1.The influence of progesterone on the extent of brain damage and neurological functions in rats with cerebral infarction

Objective To study the influence of progesterone (PROG) on infarct volume and functional outcome and to evaluate the therapeutic value of PROG on cerebral infarction in rats.Methods Health adult male Sprague-Dawley rats were randomly divided into sham-operated (control) group,ischemic group,vehicle-treated group and PROG-treated group.Permanent cerebral ischemia was induced by occlusion of the left middle cerebral artery (MCA) using an intraluminal filament technique.Sham-operated rats were subjected to the same surgical procedure,except that the filament was not advanced to occlude the MCA.Progesterone or 2-hydroxypropyl-β-cyclodextrin was injected intraperitoneally following permanent middle cerebral artery occlusion (PMCAO) of rats.Zea Longa test was used to evaluate their functional outcome at 1d,2d,3d after stroke.TTC staining was used to detect the infarct volume at 3d after stroke.Results The results of Zea Longa test showed that there were no functional deficits in all animals prior to ischemia.There were no significant changes in motor function in sham-operated animals across the 3 days assessment period.Both PROG and vehicle-treated rats experienced significant decline in scores following occlusion.However,PROG-treated rats (3.00 ± 0.63,2.83 ± 0.75,2.00 ± 0.89 )demonstrated a gradual improvement in scores compared with ischemic (4.00 ± 0.89,3.83 ± 0.75,3.16 ± 0.75 )and vehicle-treated rats ( 3.67 ± 1.21,3.50 ± 1.05,2.83 ± 0.76) at different times (P < 0.05 ).TTC staining revealed that PROG administration significantly reduced the total infarct volume in the PROG-treated rats ( ( 15.03± 3.75) % ) compared with ischemic ( (23.74 ± 4.48 ) % ) and vehicle-treated rats ( ( 24.42 ± 7.07 ) %,P <0.05).Conclusions PROG significantly reduces infarct volume and promotes the recovery of neurological functions after pMCAO,which has good therapeutic value for the rat model of cerebral infarction.
2.Clinical characteristics and pathogenic bacteria analysis of staphylococcal bloodstream infection in 70 patients with secondary immunodeficiency

Objective To understand the clinical characteristics of staphylococcal bloodstream infection in patients with secondary immunodeficiency,as well as the distribution and drug resistance of Staphylococcus aureus (SAU)and coagulase negative Staphylococcus (CNS).Methods A total of 101 patients with Staphylococcus isolated from blood sample were retrospectively reviewed in our hospital.The patients were classified into a group with secondary immunodeficiency and another group with normal immune function as control group.SPSS 18.0 statistical software was used to analyze the data,and compare the two groups of patients in terms of clinical characteristics,pathogens,antibiotic resistance,etc.Results All the 70 patients with staphylococcal bloodstream infection and secondary immunodeficiency had various degrees of fever except 2 cases (temperature < 36 ℃).Chills and shiver were reported in 30.0% of the patients,and migratory lesions in 15.5% of the patients.Lungs were the most common site of infection.About 11.4% of the patients complicated with septic shock.The case strains of SAU,of which 18 (43.9%)strains were MRSA,CNS 29 (41.4%)strains,of which 24 (82.8%)strains were MRCNS.In the control group,26 (83.9%)strains of SAU were identified,of which 11 (42.3%)strains were MRSA,and all the 5 (16.1%)strains of CNS were identified as MRCNS.The incidence of CNS infection in the patients with secondary immunodeficiency was significantly different from that in the control group (P =0.013).All the MRS strains were multidrug-resistant organism (MDRO)except one strain of Staphylococcus intermedius .The staphylococcal strains isolated from the patients with secondary immunodeficiency showed high rate of resistance to penicillin (100%), clindamycin (73.0%), erythromycin (90.0%)and gentamycin (61.0%).No strain was found resistant to tigecycline,linezolid or vancomycin.The resistance rate did not show significant difference between the patients with or without secondary immunodeficiency. Conclusions The clinical characteristics of staphylococcal bloodstream infection in the patients with secondary immunodeficiency are similar to the patients without secondary immunodeficiency,except higher incidence of septic shock and death,but the incidence of CNS infection is relatively higher,MDRO is more prevalent.Antimicrobial agents should be prescribed in clinical practice according to disease conditions and the result of antimicrobial susceptibility testing.
3.The clinical significance for the detection of plasma asymmetric dimethylarginine in patients with cerebral small vessel disease

