1.Study on efficacy and safety of Desloratadine in the treatment of allergic rhinitis

Objective:To evaluate the efficacy and safety of Desloratadine in treating allergic rhinitis(AR).Methods:A double-blind,multicenter,randomized,comparative study versus Loratadine was carried out.A total of 190 subjects were involved in the trial.Results:The efficacy of Desloratadine was 88.30%,and the excellent rate was 73.40%,while the efficacy of Loratadine was 85.42%,and the excellent rate was 65.63%.The incidences of adverse reaction assessment (ADR) of Desloratadine and Loratadine were 8.5% and 17.7% respectively.Adverse reactions mainly included mild or moderate drowsiness,dry mouth,and headache.Conclusion:The efficacy of Desloratadine and Loratadine is similar in the treatment of AR. The incidences of ADR in Desloratadine group are less than that in Loratadine group.National Desloratadine is effective and safe for allergic rhinitis.
2.Analysis of Epstein-Barr virus LMP1 gene Xhol-site mutation in plasma of patients with nasopharyngeal carcinoma in Chongqing of China

Objective:To detect and analyze the Epstein-Barr virus LMPI gene XhoI-site mutation in plasma of patients with nasopharyngeal carcinoma(NPC)in Chongqing of China.Methods:DNA extraction and PCR amplification was used in plasma of 48 NPC patients and 40 control non-NPC cases from Chongqing of China.All the PCR production was digested by enzyme XhoI,then segregated in 8% PAGE.XhoI-site mutation was analyzed by sequencing and comparing with B95-8 cell.Results: The LMP1 was amplified and digested successfully from 48 of 48 NPC cases(100%)and from 38 of 40 non-NPC cases (95%),but none of LMPI XhoI-site mutation was found.Sequencing was performed on 4 LMPI genes from NPC cases and 5 LMP1 genes from non-NPC cases,they showed the presence of the XhoI-site.Conclusions:XhoI-site in EBV LMP1 gene N terminus has no mutation in plasma of NPC or non-NPC cases in Chongqing of China.The precise relationship of XhoI-site mutation and NPC pathogenesis need further investigation.
3.Chronic rhinosinusitis and tissue remodeling.

Chronic rhinosinusitis is a common health problem of otorhinolaryngology head and neck surgery, and is characterized by persistent inflammation of the nasal and paranasal mucosal linings. In recent years, tissue remodeling has been demonstrated to play an important role in the development of chronic rhinosinusitis, and furthermore transforming growth factor-beta1 (TGF-beta1) may play a pivotal role in this progress. This review will summarize the current knowledge on the roles of tissue remodeling in chronic rhinosinusitis, therefore to offer reference to the research and help to make diagnosis and give treatment for this disease.

4.Clinical efficacy of alternating chemo-radiotherapy for locally advanced nasopharyngeal carcinoma.

OBJECTIVE:
The purpose of this study is to investigate the effective of alternating Chemo-radiotherapy for locally Advanced Nasopharyngeal Carcinoma.
METHOD:
Retrospective analysis 106 cases of patients with locally advanced nasopharyngeal carcinoma between November 2005 and March 2007. All patients received cisplatin-based chemotherapy but 15 patients received radiotherapy(RT) alone. Inducing chemotherapy (IC) + RT + adju-vant chemotherapy (AC) regimen in 36 patients, IC+RT regimen was delivered in 25 patients and AC + RT regimen in 30 patients. 61 patients received 1 to 2 cycles of inducing chemotherapy and 66 patients received 3 to 6 cycles of adjuvant chemotherapy after radiotherapy. Chemotherapy started on the first day after the end of the induction chemotherapy, adjuvant chemotherapy begun after radiotherapy for a week. All patients were treated by radiotherapy using 60 Co r-ray, the nasophyarynx primary site was given a total does of 68 -74 Gy. The lymph nodes of the neck was given 60 to 70 Gy. The prophylactic irradiation does of the neck was 48-50 Gy. RESCULT: The median follow up time was 51 months. A total of 58 patients died, the overall survival rate was 45% in whole groups. The 5-year overall survival rates were 33%, 63%, 60% and 50% in RT, IC + RT + AC, IC + RT and RT+AC group, respectively. The 5-year disease-free survival rates were 13%, 56%, 48% and 40% in RT, IC + RT + AC, IC + RT and RT + AC group, respectively. The 5-year relapse-free survival rates were 13%, 53%, 48% and 50% in RT, IC + RT + AC, IC + RT and RT + AC group, respectively. The 5-year metastasis-free survival rates were 6%, 50%, 44% and 47% in RT, IC + RT + AC, IC+ RT and RT + AC group, respectively. There was significant difference in all groups (P < 0.05). The median time to relapses were 22 months, 29 months, 28 months and 25 months in RT, IC + RT + AC, IC + RT and RT + AC group, respectively. The median time to first distant metastasis were 10 months, 19 months, 15 months and 12 months in RT, IC + RT + AC, IC + RT and RT + AC group, respectively. There was no significant difference in all groups (P > 0.05). IC + RT + AC group had heavier acute toxicity effects than other groups, but it did not affect the treatment process, all patients could be tolerated.
CONCLUSION
This retrospective study has demonstrated that alternating Chemo-radiotherapy and early radiotherapy not only can improve the survival rate for locally Advanced Nasopharyngeal Carcinoma, but also have slight toxicities and side reaction, all patients may tolerated.

