744).There were 205 cases of cesarean section and 212 cases of vaginal deliveries.The incidence of neonates hospitalization in Department of Pediatrics in vaginal delivery group with active phase ≥6 h was higher than that of those with active phase ＜6 h [20.6％ (26/126) vs 8.1％ (7/86),x2 =6.07,P＜0.05].The incidence of intrauterine infection in cesarean section group with active phase ＜6 h were higher than that of those with active phase ≥6 h [22.7％ (30/132) vs 6.8％(5/73),x2 =8.37,P＜0.01].In vaginal group with protracted active phase before 6 cm of cervical dilation,the incidences of postpartum hemorrhage (4.8％,9/186),maternal complications (3.8％,7/186) and neonates hospitalization (15.6％,29/186) were similar with those after 6 cm of cervical dilation [6.2％ (1/16),x2=0.12; 0.0％ (0/16),x2=0.01 and 12.5％ (2/16),x2=0.00; all P＞0.05] respectively.In cesarean delivery group with cervical dilation ≥6 cm,the incidence of neonates hospitalization was higher than that of those with cervical dilation ＜6 cm [60.0％ (6/10) vs 19.9％ (34/171),x2 =8.83,P＜0.05].There were no difference in maternal age,gestational age,body mass index at delivery,volume of postpartum hemorrhage and neonatal birth weight between women with cesarean section whose protracted active phase ≥4 h or ＜4 h when cervical dilation at 3 cm (P＞ 0.05 respectively).The incidence of neonates hospitalization was low in women whose indication of cesarean section was protracted active phase (11.2％,9/80),while it was high when protracted active phase complicated with intrauterine infection (42.1％,16/38).Conclusions Protracted active phase with cervical dilation less than 6 cm might not need active management if neither the mother nor the fetus is compromised.The infants born vaginally should be closely monitored if the active phase is over 6 h.

Objective:To detect differentiallyexpressed microRNAs (miRNAs)in plasma of schizophrenia and explore biomarker for diagnosis of schizophrenia.Methods:The discovery cohort tincluded 6 patients with schizophrenia meeting diagnostic criteria of schizophrenia of the Diagnostic and Statistical Manual of Mental Disor-der,Fourth Edition (DSM-IV),as well as 6 healthy control subjects,whose age and gender were matched to pa-tients.The expression of 754 miRNAs (Sanger human miRBase v14)in the discovery cohort was investigated by Taqman Array (Human MicroRNA A +B Cards Set v3.0).Then the quantitative reverse-transcription polymor-phism chain reaction (qRT-PCR)assay was conducted to validate differentially expressed miRNAs in an independ-ent replication cohort,which included 25 schizophrenia patients and 18 healthy control subjects.The student's t-tests were used to analyze the differential expression of miRNA between schizophrenia patients and controls.All analyses were performed in Significance Analysis of Microarrays (SAM)software.Results:Twenty down-regulated miRNAs were observed in discovery cohort.Four miRNAs,hsa-miR-15b-5p were down-regulated in both discovery and rep-licated cohorts.Conclusion:The present study suggests that aberrant expression of miRNA might be of potential im-portance as biomarkers in the diagnosis of schizophrenia.
