1.Adverse reactions of lansoprazole combined with Flupentixol and melitracen in the treatment of functional dyspepsia patients

Objective To analyze the curative effect and adverse reaction of lansoprazole combined with droperidol thiadiazole melitracen treatment in patients with functional dyspepsia.Methods From December 2013 to December 2015 in our hospital and accepted treatment of 90 patients with functional digestive adverse as the research object,according to the random number table were divided into the control group and the observation group,45 cases in each group.The control group by oral administration of lansoprazole enteric coated capsules,the observation group taking oral lansoprazole and droperidol thiadiazole melitracen,course lasted for 4 weeks,efficacy and adverse occurrence was compared between two groups of patients.Results After 4 weeks of treatment,the observation group with symptoms score GSRS(postprandial fullness discomfort,early satiety,abdominal pain,epigastric burning sensation were lower than the control group with postprandial fullness discomfort early satiety,abdominal pain,epigastric burning sensation,and the difference was statistically significant(P<0.05); the patients in the observation group after 4 weeks of treatment,the total effective rate(86.7%)was higher than the control group(66.7%)significantly,and difference was statistically significant(P<0.05); symptoms of adverse reactions of two groups of patients in the process of drug use were mild and did not give special treatment,in a few days after the basic relief.Conclusion Lansoprazole combined with fluorine Pai Sai tons of Murray Song Xin can effectively eliminate or reduce the symptoms of functional dyspepsia patients clinical and no serious adverse reactions,it is safe in clinical application.
2.Mechanism of T helper cell for demyelinated optic neuritis

Background Optic neuritis is one of the common clinical neuro-ophthalmic diseases.Recurrent optic neuritis can cause irreversible axonal damage and visual impairment,and no prevention method is effective up to now.Its pathogenesis is considered to be associated with the body's immune imbalance.Objective This proposal tried to establish experimental autoimmune encephalomyelitis (EAE) and explore the immunomodulatory effects of T helper (Treg),Th17 cell in the model of optic neuritis.Make the immune mechanism of demyelinating optic neuritis clear.Methods EAE model was established to observe the clinical symptoms and visual electrophysiological changes of optical neuritis in mice;hematoxylin-eosin staining to observe the histological changes in the mouse models of EAE; immunohistochemical staining to detect optic nerve axon damage markers β-amyloid peptide precurso (β-APP) protein expression quantity changes;reverse transcription PCR (RT-PCR) to detect the dynamic expression of forkhead/winged helix transcription factor p3 (Foxp3),transforming growth factor-β (TGF-β),interleukin-1β (IL-1β),IL-17,IL-10 in optic nerve tissue.Results After immunization 11 days,neurological symptoms of EAE mice started to appear,hematoxylin-eosin staining showed optic nerve tissue infiltration of inflammatory cytokines; immunohistochemical staining showed enhancement of axonal injury markers β-APP positive staining; flash visual evoked potential(F-VEP) test showed 7,11,14,19,23,28 days,compared with the normal mice,P1 prepatents of model group were prolonged after modeling,the differences were statistically significant (t =4.487,15.203,16.364,11.540,11.959,16.163,all at P<0.05).When 7 days after modeling,the difference of N1-P1 amplitude between normal group and model group was no statistical significance (t =-0.992,P =0.378); while 11,14,19,23,28 days after modeling,compared with normal mice,N1-P1 amplitudes of model mice were significantly lower,the differences were statistically significant(all at P< 0.05).The expressions of TGF-β,IL-1β,IL-17,Foxp3,IL-10 mRNA among different time points had stastistie significant differences (F =12.721,15.015,14.343,69.374,68.290,all at P =0.000),compared with the normal group,when 11 days,14 days after modeling,TGF-β mRNA levels were significantly increased ; 19 days,23 days after modeling,IL-1β mRNA levels were significantly increased;7 days,11 days after modeling,IL-17 mRNA levels were significantly increased ;7,11,14,19,23,28 days after modeling,Foxp3 mRNA levels were significantly lower; 19,23,28 days after modeling,IL-10 mRNA levels were significantly lower,the difference were statistically significant (all at P< 0.05).Conclusions Th17 cells launch demyelinating optic neuritis immune injury,and Th1 subgroup performs the function of maintaining inflammatory lesions,Treg cell subgroup is abnormal much earlier than that of the Th2 subgroup.The imbalance of Th17/Treg cell may be involved in the pathogenesis of demyelinating optic neuritis.
4.Relationship between nocturnal hypoglycemia and cognitive function in aged type 2 diabetic patients

