1.Infusion of drug via portal vein for liver cirrhosis

Medication is still the main means for treating liver cirrhosis. However, micro-circulation obstacle in the liver and dynamic abnormality of portal vein flow suggest that liver protecting drugs via peripheral veins can hardly give full requirement for their effect. The authors reviewed a new method of percutaneous transhepatic portal vein implantation of port-catheter system for infusion of drugs in the treatment of liver cirrhosis, with theoretical basis and the outlook of application.
2.Treatment of humerus surgical neck fracture with anatomy plate and locking compression plate

Objective To compare the therapeutic effect of anatomy plate and locking compression plate in the treatment of humerus surgical neck fracture. Methods Fifty one patients with humerus surgical neck fracture were enrolled into the study and divided into two groups randomly to accept anatomy plate or locking compression plate treatment. In the anatomy plate treatment group ( n = 35 ), 19 cases were type- Ⅲ fractures and 16 cases were type-Ⅳ fractures according to Neer classification. In the locking compression plate treatment group (n = 16) ,9 cases were type-Ⅲ fractures and 7 cases were type-Ⅳ fractures according to Neer classification. Results In anatomy plate group,28 cases were followed up for 4 months to 48 months ( average period was [21 ± 2] months). According to the Constent score criteria, 19 cases got excellent outcome,8 cases got good outcome,1 case got fair outcome. In locking compression plate group, 14 cases were followed up for 1 month to 24 months (average period was[12 ± 1] months). Ten cases got excellent outcome,4 cases got good outcome. Conclusion Both anatomy plate and locking compression plate treatments are good for the humerus surgical neck fracture.
3.Evaluation of fMRI functional imaging in the curative effect of the acupuncture combined with traditional Chinese medicine in the treatment for patient with cerebral infarction

Objective: To explore the relationship between the combinative intervention therapy and clinical curative effect, through used function magnetic resonance imaging (fMRI) to observe the change of detection data for patients with cerebral infarction pre and post-intervention therapy of acupuncture combined with traditional Chinese medicine (TCM). Methods: 40 patients with cerebral infarction were divided into control group (20cases, received normal treatment)and observation group (20cases, received combinative treatment of acupuncture and TCM). Pre and post therapy, the patients of every group received evaluation about state of illness and detection of fMRI. The score of national institute of health stroke scale (NIHSS) was used to evaluate the degree of neurologic impairment, and analyze the imaging manifestations of fMRI on patients with cerebral infarction at basal ganglia who received acupuncture combined with TCM. Some cerebral functional imaging techniques, included blood oxygenation level dependent( BOLD), fMRI and diffusion tensor imaging (DTI), were used to grasp the changes of relevant functional region of brain and different brain regions connected with function region of patients with cerebral infarction before and after therapy. Results: The NIHSS score of observation group was significantly less than that of the control group (t=5.450, P<0.05), and the total efficiency of observation group (95%) was significantly higher than that of the control group (75%) (x2=4.329, P<0.05). And the differences of the activation frequency, activation volume, ADCavg values and the values of FA between the observation group and the control group were statistically significant (t=5.425, 2.760, 3.453, 5.513, P<0.05). Conclusion: Acupuncture combined with traditional Chinese medicine can efficiently improve the status of neurologic impairment for patients with cerebral infarction, and the clinical efficacy before and after treatment can preferably be observed by fMRI.
4.Determination of multiple optical parameters of red blood cell suspension

Objective To develop a measurement method for determination of optical parameters of a red blood cell ( RBC) suspension based on the measurement of spatial scattered light signals without using an integrating sphere.Methods Multiple independent photoelectric sensors and light intensity modulation were used to obtain the measured values of diffuse reflectance,diffuse transmittance and collimated transmittance.The measured data results were imported into a Monte Carlo simulation based RTE to inversely determine the absorption coefficient, scattering coefficient and anisotropy factor of the measured sample using a new perturbation method.Results The described measurement method was applied to determine the optical parameters of a polystyrene microsphere suspension with a mean diameter of 2.6μm,and the results were essen-tially consistent with the calculated optical parameters by Mie code.Then, the RBC suspension was used for testing optical parameters,and the results were basically consistent with the parameters in the literature.Conclusion The system based on the measurement of spatial scattered light signals without using an integrating sphere will provide a quick and accurate approach for quantitative analysis of free hemoglobins and RBC suspensions.
5.Quantitative evaluation of salivary gland dysfunction after radioiodine therapy using salivary gland scintigraphy

Objective To assess the variation of salivary gland function in differentiated thyroid carcinoma (DTC) patients receiving different doses of 131 I therapy in the first.Methods 40 DTC patients were divided into two groups according to the application 131I doses,salivary scintigraphy was performed with 99TcmO4-on DTC patients before and 3months after 131 I therapy.Quantitative analysis of salivary gland function were performed.Results In low dose group,only the uptake ratio of 30min (UR30) of bilateral parotid decreased ( P < 0.05 ) ; but in high - dose group,the uptake ratio of 30min (UR30),excretion fraction ( EF ),excretion rate (ER) of bilateral parotid and submandibular glands were significantly decreased,excretion time(EP) significantly prolonged after 131 I therapy( all P <0.05) ;the parotid gland was more severely than the submandibular gland.Conclusion Salivary gland function was damaged of DTC patients receiving different doses of 131I therapy in the first,salivary gland dysfunction correlated well with the administered dose.
6.An analysis of the advantage of biologically equivalent dose of unconventionally fractionated radiotherapy

