1.Application of HIS Hospital Management System in Medical Equipment.

OBJECTIVETo analyze the effect of HIS hospital management system in medical equipment.
METHODSFrom April 2012 to 2013 in our hospital 5 100 sets of medical equipment as the control group, another 2013 in our hospital from April 2014 may 100 sets of medical equipment as the study group, comparative analysis of two groups of medical equipment scrap rate, usage, maintenance score and the score of benefit etc.
RESULTSControl group and taken to hospital information system, his research group equipment scrap rate, there was a significant difference, the research group of equipment maintenance score and efficiency scores were higher than those of the control group (P < 0.05), the study group of equipment maintenance score and efficiency scores were higher than those of the control group.
CONCLUSIONHIS hospital management system for medical equipment management has positive clinical application value, can effectively improve the use of medical equipment, it is worth to draw and promote.

Chronic rhinosinusitis is a common health problem of otorhinolaryngology head and neck surgery, and is characterized by persistent inflammation of the nasal and paranasal mucosal linings. In recent years, tissue remodeling has been demonstrated to play an important role in the development of chronic rhinosinusitis, and furthermore transforming growth factor-beta1 (TGF-beta1) may play a pivotal role in this progress. This review will summarize the current knowledge on the roles of tissue remodeling in chronic rhinosinusitis, therefore to offer reference to the research and help to make diagnosis and give treatment for this disease.

4.Influence of intensive nursing on working memory dysfunction in patients with early Parkinson's disease

Objective To examine the influence of 3R intensive nursing on working memory dysfunction of patients with early Parkinson's disease. Methods 30 patients with early Parkinson's disease were assessed by Working Memory Test Battery,and acceped 3R- nursing. The condition of working memory dysfunction was compared before and after nursing intervention. Results The patients with early Parkinson's disease had evident difference than the normal controls, especially the visualspatial working memory. After 3R- nursing, the subtest scores and dimensions were improved apparently. Conclusions The clinical effect of 3R- nursing method in patients with early Parkinson's disease has significant predominance ,so it is worth popularizing.
5.Clinical studies of transurethral bipolar plasma cutting and transurethral resection for benign prostatic ;hyperplasia

Objective To investigate clinical value of transurethral bipolar plasma cutting and transurethral resection for benign prostatic hyperplasia,to provide a reference for clinical treatment.Methods 100 cases with benign prostatic hyperplasia were selected,according to surgical treatment,they were divided into control group and observation group.50 patients in the control group received TURP surgical treatment,50 patients in the observation group were given TUPKP surgical treatment.The patients were followed up for 3 months,the indwelling catheter time, weight and other clinical indicators of glandular tissue were compared,serum sodium and hemoglobin levels were observed,the quality of life index,international Prostate Symptom Score,complications and other indicators were compared. Results In the observation group,operative time was (60.54 ±8.37)min,blood loss was (108.43 ±21.27)mL, hospital stay was (7.09 ±2.25)d,which were shorter than those of the control group (70.24 ±12.22)min,(188.76 ± 21.36)mL,(8.63 ±2.76)d,the differences were statistically significant (t =5.441,P =0.012;t =8.326,P =0.001;t =6.216,P =0.006).In the observation group after three months,maximum flow rate was (18.95 ±4.11)mL/s, residual urine volume was (28.74 ±4.55)mL,IPSS was (5.12 ±1.27)points,QOL was (1.43 ±0.24)scores, which were better than those of the control group (14.24 ±3.77)mL/s,(20.85 ±5.11)mL,IPSS(6.95 ±1.44)points, QOL(2.03 ±0.32)points,the differences were statistically significant (t =8.763,P =0.000;t =7.616,P =0.003;t =5.472,P =0.011;t =8.044,P =0.002).The incidence rate of complication in the observation group was 10.00%, which was significantly lower than 22.00% in the control group,the difference was statistically significant (χ2 =5.437,P =0.012).Conclusion TUPKP treatment for benign prostatic hyperplasia has significant clinical effect, which can effectively improve the clinical symptoms and quality of life with good safety,it is worthy of clinical application.
6.Clinical efficacy of exclusive right-thoracic approach in esophageal surgery

