Thrombocytopenia caused by chemotherapy and radiotherapy has become a very important factor that influence the treating of malignant tumors. Traditional Chinese medicine has a good curative effect in its prevention and treatment. Animal experiment also proved the curative effect and some of the traditional Chinese medicine's function mechanism. We introduced the animal model, the state of field and development of the thrombocytopenia caused by chemotherapy and radiotherapy.

Objective To evaluate the occluder combined with covered stent-graft and coils in treatment of intra-thoracic pseudo-aneurysm at the proximal segment of subclavain artery. Methods The ASD occluder was deployed at the orifice of the subclavain artery via the access of left humeral artery to seal the entrance. The stent-graft was deployed to give additional sealing of the entrance of the left subclavain artery via the access of femoral artery. Finally the coils were deployed at the outlet segment of the pseudo-aneurysm again via the left humorol artery to block the reflux. Results The orifice and the outlet of the left subclavain pseudo-aneurysm were both sealed thoroughly without endoleak,brain accident or arm ischemic symptoms. Conclusions The tri-interventional method utilizing the occluder,covered stent-graft,and the coils in the treatment of the pseudo-aneurysm at the proximal segment of the subclavain artery has the benefits of mini-invasion,thorough sealing,stable frame structure,and assures high security.

Objective To study the effects of the early diagnosis and treatment of sleeve fractu re of patella in children.Methods The retrospection study was done to a nalyse the effects of the diagnosis a nd treatment of ten samples of sleeve fr acture of patella in our hospital fro m 1993to 2001.Results The diagnosis was confirmed by the characteristic s of the tension swelling of the knee,high position of the patella and a sle eve fractured fragment in the patella.T he operations were the main initial t reatment to regain the proper reduction.After 4weeksfixation the patients did knee exerc ises.And the curative effects were q uite good.Conclusion The sleeve fracture of the patella in children is easy to be misdiagnosed o r missed because of the cartilage.So it is the more important to be able to con duct an early diagnosis and treatmen t for the better prognosis.

Objective To explore the short-and mid-term curative effect and influence factors of hepaticojejunostomy(Kasai operation) for biliary atresia.Methods The clinical data of 66 children with biliary atresia treated with Kasai operation were analyzed retrospectively.The jaundice fading rate,2-year survival rate and influence factors of prognostic were analyzed.Results The jaundice fading rate of 62 cases was 58.1％ after Kasai operatiou.The patients also had a 2-year native liver survival rate of 53.2％.The jaundice fading rate,2-year survival rate of type Ⅰ,Ⅱ and Ⅲ had no significant differences (χ2 =1.91,0.76,all P ＞ 0.05).The 2-year survival rate of children underwent Kasai operation as younger than 60 days was higher rate than that of the children as 61 to 90 days and older than 90 days(χ2 =4.72,P ＜ 0.05).The 2-year survival rate of children with cytoinegalovirus infection and postoperative cholangitis were 43.6％,39.5％,respectively,which were lower than those of children without cytomegalovirus infection and postoperative cholangitis (69.6％.75.0％) (χ2 =3.92,7.46,all P ＜ 0.05).Conclusion Kasai operation is still the preferred method for the treatinent of biliary atresia.Type of the disease was not related with the short-and mid-term effects of Kasai operation.Age at operation was the main factor affecting the prognosis.Therefore,it is very important to make early surgery,enhance the anti-viral treatment and prevent postoperative cholangitis.

OBJECTIVE To investigate the pathogen distribution and antimicrobial resistance in ventilator-associated pneumonia(VAP) in intensive care unit(ICU) of our hospital and provide reference for clinical treatment.METHODS Among 135 patients with VAP in ICU of our hospital,pathogens distribution and resistance rate from sputum culture were analyzed retrospectively.RESULTS Among 188 strains of isolated pathogens in 135 patients,the Gram-negative bacteria accounted for 74.5%,the Gram-positive bacteria 14.4% and the fungi were 11.2%.The predominant pathogens were Pseudomonas aeruginosa(19.7%),Stenotrophomonas maltophilia(18.6%),Klebsiella pneumoniae(16.0%),and Acinetobacter baumannii(14.4%),Staphylococcus aureus(10.6%),Candida albicans(9.6%).The percentage of meticillin-resistant S.aureus(MRSA) was 95.0% and no vancomycin-resistant S.aureus isolate was detected out.Sensitive antibiotics were cefoperazone/sulbactam,imipenem/cilastatin,fluconazole etc.CONCLUSIONS The multi-drug resistant Gram-negative bacilli are the main pathogens in VAP in ICU of our hospital.Cefoperazone/sulbactam and imipenem/cilastatin can be used as first empirical antibiotics for patients with VAP in ICU of our hospital.

