Purpose:Investigate the clinical value of tumor markers CEA, CA50, CA199 and CA242 for the diagnosis of lung cancer.Methods:The 223 serum sample included 41 cases of squamous cell carcinoma, 46 cases of adenocarcinoma, 14 cases of small cell lung cancer, 16 cases of unclassified carcinoma, 6 cases of metastatic carcinoma, 100 cases of benign lung disease. The levels of CEA, CA50, CA199 and CA242 in the plasm were measured by ELISA. Some data were analyzed by t and ? 2 test. The results were analyzed with regard to sensitivity, specificity, negative prognostic value, positive prognostic value and positive percentage.Results:In all malignant groups the value of CEA was higher than normal value,while in benign group. Objective To explore the feasibility of one-stage operation in periappendicural abscess.Methods The clinical data of 111 patients with periappendicural absecess from 1990 to 2007 were analyzed retrospectively.Results A1l operation were successful.the rate of wound infection was 7.2%(8/111),there was no operative death and no severe complication occurred.Conclusion Timely and appropriate one-stage operation in early periappendicural absecess is safe and feasible.
3.Therapeutic effect of atrovastatin combined irbesartan on cardiovascular remodeling in patients with hypertension

Objective:To analyze the therapeutic effect of atorvastatin combined irbesartan on arterial and ventricular remodeling in patients with essential hypertension (EH).Methods:A total of 114 EH patients of our hospital were selected.According to therapeutic protocol,they were divided into irbesartan group (n=72,only received irbesar-tan treatment)and combined treatment (n=42,received atorvastatin combined irbesartan therapy).After three-month treatment,carotid intima-media thickness (IMT),left ventricular end-diastolic diameter (LVEDd),inter-ventricular septal thickness (IVST),left ventricular end-diastolic posterior wall thickness (LVPWTd)and wall mo-tion score index (WMSI)were compared between two groups.Blood pressure and blood lipid levels were compared between two groups.Results:Compared with irbesartan group,there were significant reductions in IMT [(0.98± 0.13)mm vs.(0.81±0.08)mm],LVEDd [(55.2±3.9)mm vs.(43.3±2.8)mm],IVST [(14.9±2.6)mm vs. (11.5±1.8)mm]and WMSI [(1.67±0.22)vs.(1.39±0.16)],P <0.05 all;there were significant reductions in levels of blood pressure [(144±12)/ (94±8)mmHg vs.(136±16)/ (86±8)mmHg],total cholesterol [(5.87 ±0.96)mmol/L vs.(4.53±0.57)mmol/L]and low density lipoprotein cholesterol [(3.46±0.59)mmol/L vs. (2.68±0.42)mmol/L],P <0.05 all,and significant rise in level of high density lipoprotein cholesterol [(0.87± 0.13)mmol/L vs.(1.36±0.24)mmol/L,P <0.05]in combined treatment group.Conclusion:Atorvastatin com-bined irbesartan can significantly improve blood lipid,arterial and ventricular remodeling in EH patients.

5.Changes and clinical significance of peripheral blood CD34+ cells in patients with acute cerebral infarction