Objective To investigate the indications and clinical efficacy of an exclusive right-thoracic approach in esophageal surgery.Methods The clinical data of 35 patients with esophageal disease who underwent an exclusive right-thoracic approach operation from June 2008 to June 2010 at the Affiliated Hospital of Qingdao University were retrospectively analyzed.Of the 35 patients,21 were with esophageal cancer,and their ages were above 70 years,the partial pressure of oxygen of arterial blood (PaO2),maximal voluntary ventilation and forced vital capacity of the first second were (70 ± 9) mm Hg (1 mm Hg =0.133 kPa),51% ± 9% and 57% ± 11%,respectively.Fourteen patients were with benign esophageal disease or low grade malignancy,and their PaO2 maximal voluntary ventilation and forced vital capacity of the first second were (96 ± 13) mm Hg,83% ± 11% and 91%± 14%,respectively.Exclusive right-thoracic approach was selected for all the patients,and surgical procedure was selected according to the location of the lesions.Patients were followed up by out-patient examination or phone call till April 2012.Results All the operations were carried out successfully,21 patients with esophageal cancer underwent partial esophagus resection and stomach-esophagus intrathoracic anastomosis through a right-thoracic approach.Of the 14 patients with benign esophageal disease or low grade malignancy,2 patients were with esophageal leiomyoma,3 with esophageal stromal tumor and 1 with esophageal leiomyomatosis underwent partial esophagus resection + intrathoracic anastomosis ; 7 patients with esophageal leiomyoma and 1 with esophageal stromal tumor underwent tumor extirpation.The operation time and operative blood loss were (3.4 ± 1.8)hours and (160 ± 44)ml,respectively.The number of lymph node dissected of the 21 patients with esophageal tumor was 14 ± 5,including 1.1 ± 0.7 lymph nodes dissected near the left gastric artery.No anastomosis fistula and perioperative death were detected,and the duration of hospital stay was (10 ± 4)days.The postoperative PaO2,maximal voluntary ventilation and forced vital capacity of the first second were (66 ± 10)mm Hg,50%± 11% and 51% ± 13% for patients with esophageal cancer,and (94 ± 13) mm Hg,80% ± 13% and 87% ± 16% for patients with benign esophageal disease or low grade malignancy.All the patients were followed up for 1-2 years,no eating obstruction was detected,and the postoperative 1-and 2-year survival rate were 82.9%(29/35) and 77.1% (27/35),respectively.Of the 8 patients with esophageal cancer who died postoperatively,1 died of cardiovascular diseases and 7 died of tumor recurrence and metastasis.Conclusion Exclusive rightthoracic appraoch could be used for patients with benign esophageal disease,low-grade malignancy and old esophageal cancer patients with poor pulmonary function.

