1.Nursing of patients with gynecologic tumor and diabetes mellitus during chemotherapy

Objective To discuss the effect nursing of patients with gynecologic tumor and diabetes mellitus during chemotherapy in order to find effective nursing measure. Methods A retrospective study was carried out in 100 patients with gynecologic tumor and diabetes mellitus during chemotherapy. The treatment process was summarized. Results No incidence of infection and complications of diabetes mellitus were seen in these patients. The adverse effect of chemotherapy was low. Conclusions Strengthening of psychological and diet nursing, prevention of infection and health education proved to be effective measures to improve the life quality and prevent anxiety of patients with gynecologic tumor and diabetes mellitus during chemotherapy.

Postsurgical gastroparesis syndrome (PGS) is a functional disease,characterized by nauseating,vomiting,and gastric atony without of mechanical gastric outlet obstruction,and is ofen caused by operation at the abdomen,especially gastric and pancreatic operation.In recent years,the incidence rate of PGS presents ascendant tendency,and more and more doctor attach importance to PGS.A better knowledge of the etiology,pathogenesis,clinical manifestation,diagnosis and treatment of PGS can help decrease the incidence rate of PGS and improve the cure rate of PGS.Thus,the authors write this review about these aspects of PGS.
3.Approach on clinical application of visual artificial simulation splenectomy

Objective To study the three-dimensional (3D) reconstruction of pancreas based on the data of 64-row helical CT scanning and the effect of the dynamic simulation surgery of spleen.Methods The original scan data of spleen 64-slice helical CT were collected and the image program segmentation and automatic extraction were done with images of CT series by adaptive region growing algorithm.Then the image processing software was used to proceed with the 3D reorganization with the images which are segmented.Import reconstructed objectives to FreeForm Modeling System in the form of STL format were embellished and smoothed.GHOST SDK software of FreeForm Modeling System was utilized to develop various instruments needed by simulated surgeries.And the visualization of simulated surgery by PHANTOM software was studied before the splenectomy.Results By the adaptive region growing algorithm, the spleen image procedure could be calculated soon and efficiently, and the satisfactory spleen segmentation data were obtained.The clear picture structure of 3D- reorganization was a exact reproduction of spleen and the structures of vital organs and passages around;The simulated splenectomy was high simulated, vivid, exact and the operator could feel like a real surgery.Conclusions The research of 3D reorganization and visual artificial simulation surgery to spleen with the medical image process software and the FreeForm Modeling System can make surgeons know the 3D anatomical structure around spleen clearly before the surgery.And it is of great help to reduce the risk of surgery, decrease operative complications, and improve the effect of surgery.
4.Influence of tissue culture on the survival of thyroid allografts in rats

The thyroid allografts in rats were cultured for 48h in the conditions of hyperbaric oxygen (a mixture of 95% O2,5% CO2 pressurized at 2 atmospheres)and low pH(5.5 and 6.9).Uncultured and air cultured allografts were taken as control groups.The thyroid slices were transplanted under the kidney eapsulse of thyroidectomized recipients.By examining histological changes,detecting the serum T3 and T4 levels and five weeks body weight gain,the graft survival was cvaluated.Though the thyroids cultured in hyperbaric O2 were contaminated with the passenger leukocytes,the grafts had prolonged survival,suggesting this method might effectively diminish MHC immunogenicity to thyroid grafts.
5.Effect of basic fibroblast growth factor on proliferation and differentiation of monkey bone marrow mesenchymal stem cells into neuronal precursor cells :Does the concentration affect cryptotanshinone induction?

BACKGROUND:Basic fibroblast growth factor(bFGF)belongs to active peptide,which is an effective mitogenic factor.OBJECTIVE:To investigate the effect of bFGF on proliferation and differentiation of monkey bone marrow mesenchymal stem cells(BMMSCs)into neuronal precursor cells.METHODS:Monkey BMMSCs were in vitro cultured by density gradient centrifugation,and then divided into 4 groups after passaged,namely,control,bFGF with low,medium and high concentration groups.In the bFGF groups,0,3,6,10 μ/L bFGFwere applied.The proliferation of BMMSCs in each group were observed.The 5th BMMSCs were cultured with serum free L-DMEM culture medium containing 20 mg/L cryptotanshinone to differentiated into neuraMike cells.The expression of positive-nestin protein was detected by immunohistochemical method.RESULTS AND CONCLUSION:Compared with the control group,proliferation rate of BMMSCs in the bFGF groups were accelerated(P < 0.05),which showed a positive correlation to the concentration of bFGF.The positive-nestin protein could be found in the low and medium concentration groups at 0.5 hours after induction,and reached a peak at 1.5 hours,which increased obviously in the low concentration group than that of the high concentration group(P < 0.05).bFGF can promote BMMSCs proliferation in vitro,enhance inducing ratio of prophase neuron-like cells at lower concentration but inhibit differentiation at high level.
6.Observation of dendritic cell and HLA-DR antigen expression in autoimmune thyroid diseases and subacute thyroiditis

