Objective To explore the value of Ligasure and Ultracision in total laparoscopic hysterectomy (TLH). Methods From January 2002 to December 2006,totally 225 patients received total laparoscopic hysterectomy in our hospital. Among the cases,Ligasure was used in 152 patients, and Ultracision were employed in 72. The clinical data of the patients were reviewed. Results The mean operation time in the Ligasure group was significantly shorter than that in the Ultracision group [(104.9?29.2) min vs. (152.4?46.6) min,t=-9.329,P=0.000]. And the mean blood loss of the Ligasure group was significantly less than that in the other group [(32.9?14.9) ml vs. (126.4?12.1) ml,t=-46.710,P=0.000]. In both the groups,no patient was converted to open surgery or developed complications or surgical injuries. Conclusions Both Ligasure and Ultracision are safe and effective for total laparoscopic hysterectomy. Ligasure is superior in the management of uterine vessels, while the Ultracision is more effective in opening the vesicle and separating the cardinal ligament.

Objective To analyze the airflow characteristics and investigate the relationship of the structure and the function of nasal cavity by the three?dimensional reconstruction of nasal airway of patients with structural abnormalities(nasal septum deviation)and healthy people and the estab?lishment of finite element model by computer. Methods On the basis of CT imaging of the nasal cavity in patients with structural abnormalities(na?sal septum deviation,n=20)and healthy people(n=20),three?dimensional reconstruction of nasal airway was conducted by resurfacing finite ele?ment subdivision to simulate the characteristics of airflow in nasal cavity. Results The airflow mainly went through the commodious side of the nose in patients with nasal septum deviation and the maximum fluence appeared in the middle part of meatus nasi communis. The airway pressure de?creased most significantly in the most flank?curvature part of nasal septum deviation, accounting approximately 79.65%of the total pressure. In healthy people, the bilateral airflow was affected by nasal cycle and was mainly characterized by one nasal cavity, and the maximum fluence was ob?served in the middle and the inferior part of meatus nasi communis. The airway pressure decreased most significantly in limen nasi, accounting ap?proximately 58.78%of the total pressure. Conclusion Numerical modeling of nasal cavity can be used to analyze the relationship between the nasal structural abnormalities and the airflow characteristics, which is a scientific method to analyze the association of nasal structure and function with dis?ease and can be used for pre?and post?operative individual evaluation of operative therapeutic regimen targeting at optimizing airway and altering air?flow distribution.

Objective:To compare the time domain and the frequency domain of speech-evoked auditory brainstem response measured by stimulation of left and right ears, and to explore the difference and possible reasons of neural coding for speech from different ears in auditory brainstem.Method:Speech-ABRs to syllable /da/ of 31 healthy adults were recorded. Statistical analysis was performed on time-domain parameters, such as latencies and amplitudes of featured peaks, and frequency-domain ones, such as amplitudes of the fundamental frequency and the first formant of speech-ABRs ranging from 20-50 ms. A scoring criterion to grade the appearance of featured waves was proposed for waveform evaluation.Result:There were no significant difference for the latencies of binaural featured peaks and amplitudes of feature peaks(except peaks A and O). The waveform scores of right ear were greater than that of left ear. The amplitudes of fundamental frequency of binaural waves were both greater than that of the first formant. There was no significant difference of amplitudes of fundamental frequency and the first formant between two ears. Conclusion:The origins and distributions of speech-ABR are essentially symmetrical in brainstem in contrast with the hemisphere asymmetry of speech.
