1.Research on evaluation methods of the Pyretic pulmonary syndrome model based on metabonomics

Objective To estabhsh a set of consummate evaluation methods of animal models which are with syndromes of Traditional Chinese Medicine.Methods By means of Streptococcus pneumoniae-induced pyretic pulmonary syndrome model for the entry point,in the base of the conventional evaluation methods,further attempt were made through metabonomies,through analyzing metabolic fingerprint data of rats'urine in the control group together with the model group and metabolome of rats'urine in model group at different times,in order to approach the evaluation methods of animal models.Results After rats were given Streptococctts pneumoniae through nose,compared with the control group,both the body temperature variance and lencocyte count in model group were statistically significant with P <0.01,pathological changes of lung was obvious;According to the metabolic results,metabolic profiles of rats'urine in model group changed and metabolome diverged obviously in scores plot,the research results of metabonomics were coincide with the results of macroscopy physical sign and pathological biochemistry of Pyrefic pulmonary syndrome.Conclusion Metabonomics can be used to evaluate the studies of animal models with syndromes of traditional Chinese medieine.
2.Children's hearing behavior observations and high risk individual genetic screening for late-onset hearing loss early detection and intervention exploring a basic-level hospitals model.

OBJECTIVE:
To explore the methods to detect and intervene children's late-onset hearing loss early which are suitable for basic-level hospitals.
METHOD:
Udiology and imaging diagnosis had been given to the children who passed the newborn hearing screening but showed auditory behavior disorders in the growth process, and individualized interventions were given according to the results of diagnosis. Seven children with high risk for hereditary deafness were sent to superior hospital and had molecular screening of common mutations of inherited deafness carried out, then corresponding prevention guidance and intervention were given to them.
RESULT:
Fifty-two cases with late-onset hearing loss or verbal disorders were detected by auditory behavior observations,including 4 cases of auditory neuropathy, 4 cases of unilateral sensorineural deafness, 27 cases of secretory otitis media. 13 cases of bilateral sensorineural deafness and 4 cases of autism. Seven newborns with high risk of hereditary deafness were sent to the Third Affiliated Hospital of Sun Yat-Sen University and received molecular screening of common mutations of inherited deafness. One case with GJB2 compound heterozygous mutations was detected and followed up to 4 years old, he was found bilateral moderate hearing loss and accepted the hearing aids at 2 years old. Mitochondrial DNA 1555 a > G heterogeneity mutation in 2 cases and GJB2 235 delC single heterozygous mutations in 3 cases, no mutation in 1 case, all these 6 cases have been followed-up until now, their hearing are normal.
CONCLUSION
Children's auditory behavior observations and the superior hospitals referral performing high risk individual screening for newborns with high risk for hereditary deafness can detect children's late-onset hearing loss in time, this model is suitable for basic-level hospitals.

3.Use of the Y-shaped mesh in functional repair of the pelvic floor in women

BACKGROUND:The Y-shaped mesh graft material weaved using lightweight polypropylen has the appropriate porosity, which not only can make the vaginal tissues grow and fuse rapidly on the mesh, but also can maintain good biological strength to ensure the fixed strength for the presacral suspension. OBJECTIVE:To retrospectively analyze the clinical effect of Y-shaped mesh for the biological function reconstruction of the female pelvic floor. METHODS:Ten female cases of pelvic floor dysfunction were enrol ed, aged 37-73 years, and al were given the treatment of sacral colposuspension under laparoscopy. Then perioperative complications were recorded;patients were fol owed up regularly to record the Pelvic Organ Prolapse Quantification (POP-Q) score at different time points;and the subjective satisfaction was investigated using the Pelvic Floor Impact Questionnaire (PFIQ-7). RESULTS AND CONCLUSION:After at least 6-month fol ow-up, no postoperative pelvic infection, nerve damage and complications appeared, the patients healed wel , and no mesh erosion, infection and other adverse reactions occurred. The POP-Q and PFIQ-7 scores at 1, 3 and 6 months after repair were significantly improved than those before repair (P<0.05). These results suggest that the Y-shaped biological mesh repairing female midpelvic floor dysfunction has good biocompatibility, and can restore the anatomy of the pelvic floor.
4.Effect of uterine compression suture in the prevention and treatment of postpartum hemorrhage of cesarean section