Objective To investigate the effects of dendritic cell (DC) and HLA-DR antigen expression in the generation and development of Graves′ disease (GD), Hashimoto′s thyroiditis (HT) and subacute thyroiditis (SAT). Methods A morphologic study was performed on the thyroid tissue of 53 GD with pronounced lymphocytic infiltration, 52 HT and 31 SAT to investigate the pathologic changes of DC and HLA-DR antigen positive cell in the parenchymal cells and intestitial tissue by histochemical, immunohistochemical and electron microscopic technique and statistic analysis. Results The observation on DC and HLA-DR positive infiltrating cell in GD, HT and SAT showed a similar increase with the degree of cellular infiltation. The highest peaks of DC and HLA-DR positive thyroid follicular epithelium were in HT O-type and granulomatous area of SAT. The highest peaks of HLA-DR positive immunoactive cells was in HT P-type and fibrous area of SAT. Conclusion DC and HLA-DR positive cells may play an important role in the antigen-presenting step as well as in their direct cytotoxicity. These processes are related to the pathogenesis of AITD (GD and HT) and SAT. The thyroid follicules may be destroyed in the autoimmune reaction, and initially the patient presents hyperthyroidism and finally hypothyroidism follows because of the fibnosis of the follicules.
7.The influence of cognitive therapy on life quality of rural breast cancer patients during chemotherapy

Objective This study is to discuss the effects of cognitive therapy on life quality of rural breast cancer patients during chemotherapy in order to offer evidence for choosing the appropriate therapeutic measures.Methods 120 rural breast cancer patients were equally divided into the intervention group and the control group at random.The usual care plus cognitive treatment was given to the patients in the intervention group for 6 months,while only the usual care after chemotherapy was given to the control group.The quality of life assessments were carried out to both groups on the 5th day during hospitalization,and the first,third and 6th month during recheck period of time,respectively.The treatment effect was observed.Results There were statistical significant difference of the total scire of life quality assessment on the 5th day,the first,third and 6th month,respectively,between the intervention group and the control one after cognitive intervention.The downtrend of total score of quality of life in the intervention group after cognitive intervention was more significant than the control group,which indicated the quality of life of the intervention group was better than the control group. Conclusions Cognitive therapy is an efficient method to enhance the quality of life of rural breast cancer patients during chemotherapy.
8.The G4-quadruplex mutation of the core gene enhances the immunogenicity of hepatitis C virus DNA vaccine

Objective To investigate the effect of G-quadruplex (G4) RNA structure of core of hepatitis C virus (HCV) on the specific immune response. Methods Circular dichroism (CD) was usedto detect the G4 spatial structure of the G4 oligonucleotide chain RNA (named as G4R) and its mutant of G4 (named as G4RM) by G base site-specific mutation.The HCV wild-type core gene G4(DNA) sequence was mutated as G4M-core by PCR site-directed mutagenesis without changing the amino acid codon.Then wild type and mutated core genes were constructed into the eukaryotic expression plasmid pcDNA3.1-Myc, and produced as pcDNA3.1-core-G4-WT (named as pG4) and pcDNA3.1-core-G4-M (named as pG4M), the expression of core protein was examined by Western blot. The mice were immunized with the pG4 and pG4M plasmids DNA respectively, and their humoral and cellular responses were examined. Results CD results showed that the structure of G4RM was changed compared to Wild type G4R, and the melting curve analysis showed the melting temperature of GR4M was lower than that of G4R, which indicates that G4RM structure is unstable. Western blot analysis showed that pG4M had much higher protein expression level compared to pG4(P<0.05). Analysis of animal immunization showed that pG4M induced increased levels of total IgG and IFN-γ compared to pG4(P<0.05). The IgG level of the pG4M group was 1.61 times higher than that of the pG4 group. By enzyme-linked immunospot(ELISpot)assay, we found that the release IFN-γ level of pG4M was 1.39 times higher than those of pG4. Flow cytometry showed that the intracellular IFN-γ production in the splenic CD4+ T cells was 1.79 times than those of pG4. Conclusion The G-quadruplex structure of HCV core can inhibit its protein translation. The mutation of G-quadruplex of core led to increased Th1-type immune responses. This is the first report demonstrate that HCV core G-quadruplex mutation can enhance its immunogenicity and could be used as a new strategy ofexploring HCV vaccine with enhanced immunogenicity.
9.Clinical Analysis and Material Selection of Tension-free Hernia Repair in 130 Cases

Objective To discuss the operation style of filling,film-style,three-dimensional double-layer mesh tension-free for hernia and the repair materials,the selectlon of surgical operation style for reducing recurrence.Methods We used the style of plug-and-chip,two-dimensional mesh tension-free hernia repair for the treatment of 130 cases of inguinal hernia patients.Results In 130 cases of patients with 1~5 years of follow-up,there was no recurrence.Conclusion A reasonable choice of repair materials and improving the surgical operation can be effective in reducing recurrence.
10.Air Circumstance in Laminar Flow Operating Room in Beijing:A Crosssection Investigation of 18 Hospitals

OBJECTIVE To investigate the management and sanitation status of the air circumstance in laminar flow operating rooms of grade two and above hospitals in Beijing.METHODS Air surveillance methods,cleaning methods of intakes,cleaning status of pipelines and frequency of renewing filter systems were investigated through interviewing charge nurse of 18 hospitals by questionnaires.RESULTS 50.0% of pipelines had never been cleaned,14.3% of intakes had never been cleaned.Air surveillance method of all hospitals was using sedimentation,with a low sensitivity.The monitoring systems were poor in 50.0% of hospitals.CONCLUSIONS Now there is only requirements of construction standard of clean surgery region,lacking routine regulations and technologies of contamination controlling standard,nor available dynamic contamination controlling system.Potential hazard exists in routine regulations of laminar flow operating room and it demands for making contamination controlling standard urgently in hospitals.
