1.Research advances in protective effects of water soluble compounds in Salvia miltiorrhiza against myocardial ischemia

Salvia miltiorrhiza is dry root or rhizome of the labia-tae plant Salvia miltiorrhiza, whose water-soluble ingredients are recognized as its bioactive components in cardiovascular disea-ses. Based on researches at home and abroad in recent years, this article summarizes pharmacological effect resisting myocardi-al ischemia of Salvia miltiorrhiza ’ s water soluble compounds in multi-level from the prevention, treatment and anti-reperfusion damage, to provide theoretical basis for the research and devel-opment of the active monomers in Salvia miltiorrhiza and com-pound preparations.
2.Effect and mechanism of total flavonoids of bugloss on rats with myocardial ischemia and reperfusion injury.

This study is to investigate the effect of total flavonoids of Uygur medicine bugloss (BTF) on rats with myocardial ischemia/reperfusion injury, and to explore the mechanisms by which it acts. Left anterior descending (LAD) coronary artery in rats was occluded for 30 min followed by 4 h reperfusion. Meanwhile, BTF dissolved in saline was administered intraperitoneally at dosage of 10, 30 and 50 mg x kg(-1). Electrocardiograph, infarction index, serum myocardial enzymes and heart function were determined to evaluate the effect of BTF. Some other observations were carried out to explore whether inhibiting inflammation and apoptosis is involved in the mechanisms underlying BTF. Our results showed that in ischemia/reperfusion injured rats BTF could dose-dependently reduce myocardial infarction index and myocardial enzyme leakage, and enhance heart function, indicating that it possesses significant cardio protection. ELISA analysis showed that BTF could decrease the content of myocardial inflammatory cytokines such as IL-1beta, IL-6 and TNF-alpha. Western-blotting confirmed that BTF could increase the expression of anti-apoptotic protein Bcl-2 and reduce the expression of proapoptosis protein Bax. Further more, the phosphorylation level of PI3K and Akt was upregulated by BTF treatment. BTF can protect rat against myocardial ischemia/reperfusion injury. Anti-inflammation and inhibition of apoptosis through upregulating PI3K/Akt signal pathway may contribute to the protective effect of BTF.
3.Protective effect of Salvianolic acid A against isoproterenol-induced myocardial infarction in mice

Aim To investigate the effect of Salvianol-ic acid A (Sal A)on mice with isoproterenol (ISO)-induced myocardial infraction and its possible mecha-nisms.Methods The mice were subcutaneously in-jected with ISO (8 mg·kg-1 )to induce myocardial in-farction.The myocardial protective effect of Salvianolic acid A was evaluated from mortality rate,electrocardio-gram (ECG),heart function,myocardial infarction in-dex,serum myocardial enzymes and its action mecha-nisms were explored from inflammation,anti-oxidation and cells apoptosis.Results Salvianolic acid A dose-dependently enhanced the heart function of myocardial infarction mice,reduced the heart index,inhibited the myocardial enzyme leakage,showed obvious myocardi-al protection effects.ELISA results showed that Salvi-
anolic acid A could reduce the expression of myocardial inflammatory cytokines such as IL-6(interleukin-6,IL-6),TNF-α(tumornecrosis factor-α,TNF-α).West-ern-blotting confirmed that Salvianolic acid A could in-crease the expression of anti-apoptotic proteins Bcl-2, reduce the expression of apoptosis protein Bax,and raise the phosphorylation level of PI3K and Akt.Con-clusion Salvianolic acid A displays a significant pro-tective effect against isoproterenol-induced myocardial infarction and its mechanism may be related to the in-crease of PI3K/Akt signal pathway and the inhibition of cell apoptosis and inflammatory reaction.
4.Vasorelaxant effect of novel Rho-kinase inhibitors on isolated thoracic aorta rings and underlying mechanisms

