1.Experimental study on treatment of acute gout arthritis with periploca forrestii schltr

Objective:To investigate the effects of periploca forrestii schltr in the treatment of acute gout arthritis.Methods:60 healthy male SD rats were equally randomly divided into 6 groups:normal control group( NC) ,model group( M group) ,colchicine group (C group),high doses group of periploca forrestii schltr(HD group),middle doses group of periploca forrestii schltr(MD group) and low doses group of periploca forrestii schltr( LD group).Except the normal control group,model of gouty arthritis was induced in other groups by uric acid salt,colchicine(positive control) and different dose of periploca forrestii schltr were given by intragastric ad minis-tration.Swelling dimension of joints were observed at 3,5,7 days after treatment.All rats were killed after 7 days of treatment and ankle joint tissue was taken for pathological examination and the peripheral blood of rats was prepared for detecting the expression of interleukin 1β(IL-1β),IL-6,IL-8 and tumor necrosis factor(TNF-α) using enzyme linked immunosorbent test(ELISA).Results:The ankle joint swelling of periploca forrestii schltr group was significantly lower than that in the model group,and the effect of high doses group was much better than the low doses group after 7 days treatment(P<0.05);compared with model group,the inflammatory cells of each treatment groups were decreased and high doses group did not differ from that of normal control group;the levels of IL-1β,IL-6,IL-8 and TNF-αin periploca forrestii schltr group were dramatically lower than those in the model group in a dose-dependent manner.Conclusion:Periploca forrestii schltr has good therapeutic effect in rats with acute gouty arthritis and shows a dose-dependent response,and the mechanism may relate to the inhibition of inflammatory cytokines expression.
2.Role of exosomes in hepatocellular carcinoma

Exosomes are vesicles with a diameter of about 50-150 nm released by a variety of cells and contain miRNA, mRNA, and protein, with the potential to regulate the signal transduction pathways in receptor cells. Exosomes provide nucleic acids and proteins to coordinate cell-cell communication and microenvironment regulation. This article summarizes the latest research advances in the role of exosomes in hepatocellular carcinoma (HCC) and analyzes the effect of exosomes on the development and progression of HCC, including chemotherapy resistance, epithelial-mesenchymal transition, angiogenesis, metastasis, and immune response. The analysis shows that exosomes have a promising future in the treatment of HCC.
3.Immunomodulatory Effects of Tetrastigma Hypoglaucum Planch on Rats with Rheumatoid Arthritis

Objective To investigate the immunomodulatory effects of Tetrastigma hypoglaucum Planch in the treatment of rheumatoid arthritis. Methods Sixty SD rats were ramdomly divided into normal control group, model control group,low-,middle-and high-dose Tetrastigma hypoglaucum Planch groups (50,100 and 200 mg.kg-1,respectively),and tripterygium glycosides tablet group. Except for normal control group,rheumatoid arthritis model was established by using bovine typeⅡcollagen in SD rats of the other groups. Organ index,plasma levels of IL-1β,IL-6,TNF-αand anti-CⅡin rat rheumatoid arthritis models were evaluated. Results The kidney index of the low-dose/middle-dose Tetrastigma hypoglaucum Planch groups were significantly different from that of the model control group (P<0.05). Significant difference was also found in the spleen index between the high-dose Tetrastigma hypoglaucum Planch group and the normal control group ( P<0.05) . The plasma levels of IL-1β,IL-6,TNF-αand anti-CⅡantibody were significantly higher in the model control group than in the normal control group. Meanwhile, the plasma IL-1β, IL-6 and TNF-α levels of the middle-dose/high-dose Tetrastigma hypoglaucum Planch groups were significantly lower than those of the model group ( P<0.01) . The levels of anti-CII antibody were significantly lower in the low-,middle-and high-dose Tetrastigma hypoglaucum Planch groups than in the model control group. Conclusion It is worthwhile to explore and develop Tetrastigma hypoglaucum Planch since it possesses immunomodulatory effects and may be applied in the treatment of rheumatoid arthritis.
4.Evaluation on effect of novitiate of medical students in center of disease control and prevention

