1.Relationship among hypertensive cardiac change, kidney disease and hypertensive retinopathy

Objective:To analyze the relationship among hypertensive cardiac change,kidney disease and hyperten-sive retinopathy (HRP).Methods:A total of 100 patients with hypertension,who were diagnosed and treated in our hospital from Jan 2012 to Dec 2013,were enrolled,including 90 HRP patients (34 cases of class Ⅰ,49 cases of class Ⅱ,six cases of class Ⅲ and one case of class Ⅳ)and 10 cases with normal fundus (normal control group). Clinical data of all patients were analyzed,left ventricular mass index (LVMI),carotid intima-media thickness (IMT)and glomerular filtration rate (GFR)etc.were measured.Results:Compared with normal control group, there were significant thickening in IMT [(0.62±0.30)mm vs.(0.87±0.16)mm vs.(1.10±0.28)mm]in HRP class Ⅰ+Ⅱ group,class Ⅲ+Ⅳ group,the IMT of class Ⅲ+Ⅳ group was significantly thickened than that of class Ⅰ+Ⅱ group (P <0.01 all);Compared with normal control group,the incidence rates of cardiac dilation (0%vs.12.05% vs.71.43%)and kidney function damage (0% vs.24.10% vs.85.71%)significantly rose in HRP class Ⅰ+Ⅱ group,class Ⅲ+Ⅳ group,the incidence rates of cardiac dilation and kidney function damage in HRP class Ⅲ+Ⅳ group significantly rose than those of class Ⅰ+Ⅱ group (P <0.01 all).Conclusion:Severity of hyper-tension retinopathy is closely related to hypertensive cardiac dilation and kidney function damage.
2.Cerebral hemodynamic change for patients with hypertension complicated retinopathy

Objective:To explore the cerebral hemodynamic changes of patients with hypertension complicated reti-nopathy.Methods:A total of 77 patients with hypertension complicated retinopathy were enrolled as study group, another 70 patients,who received physical examination during the same period and its results were normal,were treated as normal control group.Index changes of middle cerebral artery (MCA)and anterior cerebral artery (ACA)were compared between two groups,and hemodynamic difference of ophthalmic artery (OA)and central retinal artery (CRA)were recorded.Results:Compared with normal control group:① Mean velocity of MCA and ACA [(64.28±13.29)cm/s,(20.12±7.21)cm/s vs.(129.18±20.14)cm/s,(89.35±10.98)cm/s]signifi-cantly rose in study group ,P <0.01 all;②There were significant reductions in peak systolic velocity (PSV)and end-diastolic velocity (EDV)of OA and CRA,P <0.05 all,and resistance index significantly rose in all classes of study group (P <0.05 all);③There were significant reductions in mean blood flow velocity [(21.62±7.36)ml/s vs.(13.47±5.13)ml/s],maximum blood flow velocity and minimum blood flow velocity ,and significant rise in pulse wave velocity,peripheral resistance,critical pressure,vascular characteristic impedance [(17.35±5.36)kPa ·s-1 ·m-1 vs.(31.68±6.47)kPa· s-1 · m-1 ],and dynamic resistance [(30.59 ± 6.98)kPa· s-1 · m-1 vs. (64.48±12.38)kPa·s-1 ·m-1 ],in basilar artery in study group,P <0.01 all.Conclusion:In patients with hy-pertension complicated retinopathy,cerebral blood supply indexes reduce along with blood pressure rises,CRA blood flow changes can be regarded as a judging basis for disease course development,which deserves highly atten-tion for patients and physicians.

5.Analysis of clinical features of benign paroxysmal positional vertigo secondary to sudden deafness

OBJECTIVE To retrospectively analyze the clinical characteristics of the benign paroxysmal positional vertigo (BPPV) secondary to the sudden deafness (SD) and to explore the possible pathogenetic mechanism, diagnosis and treatment. METHODS 218 cases of the SD in our department were retrospectively analyzed, and 37 patients with primary BPPV during the same period were used as control group. All patients were confirmed by the examination of Dix-Hallpike and the roll check,the primary disease (sudden deafness) of secondary BPPV patients were in line with the Chinese Medical Association diagnostic criteria. RESULTS In all these patients with SD,there were 91 cases with BPPV secondary to the SD. There were 67 cases of BPPV of posterior semicircular canal and 21 cases of BPPV of lateral BPPV semicircular canal. Besides, 3 cases were of multi-semicircular canal affected. All patients with BPPV were diagnosed as the same ears as the SD. The symptoms were alleviated and initial success rate was 68.1%. During 6-22 month follow-ups, total success rate was 100%, and recurrence rate was7.7% (7/91). The hearing recovery of high frequency is worse than that of the low frequency. There were 2 cases of recurrence in lateral BPPV group, 5 cases in posterior BPPV group. All were cured by reposositioning maneuver. All patients with primary and secondary BPPV were cured with Epley maneuver or Barbecue roll maneuver, and the curative effect of them is similar. CONCLUSION The occurrence of BPPV may follow SD,and the majority of BPPV secondary to the SD occurs in the posterior semicircular canal. The canalith repositioning is an effective therapy for secondary BPPV.
6.Clinical observation on the therapeutic efficacy of cyberknife for primary or metastastic retroperitoneal tumors