Objective To determine the level of peripheral blood CD34 positive (CD34+) cells in patients with acute cerebral infarction (ACI),and to explore its clinical significance.Methods The level of peripheral blood CD34+ cells was determined by flow cytometry within 72 hours of onset of patients with acute cerebral infarction (infarct group,n=45),cerebrovascular risk factors in patients without cerebral infarction (high risk group,n=27) and healthy subjects (control group,n=20).The neural function defect score,infarction lesion volume and carotid artery intima-media thickness (IMT) were determined in patients with infarction group.Results The percentages of peripheral blood CD34 cells in infarction group (0.034 ±0.012)% and the high risk group of patients (0.047±0.009)% were lower than that of control group(0.063±0.009)%,and were lower in infarction group than in high-risk groups (all P<0.05).The percentages of peripheral blood CD34+cells were significantly decreased compared with control group (P<0.05) in infarction patients with mild [(0.047±0.009)%],moderate [(0.036±0.009)%],severe [(0.022±0.007)%] infarction nervous function defect score.Wherein,the percentages were lower in severe group than in the moderate group,moderate group was lower than in mild group (all P<0.05).The percentages of peripheral blood CD34 cells in infarction patients with small,moderate,large infaret lesion volume were lower than in control group (P<0.05),wherein,were lower in large group than in moderate group,lower in moderate group than in treatment group (all P<0.05).Infarction patients were confirmed with carotid atherosclerosis (CAS) by carotid ultrasound.The extent of lesion were divided into carotid artery intimal thickening group [(0.043±0.010)%],carotid artery plaque group [(0.036±0.010)%],and carotid artery stenosis group [(0.023±0.009)%].The levels of peripheral blood CD34+ cells in three groups of patients were decreased compared with control group.The levels were lower in carotid artery stenosis group than in carotid artery plaque group,lower in carotid artery plaque group than in carotid artery intimal thickening group (all P<0.05).Conclusions The level of peripheral blood CD34+ cells in acute cerebral ischemia is reduced,it can become a sensitive and early indicator of cerebral ischemia,and its level is related to neurologic impairment,infarction size and the degree of carotid artery atherosclerosis.
6.Curative effect of middle and high flow intracranial -external vascular bypass on complex intracranial aneurysms and selection of grafts

Objective To explore the efficacy of middle and high (mid-high) flow intracranial-external vascular bypass in treatment of complex intracranial aneurysms and selection of grafts. Methods The clinical data of 79 patients with complicated intracranial aneurysms treated by mid-high flow extracranial-intracranial bypass in our hospital from August 2010 to October 2017 were collected retrospectively. The grafts were radial artery (n=21), saphenous vein of the calf segment (n=29) or thigh saphenous vein segment (n=29). The efficacy was determined based on Glasgow outcome scale (GOS) scores at discharge and modified Rankin scale (mRS) scores at follow-up, and the differences of occlusion in different types of grafts were analyzed. Results CTA showed patency of the grafts in all patients one d after surgery. There were 6 patients having vascular occlusion: 2 patients (the grafts at saphenous vein of the calf segment ) were occluded 3 and 4 d after surgery, without symptom; 2 patients (the grafts at the radial artery), with decreased limb muscle strength, were occluded 5 and 25 d after procedure; 2 patients ( the grafts at the saphenous veins of the calf segment) were occluded 6 months after procedure without any symptom. There were 4 patients developed cerebral ischemia after operation: one had cerebral infarction and three had vasospasm. GOS scores at discharge and mRS scores at follow-up showed that 78 patients had improved symptoms and good prognosis; one patient showed no improvement in symptoms and plant survival. Conclusion Mid-high flow extracranial-intracranial bypass for treatment of complex intracranial aneurysms is effective; the graft should be individually selected based on preoperative assessment results.
7.Endovascular aortic repair for the treatment of isolated iliac artery aneurysm: a report of 23 cases

Objective To summarize the experiences of endovascular aortic repair (EVAR) for isolated iliac artery aneurysm (ⅡAA).Methods The clinical data of 23 patients with ⅡAA undergoing EVAR from Aug 2008 to Mar 2016 were retrospectively analyzed.There were 5 cases of internal iliac artey (internal iliac artery ⅡA) aneurysm,10 cases of unilateral common iliac artery(common iliac artery CIA) aneurysm,6 cases of bilateral CIA aneurysms.Unilateral ⅡA aneurysm was treated by coil embolization and covering the entrance;CIA aneurysm without involving ⅡA was treated by EVAR with covered stent,those involved unilateral ⅡAs or combined ⅡA aneurysms were treated by EVAR after ⅡA embolization.Those with bilateral ⅡA involvement were repaired by Sandwich technique;Bilateral ⅡA was treated in two phases.Results All endovascular procedures were successfully performed;No patients died during the perioperation period.Intraoperative endoleak found in 5 cases managed by balloon dilatation,or conservatively were cured.23 patients were followed-up for 2-60 months,one rebuilding ⅡA was occluded.One CIA anurysm complicated with endoleak was cured by coils embolization and stenting.Gluteal claudication and erectile dysfunction occured in 4 out of 15 cases with unilateral or bilateral ⅡAs occlusion.Conclusion EVAR is a safe and effective treatment for ⅡAA,after a proper management for ⅡA.
8.Observation on the efficacy of CLAG regimen in treatment of relapsed/refractory acute myeloid leukemia