Objective To evaluate on novitiate of medical students in center of disease control and prevention(CDC).Methods Junior medical students of Grade 2007 were selected randomly by Cluster sampling method and classified into teaching reform group(novitiate in CDC,n=79) and control group.Traditional lecture and discussion were used in the laboratory of the school to 98 students of the control group,while 79 students of the teaching reform group became CDC trainees.Questionnaire and qualitative interviews were used to evaluate the trainees' preventive medicine cognition,their interest and employment intention.Questionnaire data was proofread and entered in EPIdata (version 3.1),and was analyzed by using Statistical Package for Social Sciences(SPSS version 17.0),and ratio or rate between groups were compared by x2 test,and scores of two groups were compared by using Wilcoxon test.Results In teaching reform group,rate of H1N1 correctly classified as category B was 76.0%,score of knowledge of Programme on Immunization,knowledge of H 1N 1 reporting procedures,knowledge of DOTs strategy,knowledge of chronic non-communicable disease management,and self-interest in preventive medicine was 4.0,4.0,3.0,5.0 and 4.0(points) respectively,and the proportion of regarding CDC as an employment options was 73.3%,respectively higher than 40.4%,2.9,2.9,2.0,4.0,3.8 points and 28.7% in control group(all P<0.05).Conclusion Novitiate in CDC is helpful to increasing students' interest in learning preventive medicine,learning knowledge of preventive medicine,and changing employment concept.
5.Urgent need for contraceptive education and services in Chinese unmarried undergraduates: A multi-campus survey.

In order to ascertain prevalence rate of premarital sexual intercourse, unintended pregnancy and abortion, and evaluate associated factors of unintended pregnancy among undergraduates from all over China, the representative sample of unmarried undergraduates was obtained by using a multi-stage, stratified, probability cluster design, and data were collected by using a survey questionnaire. 62 326 available responders were gained. 11.6% of them acknowledged having experiences of premarital sexual intercourse (standardized prevalence rate of sexual intercourse was 13.8%). 31.5% of students active in premarital sex acknowledged undergoing unintended pregnancy. 76.2% of pregnant students selected abortion to end it. Of students active in premarital sex, 46.2% used contraception at the first sexual intercourse, 28.2% replied "always" using contraception in sexual intercourse. The rate of using condoms, oral contraceptives (OCs), and withdrawal among students who had used contraception was 52.0%, 31.0%, and 27.2% respectively. "No preparation for sex" (40.3%), "pleasure decrement" (32.1%), "won't-be-pregnancy in occasional sexual intercourse" (30.2%) were their common excuses for using no contraception. The identified risk factors for unintended pregnancy among students active in premarital sex by multivariate analysis were as follows: having no steady lover [having no steady lover vs having a steady lover: odds ratio (OR), 1.875; 95% confidence interval (CI), 1.629-2.158], unaware of the course of conception (unaware vs aware: OR, 2.023; 95% CI, 1.811-2.260), considering abortion not endanger women's physical and mental health (no endangerment vs endangerment: OR, 2.659; 95% CI, 2.265-3.121), nonuse of contraception (never use vs always use: OR, 1.682; 95% CI, 1.295-2.185). Medical students were not less likely to experience an unintended pregnancy than nonmedical students (OR, 1.111; 95% CI, 0.906-1.287). The substantial proportion of unintended pregnancy among undergraduates indicates a need for convenient and targeted contraceptive education and services.
6.Clinical analysis on 64 cases of testicular torsion and literature review

Objective To investigate the clinical characteristics of testicular torsion,and to provide the strategies for its early diagnosis and timely therapy.Methods The clinical data in the patients with testicular torsion diagnosed by surgical exploration from March 2011 to December 2016 were reviewed.The clinical characteristics were compared between the testis-preserving group and testis-resection group.The latest advances in the diagnosis and treatment of testicular torsion were summarized.Results A total of 64 cases of testicular torsion were included in this study.There were 27 cases in the testicle-preserving group with the age range of(8-40) years old,the average age was(19.00±5.28) years old,the onset time range was(1-24) h,the average time was (8.78 ± 6.73) h;the intraoperative sper matic cord twist range was (180-540) ° and average (290.00 ±103.92) °;6 cases were misdiagnosis with the misdiagnosis rate of 22.22 %.There were 37cases in the testicle-resection group with the age range of(10-34),the average age was(19.00 ± 7.45) years old;the onset time range was (12-168) h,the average time was(66.92 ± 47.01) h,the intraoperative spermatic cord twist range was(180-720)° and average(457.00± 168.88)°;22 cases were misdiagnosed with the misdiagnosis rate of 59.46 %.The onset time,spermatic cord twist degree and misdiagnosis rate had statistically significant difference between the two groups(P<0.05).Conclusion Testicular torsion is an important urinary emergency condition which can not be ignored.Timely and effective treatment is the key to save the testicles.
7.Relationship between obesity and serum testosterone level in 1 166 adult males

