By means of the observation of acridine orange-stained lysosomes with fluorescence microscope, we studied the intracellular killing and digestive process after alveolar macrophage (AM) ingested living-yeast. Based on the morphological changes of the cells, we divided the process into three stages: the pre-phagosome-lysosome fusion stage, the phagosome-lysosome fusion and killing stage, and the digestive stage. The results revealed that all the values of phagocytic index, phagocytic rate, fusion index and fusion rate of the isolated AM of rabbits infected with living-BCG were higher than those of normal rabbits (P

Objective:For further exploring the feasibility,we have treated the middle and infe- rior femoral bone fracture nonunion by the transplantation of bone flap with ascending and transvers branch of the lateral femoral circumflex vessels.Method:The technique,on the basis of anatomical study have been designed to treat the femoral bone fracture nonunion by the transplantation of bone flap with ascending and transvers branch which pedicle was descendant branch of the lateral femoral circumflex vessel.Results:8 cases of middle and inferior femoral fracture nonunion had been treated by this method.At follow-up of 1 year 5 months to 4 years 6 months,the results have been satisfacto- ry.Conclusions:The method of the transplantation of the bone flap with ascending and transvers branch of the lateral femoral circumflex vessel for the treatment of middle and inferior femoral fracture nonunion has the adventages of simplicity,easiness,reliability.
3.Vacuum sealing drainage facilitates the survival of slender narrow pedicle flap

Objective To probe into the application of vacuum sealing drainage (VSD) in flap with slender narrow pedicle and to observe the survival,the shaping of blood vessel,congested edema and the expression of CD34 in the flap,and to analyze the underlying mechanism of VSD on the flap survival.Methods 64 SD rats were randomly divided into 8 groups:4-hour group,8-hour group,1-day group,2-day group,3-day group,4-day group,and 6-day group.A flap was designed and put on either side of a rat's back.The both width and length of the flap were 0.8 cm and the pedicle sized in the diameter of 3.5 cm.Both sides of the flaps on the rats' backs were randomly arranged into control side and experimental side.VSD was fixed sideways to each group after the shaping of flaps.Each of the equipments was dismantled respectively in the corresponding time after the operation.Afterwards the color and swelling degree of the flaps were observed,a sample was cut and the flap survival area was measured altogether on the 7th day.Flap histology and the expression of CD34 were observed mainly through HE dyeing and immunohistochemistry.The statistical data were treated to figure out the flap survival rate and to determine whether statistical differences existed in the microvessel density (MVD).Results As the time of the negative pressure suction went on,the survival area of the experimental side flap was increasing day by day and came to the maximum on the third day (95±4)%,but no statistical difference was discerned between on the third day and on the sixth day (P>0.05).As the time of the negative pressure suction went on,the MVD in the flap was also increasing.Statistical differences (P<0.05) existed between the experimental side of the instant group,4-hour group and the 8-hour group and those of 3-day group,4-day group,and 6-day group.Conclusions The application of VSD can advance the survival of the flap,whose survival area can reach its maximum within 3 days under application.Besides,it can promote the increase of MVD in the flap.
4.Expression of SDF-1 and CD34 and changes of MVD in survival process of narrow pedicle flap upon hypoxia and ischemia