Two hundred and eleven aged patients with type 2 diabetes were enrolled in the study.Blood glucose levels were monitored with continuing glucose monitoring system (CGMS) for 3 days and the cognitive function was evaluated with Mini-Mental State Examination(MMSE) scale.The correlation between hypoglycemia(or nocturnal hypoglycemia) and MMSE scores was evaluated.CGMS recorded hypoglycemia in 47% patients (100/211),82% of whom (82/100) had nocturnal hypoglycemia.Patients with hypoglycemia had lower MMSE scores than those without hypoglycemia (25.1 ± 5.1 vs.26.8 ± 3.0,P < 0.05).Furthermore,the incidence of hypoglycemia(or nocturnal hypoglycemia) was higher in patients with MMSE≤24 than that in patients with MMSE > 24(P <0.05).The median of hypoglycemia maximal time and nocturnal hypoglycemia maximal time were longer in patients with MMSE ≤ 24 than those with MMSE > 24 (P < 0.05).Stepwise regression analysis showed that the total time and maximal time of hypoglycemia(or nocturnal hypoglycemia),age,and education level were the main factors influencing MMSE scores in aged type 2 diabetic patients(all P <0.05).
5.Application of amplitude integrated electroencephalography in the therapeutic hypothermia of hypoxic-ischemic encephalopathy

Hypoxic-ischemic encephalopathy (HIE) is the main reason of causing neonatal death and poor prognosis of the nervous system.The treatment of HIE has gone into the era of therapeutic hypothermia,the early,continuous,specification amplitude integrated electroencephalography(aEEG) monitoring for therapeutic hypothermia to screen appropriate cases,assess early the severity of HIE,provide early nerve protection treatment and evaluate the therapeutic efficiency,early intervention and improve the prognosis of the long-term HIE.This paper review the application of aEEG in the therapeutic hypothermia of HIE.
6.Literature analysis on drug-induced diseases of vital organs caused by traditional Chinese medicine injection

ObjectiveTo study the occurrence and influence factors of drug-induced injuries of vital organs caused by traditional Chinese medicine injection.MethodsChina Hospital Knowledge Database (CHKD) from 1993-2013 and case reports concerning adverse effects and drug-induced diseases caused by traditional Chinese medicine injection from Wanfang Data Knowledge Service Platformwere retrieved. Indicator systemwasestablished and relevant contentsweresummarized and analyzed.Results34 types of traditional Chinese medicine injectionswereincluded and 699 drug-induced diseasesweresummarized, taking up 53.98% of the total adverse effects. Among them, the top three included in the drug-induced diseaseswere acanthopanax injection, safflower injection and Mailuoning injection. The non-conformance between the traditional Chinese medicine injections in the literature and instructions mainly reflects unreasonable solvent selection, large compatibility concentration and usage and dosage beyond those specified in the instructions. ConclusionThe occurrence of drug-induced injuries of vital organs caused by traditional Chinese medicine injectionis relatively high, drug instructions shall be strictly followed in accordance with requirements during clinical application.
7.Application of virtual touch tissue quantification technique in staging of chronic kidney disease

Objective To investigate the application value of virtual touch tissue quantification (VTQ) technique in staging of chronic kidney disease (CKD). Methods VTQ examination was performed on a random sample of 96 CKD patients at different clinical stages and 45 healthy volunteers (control group). Shear wave velocity (SWV) of renal parenchyma was measured and compared. Correlation between SWV and clinical stage and glomeru-lar filtration rate (GFR) of CKD patients was analyzed. Results The SWV value of renal parenchyma decreased gradually, along with the progression of CKD disease and impairment of renal function. There was no significant difference(P>0.05)between the control group and the CKD1 group ( P=0.061), the CKD2 group and the CKD3 group (P = 0.238), and the CKD4 group and the CKD5 group (P = 0.624). The difference of SWV between the remaining groups was statistically significant(P < 0.05). SWV of renal parenchyma was negatively correlated with CKD stage (r =-0.65, P < 0.001), which means the higher the stage of CKD was associated with the lower the SWV. SWV was positively correlated with GFR (r = 0.73, P < 0.001), which means the lower the GFR was related to the lower the SWV. Conclusion VTQ technique can noninvasively reflect the changes of renal parenchyma elas-ticity and evaluate the impairment of renal function in CKD patients. It can provide a new quantitative evaluation index for the clinical staging of CKD.
8.Development and application of holding mechanism for drainage bag