Objective To understand the iodine nutritional status of urban and rural pregnant women in Shijiazhuang.Methods Urban pregnant women were selected from 8 neighborhood committees of Chang' an District,Shijiazhuang,while the rural pregnant women were selected from 72 villages of 18 counties in Shijiazhuang.These research subjects were grouped into women with early pregnancy at 1-12 weeks,mid pregnancy at 13-28 weeks and third trimester at 32-40 weeks.Urine samples of the subjects were collected.Arsenic-cerium catalytic spectrophotometry (WS/T 107-2006) was used to determine urinary iodine concentrations (UIC).Results Seven hundred thirty-seven urban pregnant women and 898 cases from the rural areas,totally 1 635 subjects were included into the survey.The median UIC of urban subjects,rural cases,and all pregnant women in Shijiazhuang was 169.0,145.9 and 156.7 μg/L,respectively.The median UIC of the rural pregnant women was significantly lower than that of their urban peers (x2 =54.569,P ＜ 0.05).Urban pregnant women with UIC less than 150 μg/L accounted for 40.30％ (297/737) of the total cases,and rural pregnant women 51.89％ (466/898).The difference was statistically significant between these subjects (x2 =21.864,P ＜ 0.05).The median UIC of pregnant women at third trimester of the urban and the rural areas was 113.5 and 145.6 μg/L,respectively.Though their UICs were not up to the national criteria (150 μg/L),the difference was statistically significant between the urban and the rural subjects (x2 =8.365,P ＜ 0.05).UIC less than 150 μg/L of the urban and the rural pregnant women [60.19％ (65/108) vs 51.96％ (93/179)]at third trimester was not significantly different (x2 =1.844,P ＞ 0.05).Conclusions Iodine deficiency is present in pregnant women in Shijiazhuang City.Iodine deficiency among rural pregnant is more severe than that of peers of urban.The iodine deficiency is more severe in the pregnant population at late pregnancy.Besides iodized salt,food with rich iodine should be supplemented for the pregnant,especially those at the third trimester.It is important to monitor iodine nutritional status dynamically during the pregnancy to keep a proper UIC for the pregnant.

Objective To evaluate the application value of non-invasive hemodynamic monitoring by thoracic electrical bioimpedance (TEB) in elderly non-cardiac surgery patients after general anaesthesia.Methods Fifty-six elderly non-cardiac surgery patients after general anaesthesia were divided into control group and TEB group with 28 patients in each group by random digits table. All patients received standardized care including central venous pressure (VAP). Besides these monitoring, hemodynamic monitoring by TEB was used to evaluate the hemodynamic state of patients in TEB group. Diuretics, inotropic agents, vasoactive drugs or intravenous fluid therapy were used according to monitoring guidance. The heart rate (HR), mean arterial pressure (MAP), VAP, urine output (UO) were recorded in different time. The length of mechanical ventilation and ICU stay were also recorded. The difference in HR, MAP, VAP, UO, the length of mechanical ventilation and ICU stay between two groups were analyzed. Results The differences between two groups had no significance in HR, MAP, VAP, UO at the same time (P ＞ 0.05 ). The length of mechanical ventilation of TEB group [ (19.5±15.9)h] was shorter as compared to that of control group [ (25.5 ± 16.5) h ], but the difference was not significant (P =0.173). The ICU stay of TEB group [(2.8 ± 1.0) d ] was shorter as compared to that of control group[(3.6±1.6)d] and the difference was significant (P=0.032). Conclusion Non-invasive hemodynamic monitoring by TEB can monitor the changes of hemodynamics and direct treatment in elderly non-cardiac surgery patients after general anaesthesia.

Objective To explore the direct effect of attachment and the indirect effect through coping style and social support on mental health in technical school students. Methods 372 technical school students were assessed by Experiences in Close Relationship' s Inventory ( ECR), Symptom Check Scale List-90 ( SCL90) ,Coping Style Scale For Secondary School Students( CSS)and Perceived Social Support Scale(PSSS). Results ( 1 ) Total score of SCL-90 had significant correlations with all index of attachment anxiety, parents attachment avoidance( r=0. 165 ～0.370, P＜0.01 ) ,with coping style focus on problem ( r=0.291 ～0.552, P＜0. 01 ) and perceived social support( r = -0. 245, P ＜ 0.01 ). (2)Coping style focus on emotion had significant correlations with attachment anxiety( r=0.237 ～0.383, P＜0.01 ) and coping style focus on problem had significant correlations with attachment avoidance( r= -0. 267 ～ -0. 403, P＜0. 01 ). (3) Perceived social support had significant negative correlations with attachment avoidance ( r= - 0. 425 ～ - 0. 459, P ＜ 0.01 ). (4) The coping style focus on emotion had 65.2% mediation effect between attachment anxiety and mental health and perceived social support was moderated by attachment avoidance. Conclusions ( 1 ) Attachment anxiety and attachment avoidance have direct effect on mental health. ( 2 ) The indirect effect of attachment anxiety on mental health is a lot mediating effect performed by the coping style focus on emotion. Attachment avoidance also indirectly influences on mental health by moderating perceived social support.

Objective To study MRI differential diagnosis of peripheral hemangioma and vascular malformations.Methods MRIfindings of peripheral hemangiomas and vascular malformations proved by operation in 61 cases were retrospectively analyzed in comparison with pathological results.Results In 29 cases of peripheral hemangiomas,20 cases showed equal signal intensity (equal to muscle) and 9 cases showed heterogeneous signal intensity on T1-weighted images; 24 cases showed increased signal intensity (approach fat signal) and 5 cases showed markedly increased signal intensity (increase fat signal) on T2-weighted images,25 cases had septa as low signal network on T2-weighted images,4 cases showed enhancement separated mass enhancement after intravenous contrast injection.In 32 cases peripheral vascular malformations,16 cases showed equal signal intensity and 16 cases showed heterogeneous signal intensity on T1-weighted images.3 cases showed high signal intensity and 29 cases showed markedly increased signal intensity on T2-weighted images,15 cases showed inhomogeneous enhancement after intravenous contrast injection.Conclusion MRI plays an important role in differential diagnosis of peripheral hemangiomas and vascular malformations.
