1.Efficacy comparison of lamivudine and telbivudine combined with hepatitis B vaccine in blocking vertical transmission of hepatitis B virus

Objective To compare the efficacy of lamivudine and telbivudine combined with hepatitis B vaccine in blocking vertical transmission of hepatitis B virus (HBV) in pregnant women with HBV infection during late stage of pregnancy.Methods From January 2010 to December 2013,195 pregnant women with HBV infection who received prenatal examination and institutional delivery were enrolled and randomized into three groups based on random number table method,lamivudine group,telbivudine group and control group,65 cases in each group.The subjects in lamivudine group and telbivudine group orally took lamivudine 100 mg/d and telbivudine 600 mg/d respectively,from the 28th week of gestation to 1 month after delivery.The primary end point was HBsAg positivity of infants at 12 months of age.Results Serum lgHBV DNA level of mothers in lamivudine group was 3.7 ± 1.2,and in telbivudine group was 3.6 ± 1.5,in control group was 8.4 ± 1.7.The serum lgHBV DNA level of mothers in lamivudine group and telbivudine group was significantly lower than that in control group (P < 0.01).HBsAg positivity rate of infants at 12 months of age in lamivudine group was 3.1%(2/65),in telbivudine group was 4.6%(3/65),and in control group was 15.4% (10/65).HBsAg positivity rate of infants at 12 months of age in lamivudine group and telbivudine group was significantly lower than that in control group (P < 0.05).Conclusion Lamivudine and telbivudine are both effective in preventing vertical transmission of HBV for pregnant women with HBV infection in late stage of pregnancy,and further clinical study is required.
3.Introduction and evaluation of DSM-5 cross-cutting symptom measures

The DSM-5 cross-cutting symptom measures were developed by the DSM-5 Task Force and Work Groups to serve as a review of mental systems in each patient who presents for mental health evaluation and treatment.The cross-cutting symptom measures have two levels.Level 1 questions are a brief survey for adult patients and for child and adolescent patients.Level 2 questions provide a more in depth assessment of certain domains.The comprehensive symptoms could be evaluated through the objective assessment other than symptoms fit nearly into the diagnostic criteria.The following are briefly introduced.
4.Content Determination of Polysaccharide in Pinellia Tuber

Objective To determine the content of polysaccharide of prepared pinellia tuber. Methods The content was determined by sulfuric acid-anthrone colorimetric method with wavelength at (624?1)nm. Results The content of polysaccharide was 2.533 mg/g in prepared pinellia tuber. The average recovery was 99.54% and RSD was 1.69%. Conclusion The methods was simple, steady and reproducible.
5.Endoscopic removal of a dental foreign body from maxillary sinus via anterior prelacrimal recess approach: a case report.

We present a rare case of dental foreign body from maxillary sinus in a 21-year woman who was hospitalized because of oral cavity and nasal sinus leak for 3 months when doing cheek-bulging action. Admission diagnosis :dental maxillary sinus"foreign body" (left); chronic maxillary sinusitis (left). Computed tomographic scan showed irregular high density shadow in the left maxillary sinus. The "foreign body" was removed via anteri- or prelacrimal recess approach, which was supposed to be the iatrogenic foreign body - alveolar bone.

6.Uncoupling protein 4 and its roles in the central nervous system diseases

Uncoupling protein 4 (UCP4) is a member of the multigenic uncoupling proteins (UCPs), specific expressing in the cerebral cortex and hippocampus. UCP4 plays an important role in Parkinson's disease, multiple sclerosis, epilepsy, stroke, brain trauma and other central nervous system diseases by uncoupling, decreasing mitochondrial membrane potential,regulating Ca2+ homeostasis and oxidative stress. This article reviews UCP4 and its roles in the central nervous system diseases in order to provide certain basis for the development of UCP4targeted medication.
7.Associations between brain-derived neurotrophic factor gene polymorphisms and cognitive disorder in depression

Objective To explore the relationship between polymorphisms of brain-derived neurotrophic factor(BDNF) gene (rs6265 and rs12273539) and cognitive impairment in depressive disorder.Methods All participants including 73 depressed patients and 71 healthy controls were received clinical and cognitive assessments at admission,and then the depression group was divided into two groups by the score of Beijing version of the Montreal Cognitive Assessment (MoCA-BJ).One was depression with cognitive dysfunction group,and the other was depression without cognitive dysfunction group,with 36 and 37 cases respectively.The polymorphisms of BDNF gene was identified by PCR-RFLP.Results No significant difference for rs6265 gene types(x2=5.18,P=0.27),A allele carries (x2 =4.28,P=0.12) and G allele carries (x2 =1.95,P=0.38) among the three groups.There was no significant difference for rs12273539 gene types,allele carries between patients without cognitive dysfunction and controls groups(P>0.05).There was much more C-allele carries (x2=5.40,P=0.02)and less T-allele(x2=6.06,P=0.01) in patients with cognitive disorder than those in health and it was different in rs12273539 gene types between the two groups(x2=8.38,P=0.02).CC/CT/CT gene type performed different on attention function (P<0.01).Conclusion BDNF rsl2273539(T/C) gene type has relationship with the onset of cognitive disorder in depressed patients,and there are more C-allele carries in depressive patients.The depression patients with CC gene type are worse on the attention function impairement.
8.The correlation of cognitive dysfunction with serum brain-derived neurotrophic factor level in depression patients

