BACKGROUND: Scholars disagree with each other about the small saphenous vein effects on skin flap and how to dispose vascular proximal pedicles.OBJECTIVE: To analyze effects of different methods of the small saphenous vein disposal on flap survival using sural neurocutaneous island flap retrograde metastasis for repairing defects of soft tissue of instep, heelstick and ankle.DESIGN, TIME AND SETTING: The case control observation experiment was performed at the General Hospital of Guangzhou Military Area Command of Chinese PLA from March 1998 to April 2007.PARTICIPANTS: A total of 56 patients with defects of soft tissue of instep, heelstick and ankle were divided into 2 groups, small sapbenous vein deligation in proximal pedicle flap group (group A) (n=38), and proximal small saphenous vein and great saphenous vein or tributaries at recipient site anastomosis flap group (group B) (n=18).METHODS: During sural neurocutaneous island flap retrograde metastasis for repair,(3.5×4.0)cm-(4.0×4.5) cm flap was obtained in 35 cases, and (4.0×4.5) cm -(10.0×12.0) cm in 21 cases.MAIN OUTCOME MEASURES: Outcome of flap survival at different incision area and implanted methods.RESULTS: No vein articulo occurred in 21 cases, which flap areas were (4.0×4.5) cm -(10.0×12.0) cm. Five of 35 cases which flap areas were (3.5×4.0) cm -(4.0×4.5) cm developed vein articulo. The necrotic rate of flaps in group B was significantly lower compared to the group A (P=0.017 67).CONCLUSION: When the area of skin flap is smaller than (4.0×4.5) cm, the proximal end of the small saphenous vein should be anastomosed with the great saphenous vein or tributaries connecting with the great saphenous vein at recipient site. The small saphenous vein is not a superficial vein, which only cross the skin flap, but it has trophic action on the skin flap.

Objective To investigate the effect of Buyanghuanwu decoction(BYHWD) on angiogenesis and Ang-1/Tie-2 expression after focal cerebral ischemia in mice. Methods Focal cerebral ischemia was induced by 30 min of middle cerebral artery occlusion followed by reperfusion. BYHWD (20 g/kg) was administered orally 24 h after ischemia and once a day. The neurological score and the corner test were used to evaluate sensorimotor function during 28 days after ischemia. The microvessels density and the expression of Ang-1/Tie-2 in the ischemic boundary zone were determined by immunohistochemistry on 28 days after ischemia. Results Compared with the model group, BYHWD significantly ameliorated neurological dysfunction and reduced the number of right turn during 3 to 28 days after ischemia(P<0.05 ). In addition, the numbers of CD31 ,Ang-1 and Tie-2 immunopositive cells were (437 ±59) ,(389 ±61 ) and (251 ±42) at the boundary zone in model group 28 days after ischemia, respectively. However, BYHWD treatment significantly increased the numbers of CD31(609 ± 68 ), Ang-1 (551 ±66) and Tie-2 (342 ±46) immunopositive cells(P<0. 01 ). Conclusion BYHWD promotes angiogenesis, which may contribute to recovery of neurological function after focal cerebral ischemia, and Ang-1/Tie-2 pathway appears to mediate BYHWD-induced angiogenesis.

Objective To study the value of renal CT scan in evaluating split renal function. Methods 147 patients undergone CT scan from June 2009 to June 2011 were involved in this study. There were 73 cases of obstructive hydronephrosis and 74 cases of renal tumors.66 patients were males and 81 were females with a mean age of 53 years ( range 17 - 87 years).GFR detected by single-photon emission computed tomography (SPECT) was used as the reference of split renal function. The kidneys were divided into 3 groups according to the GFR:normal renal function (113 cases,GFR ≥ 34 ml/min),mildly impaired renal function (66 cases,GFR:20 -34 ml/min) and severely impaired renal function (41 cases,GFR <20 ml/min).One-way ANOVA and linear regression analysis were used to analyze the relationship between the results of CT scan and split renal functions. Results There were significant differences in the cortical thickness among the normal renal function, mildly impaired renal function and severely impaired renal function groups. The cortical thicknesses were (0.62 ±0.11) cm,(0.45 ±0.10) cm and (0.26 ±0.07) cm,respectively (P < 0.01 ).The renal cortical thickness was strongly correlated with GFR (r =0.806,P <0.01 ).There were significant differences in the enhancement during the cortical phase among the 3 groups, which were (162.1 ±25.3) HU,(121.6 ±21.0) HU and (63.5 ±20.0) HU,respectively (P<0.01).The enhancement during the cortical phase was strongly correlated with the GFR (r =0.851,P < 0.01 ).The enhancement during the parenchymal phase and excretory phase was moderately correlated with the GFR ( r =0.467 and r =0.451,P < 0.01 ). Conclusions The renal cortical tbickness and the enhancement during the cortical phase detected by CT scan might be useful for the clinical evaluation of split renal function.