ObjectiveTo study the relationship between the plasma level of asymmetric dimethylarginine (ADMA) and the damage degree of white matter and the changes of cognitive function in patients with cerebral small vessel disease.Methods120 patients with white matter lesions,induced by cerebral small vessel disease,were randomly selected.All patients were divided into group with or without cognitive dysfunction according to the results of MoCA scale.The plasma level of ADMA was detected with high-perfomance liquid chromatography method.Scheltens semi-quantitative evaluation was used to detect the damage degree of white matter.Correlation analysis was used to evaluate the relationship between the plasma level of ADMA and white matter lesions and cognitive dysfunction.ResultsThere were 42 patients with cognitive dysfunction and 78 patients without cognitive dysfunction.The plasma level of ADMA in patients with cognitive dysfunction ( ( 1.57 ±0.56) μg/ml) was significantly higher than patients without cognitive dysfunction ( (0.89 ±0.23 ) μg/ml) (P < 0.01 ).Scheltens semi-quantitative evaluationfor patients with cognitive dysfunction and patients without cognitive dysfunction was 8.01 ± 2.50and 2.49 ± 0.59,respectively.There was significant difference between this two groups (P < 0.01 ).The plasma level of ADMA positively correlated with the damage degree of white matter lesions and the changes of cognitive function.The correlation coefficients were 0.913 and 0.837,respectively ( All P < 0.01 ).ConclusionADMA may be involved in the process of white matter damage caused by cerebral small vessel disease.The detection for plasma levels of ADMA may contribute to the prevention and early diagnosis of patients with cognitive dysfunction caused by cerebral small vessel disease.

5.Changes of cardiac aldosterone and mineralocorticoid receptor in rats with chronic heart failure induced by isoproterenol

Objective To investigate the changes of cardiac aldosterone and mineralocorticoid receptor (MR) in Sprague-dawley (SD) rats with chronic heart failure (CHF) induced by isoproterenol (ISO). Methods The SD rats were randomly divided into CHF group (n=9) and normal control(NC) group (n=10). The experimental CHF group was induced by subcutaneous injection of ISO, and the NC group received same dose injection of sodium chloride. The heart function was evaluated with both echocardiography and hemodynamics. The contents of aldosterone in both plasma and heart were assessed by radioimmunoassay. The expression of MR was measured by Western blot and immunohistochemistry staining. Results Compared with NC group, the heart function was decreased in CHF group, the left ventricular ejection fraction was (38.8%±4.0%) in CHF and(79. 4%±4.6%), in NC group. The maximal rate of increase of ventricular pressure (+dp/dtmax) was (7164.4±502.6) mm Hg(1 mm Hg=0.133 kPa)/s in CHF and (10199.5±462.9) mm Hg/s in NC group (both P<0. 01 ). The contents of aldosterone both in plasma and heart were higher in CHF group than in NC group [(0.63±0.06)μg/L vs. (0.3±0.07) μg/L, (0.41±0.05) μg/kgvs. (0.08±0.01)μg/kg, both P<0. 01]. The MR expression was increased in CHF group versus in NC group (P<0.01). Conclusions The heart function is decreased in rats with CHF induced by ISO, which is similar to dilated cardiomyopathy. The higher levels of aldosterone both in circulation and in heart as and well as MR expression upregulation in heart may play important roles in the pathogenesis of CHF induced by ISO.
6.Interleukin 33 prevents myocardial ischemia and reperfusion injury in rats by activating P38-MAPK pathway