Objective To investigate the characteristics of cognitive dysfunction in patients with depression,and identify the correlation between cognitive dysfunction and serum brain-derived neurotrophic factor (BDNF) level.Methods All participants including 73 depressed patients and 71 healthy controls were received clinical and cognitive assessments at admission,the depression group was divided into two groups by the score of Beijing version of the Montreal Cognitive Assessment (MoCA-BJ),one was depression with cognitive dysfunction group which had 36 cases,the other was depression without cognitive dysfunction group which had 37 cases.Concentration of BDNF was measured by the ELISA method.Results Cognitive impairments were found in numerous cognitive domains of depressed patients,including visuospatial and executive abilities,attention,delayed recall and orientation(P < 0.05).The incidence of cognitive dysfunction in depression was 49.3%.There was not significant difference between two depressive groups (depression with cognitive disorder(12.08 ± 7.08)ng/ml,depression without cognitive disorder (12.22 ± 7.93)ng/ml,P > 0.05),and the levels were significantly lower than that in healthy people ((16.55 ± 7.47) ng/ml,P< 0.01),and serum BDNF level had no positively relevant with different cognitive functions (P > 0.05).Conclusion Depression patients have cognitive dysfunction in numerous cognitive domains,including visuospatial and executive abilities,attention,delayed recall and orientation.Serum BDNF level is closely related with depression,while,it has no obvious relationship with cognition function in depression.
9.Ischemia-reperfusion injury induces autophagy inhibition in aged rats

Objective To explore the mechanism of hepatic autophagy inhibition induced by ischemia-reperfusion injury in the aging liver.Methods The healthy male Lewis rats aged 3 months (3M) and 24 months (24M) were selected,and then were randomly divided into 3M IRI group,3M sham operation group,24M IRI group,24M sham operation group.In the experimental group,noninvasive vascular clamp was used to clamp the left and middle hepatic lobes (about 70o% hepatic ischemia).The liver was subjected to ischemia at 37 0.5℃ for 30min and reperfusion for 6h.The hepatic duodenal ligament was dissected only by sham operation.The serum levels of serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were measured at 6 h after operation.Liver tissues of each group were examined by liver pathology and the number of autophagosome of LC3B in liver tissue of each group was observed under confocal microscopy.The changes of autophagyrelated protein were analyzed by Western blotting.Results The levels of serum ALT and AST in 24M IRI group were significantly higher than those in 3M IRI group,the difference was statistically significant (P<0.05).Pathological analysis showed that 3M IRI group showed spotty necrosis,the 24M IRI group showed massive necrosis and the infiltration of the inflammatory cells;Confocal microscopy showed that the number of autophagosome in the liver tissue of the 24M sham group was slightly lower than that of the 3M sham operation group and the number of autophagosome in the 24M IRI group was significantly lower than that in the 3M IRI group (P<0.05).The levels of autophagyrelated proteins (Beclin1 and ATG4B protein) in 24M IRI group were significantly down-regulated compare to 3M IRI group (P<0.05).Conclusion The ischemia-reperfusion injury of liver in aged rats inhibits autophagy,and its mechanism may be related to the decrease of autophagy-related protein level in hepatic ischemia-reperfusion injury.
10.The diagnosis and treatment primary malignant tumor of the duodenum:a report of 54 cases

Objective To investigate improvements in the diagnosis and treatment of primary malignant tumor of the duodenum. Methods The clinical data of 54 patients with primary malignant tumor of the duodenum treated during a period of 6 years were analysed retrospectively. Results Tumors located in the first, second, third and fourth parts accounted for 6 cases(11.1%), 44 cases(81.5%), 2 cases(3.7%), and 2 cases(3.7%) respectively; and among them, ampullary tumor accounted for 38 cases(86.4%) of tumor of descending portion of duodenum and 70.4% of the total number of duodenal tumors. The main clinical presentation included jaundice, upper abdominal pain, weight loss, abdominal distention, and gastrointestinal obstruction. Upper abdominal pain, no positive signs and abdominal mass were found in 72.0%, 20.0% and 8.0% respectively on abdominal examination. Preoperative associated cholecystopathy accounted for 37.0%. The accuracy rate of duodenoscopy and ERCP in preoperative diagnosis was 94.4%, and 77.8% respectively. Preoperative diagnostic accuracy rate was 27.8%. Panceaticoduodenectomy was performed in 38 cases, duodenectonmy in 1 case, palliative resection of tumor in 9 cases, and tumor was inoperable in 6 cases. Radical resection rate was 72.2%. The postoperative 3- and 5-year survival rate was 40.6% and 21.9%, respectively. After palliative resection, death occurred from 1month to 24months. Conclusions The tumors in the ampullary region account for the majority of primary malignant tumors of the duodenum and are mainly adenocarcinoma. The symptoms of advanced stage are complicated, and specific signs on abdominal examination are few. Associated cholecystopathy is relatively ferquent and preoperative accurate diagnostic rate is low. The examinations of first choice are duodenoscopy and ERCP, and radical panceaticoduodenectomy can extend survival time.