Objective:To analyze the efficacy and safety of CLAG regimen (cladribine, cytarabine, granulocyte colony-stimulating factor) in treatment of relapsed/refractory acute myeloid leukemia (AML).Methods:The clinical data of 30 patients with relapsed/refractory AML who were admitted to Suzhou Hongci Blood Disease Hospital from January 2018 to February 2021 were retrospectively analyzed, and the efficacy and adverse reactions were observed.Results:Among 30 patients, 1 patient was not evaluated for efficacy due to treatment-related death. Among the 29 evaluable patients, 17 patients (58.6%) achieved complete remission (CR), 5 patients (17.3%) achieved partial remission (PR) and 7 patients (24.1%) had no remission (NR). One patient in low-medium-risk group achieved CR; the CR rate in medium-risk group was higher than that in high-risk group [68.8% (11/16) vs. 41.7% (5/12)], but the difference was not statistically significant ( P = 0.250). Seven patients with M 5 achieved CR. Five patients with positive MLL gene rearrangement achieved CR. Among 6 patients with extramedullary invasion of leukemia cells, 4 patients achieved CR, and among them 3 patients with central nervous system invasion achieved CR. Among 6 patients with cladribine 3.5 mg·m -2·d -1×5 d, 1 patient died of infection after chemotherapy, 1 patient had NR, and the rest achieved CR. Among 24 patients with cladribine 5 mg·m -2·d -1×5 d, the CR rate in patients with cytarabine 2 g·m -2·d -1×5 d was higher than that in patients with cytarabine 1 g·m -2·d -1×5 d [70.0% (7/10) vs. 42.9% (6/14)], but the difference was not statistically significant ( P = 0.240). All patients developed grade Ⅳ myelosuppression after chemotherapy. 12 patients underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT) after CR, and 9 patients were still alive. Conclusions:CLAG regimen is effective and well tolerated in the treatment of relapsed/refractory AML. In order to improve the prognosis, allo-HSCT should be performed as soon as possible after CR.
9.Effects of C10orf10 on growth and prognosis of glioma under hypoxia.

OBJECTIVES:
Glioma is the most common malignant tumor in the central nervous system, and the hypoxic microenvironment is prevalent in solid tumors. This study aims to investigate the up-regulation of genes under the condition of hypoxia and their roles in glioma growth, as well as their impact on glioma prognosis.
METHODS:
The hypoxia-related dataset with glioma was screened in the Gene Expression Omnibus database (GEO), and the differentially expressed genes were analyzed between hypoxia and normoxia through bioinformatics, and chromosome 10 open reading frame 10 (C10orf10) was verified and screened in hypoxia-treated cells through real-time PCR and Western blotting. The Cancer Genome Atlas (TCGA) and the Chinese Glioma Genome Atlas (CGGA) datasets were downloaded to analyze the mRNA expression of C10orf10 in different grades of glioma and its impact on prognosis. The glioma specimens and follow-up data of 68 gliomas who underwent surgical treatment in Xiangya Hospital of Central South University from March 2017 to January 2021 were collected, and real-time PCR was used to detect the mRNA expression of C10orf10 in different grades of glioma, and the Kaplan-Meier method was used to analyze the relationship between the expression C10orf10 and prognosis. The glioma cells, which could interfere the expression of C10orf10, were constructed, and the effect of C10orf10 on the proliferation of glioma cells was evaluated by cell counting kit-8 (CCK-8) and colony formation assays.
RESULTS:
Compared with the condition of normoxia, the expression levels of C10orf10 mRNA and protein were significantly up-regulated in glioma cells under hypoxia (P<0.001), and the mRNA expression level of C10orf10 in glioma tissues was up-regulated with the increase of WHO grade in glioma (P<0.001). Based on Kaplan-Meier survival analysis, the higher the mRNA expression level of C10orf10 was, the shorter the survival time of the patient was (P<0.05). And the expression of C10orf10 mRNA was higher in recurrent gliomas than that in primary gliomas in the CGGA database (P<0.001). Knockdown of C10orf10 could significantly inhibit the growth of glioma cells both under hypoxia and normoxia (both P<0.001).
CONCLUSIONS
The expression level of C10orf10 can promote the proliferation and prognosis of glioma, which is expected to become a prognostic marker and therapeutic target for glioma.

