1.Investigation of Unreasonable Medical Orders at Pharmacy Intravenous Admixture Service of Our Hospital

Objective:To analyze the unreasonable medical orders at the pharmacy intravenous admixture service ( PIVAS) of our hospital to improve clinical medication. Methods: The medical orders from January to October in 2013 were collected and reviewed, the unreasonable medical orders were analyzed and the results were informed to the clinics. Results:Totally 323 unreasonable medical orders were found, which accounted for 0. 03%. The unreasonable aspects included unreasonable compatibility, input error, irregular medical orders, over dosage or concentration, unsuitability between dosage and age and the disproportion between dosage and diagno-sis. Conclusion:The irrationality in intravenous medical orders still exists in clinics, which shows drug use risks. The unreasonable medical orders can be avoided or reduced through the review of medical orders by pharmacists at PIVAS to improve the safety of clinical drug use.
2.Simultaneous Determination of Six Saponins Constituents in Xinling Pills by HPLC-ELSD

Objective: To develop an HPLC-ELSD method for the simultaneous determination of six saponins constituents including notoginsenoside R1, ginsenoside Rg1, ginsenoside Re, ginsenoside Rb1, ginsenoside Rc and ginsenoside Rd in Xinling pills.Methods: HPLC-ELSD was used, and the chromatographic separation was performed on an Agilent Eclipse XBD-C18 column (150 mm×4.6 mm,5 μm) with acetonitrile-water as the mobile phase with gradient elution at the flow rate of 1.0 ml·min-1, the column temperature was maintained at 20℃, the drift tube temperature was 60℃, and the gas pressure was 4.00 bar.Results: Notoginsenoside R1, ginsenoside Rg1, ginsenoside Re, ginsenoside Rb1, ginsenoside Rc and ginsenoside Rd was linear within the range of 0.30-6.00μg(r=0.999 5), 1.14-22.80μg (r=0.999 6), 0.17-3.40 μg (r=0.999 7), 0.81-16.20 μg (r=0.999 7), 0.08-1.60 μg (r=0.999 8) and 0.07-1.40 μg (r=0.999 8), respectively.The average recovery was 98.23%, 97.98%, 99.14%, 99.15%, 98.72% and 98.37%, and the RSDs were 1.56%, 1.31%, 1.16%, 1.07%, 0.73% and 0.92%(n=6), respectively.Conclusion: The method is convenient, accurate and reproducible in the quality control of saponins components in Xinling pills
3.Establishment of Quality Standard System for Gui Erbai Gel Based on a Method of A System to Multiple Evaluation

Objective:To establish the quality standard system of Gui Erbai gel based on a method ofa system to multiple evalu-ation and discuss the feasibility of the method used for the quality standard for traditional Chinese medicine. Methods:TLC identifi-cation of Gui Erbai gel was established by one thin layer system. An HPLC method was used to detect 6 active components in Gui Erbai gel. Results:Five active components in the gel could be identified by one thin layers system simultaneously with clear spots and good separation. Six active components in the gel could be determined by the same HPLC system with high accuracy. The average content of podophylotoxin,quercetin,kaempferol,imperatorin,dictamnine and rutin is as follows 0. 154,0. 052,0. 138,0. 051,0. 060,0. 048 mg· g-1 . RSD<3%. Conclusion:The established method based on a system to multiple evaluation can be used for the quality standard establishment for Gui Erbai gel with the properties of promising feasibility, simple operation, low cost, high accuracy and good stabili-ty.
4.Effect of Effective Fraction from Radix Vitis Davidii Foex. on Fracture Healing

Objective To investigate the effect of effective fractions from Radix Vitis Davidii Foex. on fracture healing. Methods SD rats were randomly divided into model control group,Yunnan Baiyao group,and effective fraction group (n= 36 each group). Fracture models were established. Rats in the Yunnan Baiyao group and effective fraction group were given 3.0 g?kg-1?d-1 of Yunnan Baiyao and effective fraction from Radix Vitis Davidii Foex.,respectively,and rats in the model control group was treated with the same volume of pure water. Alkaline phosphatase and Ca concentration in serum were determined.For radius fracture structure,digital radiography and RT-PCR detection of VEGF mRNA expression were performed. Results The peak concentration of the alkaline phosphatase and Ca concentration in serum were significantly higher in the effective fraction group than in the Yunnan Baiyao group,and peak value appeared earlier,fracture healing score was higher, fracture line disappeared earlier,and VEGF mRNA expression was significantly higher in three weeks after the fracture (all P<0. 01) . Conclusion The effect of the effective fraction from RadixVitis Davidii Foex.on the fracture healing is strong.
5.Screening of active fractions of Jiegu ointment for promoting fracture healing in New Zealand rabbits

OBJECTIVE To screen the active site of Jiegu ointment in promoting fracture healing in New Zealand rabbits. METHODS The ethanol extract of Jiegu ointment, as well as the ethyl acetate and n-butanol parts, were prepared and mixed with honey to form a plaster with appropriate viscosity. The radial fracture model of left forelimb in New Zealand rabbit was established and divided into model control group, ethanol extract group, ethyl acetate fraction group and n-butanol fraction group, with 6 rabbits in each group. Except for model control group, rabbits of all other groups were treated with corresponding polar part of Jiegu ointment for external application, for 4 weeks. The radial fracture healing of rabbits was studied by X-ray examination. Enzyme-linked immunosorbent assay was used to detect the serum levels of interleukin-6 (IL-6), tumor necrosis factor α (TNF-α), osteocalcin (OC), vascular endothelial growth factor A (VEGFA), basic fibroblast growth factor 2 (bFGF2) and alkaline phosphatase (ALP). HE staining was adopted to observe the changes of pathological morphology of rabbit fracture site, and immunohistochemical method was used to detect the protein expression of bFGF2 in fracture site of rabbits. RESULTS The healing speed of the fracture site in the n-butanol fraction group was the fastest, followed by ethanol extract group, and the ethyl acetate fraction group was the slowest; the serum levels of TNF-α and IL-6 in n-butanol fraction group decreased the fastest, while the levels of ALP, bFGF2, OC and VEGFA increased the fastest [significant increase compared with ethanol extract group (P<0.01)]; the chondrocytes at the fracture fraction completely disappeared, forming a large number of bone marrow cavities, and the bone trabeculae in the bone marrow cavity were officially formed. The expression level of bFGF2 was also higher than ethanol extract group. CONCLUSIONS The effect of n-butanol fraction on promoting fracture healing is more significant than ethyl acetate fraction and ethanol extract, and n-butanol fraction is the active fraction of Jiegu ointment to promote fracture healing.
