1.Beta-lactamase Gene and intⅠ1 Gene in ESBLs-producing Klebsiella pneumoniae

OBJECTIVE To investigate the beta-lactamase gene and intⅠ1 gene in ESBLs-producing Klebsiella pneumoniae.METHODS Beta-lactamase gene and intⅠ1 gene were analyzed by PCR.RESULTS The positive rates of TEM,SHV,OKP,CTX-M-1group,CTX-M-2group,CTX-M-9group,GES,PER,VEB,OXA-10,ACT-1,LEN,DHA,and intⅠ1 were 51.4%,0%,0%,17.1%,0%,11.4%,0%,0%,0%,0%,0%,5.7%,17.1%,and 60%,respectively.Beta-lactamase gene was found in 62.9%,intⅠ1 gene was found in 60%.CONCLUSIONS There are very high positive percentages of beta-lactamase gene and intⅠ1 gene in ESBLs-producing K.pneumoniae.
2.Mono-modality image registration based on modified mutual information

With modified mutual information(MI) measure,Powell search algorithm is utilized to realize mono-modality image registration.The MI's formulae are improved to reduce computation cost,and the replace of Powell's search direction is also rectified to keep the original search direction and eliminate local maxima.With rotation set in advance,local maxima can be avoided.The influence on registration velocity by Powell convergence threshold is also discussed.The results show that the modified MI measure can reach sub-pixel precision with obviously accelerated speed.It has good robustness,high speed and high precision.

Extraction process plays an important role in Chinese herbal medicine applications. Traditional extraction method has some disadvantages, such as long processing time, high solvent consumption and low efficiency. Thus, some new technologies and methods have been introduced in this paper, including ultrasonic extraction, microwave extraction, supercritical fluid extraction. On the basis of advantages and disadvantages of traditional Soxhlet extrac-tion, this paper mainly described the principles, characteristics, influencing parameters and applications.

5.Clonal Spread of Carbapenem Non-susceptible Acinetobacter baumannii in an Intensive Care Unit in a Teaching Hospital in China.

BACKGROUND: This study was aimed to investigate the genetic diversity and antibiotic resistance profile of the nosocomial infection agent Acinetobacter baumannii from a medical intensive care unit (ICU) in a teaching hospital in Suzhou, China. METHODS: The genetic relationship among A. baumannii isolates in an ICU was investigated using multilocus sequence typing (MLST). The antibiotic resistance pattern was determined by performing an antibiotic susceptible test, which included an agar dilution method and an E-test method. Resistant determinants, e.g., carbapenemase genes, metallo-beta-lactamases, and class 1 integron, were analyzed by specific PCR and DNA sequencing. RESULTS: In the present study, 33 non-duplicate isolates were identified as 5 existing sequence types (STs) (ST92, ST75, ST112, ST145, and ST345) and 1 new sequence type STn, which has a G-A mutation at nt268 on ropD40 of ST251. These results reveal limited diversity in carbapenem non-susceptible A. baumannii (CNSAb) isolates in our ICU, which are comprised of only 2 distinct STs, with ST92 and ST75 clustering into a clonal complex (CC) 92. Most CNSAb isolates (94.4%, 17/18) harbored the OXA-23 gene, while no carbapenem-susceptible A. baumannii (CSAb) isolates harbored it. In addition, 66.7% (22/33) isolates were positive for class 1 integrase, and gene cassette analysis showed there are 3 gene arrays among them, i.e., aacA4-catB8-aadA1 (77.3%, 17/22), aacA4 (22.7%, 5/22), and aacC1-orfX-orfX'-aadA1 (4.5%, 1/22). CONCLUSIONS: When all these data are combined, the antibiotic resistance and wide distribution of CNSAb isolates in our ICU are probably caused by expansion of the CC92 clone.
6.Evaluation of a Novel Array-Based Toxoplasma, Rubella, Cytomegalovirus, and Herpes Simplex Virus IgG Enzyme Linked Immunosorbent Assay and Its Comparison with Virion/Serion Enzyme Linked Immunosorbent Assays.

