1.Development of a performance indicator evaluation system for the Healthcare Improvement Initiative

Objective To develop a set of indicators for measuring the performance of China's Healthcare Improvement Initiative.Methods Such methods as literature review,expert consultation and onsite preliminary experiment were used,to study and demonstrate the indicators,the evaluation framework,the weight,evaluation indicators,and evaluation standards.Results The final evaluation indicators for medical institutions included nine class-1 indicators,29 class-2 indicators,and 56 class-3 indicators;those for healthcare administrations included six class-1 indicators,nine class-2 indicators,and 13 class-3 indicators.Conclusions The system takes into full account the special nature of healthcare,and mission of the Initiative,as the well as the quality,safety,efficiency and equity of healthcare,to make the system scientific and operational.
2.The effect of different BIS value on the early postoperative cognitive function and S100βprotein in elderly patients undergoing abdominal surgery

Objective To investigate the effects of different BIS values on postoperative cogni-tive dysfunction (POCD)and S100βprotein(S100β)in the early stage of postoperation.Methods Fifty patients who were scheduled for selective abdominal surgery under general anesthesia (male 34 cases, female 1 6 cases,aged 65 to 75 years,ASA Ⅰ or Ⅱ)were randomly divided into two groups:light anesthesia group (group L,n =25,BIS value was maintained at 50 to 59)and deep anesthesia group (group D,n =25,BIS value was maintained at 30 to 39).BP,HR,SpO 2 ,ECG,PET CO 2 ,inhaled anes-thetic concentration and BIS values were recorded on time points of 5 minutes after the patients ente-ring the operating room (T0 ),before endotracheal (T1 ),intubation (T2 ),incision (T3 ),two hours after incision (T4 ),three hours after incision (T5 )and at the end of surgery (T6 ).The procedure du-ration,anesthesia time,dosages of propofol,fentanyl,midazolam and VAS scores on 1 d after sur-gery were also recorded.Blood samples were collected on time points of 10 min before anesthesia,im-mediately after surgery and 24,48 h after operation.S100β concentration were detected.Mini-mental State Examination (MMSE)score and Trail Making Test (TMT)completion time were recorded on 1 d before surgery and 1,3,7 d after surgery.Results BIS value of group D were lower than group L on T2 ,T3 and T4 .The propofol dosage of group D was significantly greater than that in group L (P <0.05 ).The concentration of serum S100βincreased significantly immediate and 48 h after operation in both groups compared with 10 min before anesthesia(P < 0.05).It was still higher 24 hours after op-eration than before anesthesia.But there was no statistic difference.Compared with the end of surger-y,the concentration of serum S100βin two groups on 24 h after surgery were significantly decreased (P < 0.05 ).The concentration of serum S100β in group L on the end of surgery and 24 h after surgery were higher than that in group D significantly (P <0.05).Compared with 1 d before surgery, postoperative 1 d MMSE scores in two groups and postoperative 3 d MMSE score in group L de-creased significantly (P <0.01).Compared with postoperative 3 d,postoperative 7 d MMSE score in group L increased significantly (P <0.01).Postoperative 1,3 d MMSE score in group D were signifi-cantly higher than group L (P <0.05).Compared with 1 d before surgery,TMT completion time in two groups on 1 d after surgery were significantly prolonged (P <0.01 ).Compared with 1 d after surgery,TMT completion time in two groups on 3 d after surgery were significantly shortened (P <0.01).Compared with 3 d after surgery,TMT completion time in group L on 7 d after surgery was significantly shortened (P <0.01 ).TMT completion time in group D on 1,3 d postoperative were significantly shorter than group L (P <0.05).POCD incidence of group D on 1 d after surgery was lower than that in group L (P < 0.05).Conclusion Different depth of anesthesia can ensure hemo-dynamic balance in old patients during surgery and after surgery.When BIS value was maintained at 30 to 39,it had lower S100βprotein levels,lower incidence of early POCD and a lesser degree of post-operative cognitive dysfunction.
3.Effect of TGF-β1on the expression of MMP-9 in mouse endometrial stromal cells