Objective To analyze the advantage of altered fractionation radiotherapy by calculating the accumulative effects of daily biologically effective dose (BED) to find out the difference between conventional fractionated radiotherapy and altered fractionation radiotherapy.Methods The data in the report of hyperfractionated or accelerated radiotherapy for head and neck cancer published by Cochrane Collaboration in 2010 was analyzed.Based on the radiotherapy processes mentioned in this report,the accumulative effects of daily BED were calculated and compared in different radiotherapy processes by using linear-quadratic mode.The variation of BED in different radiotherapy processes was find out.Results In total dose of unity as the premise of 70 Gy,altered fractionation especially the hyperfractionated accelerated radiotherapy could give a higher BED to the tumor during a shorter period,hyperfractionated radiotherapy could give a lower BED to normal tissues,and hyperfractionated radiotherapy with split course could give higher BED to the tumor while lower BED to normal tissues.Conclusions The variation of BED in different radiotherapy processes can be shown clearly by linear-quadratic mode.It can be simple and shortcut through mathematical models for the evaluation of different radiotherapy plan,on clinical symptomatic selection play a guiding role in tumor therapy.
7.Experience of SHAO Zhao-di in Treating Hydronephrosis

Hydronephrosis is a common and frequent clinical disease. With the extensive application of minimally invasive techniques of Western medicine, it can quickly remove the obstruction and eliminate water. However, there may be problems of easy recurrence after surgery, causing more serious irreversible kidney damage. TCM has a unique advantage in the treatment of hydronephrosis, which can radically relieve the pain of patients, prevent recurrence, and protect the kidney function. Professor SHAO Zhao-di thinks that the cause of hydronephros is deficiency of kidney qi, which affects the function of gasification. Therefore, the treatment should be nourishing qi to invigorate spleen and reinforcing kidney for diuresis. In clinical practice, Bixie Fenqing Decoction can be used to treat hydronephrosis, which can obtain good efficacy.
8.Effect of leptin on inducible NO and MMP-13 in rabbit articular chondrocytes in vitro

Objective To observe the in vitro effect of leptin, alone or in combination with tumor necrosis factor-alpha (TNF-α) on inducible nitric oxide (NO) and on inducible matrix metallo-proteinase-1 3 (MMP-13) in rabbit articular chondrocytes. Methods The chondrocytes from the articular cartilage of 2-month-old rabbits were cultivated and identified, and the second filial generation chondrocytes were cocultured on plates with different concentrations of leptin alone or in combination with TNF-α for 48 h or 96 h after 12 h starvation. The concentration of NO and MMP-13 was measured in the chondrocytes culture supernatant fluid. The results were statistically analyzed. Results There was no significant difference in the concentrations of NO between the different concentrations of leptin alone groups and the blank control group (P > 0. 05). In combination with the same concentration of TNF-α (10 ng/mL), leptin could dose-dependently increase the concentration of NO in the chondrocytes culture supernatant fluid in vitro. There was significant value in average concentration of MMP-13 on the main effect of both time and dose (P <0. 05) . No MMP-13 was detected in the blank control group. Conclusion Leptin can induce MMP-13 and have synergistic induction effect on NO with TNF-α in rabbit articular chondrocytes in vitro.
9.Application of percutaneous transhepatic portal vein port-catheter system implantation in preventing small hepatocellular carcinoma recurrence after curative treatments

Objective To investigate the technical outline and clinical value of percutaneons transhepatic portal vein port-catheter system implantation in preventing small hepatocellular carcinoma recurrence after curative treatments. Methods Fifteen patients with small hepatocellular carcinoma after curative treatment were included in this study. Guided by ultrasound and fluoroscopy, left branch of the portal vein were punctured and port-catheter system were implanted. Then drugs infusion into portal vein system was done for preventing recurrence of hepatic carcinoma. Results Interventional operations were succeed in all 15 cases. Drugs could drop into portal vein smoothly. No operating complications occurred. Conclusion Percutaneous transhepatic portal vein port-catheter system implantation was an easy operating and micro traumatic method. This technique could play an important role in preventing recurrence.
10.Early diagnosis value of MRI combined with DSA in cancer nodules from cirrhotic nodules

Objective To evaluate the early diagnosis value of MRI combined with DSA in cirrhotic nodules (regenerative nodules, dysplastic nodules) becoming small hepatocellular carcinoma. Methods 40 patients diagnosed as cirrhosis and whose liver nodules detected by ultrasound were followed up as MR dynamic study. All patients were followed up by three dynamic contrast-enhanced MR scanning once every 3 months,and the changes of cirrhotic nodules were observed and analyzed in the signal. If MRI suggested cancerous nodules cirrhosis, hepatic artery DSA would be carried out. Results 40 patients were followed up for 1.5 to 3 years,all patients MR nodules were found in liver regeneration. Follow-up process, the dysplastic nodules were founded in 28 cases and the small hepatocellular carcinoma were founded in 18 patients. 16 cases of 18 patients with small hepatocellular carcinoma carried out routine DSA all had typical of hepatic arterial blood supply and angiogenesis, and were given to Integrated Traditional and Western intervention simultaneously. Conclusion Combined use of MRI-DSA in the evaluation of cirrhotic nodules had a definite value, and could find smaller hepatocellular carcinoma,provide the basis for smaller hepatocellular carcinoma therapy.