BACKGROUND: The dramatic increase in use of the IgG test for toxoplasma, rubella, cytomegalovirus (CMV), and herpes simplex virus (HSV) [TORCH] has led to the requirement for a high-efficiency method that can be used in the clinical laboratory. This study aimed to compare the results of BGI-Array ELISA TORCH IgG (BGI-GBI, China) screening method to those of Virion/Serion TORCH IgG ELISA (Virion/Serion, Germany). METHODS: Serum specimens (n=400) submitted for routine IgG testing by Virion/Serion ELISA were also tested using the BGI-Array ELISA method. The agreements of these two kinds of method were analyzed by kappa-coefficients calculation. RESULTS: Following repeat testing, the BGI-Array ELISA TORCH IgG assays demonstrated agreements of 99.5% (398/400 specimens), 98% (392/400 specimens), 99% (396/400 specimens), and 99.5% (398/400 specimens), respectively. The BGI-Array ELISA IgG assays provided results comparable to Virion/Serion ELISA results, with kappa-coefficients showing near-perfect agreement for the HSV (kappa=0.87), rubella (kappa=0.92) and CMV (kappa=0.93) and substantial agreement for the toxoplasma (kappa=0.80) IgG assays. The use of the BGI-Array ELISA TORCH IgG assays could reduce the turnaround time (1.5 hr vs. 5 hr by Virion/Serion ELISA for 100 specimens) and were easy to use. CONCLUSIONS: BGI-Array ELISA TORCH IgG shows a good agreement with Virion/Serion ELISA methods and is suitable for clinical application.

7. Selection and application opportunity of antibiotics in children with respiratory tract infections

Respiratory tract infection is the most common disease in children, and it is also the most common disease of irrational use of antibiotics.The rational application of antibiotics is to reduce the rate of bacterial resistance and its adverse events.The emergence of drug-resistant strains is closely related to the wide application of antibiotics.In order to reduce the irrational use of antibiotics in children′s respiratory tract infection, pediatricians need to know the timing of application of antibiotics.The respiratory tract infection can be divided into upper respiratory tract infection and lower respiratory tract infection, including common cold, pharyngitis, sinusitis, otitis media, bronchitis and bronchitis, and pneumonia.This article reviews the application timing of antibiotics in the common respiratory tract infection of these children.

Objective To establish the central tube preparation system for the blood samples from hospitalized patients in clinical laboratories and explore its application value.Methods A central tube preparation system for clinical specimen was researched and developed.The system was directly connected with the laboratory information management system (LIMS) and hospital information management system (HIMS) for monitoring and tracking management on-line with standardization,intellectualization and informatization during the whole process of collection,detection and report of clinical specimens.Results The central tube preparation system for blood samples from hospitalized patients in clinical laboratories was successfully established.The development and application of this system optimized the blood collection process,avoided incidental human mistakes in traditional blood collection process,realized the real-time monitoring of the clinical samples during the whole process and reduced the labor intensity of nurses.Therefore,the working efficiency and degree of clinical satisfaction were increased greatly.Conclusion The developed system could process the specimens intelligently,improve the management level of hospital and department and provide guarantee for standardized management of clinical laboratories.
10.Clinical study of limited internal fixation contrasting micro?external fixator in the treatment of fractures around the hand

Objective To analyze the effect of limited internal fixation contrasting micro -external fixator in the treatment of fractures around the hand.Methods Sixty patients with fractures around the hand treated in the First People′s Hospital of Huizhou from May 2015 to May 2017 were selected and randomly divided into the internal fixation group and the external fixation group,and then were treated with effective internal fixation and mini external fixator respectively.The curative effect,operation condition,postoperative recovery and complications of the two groups were compared.Results The treatment effect(excellent and good rate was 96.67%(29/30)),fracture recovery time((6.37 ± 1.25)weeks),hospitalization time((4.32 ±1.23)d)and postoperative complication rate(10.0%(3/30))in the external fixation group were superior to those in the internal fixation group(76.67%(23/30),(8.87 ± 2.12)weeks,(7.29 ± 2.15)d,33.3%(10/30)),the differences were statistically significant(P=0.032;t=15.459,P=0.005;t=17.788,P =0.001;P=0.012).However,there were no significant differences between the external fixation group and internal fixation group in the operation time and the blood loss during operation((28.41±2.87)min vs.(27.67±1.42)min;(16.87 ± 3.71)ml vs.(16.43 ± 2.89)ml)(t=2.459,P=0.423;t=1.788,P =0.619). Conclusion Compared with limited internal fixation,the mini external fixator is reliable and effective,with less complications,and is more conducive to the early activity and functional recovery of the patients with hand fractures.