5.The secretion and use of eosinophil cationic protein from patients with chronic nasal disease

Eosinophil cationic protein, a sensitive marker of activated eosinophils, plays an important role in eosinophila inflammation, such as asthma, allergy disease and so on. To get acknowledges of ECP of secretion and use in nasal general chronic disease(NP, CS and AR),we review many articles and summary the present situation of this topic of its properties and uses. We find a critical relationship between ECP and nasal general chronic disease. Moreover, the measurement concentration of ECP in nasal fluid can help evaluate degree, turnover and prognosis of these diseases.
6.Comparison of curriculum and degree conferment in higher medical education between Germany and China

The higher medical education in China starts late and needs further exploration and improvement.While in Germany,it is elite education with a long history.Compared with China,in curriculum setting,Sociology,Medical health security,Humanities and Social Sciences and Organ system classification courses are more emphasized,and in terms of degree,it only has Doctor degree but with higher quality in Germany.Higher medical education in China will also take the path of the elite education.We can learn from the German medical education development experience and pay attention to solving the lack of the curriculum by supplementing social medicine and medical health security,increase the autonomy of the instructors in the training process,strengthen the medical students' social responsibility,and improve the quality of higher medical education in China.
7.Purification of bone marrow mesenchymal stem cells by a Colony-Forming system

Objective To purify BMSCs by a simple Colony-Forming system and identify BMSCs in vitro.Methods BMSCs were planted at low-density ( 10/cm2 ) in the early stage of isolation,additional cells were scraped until a colony formed,secondly seeded those Colony-Forming cells at low-density.This process was repeat again when passaged cells formed another colony.This so called Colony-Forming selection was repeated several times until highly purified cells were obtained.Biological characters of BMSCs were observed.The surface antigens of BMSCs were identified by flow cytometry.The multipotentiality of BMSCs was assayed for differentiation into either osteoblasts or adipocytes.Results Homogeneous cells were obtained after BMSCs were passaged twice to thrice by Colony-Forming system.These BMSCs highly expressed positive surface antigens of BMSCs ( CD29 in 98.8%,CD90 in 98.4% ) meanwhile seldom expressed negative surface antigens of BMSCs (CD31 in 2.6%,CD45 in 3% ).BMSCs efficiently differentiated into osteoblasts and adipocytes.Conclusions Colony-Forming system is a simple and effective way to get highly purified BMSCs in a short time.
8.MR guided focused ultrasound for treating uterine fibroids:a Meta analysis

Objective To investigate the recent effect and safety of magnetic resonance guided focused ultrasound (MRgFUS) for treating uterine fibroids with a Meta analysis. Methods Such databases as Cochrane Library,PubMed,EMbase, CBM,CNKI,CQVIP and Wanfang were searched from their establishment. After thestudy selected, assessment and data extraction conducted by two reviewers independently, Meta-analysis were performed by using the Comprehensive Meta-analysis V2 software. The volume of non-perfusion ratio (NPVR), fibroid shrinkage, clinical symptom severity (SSS score) and adverse reactions were analyzed.Results Nighteen retrieved articles were included, with a total of 1 549 patients. The results of Meta-analysis showed that:(1)nonperfused volume ratio (NPVR),Mean=57.1%(95%CI 45.0% —69.2%).(2)Percentage of FBs shrinkage:mean=31.4% (95% CI 29.2% —33.6%).(3)Symptom severity score (SSS):baseline mean=51.00(95%CI 41.61—60.40),3 months mean=32.50 (95%CI 25.90—39.10),6 months mean=28.62(95%CI(23.57—33.66),12 months mean=24.46(95%CI 15.44—33.49), the average of the severity score of the patient symptoms had a tendency to reduce.(4)The degree of adverse reactions were mild and could relieve itself, there was fewer serious adverse reactions. Conclusions The current evidence indicates that MRgFUS in treatment of uterine fibroidsis reliable and safe. MRgFUS can significantly reduce the volume and the symptom severity score ofthe patients.
9.Application of Medical Imaging in Nasal Endoscopic Surgery Teaching

The application and expanding of nasal endoscopic surgery,an epoch-making transform of rhinology,have a very close associations with medical imaging,which includes anatomy photograph,conceptual diagram,radiological imaging,nasal endoscopic image display system,operation photographic recording and computer assisted instruction courseware,etc.It is important to collect the medical imaging through various means and set up medical imaging resources library for nasal endoscopic surgery teaching.
10.Experiences of Clinical Teaching of Advanced Otolaryngology-head & Neck Surgeons