Objective To investigate the effect of transforming growth factor-β1(TGF-β1) on the expression of matrix metal-loproteinase-9(MMP-9) in cultured mouse endometrial stromal cells .Methods After separation and purification ,the mouse endom-etrial stromal cells were cultured with different concentrations of TGF-β1 ,and the final concentrations were 0 .1 ,2 .5 ,5 .0 ,10 .0 , 20 .0 ng/mL ,respectively .ELISA method was used to detect the content of MMP-9 in the the culture medium after 48 h culture . Results Compared with the control group ,the expression of MM P-9 in mouse endometrial stromal cells in the experimental groups increased with the TGF-β1 concentration elevation ,and the difference was statistically significant (P < 0 .01) .Conclusion In the mouse endometrial stromal cell system ,TGF-β1 may be involved in the regulation of extracellular matrix metabolism through regu-lating the expression of MMP-9 .
4.Attempts made by Xiamen Cardiovascular Hospital in building a regional first-aid network for chest pain rescue

Xiamen Cardiovascular Hospital set up the first chest-pain center in Fujian province,and launched the first citywide regional chest pain network in China.Their experiences prove that the first-aid network can effectively integrate ambulances,primary hospitals,general hospitals,and special hospitals for seamless connection between EMS and in-house rescue.These efforts can minimize the mortality and disabilities of myocardial infarction,effectively elevating the myocardial infarction treatment efficiency and capabilities of the region.
5.Survey on awareness and willingness of breast cancer screening between Han and Mongolian women in Ordos , Inner Mongolia Autonomous Region

Objective To investigate and explore the awareness and willingness of breast cancer screening between Han and Mongolian women in Ordos, Inner Mongolia Autonomous Region, and to analyze the influence factors. Methods The patients answered investigation questionnaires face to face, and the content of questionnaire included demographic information, breast cancer awareness and relevant affecting factors on the awareness and willingness. Results 11162 (88.2 %) Han women and 1495 (11.8 %) Mongolian women were enrolled in the survey, with the average age of (45.4 ±7.1) years old and (45.4 ±6.8) years old. The education level (high school and above) of Han women was lower than that of Mongolian women [4456 women (40.0%) vs. 750 women (50.2%), χ2= 57.041, P< 0.001]. The Main way to delivery information on breast cancer screening in Han and Mongolian women was from their family members or friends [5002 women (44.9%) vs. 688 women (46.1%),χ2=40.699, P<0.001]. The proportions of never attending screening in the Han and Mongolian women were 41.7 % (4640 women) and 45.9 % (683 women), respectively (χ2=14.075, P=0.001). The breast self-examination rate of Han women were higher than that of Mongolian [71.1 % (7926 women) vs. 58.5 % (872 women), χ2= 98.466, P< 0.001]. According to logistics regression analysis, the cognitive level of women with high education level and breast disease history was higher (all P<0.001). Conclusions The cognitive level of breast cancer screening in Han women is better than that in the Mongolian, however, the overall level are low. It is necessary to strengthen the propaganda and education, and to encourage women to actively participate in breast cancer screening.
6.Effects of tracer methodology on intravenous thrombolysis in patients with acute ischemic stroke

