Study of the strategic objectives chosen by hospitals from the perspective of stakeholders is now under the spotlight of researchers at large. To identify whether there exist differences in strategic objectives among various groups in the hospital and probe into the causes, the author called into play the competing values framework as the analysis tool. An evidenced-based study on public hospitals in Hangzhou identified marked differences on choices of strategic objectives among various internal stakeholders in the hospital For example, hospital administrators tend to focus on internal control, while medical doctors tend to focus on interactions between the hospital and the external environment. On this basis, further studies are proposed.
4.Effect of passive smoking during pregnancy on the expression of TNF-? and CINC in the lungs of neonatal rats

Objective To determine the effect of passive smoking during pregnancy on the expression of tumor necrosis factor-? (TNF-?) and cytokine-induced neutrophil chemoattractant (CINC) in the lungs of neonatal rats. Methods Sprague Dawley rats were exposed to sidestream cigarette smoke on the 1, 8 and 15 day after pregnancy for a total of 2 h each day. Those rats who were put into the box without smoke exposure were as control. The neonatal rats were delivered by cesarean section on the 20th day of gestation and birth weight was measured. Reverse transcription polymerase chain reaction (RT-PCR) and enzyme-linked immunoadsorbent assay (ELISA) was performed to evaluate the expression of TNF-?、CINC mRNAs and TNF-? protein. Objective To investigate the changes and effects of intercelluar adhesion molecule-1 (ICAM-1) and inhibitor protein of nuclear factor-?B(I?B?) in pulmonary hemorrhage induced by endotoxin lipopolysaccharide (LPS) in neonatal rats. Methods Sixty-eight seven to ten-day-old Wistar rats were separated into 2 groups randomly. In the experimental groups, LPS was injected intraperitoneally at the dosage of 5 mg/kg and the normal saline (NS) control group was injected with equal amount of NS intraperitoneally. The rats were sacrificed at 30 min, 1, 2, 4, 8, 16 and 24 h post injection. The histology of lung tissues were examined by microscope and electron microscope. The expression of ICAM-1, I?B?protein and mRNA expression were observed using Western blot and in situ hybridization. Results One hour after LPS administration, pulmonary hemorrhage and inflammatory responses were observed in the rat lungs. 8.Effects of different medication forms of dexmedetomidine on cognitive function in elderly patients undergoing radical operation of stomach neoplasms and Ramsay sedation score

Objective:To evaluate effects of different medication forms of dexmedetomidine on perioperative cognitive function in elderly patients undergoing radical operation of gastric carcinoma, and Ramsay sedation score.Methods:A total of 150 patients aged 65 years and 75 years scheduled for elective radical operation of gastric carcinoma from December 2018 to December 2019 in Shanxi Provincial Cancer Hospital were enrolled, and they were divided into 3 groups according to random number table method: different medication forms of dexmedetomidine groups (group A, group B) and the control group (group C), 50 cases in each group. The patients in group A and group B continued to pump dexmedetomidine at a rate of 0.5 μg·kg -1·h -1 and 0.3 μg·kg -1·h -1 respectively for 15 min before induction of anesthesia. And the patients in group C were given intravenous pumping of an equal volume of 0.9% NaCl for 15 min. Subsequently, patients in group A and group B received continuous intravenous infusion of 0.3 μg·kg -1·h -1 for 30 min before the end of operation, the patients in group C received 0.9% NaCl infusion of equal volume until the end of operation. The cognitive function of the patients was measured by using the Monterey cognitive assessment scale (MoCA) the day before surgery and on day 1,3 and 7 after surgery, and the incidence of perioperative neurocognitive disorder (PND) was counted. Ramsay sedation score at 30 min, 24 h and 48 h after surgery was compared among the three groups. Results:There were 3 patients in group A and 3 patients in group C were excluded because they were transferred to ICU due to serious postoperative complications. There were significant differences in MoCA score among the three groups on day 1, 3, 7 after operation (all P < 0.01); MOCA score of group A, B and C on day 7 after operation was (26.9±0.7) scores, (26.6±1.0) scores, (26.3±1.2) scores, respectively, and the difference between group A and group C was statistically significant ( P < 0.01). The incidence of PND among the three groups on day 1, 3, 7 after surgery had statistically significant differences (all P < 0.05), and the incidence of PND in group A was lower than that in group B and C (all P < 0.05). Ramsay sedation score among the three groups at 30 min and 24 h, 48 h after operation showed statistically significant differences (all P < 0.01), and that in group C was lower than that in group A and group B, and the differences were statistically significant (all P < 0.01). Ramsay sedation score at 24 h after operation in group A was high than that in group B and group C (all P < 0.01). Conclusions:Dexmedetomidine assisted with anesthesia can reduce the incidence of PND in elderly patients undergoing radical operation of gastric carcinoma and enhance the sedative effect. What's more, the most obvious effect is the infusion of 0.5 μg·kg -1·h -1 before anesthesia induction.
9.Pharmacological effects of anti-melanoma DC vaccine sensitized by fusion proteins of G3G6 and HST1

This study aimed to investigate the effects of fusion proteins GnRH-GRP(G3G6)and HSP65-STEAP1(HST1)on dendritic cells(DC)and the sensitization of DCs to B16F10 melanoma. The fusion proteins G3G6 and HST1 were obtained using the previous engineering strains in our laboratory. Group by unsensitized DC(US-DC), the G3G6 fusion protein sensitized DC, the HST1 fusion protein sensitized DC(HST1-DC)and the combined sensitized DC(GH-DC), the mouse bone marrow-derived DCs were sensitized with fusion protein to obtain the fusion protein sensitized DC vaccines. B16F10 melanoma cells were transplanted into C57BL/6J male mice to construct a melanoma model(1×106 cells per mouse), and DC vaccine was injected for treatment. The antitumor efficacy of DC vaccine was explored by in vitro and in vivo experiments. Flow cytometry analysis showed that the fusion protein can effectively stimulate DC into differentiation and maturation; in the animal experiment, the inhibition rate of melanoma treated with G3G6-DC was 35. 75%, that of HST1-DC group and combination group were 34. 03% and 55. 74%. It was initially proved that both G3G6-DC and HST1-DC can effectively inhibit the growth of transplanted tumors of melanoma B16F10 cells in mice, and the combination therapy is superior to the single therapy.