10.Metabonomic Analysis of Wenfei Huaxian Granules Against Idiopathic Pulmonary Fibrosis in Mice

ObjectiveTo investigate the changes of differential metabolites in the serum of mice at different stages of bleomycin sulfate(BLM)-induced pulmonary fibrosis modeling and administration, and the mechanism of Wenfei Huaxian granules(WHG)against idiopathic pulmonary fibrosis. MethodMice were randomly divided into control group, control group of 14 days, model group, model group of 14 days, low-dose WHG group and high-dose WHG group. BLM(0.04 U per mouse)was injected into the trachea of mice in the model group, model group of 14 days, low-dose WHG group and high-dose WHG group, and sterile normal saline was injected into the trachea of mice in the control group and control group of 14 days. Mice of low-dose WHG group and high-dose WHG group were given different doses of WHG by gavage every day after injection of BLM, and mice of control group, control group of 14 days, model group and model group of 14 days were given sterile water by gavage every day. The peripheral blood of mice in the control group of 14 days and model group of 14 days were taken to prepare serum after injection of BLM for 14 days, and the peripheral blood and other materials of mice in the other groups were taken after continuous administration for 28 days. The bronchoalveolar lavage fluid(BALF)was collected for leucocyte differential count, the pathological examination and hydroxyproline(HYP)content determination of lung tissues of mice were performed, and the small molecule metabolites in serum samples of mice in each group were determined by ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS). Principal component analysis(PCA)and orthogonal partial least squares-discriminant analysis(OPLS-DA)were conducted to screen differential metabolites and their biological functions were analyzed. ResultCompared with the control group, a large number of continuous fibrotic foci appeared in the lung tissue of mice in the model group, the alveolitis score, fibrosis degree score and HYP content increased significantly(P<0.01), and the total number of leukocytes, macrophages and lymphocytes in BALF increased significantly(P<0.05). A total of 33 differential metabolites were screened between the control group of 14 days and model group of 14 days, mainly lipid metabolites, which were mainly involved in oxidative damage and inflammatory process. A total of 34 differential metabolites, mainly amino acid metabolites, were screened between the control group and model group, mainly involving nucleic acid damage and inflammatory process. Compared with the model group, the HYP content, fibrosis score and alveolitis score in the lung tissue of mice from high-dose WHG group decreased significantly(P<0.05, P<0.01), and the total number of lymphocytes in BALF decreased significantly(P<0.05). Compared with the model group, 27, 40 differential metabolites were identified in the serum of mice from the low-dose WHG group and high-dose WHG group separately. There were totally 9 common differential metabolites between the model group and low-dose WHG group/high-dose WHG group, which mainly involved in the metabolic pathways of inflammation related lipids metabolism, arginine and tryptophan metabolism, and the change trends in low-dose WHG group and high-dose WHG group were significantly back-regulated compared with the model group. ConclusionWHG can alleviate BLM-induced pulmonary fibrosis, collagen deposition and inflammatory reaction in mice, and its anti-fibrotic effect may be related to the adjusting of inflammatory factors, nitric oxide and oxidative stress related metabolic pathways.