Objective To observe the effects of tracer methodology when used to trace and manage emergency intravenous thrombolysis processes in patients with acute ischemic stroke (AIS). Methods The patients' attendance process was traced, improved and constantly optimized according to the theoretical basis of tracer methodology. A total of 112 AIS patients who received thrombolysis from September 2015 to August 2016 (before tracer methodology was implemented) were divided into the control group, while another 160 AIS patients who received thrombolysis between September 2016 and August 2017 (after tracer methodology was implemented). The time between arrival and thrombolysis administration and hospital treatment were compared between the two groups. Results After tracer methodology was carried out, the time used between arrival and admission by our stroke treatment team in the observation group was (4.1±1.2)min; the time between arrival and imaging examination was (18.8±11.2)min; the time between imaging examination and thrombolysis administration was (22.5±10.2)min; and the time between arrival and thrombolysis administration was (44.6±12.5)min, all shorter than that of the control group (P<0.05). Conclusions The application of tracer methodology effectively reduces door to needle time, improves the efficiency of intravenous thrombolysis in AIS patients.
7.Changes in the practice environment from the perspective of doctors in 136 tertiary public hospitals in China

Objective:To understand the changes in the practice environment from the perspective of doctors in 136 tertiary public hospitals, and to provide evidence for formulating strategies and measures to improve the practice environment of doctors.Methods:The data was collected from five rounds of China Healthcare Improvement Evaluation Survey from December 2015 to March 2021. The sample size was 17 945, 19 774, 20 786, 23 289, and 22 836, with a total of 104 630. The self-designed questionnaire on four dimensions of doctor-patient relationship, workload, administrative logistics support and professional identity was performed.Results:From the 1st to the 5th survey, the approval ratio of " the administrative and logistics department of this hospital is highly efficient" showed the characteristics of " U-shaped distribution" , and the approval ratio of " approve or encourage children to continue to engage in medical work" increased from 13.38% to 23.91%.From the 2nd to the 5th survey, the percentage of doctors who experienced doctor-patient disputes dropped from 28.96% to 23.20%, and the percentage of doctors agreeing that the relationship between doctors and patients in this hospital was improving continued to increase from 33.20% to 60.45%.From the 3rd to the 5th survey, the proportion of doctors experiencing medical troubles dropped from 15.10% to 14.19%, and the recognition rate of " the cafeteria of this hospital provided medical staff with nutritious and delicious meals" increased from 36.25% to 47.52%, and the approval proportion of " my current work load is heavy" dropped from 61.55% to 45.24%. The results of the 5th survey showed that there were significant differences in the approval ratios of each dimension in different genders, job titles and hospital types( P<0.001). Conclusions:From the perspective of doctor-patient relationship, workload, administrative logistics support, and professional identity, the improvement of doctors′ practice environment is still insufficient. Hospital administrators should continue to focus on the improvement of the doctor-patient relationship, and take necessary measures to reduce doctors′ workload and improve doctors′ professional identity.
8.Effect of CO 2 fractional laser on the structure and function of pelvic floor in female vaginal laxity