Objective To analyze the clinical effect of uterine artery ligation combined with modified uterine compression suture in the prevention and treatment of postpartum hemorrhage.Methods A total of 107 patients with refractory cesarean section who were treated with uterine artery ligation combined with modified uterine compression suture were selected as treatment group.Of the patients treated with conventional treatment,83 patients were included in the control group.The effective hemostasis rate,clinical curative effect,incision infection,postpartum hemorrhage,blood transfusion rate,mattress rate and postoperative morbidity were compared between the two groups.Results The amount of intraoperative blood loss (1 106 ± 458.78) mL in the treatment group was significantly higher than (753.6 ±302.30)mL in the control group,the difference was statistically significant (t =1.892,P < 0.05).The 24h total blood loss of the the treatment group [(1 028.89 ± 375.87) mL] was significantly higher than (813.87 ± 312.15) mL of the control group (t =3.872,P < 0.05).The postoperative 24h blood loss in the treatment group [(70.86 ±19.30)mL] was lower than (73.50 ± 21.20)mL in the control group,and the difference was not statistically significant (t =0.045,P > 0.05).The transfusion rate in the treatment group was 27.10%,which in the control group was higher than 10.84%,the difference was statistically significant between the two groups (x2 =7.722,P < 0.05).The effective hemostasis rate in the treatment group was 99.66% (106/107),which was significantly higher than 96.39% in the control group (x2 =6.263,P < 0.05).The rate of puerperal infection was 7.48% in the treatment group,which in the control group was 6.02%,there was no significant difference between the two groups (P > 0.05).The incision infection rate and postoperative morbidity rate were 0.00% in the treatment group and the control group,and the two groups were treated with Ⅱ / A healing.Conclusion Uterine artery ligation combined with improved uterine compression suture in the prevention and treatment of postpartum hemorrhage has good hemostasis effect,hysterectomy rate is low,it is worthy of promoting and application.
5.A study on the current situation and influencing factors of career calling of nurses

Objective:To understand the status quo of nurses' professional mission and explore the influencing factors of nurses' professional mission.Methods:From November to December, 2018, 316 nurses from three tertiary hospitals in Tianjin were selected as the research objects. The occupational mission scale was used to investigate the sense of professional mission, the nursing work environment scale was used for the nursing work environment survey, and the work family conflict scale was used for the work family conflict investigation. Pearson correlation analysis was used to analyze the correlation among nurses' sense of professional mission, nursing work environment and work family conflict; multiple linear regression analysis was used to analyze the influencing factors of nurses' sense of professional mission.Results:The score of professional mission of nurses was (2.90±0.56) . Average monthly income, nursing work environment and work family conflict were the influencing factors of nurses' professional mission ( P<0.05) . The results of hierarchical regression showed that the higher the average monthly income ( β=0.252) , the higher the sense of professional mission of nurses ( R2=0.064) ; after controlling general data, the two dimensions of nursing work environment: Nurses' participation in hospital affairs ( β=0.263) , high-quality nursing service foundation ( β=0.368) , and work family conflict ( β=-0.145) could explain 43.1% of the total variation of professional mission. Conclusion:The sense of professional mission of nurses is above the middle level. Nursing managers should start with the influencing factors such as average monthly income, nursing working environment and work family conflict, so as to stimulate or improve nurses' sense of professional mission.

7.Relationship between nurses' calling and work engagement and their career success

Abstract]? Objective? To investigate effects of calling and work engagement on the career success of nurses, and construct a three-variable relationship model to provide theoretical basis for nursing management of clinical nurses. Methods? From July to October 2018, a total of 320 nurses from 3 Class ⅢGrade A hospitals in Tianjin were selected by the convenience sampling method, and the Calling Scale, Gallup Q12 Employee Engagement Survey, and Career Success Scale were used to measure the three variables. Results? A total of 320 questionnaires were distributed in this study, and 300 valid questionnaires were returned. The Calling Scale score of 300 nurses was (3.52±0.77), the score of Gallup Q12 Questionnaire was (2.20±0.47), and the score of Career Success Scale was (3.19±0.68). Correlation analysis showed that nurses' calling, engagement and career success were positively correlated (P<0.01). The path analysis model of work engagement as the mediator in the relationship between calling and career success showed that calling had direct predictive effect on career success, and the path coefficient was 0.41. Work engagement mediated the relationship between calling and career success, and the path coefficient was 0.18. Conclusions? Nursing managers should take various measures to improve the level of calling and work engagement of clinical nurses in order to promote their career success.
8.Clinical analysis of Blinatumomab on the treatment of refractory or relapsed precursor B-cell acute lymphoblastic leukemia