Aim To evaluate the vasorelaxant effect of two new chemical entities, J35242 and J35243, on iso-lated rat thoracic aorta rings as Rho-kinase inhibitors, and further to explore the underlying mechanisms of these two compounds. Methods Isolated rat thoracic aorta rings pre-contracted by KCl or norepinephrine ( NE) were used to evaluate the vasodilatory effect of J35242 and J35243 . Through the interventions of sev-eral tool drugs, the mechanisms of compounds concern-ing endothelium, K+ channels and Ca2+ were studied. Results J35242 and J35243 showed potent relaxant effect on both KCl and NE pre-contracted vessels, and exhibited partial endothelium dependency. L-NAME and Methylene Blue( MB) could influence the relaxant effect of these compounds. Meanwhile, the compounds could inhibit intracellular Ca2+ release and extracellu-lar Ca2+ influx, which indicated that the compounds might block the calcium channels to relax the vessels. In addition, the two compounds probably did not dilate the aorta rings through opening potassium channels. Conclusions J35242 and J35243 have vasorelaxant effects on vessels in vitro and the potency of J35242 is stronger than that of J35243 . The underlying mecha-nisms might be endothelium-dependent. Also the com-pounds might block Ca2+ channels, lowering intracel-lular Ca2+ concentration to relax the vessels.
5.The analysis of identification for non-tuberculous Mycobacterium with molecular assay genotype Mvcobacterium kit

Objective To identify non-tuberculous Mycobacterium(NTM) rapidly with HAIN molecular assay genotype Mycobacterium kit,and investigate the advantages and disadvantages of this method.Methods Seventy-four clinical NTM isolates were collected from hospitals in Zhejiang and Anhui province.Clinical strains were identified with HAIN molecular assay genotype Mycobacterium kit.16S rRNA gene sequencing was used to estimate and compare with this method.Results The results of kit showed that there were thirty-one M.intracellulare strains,twelve M.chelonae strains,eight M.fortuitum strains,six M.kansasii strains,five M.avium strains,three M.smegmatis strains,two M.phlei strains,two M.scrofulaceum strains and one M.gordon strain.Four strains were identified as Mycobacterium without further identification.Eight M.tuberculosis strains were identified correctly too.Compared with 16S rRNA gene sequencing,except for four strains identified as Mycobacterium,others 70 strains got the same results as 16S rRNA gene sequencing,the coincidence was 94.59%,and it could further identify thirteen Mycobacterium chelonae complex and eight Mycobacterium kansasii complex to subspecies M.abscessus and M.kansasii,respectively.If only to identify strains under the identification range of this kit,the coincidence reach to 100%,Conclusion The method of HAIN molecular assay genotype Mycobacterium kit is simple and accurate,the time is shorter and should widely be applied clinically.
6.379 Cases of HCV infected with hepatitis c virus and anti HCV and liver function index correlation analysis

Objective Hepatitis c patients with HCV RNA loads and anti HCV and liver function index of correlation.Methods Retrospective analysis in January 2011 to December 2013 in our hospital outpatient,hospital,medical examination in patients with hepatitis c antibody positive,male 150,female 229,age 32-87,application of real-time fluorescent quantitative PCR detection of HCV RNA viruses,automatic biochemical analyzer test liver function 8 indicators,according to the number of HCV RNA viral load points HCV RNA negative groups,HCV RNA in the low level group,high levels of HCV RNA.Results Three groups of age distribution proportion by the statistical test has no obvious difference (P > 0.05),but low in the proportion of women is slightly higher than that of males,Three groups of liver function and biochemical indicators is not identical results (P < 0.05 or P < 0.01),of which the HCV RNA in the low level group and high level of serum ALT,AST,GGT and ALP,TBIL compared with negative groups have difference (P < 0.05 or P < 0.01) ; TP,propagated,DBIL comparison,though no significant statistical differences between the three groups,but the low level and high level group results in a downward trend,in the low level and high level group comparison ALT,AST,TBIL also have obvious difference (P < 0.05),viral load,the higher the ALT,AST results,after treatment with interferon with loads of decline index of liver enzymes.Conclusion No matter the discretion of the viral load to last as long as HCV RNA positive,can cause the damage of liver cells and liver function abnormalities,clinical should be antiviral treatment as soon as possible.Real-time PCR fluorescence probe technique is advanced,mature technology,stable results,to early diagnosis of HCV infection,help clinical early antiviral treatment,prevent the spread of HCV and development.
7.67 cases of chronic hepatitis C treatment process and follow-up serum ALT, viral load and the change of liver fibrosis indexes

