1.Research review about inflammation promoting neoplasm metastasis
Journal of International Oncology 2010;37(6):422-425
In the tumor microenvironment, inflammatory cells regulate intracellular signaling transduction mediated by the corresponding nuclear transcription factor by releasing inflammatory factors ,which leads to transformation of epithelial cells to mesenchymalcellsand eventually promote tumor metastasis. Because of the intimate relationship between inflammation and tumor metastasis , it would be helpful for further enhancement of effectiveness of tumor therapy if we could better understand both the biological character of tumor and the related inflammatory factors.

Objective To analyze the clinical significance of aberrant hepatic artery which originated from superior mesenteric artery in liver cancer and gastric cancer operations.Method Four hundred liver cancer patients who underwent digital subtraction angiography (DSA) and multislice spiral computed tomography angiography (MSCTA) and 86 gastric cancer patients who underwent MSCTA between June 2008 and June 2010 and operated at the First Affiliated Hospital of Guangxi Medical University were included in this study.Preoperatively, the origins of the aberrant hepatic arteries from superior mesenteric arteries were detected by medical imaging.Postoperatively, immunohistochemistry of lymphoid tissues around the aberrant hepatic arteries of the gastric cancer patients were performed using recombinant human cytokeratin 20 (CK20) and carcino-embryonic antigen (CEA) to determine the incidence of lymph node metastases around the aberrant arteries. Results Among 486 patients,49 patients with liver cancer and 14 patients with gastric cancer had an aberrant hepatic artery which originated from the superior mesenteric artery. The rate was 12.96% (63/486).The hepatic artery ran in front of the pancreas in 2 patients (3.17%) and behind the pancreas in 61 patients (96.83%).Immunohistochemical analyses of CK20 and CEA were negative which revealed no metastases in the lymphoid tissues surrounding the aberrant arteries. Conclusions Aberrant hepatic artery originated from the superior mesenteric artery can be classified into the pre-pancreas type and postpancreas type. The majority of aberrant hepatic artery belonged to the post-pancreas type. The clinical significance of aberrant hepatic artery is that the hepatic hilar lymph nodes should be dissected in liver cancer and gastric cancer operations.

The development of primary or acquired taxane resistance inevitably becomes to be the main problem.Ixabepilone is effective in metastatic breast cancer (MBC) patients including those heavily pretreated or resistant to taxanes.Eribulin has been used for the treatment of MBC patients who have received at least two prior chemotherapy regimens.New microtubule-targeting agents are promising to be effective options for patients progressing after standard taxane-containing chemotherapy.

Objective To detect the expression of FOXM 1 transcription factor in cervical cancer tissues and to evaluate its clini-cal significance .Methods Immunohistochemical staining was employed to detect the expression of both FOXM 1 and Ki-67 in a va-riety of cervical tissues respectively from 38 patients with cervical cancer ,22 with cervical intraepithelial neoplasia (ranging from CINⅠ to CIN Ⅲ) and 17 with normal cervical epithelium .Results Abnormal expression rate of FOXM 1 was respectively 5 .88% , 63 .6% and 92 .1% in normal cervix ,CIN and cervical cancer ;Deregulated FOXM1 expression in cervical cancer tissues was signifi-cantly correlated with patients’ pathological differentiation ,clinical stage ,post-operational recurrence and Ki-67 expression level . Conclusion FOXM1 expression may be correlated with the deregulated proliferation ,malignant progression ,staging and prognosis in cervical cancer .

Objective To explore the effects of low frequency electric stimulation on the clinical manifes- tation and its influence on the level of plasma Endothelin (ET),atrial natriuretic peptide (ANP),thromboxane B_2 (TXB_2),angiotensinⅡ(AngⅡ),Calcitonin Gene Related Peptide(CGRP) and Neuron-specific Enolase(NSE) and its clinical significance in the patients with vertebrobasilar insufficiency(VBI). Methods One hundred and tewenty cases of VBI patients were randomly divided into two groups:a treatment group and a control group. Both groups received routine drug treatment. The treatment group (n=60) was also treated with low frequency electric stimulation at bilateral mastoid processfes in addition. The improved rate of the clinical symptom, the plasma level of ET,ANP,TXB_2,AngⅡ,GRP and NSE were observed after treatment. Results The patients treated in the treatment group had significantly better outcome than the control group in terms of symptom relief. The effective rate was 87.93% and 67.80% in the treatment group and control group, respectively(P

Objective To evaluate the efficacy and safety of management of kidney calculi in non-uronephrosis by percutaneous nephrolithotripsy (PCNL) under ultrasound guidance. Methods From July 2005 to June 2008, 97 cases of kidney calculi in non-uronephrosis were performed by percutaneous nephrolithotripsy. A tube was first inserted into the pelvis through cystoscope, and saline was instilled to dilate collecting system. Antegrade percutaneous access was obtained under ultrasound guidance. A combination of pneumatic and ultrasonic lithotrite was used to disintegrate and remove stone under direct vision. Clinical data including operation time, complications and stone free rate were analyzed retrospectively. Results The perutaneous renal access was successfully established under ultrasound guidance in all patients, immediate phase Ⅰ lithotripsy was performed in 95 cases and delayed phase Ⅱ lithotripsy in 2 cases. Operation time was 70-180 min, average time was (96±23 ) min. The average blood loss was 60 ml (20-500 ml), 4 cases had transfusion during the PCNL and average 400 ml. Minor pyrexia ( < 39℃) was seen in 24 cases, whereas serious pyrexia was noted in 3 cases. Conservatively administered with appropriate antibiotics, the fever disapeared in 27 cases within 5 days postoperatively. Severe complications did not occur during nephrolithotripsy. Stones were cleared completely in 78 out of 97 cases (80.4%)during immediate phase Ⅰ lithotripsy, residual stone fragment was found in 19 cases. Conclusion The management of kidney calculi in non-uronephrosis by PCNL appears to be efficacious and safe under ultrasound guidance.