Objective:To observe the changes of pelvic floor structure and function in female vaginal laxity after CO 2 fractional laser transvaginal treatment. Methods:This study reviewed the improvement of pelvic floor structure and function after CO 2 fractional laser transvaginal treatment in 28 female patients [aged 26-59 (37.5±8.3) years] with vaginal laxity syndrome seen at the Department of Dermatology, the First Affiliated Hospital of Air Force Medical University from March 2020 to November 2021. A total of 28 female patients with vaginal laxity syndrome underwent intravaginal treatment by CO 2 fractional laser instrument once/month for 3 times. The clinical efficacy and safety were evaluated according to the pre- and post-treatment transverse vaginal diameter, FSFI score of female sexual function, VHIS score of vaginal environment, vaginal tactile imaging system (VTI), pelvic ultrasound and MRI, tissue biopsy, patient satisfaction, pain score, and postoperative adverse effects. Results:Twenty-one of the twenty-eight female patients with vaginal laxity syndrome showed significant improvement in symptoms related to vaginal laxity syndrome after intravaginal treatment with CO 2 fractional laser therapy. All patients showed improvement in all indexes before and 1 month after treatments the mean vaginal transverse diameter decreased from (3.00±0.39) fingers to (2.71±0.40) fingers ( P<0.05), VHIS increased from (17.12±3.97) to (21.69±3.61) ( P<0.05), FSFI score improved from (23.11±3.70) to (27.43±5.33) ( P<0.05), and VTI examination showed that vaginal muscle strength and elasticity were improved to different degrees, and there was a statistical difference compared with that before treatment (total contractility of the anterior vaginal wall: t=26.23, P<0.001; total contractility of posterior vaginal wall: t=39.02, P<0.001; the mean contractility of the anterior vaginal wall: t=17.92, P<0.001; the mean contractility of the posterior vaginal wall: t=22.57, P<0.001). At the same time, questionnaire score of international consultation on incontinent questionnaire short form (ICI-Q-SF scale) of 13 patients with combined mild to moderate stress urinary incontinence showed a statistically significant decrease compared with those before treatment (8.97±2.99 before treatment and (7.18±1.79) one month after treatment; t=2.792, P<0.01). Pelvic ultrasound and magnetic resonance examination indicated a tightening of the vaginal wall structure, and pelvic ultrasound observed a significant decrease in bladder neck mobility and a significant decrease in vesicourethral rotation angle. Vaginal tissue biopsy indicated an increase in the thickness of the vaginal mucosa and an increase in the number and more regular arrangement of collagen fibers after treatment. All patients had high treatment satisfaction and there were no adverse effects such as infection and bleeding during the treatment. Conclusions:Transvaginal CO 2 fractional laser treatment can improve the pelvic floor structure and function around the vagina, treat female vaginal laxity syndrome, stress urinary incontinence and female sexual dysfunction, with significant clinical efficacy and good safety.

10.Inhibitory effect and mechanism of total flavonoids from Melicope pteleifolia on transplanted tumor of colorectal cancer in nude mice

OBJECTIVE To study the inhibitory effect and mechanism of total flavonoids from Melicope pteleifolia (TF-MPL) on transplanted tumor of colorectal cancer in nude mice. METHODS The transplanted tumor model of colorectal cancer was induced by injecting 0.2 mL colorectal cancer cell LoVo subcutaneously via the right armpit of nude mice. After successful modeling, nude mice were randomly divided into model group, 5-fluorouracil group (positive control, 10 mg/kg), TF-MPL high- dose and low-dose groups (25, 12.5 mg/kg); a normal group (normal saline containing 0.3% carboxymethyl cellulose sodium) without modeling was additionally set up, with 6 mice in each group. Each group was intraperitoneally injected with the corresponding drug solution/solvent for 21 consecutive days. The inhibitory rate of the transplanted tumor, liver and spleen index, and the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in serum were detected after the last medication; the morphological changes of tumor tissue were observed; immunohistochemical staining was used to detect protein expressions of Toll- like receptor 4 (TLR4) and nuclear factor-κB subunit p65 (NF-κB p65) in tumor tissue of nude mice. Western blot assay was used to detect protein expressions of TLR4, myeloid differentiation factor 88 (MyD88), TNF receptor-associated factor 6 (TRAF6), interleukin-1 receptor-associated kinase 1 (IRAK-1), NF-κB p65 and caspase-3 in tumor tissue of nude mice. RESULTS Compared with the model group, TF-MPL high-dose group showed a significant decrease in tumor weight (inhibitory rate of 36.91%), liver and spleen index, serum levels of TNF-α and IL-6 and protein expressions of TLR4, MyD88, TRAF6,IRAK-1 and NF- κB p65 (P<0.05 or P<0.01); the expression of caspase-3 protein was increased significantly (P<0.05), and more tumor cell shrinkage and deformation, nuclear pyknosis and fragmentation were observed. CONCLUSIONS TF-MPL can significantly inhibit the growth of transplanted tumor of colorectal cancer in nude mice, the mechanism of which may be associated with reducing inflammatory response, inhibiting TLR4/MyD88/NF-κB signaling pathway, and promoting apoptosis in colorectal cancer cells.