ObjectiveTo evaluate the early response rate and radiation toxicity of cyberknife in the treatment of primary or metastasticretroperitoneal tumors.MethodsTwenty-eightpatientswith retroperitoneal tumors were treated with cyberknife.The total doses were 2000-6000 cGy ( median 4500 cGy) and biological effective doses were 3750-10080 cGy (median 7680 cGy) in 2-10 fractions (median 5).Of all patients,3 received three dimensional conformal radiotherapy (3DCRT) or intensity modulated radiotherapy (IMRT) boost,1 was treated as second-course radiotherapy,and others were treated with cyberknife only.The survival rates were calculated by Kaplan-Meier method and compared with Logrank test.ResultsThe complete response,partial response,stable disease and progression disease rates were 43%(12/28),6% ( 10/28),18% ( 5/28 ),4% ( 1/28 ),respectively.The overall response rate was 96%.The number of patients who were followed up more than 1,2,3 years were 17,9,7,respectively.The 1-,2-and 3-year local control rates were 92%,86% and 86%,respectively.The 1-,2-and 3-year overall survival rates were 60%,49% and 49%,respectively.The difference between local progression-free survival and overall survival was not significant ( median 9.5 and 12.0 months,x2 =0.17,P =0.680).Moreover,if the patients did not have metastasis elsewhere and local treatment was effective,there was no significant difference between local progression-free survival and progression free survival (median 17 and 11 months,x2 =0.13,P=0.720).Acute radiation-induced side effects (≥ 2 grade) such as fatigue,anorexia,nausea,vomiting and epigastric discomfort occurred in 9,9,7,7 and 2 patients,respectively.Intestinal stenosis of 1 grade occurred in 1 patient.Conclusions Radiotherapy for retroperitoneal tumors with cyberknife has provided a high response rate with minimal side effects.It is a safe and effective local treatment method for retroperitoneal tumors.
7.Exploration of the Profitability Path of Listed Pharmaceutical Enterprises in China Based on DuPont Anal-ysis

OBJECTIVE:To investigate the profitability of listed pharmaceutical enterprises in China,and to investigate the ba-sic profitability path. METHODS:With the help of DuPont financial analysis system,the financial data of the listed companies and other manufacturing industry during 2007-2013 were collected and compared in terms of the return on equity(ROE),profit margin (PM),asset turnover (ATO) and equity multiplier (EM),etc. RESULTS & CONCLUSIONS:During 2007-2013,average ROE and PM of listed pharmaceutical companies both were higher than those of other manufacturing industry(10.00% vs. 6.40%, 11.50% vs. 6.49%);while average ATO and EM were lower than other manufacturing industry (0.65 vs. 0.73,1.90 vs. 2.19), with statistical significance(P<0.05). It has advantages in core earnings and product profitability while disadvantage in asset turn-over and financial leverage for the listed pharmaceutical companies. Listed pharmaceutical companies’gross margin was higher (42.87% vs. 22.07%),but was offset by the higher ratio of expenses to sales(18.24% vs. 5.61%)and insufficient utilization ratio of circulating assets. To change this situation,listed pharmaceutical enterprises should focus on circulation cost saving and the im-provement of assets operation ability,especially enhancing the utilization ratio of current assets.

Objective To observe the effects of low frequency electrical stimulation (LFES) on the proliferation of endogenous brain neural stem cells (NSCs) and on the expression of basic fibroblast growth factor (bFGF)and epidermal growth factor (EGF) in rats with acute cerebral infarction; to explore the therapeutic mechanism of LFES in improving neural function. Methods Fifty-four rats were randomly divided into a LFES group, a placebo stimulation group and a sham-operated group. Each group was further divided into 3rd day, 7th day and 14th day subgroups, with 6 rats in each subgroup. An acute cerebral infarction model was induced in the rats of the LFES and placebo stimulation groups by middle cerebral artery occlusion (MCAO). Three days after the operation, rats in the LFES group began LFES treatment (frequency 30 Hz, pulse width 250 μs, current intensity 3 mA, 10 min/d) ,while the placebo stimulation group was treated identically but without electricity. The rats in the sham-operated group had no special treatment. The expression of nestin positive cells in the subgranular zone of the hippocampus and the subventricular zone was detected by immunohistochemical staining. The expression of bFGF, EGF proteins and mRNA in the ischemic hemisphere was detected by Western blotting and RT-PCR analysis. A screen test was applied to evaluate motor function. Results Nestin-positive cells in the subgranular and subventricular zones of rats in the LFES group increased significantly more than in the placebo stimulation group at the 7th and 14th day. The expression of bFGF, EGF proteins and mRNA in the ischemic hemisphere was up-regulated compared to the placebo stimulation group at the 7th and 14th day. At the 14th day a difference in motor function was observed in rats in the LFES group compared with the placebo stimulation group. Conclusion LFES can promote the proliferation of endogenous brain NSCs and the expression of bFGF and EGF in rats with acute cerebral infarction. It can also improve motor function and enhance neural plasticity in the brain.