Objective To study expression of extracellular signal-regualated kinase in intraepithelial neoplasia,prostatic cancer and BPH tissue. Methods Expression of extracellular signal-regualated kinase (ERK) was determined in 12 prostatic intraepithelial neoplasia (PIN),11 prostate cancer and 16 benign prostatic hyperplasia specimens.ERK expression was determined by immunohistochemistry. Results ERK expression was positive in all the 12 PIN specimens,6 of which being overexpressed.In the 11 prostate cancer specimens,overexpression was observed in only one while the other 10 under expressed.Overexpreassion was not observed in all the BPH specimens whereas 12 of the 16 showed underexpression. ObjectiveTo evaluate the influence of the resected prostate tissue volume (RPV) on the improvement of International Prostate Symptom Score (IPSS), quality of life (QOL), and voiding function after transurethral resection of the prostate (TURP).
METHODSThis study included 82 men with benign prostatic hyperplasia treated by TURP. Before and three months after TURP, we obtained the IPSS, QOL score, post-voiding residual urine volume (PVR), and maximum urinary flow rate (Qmax) from the patients. We measured the total prostate volume (TPV) and transition zone volume (TZV) by transrectal ultrasound preoperatively and investigate the influence of the RPV, RPV/TZV ratio, and RPV/TPV ratio on the efficiency of TURP.
RESULTSAt three months after TURP, the mean Qmax increased by 9.27 ml/s, IPSS decreased by 15.86, QOL score increased by 3.47, PVR decreased by 87.1 ml, and 72.0% of the patients felt satisfied with the surgical results. There was no statistically significant difference in RPV between the patients satisfied and those dissatisfied with the results. Both the RPV/TPV and RPV/TZV ratios significantly increased in the satisfaction group as compared with the dissatisfaction group (P=0.002 and P=0.004). The areas under the ROC curve for the RPV/TPV and RPV/TZV ratios were 0.793 (P=0.001) and 0.687 (P=0.009), respectively.
CONCLUSIONSRPV is closely related to the short-term outcomes of TURP, and the ratios of RPV/TPV and RPV/TZV may be used as new markers to predict the outcomes of TURP.

9.Remolding of the reconstructed finger by toe to hand transfer

Objective To design technique of local flap transposition to refine the aesthetic appearance of reconstructed fingers by toes transfer.Methods Nine cases with 21 reconstructed finger were included,which involved 6 males and 3 females with an average age of 21.8 years(range,18-34years).A lingual contour flap with a lateral pedicle Was shifted from the inflated distal pulp to the narrow middle part of the"finger"to refine the aesthetic appearance.Overall results were evaluated in terms of the survival of the flap,the appearance improvement and the functional influence of the reconstructed finger.Results All of the flaps survived and healed perfectly.After a mean follow-up of 9.3 months(range,6-12months),the appearance of the reconstructed fingers were impmved apparently.There was little influence on the function of the finger.The results showed that all the patients gained more acceptable fingers.Conclusion From our experience,local flap transposition is a useful method for remolding of reconstructed fingers by toes transfer.
10.Medium term follow-up of phrenic nerve transfer to the posterior division of lower trunk to recover elbow and finger extension in patients with brachial plexus root avulsion

Objective To observe effect of phrenic nerve transter to the posterior division ot lower trunk (PDLT) for recovering elbow and finger extension in patients with brachial plexus root avulsion.Methods From June 2005 to December 2008,43 patients with brachial plexus root avulsion were treated with phrenic nerve transfer to PDLT to recover elbow and finger extension.There were 36 males and 7 females,aged from 4 to 44 years (average,23.5±9.9 years).The interval from injury to operation ranged from 1 to 12months (average,3.7±1.9 months).There were 32 cases of total nerve roots avulsion,5 cases of middle and lower trunk avulsion accompanied with upper trunk normal or partial injury,and 6 cases of C6 to T1 nerve root avulsion accompanied with C5 nerve root rupture or partial injury.The normal function of the phrenic nerve in the injured side should be proved preoperatively by radiographic and electromyographic examination.The entire brachial plexus in injured side was exposed through the combined incision.The posterior division of lower trunk was identified and severed as proximal as possible,and the posterior cord and radial nerve were dissociated distally until to the level of midpoint of humerus.Then the branches of the posterior cord except the radial nerve were sectioned.Direct anastomosis of the phrenic nerve and PDLT was performed in 33 patients,and indirect anastomosis through bridge grafting using sural nerve was performed in remaining 10 cases.Results All patients were followed up for 36 to 73 months (average,39.7±7.l months).The percentage of muscle strength ≥grade 3 in elbow,finger and thumb extension was 81.6%,41.9% and 39.5%,respectively.Conclusion Satisfactory functional recovery of elbow extension had been achieved after the phrenic nerve transfer to the PDLT in patients with brachial nerve root avulsion injury,however,the functional recovery of finger and thumb extension was not as satisfactory as anticipated.
