3.The investigation of doctor-patient communication skills of medical students and the exploration of training measures

ObjectiveTo investigate and analyze the doctor-patient communication skills of medical students and hereby to explore the efficient training system.MethodsUsing the questionnaires and statistic analysis,we focused on the doctor-patient communication skills of the observed 914 medical students in our college,their awareness of doctor-patient communication and the existing problems in the field.Results96.6% of medical students realized the importance and necessity of doctor-patient communication,while 62.7% of them lacked such kind of communication skills.Additionally,some other problems are exposed in medical history taking.ConclusionDoctor-patient communication skills might be the most important prerequisite for establishing harmonious relationship between doctors and patients,improving the quality of medical service and reducing the number of medical disputes.Consequentially,it's time to redesign relevant curriculum,to improve the old-fashioned training methods,to enhance the practice of doctor-patient communication training.Furthermore,improvements of teaching quality and a scientific evaluation system are also effective approaches to resolving the issue.
4.Effects of soft contact lenses on corneal diseases

Objective To evaluate the therapeutic efficacy of soft contact lenses on corneal diseases. Methods A total of 70 cases (82 eyes) of corneal diseases in the treatment group were treated with therapeutic soft contact lenses combined with topical antibiotic eye drops and artificial tears, but 64 cases (81 eyes) suffering from corneal diseases in the control group were treated with eye drops and eye ointments. ? 2 test was used to compare the cure rate of corneal epithelial defects and corneal ulcers caused by chemical and thermal burns between the two groups. Double sample t test was used to compare the average treatment time between the two groups. Results The cure rates of the corneal epithelial defects and corneal ulcers caused by chemical injury and thermal burn in the treatment group and the control group were 83% and 57%, respectively. In the treatment group, 100% patients with bullous keratopathy and 90% patients with dry eye disease were free from symptoms after wearing the lenses, but in the control group, the percentages were 50% and 63.6%, respectively. In the treatment group, 3 cases (4 eyes) of corneal epithelial defects caused by explosive injury and 3 cases of corneal epithelial defects after lameller keratoplasty were all cured, and 4 cases(4 eyes) of corneal perforations were healed. In the control group, 1 out of the 2 cases of corneal epithelial defects caused by explosive injury was cured, and 4 cases of corneal epithelial defects after lamellar keratoplasty surgery were cured, but 2 cases (2 eyes) of corneal perforation failed in the therapy, and corneal surgery was performed. In the treatment group, the cure rates of corneal epithelial defects or corneal ulcers caused by chemical injury and thermal burn were significantly higher than those in the control group ( P
5.InVance bulbourethral sling for the treatment of stress urinary incontinence after radical prostatectomy

Objective To evaluate the efficacy and safety of InVance bulbourethral sling for stress urinary incontinence after radical prostatectomy.Methods Six male patients with stress urinary incontinence after radical prostatectomy were treated with perineal sling (InVance) from March 2005 to March 2011,and the clinical data were analyzed.Results The operation time was 80-120 min and intra-operative blood loss was 20-50 ml.The catheter was removed at the second day after surgery.In a follow-up of 12 to 29 months,4 patients were dry,1 patient was improved,and 1 patient suffered treatment failure.There were 2 patients transient acute urinary retention and 1 patient short-term perineal pain in the main post-operative complication.In six months after the operation,urodynamic examination showed (15.9 ± 7.7) ml/s at Qmax and 0-20 ml/s at postvoid residual.Conclusion InVance bulbourethral sling for the treatment of stress urinary incontinence after radical prostatectomy appears to be safe,simple in the procedure and good effect to mild and middle incontinence.
6.Effects of Furong-Tongmai capsules on the expressions of interleukin-1 and tumor necrosis factor-α in the sciatic nerve in diabetic rats

Objective To investigate the effects of Furong-Tongmai capsules on the expressions of interleukin-1 (IL-1) and tumor necrosis factor-α (TNF-α) in the sciatic nerve in diabetic rats.Methods A total of 50 rats were randomly divided into the normal control group,model group,and low-,medium-,and high-dose of Furong-Tongmai groups,10 rats in each group.Diabetes mellitus in rats were induced by intraperitoneal streptozotocin injection (60 mg/kg).The rats in the low-,medium-,and high-dose of Furong-Tongmai groups were intragastric administrated with Furong-Tongmai suspension 0.7,1.4 and 2.8 g/kg daily for 8 weeks,respectively.The rots in the normal control group and model group were intragastric administrated with equal-volume normal saline daily for 8 weeks.The expression levels of IL-1 and TNF-α in the sciatic nerve were detected with immunohistochemistry staining.Results The expression levels of IL-1 (1.43% ± 0.17% vs.0.21% ± 0.09%;P<0.05) and TNF-α (1.98% ± 0.12% vs.0.35% ± 0.03%;P<0.05) in the model group were significantly increased than those in the normal control group.The expression levels of IL-1 (0.54% ± 0.14%,0.51% ± 0.13% vs.1.43% ± 0.17%;all P<0.05) and TNF-α (0.57% ± 0.17%,0.49% ± 0.15% vs.1.98% ± 0.12%;all P<0.05) in the medium-,and high-dose of Furong-Tongmai groups were significantly decreased than those in the model group and low-dose of Furong-Tongmai group (1.08% ± 0.18% in IL-1,1.11% ± 0.09% in TNF-α;all P<0.05).Conclusion Furong-Tongmai capsules can reduce the expressions of IL-1 and TNF-α in sciatic nerve in diabetic rats.
7.Influence ofFurong-Tongmai capsules on myocardial expression of LN and CollagenⅢ in diabetes mellitus rats