Objective Observation of polyethylene glycol (peg) interferon alpha 2 b plus ribavirin treatment of chronic hepatitis C liver function,viral replication and liver fibrosis index in the process of change.Methods Detection of 67 cases of chronic hepatitis C patients in the treatment with interferon plus ribavirin end of week (0),(48 weeks) and drug withdrawal 12 weeks (60 weeks) in serum alanine aminotransferase (ALT),hepatitis C virus (HCV) RNA (HCV RNA),hyaluronic acid (HA),a former Ⅲ type collagen peptide (PC Ⅲ),collagen type Ⅳ (Ⅳ-C) and laminin (LN) level. Results Complete response after treatment group (CR-S,43/67) ALT,4 items of HCV RNA and serum fibrosis indexes were significantly decreased (P < 0.05 or P < 0.01),part of the response group (CR-R,13/67) and no answer (NR,11/67) ALT,4 items of HCV RNA and serum fibrosis index change is not obvious,bounce is even higher.Conclusion Polyethylene glycol interferon alpha 2 b plus ribavirin treatment of chronic hepatitis C response,about 65% of patients completely as the improvement of liver inflammation,viral RNA drops and liver fibrosis index level declined obviously,suggest that interferon plus ribavirin can inhibit HCV RNA replication,regulate the body's immune function,reduce liver inflammation,improve liver function,reduce liver fibrosis.
8.Primary screening of children with vaccination contraindications and referral recommendations: suggestions for primary care of children in Shenzhen

Guideline or consensus for the vaccination of children with special conditions like immunocompromised children may be more suitable for pediatricians.However, the vaccination of children in China is mainly performed by general practitioners or child health care practitioners in community health service centers.They need to master the screening knowledge of contraindications and precautions for the vaccination of children, and make the decision to referral to specialists.Based on the technical guidelines for Immunization of National Health Commission of the People′s Republic of China, Best Practices Guidance of the Advisory Committee on Immunization Practices and contraindications and precautions proposed by the Immunization Action Coalition, 20 suggestions for primary screening of children with vaccination contraindications and referral recommendations for primary care providers were developed by experts from the Integration of Medicine and Prevention in Children of Health Commission of Shenzhen Municipality.
9.Progress on the pathogenesis of neurofibromatosis type 1

Neurofibromatosis type 1(NF1)is one of the most common autosomal dominant disorders. The disease is caused by mutations in the NF1 gene,which can involve multiple systems and have a variety of clinical manifestations,including café au lait macules,lisch nodules,neuroglioma,autism spectrum disorder,learning difficulties,neurofibromas,and skeletal dysplasia,et al.In previous studies on the pathogenesis of NF1,most of them have focused on the regulation of the RAS signaling pathway by neurofibromin. In recent years,researchers start exploring pathways other than RAS signaling to explore the potential functions of neurofibromin. This article reviews the research progress on the pathogenesis of NF1 in recent years,aiming to provide new ideas for treatment.
10.Comparison of clinical characteristics and outcomes of infants with moderate and severe acute respiratory distress syndrome diagnosed according to baseline oxygenation index

Objective:To analyze the difference of clinical characteristics and outcomes of infants with moderate and severe pediatric acute respiratory distress syndrome(PARDS)diagnosed according to baseline oxygenation index(OI) in pediatric intensive care unit(PICU).Methods:Second analysis of the data collected from the "Efficacy of pulmonary surfactant (PS) in the treatment of children with moderate and severe ARDS" program.Retrospectively compare of the differences in clinical data such as general condition, underlying diseases, OI, mechanical ventilation, PS administration and outcomes among infants with moderate and severe PARDS divided by baseline OI who admitted to PICUs at 14 participating tertiary hospitals from 2016 to December 2021.Results:Among the 101 cases, 55 cases (54.5%) were moderate and 46 cases (45.5%) were severe PARDS.The proportion of male in the severe group (50.0% vs.72.7%, P=0.019) and the pediatric critical illness score(PCIS)[72 (68, 78) vs.76 (70, 80), P=0.019] were significantly lower than those in the moderate group, while there was no significant difference regarding age, body weight, etiology of PARDS and underlying diseases.The utilization rate of high-frequency ventilator in the severe group was significantly higher than that in the moderate group (34.8% vs.10.9%, P=0.004), but there was no significant difference in PS use, fluid load and pulmonary complications.The 24 h OI improvement (0.26±0.33 vs.0.04±0.34, P=0.001) and the 72 h OI improvement[0.34 (-0.04, 0.62) vs.0.15 (-0.14, 0.42), P=0.029)]in the severe group were significantly better than those in the moderate group, but there was no significant difference regarding mortality, length of hospital stay and intubation duration after diagnosis of PARDS between the two groups. Conclusion:In moderate and severe(divided by baseline OI) PARDS infants with invasive mechanical ventilation, children in severe group have better oxygenation improvement in the early stage after PARDS identified and are more likely to receive high frequency ventilation compared to those in moderate group.Baseline OI can not sensitively distinguish the outcomes and is not an ideal index for PARDS grading of this kind of patient.