[Objective]To explore the potential mechanism of action of Huangqi Sijunzi Decoction in the treatment of hepatocellular carcinoma(HCC)by network pharmacology combined with bioinformatics methods.[Methods]The bioactive compounds of drugs and their targets were identified using the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP)database.Differential gene and module gene of HCC were identified by differential analysis and weighted gene co-expression network analysis(WGCNA).On this basis, the network of"Components-targets of Chinese drug"was constructed.Subsequently, the biological function of the target was investigated and protein-protein interaction(PPI)network was constructed to identify key targets for drug therapy of HCC.Finally, the interaction between the target and the compound was explored by molecular docking.[Results]A total of 156 compounds and 227 targets of 5 Chinese drugs were identified,as well as 2 477 and 685 differential genes in The Cancer Genome Atlas(TCGA)and Gene Expression Omnibus(GEO)datasets, and 2 104 and 2 165 WGCNA association module genes. Then, the Chinese drug component-target network containing 4 drugs,85 compounds and 9 targets was constructed.Biological function analysis showed that the 9 targets were mainly related to cell cycle,p53 signaling pathway and cell senescence.Meanwhile, two core components of kaempferol and quercetin and five core targets of cyclin A2(CCNA2),estrogen receptor alpha(ESR1),cyclin B1(CCNB1),cyclin-dependent kinase 1(CDK1)and topoisomerase Ⅱ alpha(TOP2A)were screened.Molecular docking results showed that the kaempferol and quercetin had stable binding ability to the corresponding core targets.[Conclusion]Huangqi Sijunzi Decoction may play a therapeutic role in HCC by regulating pathways and proteins related with cell cycle. This study provides research ideas and theoretical support for the treatment of HCC with Huangqi Sijunzi Decoction.

Journal of Biomedical Engineering 2013;30(5):1083-1090
In medical imaging field, doctors often complete the intra-subject registration of multi modality images by choosing a pair of anatomic landmarks. It is hard to choose the same landmark accurately in different imaging modality. For multi-modality image registration, mutual information measure is widely used because it suffers little from the difference among different modalities, but it has disadvantages of large amount of calculation and slow converges. In this paper, considering the convenience in practice, we firstly register different modality images by choosing a pair of corresponding landmarks. In order to decrease the error during choosing the landmarks, we further do the registration using mutual information method. The advantage of this solution is that the landmark based registration presents a good starting point for further mutual information (MI) registration, while the MI method decreases the difficulty of choosing landmarks. Experimental results showed that this registration solution was fast, accurate, and would have a good clinical potential application in the future.

Objective To evaluate the application value of ultrasound in vascularization of different artificial bones . Methods A total of 15 New Zealand rabbits were utilized for model establishment of classic segmental bone defect in bilateral radius . Recombinant human bonemorphogenic protein-2 ( rhBMP-2 ) coralline hydroxyapatite(CHA) and CHA were implanted into left and right limbs . Each CHA was divided into 4 equal parts which were examined with conventional ultrasonography and contrast-enhanced ultrasonography( CEUS ) on 3 d ,7 d ,11 d ,15 d ,30 d and 45 d respectively . CEUS quantitative was performed by time-intensity curve(TIC) ,which parameters including the basic intensity(BI) ,peak intensity (PI) ,increased signal intensity ( ΔSI) and time to peak ( TTP) . Then the results were analyzed and compared to pathology . Results Within the same duration ,the vascularization degree in rhBMP-2 group was stronger than that in the ordinary group with advanced vascularization time . Positive correlation was detected between ΔSI and time of both groups ( r =0 .938 ,0 .890 ;P =0 .000) ,and negative correlation was found between BI/PI or TTP and time ( BI/PI: r = -0 .798 ,-0 .899 ; P = 0 .000 ;TTP= r -0 .874 ,-0 .868 ;P = 0 .000 ) . No statistical significance was observed among four observation points of both CHA ,which indicated no obvious difference in vascularization degree of each observation point . Conclusions The structure of bone graft can be clearly displayed by conventional ultrasound ,and CEUS is able to show the early blood perfusion in two CHA grafts and to accurately evaluate the difference of CHA microvascular growth before and after rhBMP-2 application . The combination of these two techniques is a promising approach of evaluating bone graft vascularization in clinical practice .