Objective To explore the the influence ofFurong-Tongmai capsules on myocardial expression of LN and CollagenⅢ in diabetes mellitus (DM) rats.Methods A total of 60 rats were randomly divided into the control group, the model group, the low-, middle- high-dosageFurong-Tongmai capsules group (n=10). The low-, middle-high-dosageFurong-Tongmai group was given 1.4, 0.7, 2.8 g/(kg body weight) Furong-Tongmai capsules. The other two groups were given the same dose of purified water. After 8 weeks treatment, the myocardial was taken to make pathology slice with SP immunohistochemistry staining. The expression of LN and CollagenⅢ were detected.Results Compared with model group, the expression of LN (0.67% ± 0.04%,0.65% ± 0.09%vs. 1.08% ± 0.13%) and CollagenⅢ (0.67% ± 0.15%, 0.69% ± 0.13%vs. 1.17% ± 0.12%) in the middle-high-dosageFurong-Tongmai groups significantly decreased (P<0.05). However, there were no statistical differences between the low-, middle- high-dosageFurong-Tongmai groups and the model group (P>0.05).Conclusions TheFurong-Tongmai capsules could inhibit the expression of LN and CollagenⅢ in DM rats.
8.Evaluate the effectiveness of the treatment of high risk benign prostatic hyperplasia with transrectal high-intensity focused ultrasound by urodynamic test

Objective To evaluate the effectiveness of the treatment of high risk benign prostatic hyperplasia (BPH) with transrectal high-intensity focused ultrasound (TR-HIFU) by urodynamic test. Methods One hundred and thirty patients with BPH, most of who suffering with diabetes mellitus,cardio-cerebrovascular disease, respiratory disease. All patients were given the operation with Sonablate-500 high-intensity focused ultrasound (HIFU). International prostate symptom score (IPSS), quality of life (QOL), maximum urinary flow rate (Qmax), post void residual urine volume (PVR), pressure-flow tests (A-G) were employed for the evaluation of curatives effect on preoperation, 30th and 90th day after the treatment. Results The duration of the TR-HIFU prostate ablation treatment was 15-85 rain. One hundred and seven patients after treatment were followed up for 3 months. Compared the effect before treatment with that after treatment, IPSS improved from (27.0 ±11.5) scores to (8.5±1.4) scores, Qmax increased from (5.7±2.3) ml/s to (14.1±4.2) ml/s, PVR decreased from (118.5±78.0) ml to (30.0±15.0) mI,A-G parameters reduced from 62.4±19.3 to 25.6±13.0 and urine volume (VV) increased from (85.8 + 42.0) ml to (194.0±52.0) ml. The parameters all altered obviously (P < 0.05). Conclusions TR-HIFU appears highly attractive as a new optional, safe and efficacious and minimally invasive treatment for the high risk BPH. Main post-operative urodynamie parameters are more obviously improved than those of the preoperation.
9.Effect of Qingrequzhuo capsule on blood pressure of patients with type 2 diabetes metabolism syndrome

Objective To observe the effect of Qingrequzhuo capsule on blood pressure of patients with type 2 diabetes metabolism syndrome. Methods 72 cases with type 2 diabetes and metabolic syndrome were randomly recruited into a control group and a treatment group. Based on the conventional treatment, the treatment group was treated with Qingrequzhuo capsule, 5 tablets/time, 3 times/d; While the control group was treated with ramipril tablets 2.5 mg, 1 times/d, respectively. After 3 months, the value of blood pressure and blood lipids were observed. Results After treatment, the blood pressure and blood lipid were significantly improved in both groups(P<0.05), but no significant difference between the two groups (P>0.05). Conclusion Besides improving blood lipids, Qingrequzhuo capsule can also reduce high blood pressure and improve a variety of risk factors to cardiovascular disease.
10.Effectiveness of the treatment of slight and moderate benign prostatic hyperplasia with large doses of cernilton by urodynamic test

Objective To evaluate the effectiveness of the treatment of slight and moderate benign prostatic hyperplasia(BPH)with large doses of cemilton by urodynamie test.Methods A systematic review of using large doses of cernihon only for 80 patients with BPH with 7 grade < International Prostate Symptom Score(IPSS)≤20 grade.All cases with cemilton had been administered in a dosage of 750 mg,three times a day for one year.All were respectively given IPSS,postvoiding residual urine(PVR),maximum flow rate(Qmax)and pressure-flow(A-G)test in order to assess the effectiveness at pre-treatment and 3,6,12 months after treatment.Results All patients had followed up for(1.2±1.6)years,additionally 26 of those were removed and others had received complete follow-up.Before the treatment IPSS was(20.5±4.5)grade,PVR was(42.5±8.2)ml,Qmax was(10.2±2.3)ml/s,A-G was 56.5±12.8,PQmax was (54.8±13.7)mm Hg(1 mm Hg = 0.133 kPa),and 12 months later,IPSS was(11.0±3.3)grade,PVR was (15.9±7.4)ml,Qmax was(15.7±2.5)ml/s,A-G was 49.2±13.6,PQmax was(43.6±14.9)mm Hg.There were significantly difference between pre-treatment and post-treatment data of the urodynamics(P < 0.05).Conclusions The effectiveness of the treatment of slight and moderate benign prostatic hyperplasia with large doses of cemihon are relatively satisfactory.Main post-treatment urodynamic parameters obviously improves than that of the pre-treatment.
