Alpha-lipoic acid is a potent antioxidant.Its protective effects on oxidative stress induced by diabetes and othe factors are mainly achieved via its antioxidant property.Such effects have been observed in cardiovascular system,kidney,and liver.This article reviews the recent advances in the anti-oxidation mechanisms and protective effects of alpha-lipoic acid in peripheral organs.

Objective To analyse effect of different dose of sustained-release theophyline on prealbumin, C-reactive protein and white blood cell count in patients with pediatric asthma.Methods 58 children with acute asthma attack in our hospital were collected.All children were randomly divided into high dose group and low dose group,29 cases in each group.On the basis of conventional treatment, high dose group were treated with high dose theophylline sustained release tablets 5 mg/kg, one times per eight hours,orally,and low dose group was treated with low dose theophylline sustained release tablets,2 mg/kg, one times per eight hours,orally.Two groups were treatment for two weeks.After the treatment, the serum levels of pre albumin, C-reactive protein and veinal blood level of white blood cell count were detected in all children.Results Compared with high dose group post-treatment, the serum level PA was higher in low dose group (P<0.05);the serum level of CRP was lower in low dose group (P<0.05);the veinal blood level of WBC, percentage of neutrophils and percentage of eosinophils was lower in low dose group (P<0.05) .Conclusion Compared with high dose of sustained-release theophyline,the low dose of sustained-release theophyline can significantly reduce the serum CRP level and veinal blood level of WBC, percentage of neutrophils and percentage of eosinophils, improve serum PA level in patients with pediatric asthma, reduce inflammation in patients.

Objective To explore the efficacy of transvaginal color Doppler sonography(TV-CDS)in the diagnosis and differential diagnosis of cornual pregnancy.Methods The characteristics of gray-scale and color Doppler ultrasonography of 30 patients with cornual pregnancy confirmed by surgery and pathology,were studied retrospectively.The position,size,figure,echogenicity and vascular pattern of the pregnancy tissue were evaluated,and the resistance index was calculated.Results The 30 patients were classified into two groups:group A with typical gestational sac and group B with abnormal mass in the area of corner.The overall diagnostic accuracy for these patients was 43.3%(13/3)and classified as 90%(9/10)accuracy for group A and 20%(4/20)for group B.Among the 17 patients who were misdiagnosed or diagnosed unclearly by TV-CDS,one patient in group A was misdiagnosed as a normal pregnancy,11 patients in group B were misdiagnosed as gestational trophoblastic disease and the rest 5 patients in group B were diagnosed unclearly.Conclusions TV-CDS is useful for differential diagnosis of cornual pregnancy.The cornual pregnancy with abnormal mass is usually misdiagnosed,and therefore clinical features and laboratory data should be fully evaluated for its diagnosis.

Objecfive To identify what factors contribute to the occurrence of avascular necrosis (AVN) of the femoral head after pediatric femoral neck fractures. Methods A series of 33 pediatric femoral neck fractures who had been admitted to our institution between June 1994 and April 2006 were retrospectively reviewed. There were 1 case of type Ⅰ,15 type Ⅱ,14 type Ⅲ and 2 type IV according to Delbet classification. The mean age at injury was 10.7(3.7 to 14.1) years, and the mean follow-up was 3.4(1 to 12.2)years. All patients were followed up until union was achieved. AVN occurred in 16 cases, including 8 cases of Ratliff type Ⅰ,3 of Ratliff type Ⅱ,4 of Ratliff type Ⅲ and 1 unclassified. Age at injury, fracture type, degree of displacement, quality of reduction and type of fixation were calculated and analyzed with respect to AVN. Results AVN occurred in 4 cases (36.4%)under 10 years old and in 12 cases (54.5%)10 years old or more. Eight cases (53.3%)of AVN occurred in Delbet type Ⅱ and 6 cases (42.9%) of AVN occurred in type Ⅲ.One case (25.0%)of AVN occurred in non-displaced group and 14 cases(53.8%)in displaced group. Ten cases(50.O%)of AVN occurred in the group whose reduction quality was satisfactory and 6 cases(66.7%)in unsatisfactory reduction group. Thirteen cases (48.1%)of AVN occurred in the unstable fixation group and 2 cases (50.0%)in the stable fixation group. Conclusion Fracture type, degree of displacement, quality of reduction and age at injury can affect the occurrence of AVN of the femoral head after pediatric femoral neck fractures.

Objective To determine the threshold of stroke volume variation (SVV) in determining the volume expansion responsiveness during fluid therapy in patients ventilated with different tidal volumes. Methods Fifty ASA Ⅰ or Ⅱ patients aged 20-75 yr undergoing elective gastrointestinal surgery under general anesthesia were randomly divided into 2 tidal volume groups (n = 25 each):group Ⅰ VT 8 ml/kg (group V1) and group ⅡVT 10 ml/kg (group V2). Radial artery was cannulated and connected to Vigelo monitor for continuous monitoring of cardiac index (CI), stroke volume index (SVI), systemic vascular resistance index (SVRI) and SVV. Internal jugular vein was cannulated for CVP monitoring. Anesthesia was induced with milazolam, propofol, fentanyl and rocuronium and maintained with intravenous propofol and remifentanil infusion. BIS was maintained at 40-50 during anesthesia. The patients were intubatel and mechanically ventilated (VT 8/10 ml/kg, RR 8-12 bpm, oxygen flow 2 L/min). 6％ HES 130/0.4 7 ml/kg was infused iv at a rate of 0.4 ml·kg-1 ·min-1 after induction of anesthesia. MAP, HR, CVP, CI, SVV, SVI and SVRI were recorded before and at 3 min after fluid therapy. The changing rate of SVV (△SVV) and CI (△CI) were calculated. The criterion for effective volume expansion was △CI 15%. The ROC curve for SVV in determring the volume expansion responsiveness was plotted and the diagnostic threshold was determined. Results ROC curve showed that the diagnostic threshold of SVV was 10.5 % in group V1 and 13.5% in group V2. The sensitivity and specificity in determining effective volume expansion were 93.3 % and 75.0 % in group V1 and 87.5 % and 85.7 % in group V2 respectively. The area under the curve for SVV and 95% confidence interval (CI) were 0.946 (0.860-1.031) in group V1 and 0.951 (0.868-1.034) in group V2. △SVV was negatively correlated with △CI in group V1 (=0.553) and V2 (= 0.602). Conclusion The threshold of SVV in determining the volume expansion responsiveness during fluid therapy is 10.5% and 13.5% in mechanically ventilated patients with tidal volume of 8 and 10 ml/kg respectively.