Objective To investigate the protective mechanism of Interleukin 33 (IL-33) in preventing myocardial ischemia and reperfusion injury in rats. Methods A rat model with myocardial I/R with 32the adult male SD rats , which were randomly divided into 4 groups: SO group , I/R group , IL-33 + I/R group , SB230580 + IL-33 + I/R group. The levels of LDH, CK, TNF-α, IL-6, HMGB1, Bcl-2, total caspase-3, cleaved caspase-3 and P-P38 were detected. Results After reperfusion, IL-33 significantly decreased the levels of serum LDH and CK and the expression of TNF-α, IL-6 , cleaved caspase-3 , but significantly increased the expressions of Bcl-2, p-P38 (P < 0.05). SB230580 attenuated the protective role of IL-33 on myocardial I/R in a certain degree. Conclusions IL-33 may prevent myocardial I/R injury via inhibiting inflammation and cardiocyteapoptosis by way of P38 MAPK signaling pathway.
7.ELECTRON MICROSCOPIC STUDY OF THE CHANGES OF CONNECTIVE TISSUE CELLS DURING THE SKIN WOUND HEALING IN RATS

Open skin wounds were made in 28 Wistar rats, and the changes of the connective tissue cells were observed by electron microscopy during wound healing. The fibroblasts and macrophages were increased in the early stage, later on the fibroblasts might be transformed into myofibroblasts which became preponderant cells in the wound gradually and led to wound contraction and scar formation. The intercellular substances were phagocytosed by fibroclasts simultaneously. The mast cells were closely apposed to the fibroblasts or myofibroblasts and represented active functions, such as degranulation. Considerably they released the granule contents to the extracellular space and effected the other cells functions. It shows that the cells mentioned above interplay and take part in the wound healing.
8.Effects of preconditioning with sodium butyrate on myocardial ischemia/reperfusion injury in rats

Objective To investigate the effect of preconditioning with sodium butyrate on myocardial I/R injury. Methods Anesthetized rats were treated with sodium butyrate (100 or 300 mg/kg, i.p.) 30 mins before ischemia, and then subjected to ischemia for 30 min followed by reperfusion for 4 h. LDH, CK, TNF-α, IL-6, HMGB1, infarct size, MDA and SOD activity were measured. The infracted size was tested by TTC assay; The expression of HMGB1 was observed by western blot. Results After 4 h reperfusion, pretreatment of sodium butyrate (300 mg/kg) could significantly reduce the infarct size and the levels of LDH and CK (P<0.05)comparing to the control group; inhibit the increase of the MDA level and the decrease of the SOD level(P<0.05), also inhibit the expression of TNF-α, IL-6 and HMGB1 (all P < 0.05) induced by I/R. Conclusion Preconditioning of sodium butyrate can attenuate myocardial I/R injury by inhibiting inflammation response.
9.Clinical efficacy of complete mesogaster excision in the radical gastrectomy for gastric cancer

Objective To investigate the clinical efficacy of complete mesogaster excision in the radical gastrectomy for gastric cancer.Methods The clinical data of 100 patients with distal gastric cancer who were admitted to the First Affiliated Hospital of Harbin Medical University from January 2011 to December 2012 were retrospectively analyzed.All the patients underwent complete mesogaster excision in D2 radical gastrectomy for gastric cancer.The operation quality was evaluated according to operation time,volume of intraoperative blood loss,mean number of lymph nodes dissected,time to flatus,volume of drainage and duration of postoperative hospital stay.Patients were followed up by outpatient examination and telephone interview till May 2014.Results Complete mesogaster excision in the radical gastrectomy for gastric cancer was successfully carried out on all the 100 patients.The operation time,volume of intraoperative blood loss,mean number of lymph nodes dissected,time to flatus,volume of drainage and duration of postoperative hospital stay were (118 ± 34) minutes (range,90-160 minutes),(80±25)mL (range,45-135 mL),38± 10 (range,25-52),(3.0 ± 1.2)days (range,1.5-4.5 days),(62±15)mL (range,15-85 mL) and (7.0±1.5)days (range,4.0-11.5 days),respectively.According to the postoperative pathological results,there were 36 patients with high differentiated gastric carcinoma,38 with moderate and/or low differentiated gastric carcinoma,17 with low differentiated gastric carcinoma and 9 with signet ring cell carcinoma.After operation,3 patients had gastroplegia,2 with poor healing of abdominal incision,2 with duodenal stump fistula,1 with pancreatic fistula,and all of them were cured by conservative treatment.All the 100 patients were followed up for a mean time of 25.6 months (range,17.6-39.2 months).There was no tumor recurrence.Conclusions Complete mesogaster excision in the radical gastrectomy for gastric cancer is safe and feasible,with the advantage of minimal trauma,low morbidity and quick recovery during the follow up.