Objective To explore the relationship between obesity and serum testosterone level among community adult males.Methods The stratified cluster sampling method was adopted to extract 1 166 male residents aged 20 years old and over in Zunyi City and conduct the questionnaire survey and physical examination.Fasting venous blood samples were collected for detecting the serum total testosterone(TT),sex hormone binding globulin(SHBG) and luteinizing hormone(LH).Then free testosterone index(cFT),free testosterone index(FTI) and testosterone secreting index(TSI) were calculated.Results The average levels of serum TT,SHBG,cFT,FTI,TSI and LH in 1 166 adult males were(16.83±4.90)nmol/L,(42.15 ± 20.21)nmol/L,(0.31 ±0.10)nmol/L,(0.46 ± 0.19) IU/L,(3.40 ± 2.04) IU/L and(6.77 ± 5.14) IU/L respectively;the Pearson correlation analysis showed that WHtR,BMI and WC were negatively correlated with TT,SHBG,TSI and LH,and were positively correlated with FTI,WHtR was negatively correlated with cFT,BMI and WC were negatively correlated with LH,the differences were statistically significant(P<0.05).The multivariate linear regression analysis shown that WHtR was negatively correlated with TT,SHBG,TSI and LH;and was positively correlated with FTI,the differences were statistically significant(P<0.01).Conclusion Adult male central obesity may be associated with the decrease of serum TT,SHBG,TSI and LH levels,and increase of FTI level.

9.A prospective birth cohort study on the association between gestational blood pressure and neurodevelopment in 2-year-old children

Objective:To investigate the association between gestational blood pressure and neurodevelopment in 2-year-old children.Methods:Based on the"Wuhan Healthy Baby Birth Cohort", 3 754 mother-infant pairs were enrolled in this study. Based on multiple blood pressure measurements during pregnancy, the mean, cumulative, and variability of blood pressure throughout the entire pregnancy and each trimester were calculated. Blood pressure variability was evaluated using standard deviation (SD), coefficient of variability (CV), and variability independent of mean (VIM). Follow-up testing of neurodevelopment in infants and young children at the age of two was conducted to obtain the Mental Development Index (MDI) and the Psychomotor Development Index (PDI). The multivariate linear regression and generalized estimation equation were used to analyze the association between gestational blood pressure data and neurodevelopmental index.Results:The age of 3 754 pregnant women was (29.1±3.6) years, with a pre-pregnancy BMI of (20.9±2.7) kg/m2 and a gestational age of (39.3±1.2) weeks. The birth weight of 3 754 children was (3 330.9±397.7) grams, and the birth length was (50.3±1.6) centimeters. The results of the multivariate linear regression analysis showed that after adjusting for relevant confounding factors, the mean blood pressure, cumulative blood pressure, standard deviation of blood pressure, coefficient of variation of blood pressure, independent blood pressure variability of systolic blood pressure, diastolic blood pressure, and pulse pressure throughout pregnancy were negatively associated with the MDI and PDI scores of 2-year-old children. The analysis results of the generalized estimation equation showed that after adjusting for relevant confounding factors, the average systolic blood pressure in the first, second, and third trimesters was negatively associated with MDI/PDI. The negative association between cumulative blood pressure and MDI/PDI was only found in the first trimester. The negative association between blood pressure variation during pregnancy and MDI/PDI was mainly concentrated in the second and third trimesters.Conclusion:There is a negative association between gestational blood pressure and the neurodevelopmental index of 2-year-old children.