Objective:To evaluate the clinical efficacy and safety of Blinatumomab on the treatment of refractory or relapsed precursor B-cell acute lymphoblastic leukemia (R/R BCP-ALL) in children.Methods:Clinical data of children with R/R BCP-ALL treated with Blinatumomab in the Department of Hematology, Children′s Hospital of Soochow University, from August 2021 to June 2022 were retrospectively analyzed.Children were divided into<45 kg group and ≥45 kg group according to their weight at admission.They were treated with different dosages of Blinatumomab, and bone marrow remission was assessed at about 15 days.Clinical indicators and adverse events during the treatment period were recorded.The rank sum test of two independent samples were used to compare the differences between groups.The Fisher′ s test was used for comparing categorical variables. Results:Among the 16 children with R/R BCP-ALL, 12 cases (75%) achieved complete response (CR) and minimal residual lesion (MRD) turned negative at about 14 days.Among them, 5 out of 9 children with bone marrow primitive naive cell ratio≥0.5 achieved CR, and 7/7 children with bone marrow primitive naive cell ratio<0.5 achieved CR.The peak value of interleukin-6 (IL-6) in children with CR was significantly higher than those without CR ( Z=2.50, P=0.012). Twelve cases achieved CR on bone marrow assessment around day 15, and 3 cases who did not achieve CR remained in remission on day 28, with an efficacy prediction accuracy of 93.8%(15/16). Adverse events included fever, neutropenia, hypokalemia, abnormal liver function, hypocalcemia, edema, rash, hypertension, myocardial damage, abdominal pain, hypotension, and cytokine release syndrome, which were all grade 1.Neurotoxicity and death were not reported. Conclusions:The remission rate of R/R BCP-ALL in children treated with Blinatumomab was high, especially in patients with a low tumor load.The toxicity and adverse events of Blinatumomab treatment are minor and controllable.Day 15 is the optimal time point to evaluate the efficacy of Blinatumomab on children with R/R BCP-ALL, and a higher IL-6 peak can be served as a predictor of its efficacy.
9.Signal Conversion and Isolation Processing Technology Used in the Cerebrovascular Stroke Detector.

By using Doppler sensor and pressure sensor, the cerebrovascular stroke detector can be used to measure the blood flow velocity and blood pressure of the carotid artery. In this study, a variety of signal conversion and isolation processing techniques are proposed for processing and feature extraction of the output signals from the sensors. Finally, effective signal output waveforms that can be used to evaluate the cerebrovascular hemodynamics index (CVHI) are obtained, and the sound signal outputs that can reflect the change characteristics of blood flow velocity and blood pressure signals are generated, which realizes the application functional requirements of the detector.

10.A study on the correlation between medical staff engagement, perceived organizational support and turnover intention

Objective:To explore the relationship between medical staff's engagement, perceived organizational support and turnover intention.Methods:In May 2019, 600 medical and nursing staff from a tertiary hospital in Tianjin were selected by random sampling method. The questionnaire survey was conducted by using Gallup Q12 questionnaire, perceived organizational support scale and turnover intention scale. The correlation between engagement, organizational support and turnover intention was analyzed by person correlation analysis and regression analysis, and multivariate stepwise linear analysis was used for multiple factor analysis Return.Results:the total average scores of engagement, sense of organizational support and turnover intention were (2.18±0.56) , (3.48±0.72) and (2.71±0.67) respectively. There was a positive correlation between engagement and perceived organizational support ( r=0.674, P<0.01) , and negatively correlated with turnover intention ( R=-0.416, -0.487, P<0.05) . The factors of turnover intention were engagement and organizational support (F=54.673, 81.558, P<0.01) . Perceived organizational support partially mediated the relationship between engagement and turnover intention ( P<0.01) . Conclusion:The engagement and organizational support of medical staff are related to turnover intention.