Objective To develop a holding mechanism for the drainage bag to reduce the accident rate of tube falling off during CT examination.Methods The mechanism was composed of a bed body,a mobile bed board,a connecting board,a rod and kink section.CT bed board had a sandwich structure with carbon fibre composite and foam material,and the connecting board,kink section,rod and its hinge joint were made of polycarbonate material.Results The mechanism decreased the risk for tube falling off during CT examination and increased the patient comfort.Conclusion The mechanism gains advantages in stability,mobility,convenience and practicability,and thus is worthy promoting practically.
9.Clinical observation of mosapride combined with acupoint massage in treating senile type 2 diabetic gastroparesis

Objective To investigate the clinical effect of mosapride combined acupoint massage on diabetic gastroparesis(DGP)in elderly patients with type 2 diabetes.Methods 150 cases of DGP in our hospital from April 2015 to May 2016 were collected and randomly divided into three groups:the conventional treatment group(n=35,in insulin treatment),the control group(n=52,in mosapride treatment),and the experimental group(n=63,in mosapride combined with acupoint massage treatment).All patients received the conventional diabetic therapy.Clinical efficacy,gastric emptying rate and the levels of serum gastrin and fasting blood glucose were observed and compared before versus after treatment among the three groups.Results After treatment,the total effective rate was 92.1% in the experimental group,versus 51.4% and 73.1% in the conventional treatment group and control group respectively,showing a statistically significant difference(P<0.05).There were significant differences in gastric emptying rate,the levels of serum gastrin and fasting blood glucose among the 3 treatment groups before versus after treatment.And gastric emptying rate,the levels of serum gastrin and fasting blood glucose were improved in the experimental groups compared with the conventional treatment and control groups(all P<0.05).The follow-up results showed that the adverse reactions and the recurrence rates were significantly lower in the experimental group than in conventional treatment group and control group(all P<0.05).Conclusions Mosapride combined with acupoint massage in the treatment of diabetic gastroparesis have significant clinical effect,low recurrence rate and high safety,and is worthy of promotion.
10.On pathogenesis of rotavirus diarrhea in suckling mouse

Objective To study the pathogenesis of rotavirus(RV) diarrhea.Methods Simian rotavirus(SA-11) was grown in cultured MA-104 cell.The viral titers of the culture supernatant were determined by plaque forming assay.KM mice aged 7 days were inoculated with the viral supernatant via feeding tube(gavage).Histological and ultramicrostructure changes of the small intestines were observed under light microscope and electron microscope.The values of crypt depth and villi height were measured with software(image pro plus 5.1,IPP5.1).The distribution of the RV antigen in small intestine and the filamentous actin of the small intestine chorioepithelium were observed with immunohistochemical techniques.The apoptosis of the small intestine epithelium cells was observed with an in situ apoptosis detection kit.Results There were mild hyperemia,dropsy and extensive vacuolar degeneration of small intestine villi under light microscope.Plenty of lipid droplet-like structure at the top of the villi,microvilli malalignment or defluxion and enterocyte defluxion could be seen by electron microscope,but no obvious structure changes at the cell junctions were seen.The RV antigen mainly distributed at the top of the villi.The quantity of small intestine filamentous actin decreased and enterocyte apoptosis increased after RV infection.Conclusion RV mainly infects the mature villous epithelium.The presentation of RV diarrhea relates to the lesion of cytoskeleton,the microvilli lesion of the small intestine,enterocyte apoptosis and defluxion,villi atrophy,etc.,but may have no relationship with the structural changes of cell junctions in the small intestine epithelium.