Objective To detect the expression of SDF-1 and CD34,and MVD changes at slen der narrow pedicle flap of hypoxia and ischemia during the survival process,and to investigate the role of SDF-1 at flap neovascularization.Methods The slender narrow pedicle flaps,which pedicle's length-breadth ratio were 4 ∶ 2,and the flap size of each took an area of 2 cm × 2 cm (group A),3 cm ×3 cm (group B),4 cm × 4 cm (group C),5 cm × 5 cm (group D) and 6 cm × 6 cm (group E),were designed and elevated on each pigs' double dorsum.The flaps were served as a hypoxia/ischemia flap model.The survival area,histologic analysis,SDF-1 and CD34 enzyme-linked immunosorbent assay at the distal flap were evaluated at days 0,3,5,7,and 14 after operation,respectively.Results In the each group of flaps,the expression of SDF-1,CD34 and MVD increased with time,reached the plateau level after 5 days (maximal values of SDF-1:124.80 ± 4.05 ingroup A,137.85 ±3.03 in group B,166.53 ± 2.98 in group C,72.80 ± 2.63 in group D and 62.79 ± 2.20 in group E),7 days ( maximal values of CD34:16.76 ± 0.62,17.60 ± 0.72,18.48 ± 0.55,12.70 ± 0.60,and 11.51 ± 0.70,each group),and 7 days (maximal values of MVD:52.45 ± 2.78,59.34 ± 3.12,61.14 ± 3.35,25.25 ± 3.78,and 24.46 ± 7.46,each group),and then gradually decreased.In the different groups of flaps,when the flap area increased,the expression of SDF-1,CD34 and MVD increased,but the parameters decreased at the area of 5 cm × 5 cm,and the flaps were partial necrosis.Conclusions SDF-1 may play an adjusted role in the survival process of the slender narrow pedicle flap.
5.Expression of HIF-1α and survivin in gastric cancer

Objective To study the expression of HIF-1α and survivin in gastric cancer,and to study their role in tumor angiogenesis,invasion and metastasis.Methods Immunohistochemieal technique was used to detect the expression of HIF-1α and survivin in 54 cases of gastric cancer.Results The expression rate of HIF-lα in gastric cancer (74.1%) was higher than that in normal gastric tissue (O),The expression of HIF-1α in gastric cancer was associated with TNM staging,invasive depth and lymph-node metastasis (P<0.05),The expression of survivin in gastric cancers was high (72.2%),and increased with invasive depth (P<0.01),clinical stages (P<0.01) and lymphatic metastasis (P<0.05).There was a positive correlation between expression of HIF-1α and Survivin (r:0.31,P<0.05).Conclusion HIF-1α and Survivin could be factors in the judgement of the degree of malignancy of gastric cancer and prediction of its prognosis.
6.Expression of platelet-derived growth factor receptor in periapical granuloma

Objective: To investigate the expression of platelet-derived growth factor receptor (PDGF-R) in periapical granuloma.Methods:Immunohistochemical staining was conducted on prepared specimens of 15 cases of human periapical granuloma. Results:The expression of PDGF receptor ? was detected in fibroblasts,capillary endothelial cells,plasmacytes and macrophage cells in all the 15 cases of human periapical granuloma.Conclusion:These results suggested that these cells are the target cells of PDGF, and PDGF may play an important role in the lesion.
7.Plasma exchange treatment using MELD scoring system improve the prognosis of fulminant Hepatitis in Chinese patients

Objective To study prognosis of patients with fulminant hepatitis after plasma ex- change treatment using model for end-stage liver disease(MELD)scoring system.Methods 160 pa- tients were randomly divided into plasma exchange group and control group,and MELD score was calculated according to the original formula for each patient.The efficacy of plasma exchange was as- sessed by mortality and improvement in biochemical parameters and MELD score.Results The levels of total bilirubin(TBIL),INR and MELD score of patients whose MELD scores were between 30 and 40[TBIL,(379.4?40.4)?mol/L; INR,2.5?0.2; MELD,30.8?3.8]were lower than before PE treatment[TBIL,(509.7?64.6)?mol/L;INR,3.5?0.3;MELD,37.3?3.5].The levels of TBIL and INR and MELD score of patients whose MELD scores were higher than 40 [TBIL,(595.6?61.5)?mol/L;INR,3.8?0.4;MELD,39.8?3.5]were lower than before PE treatmem [TBIL, (650.4?66.3)?mol/L;INR,4.4?0.6;MELD,45.2?4.2].The mortality of patients in PE group with MELD score from 30 and 40 was 50.0%,while it was 86.7% in control group,showing significant differ- ence between PE group and control group(P<0.01).The mortality of patients with MELD scores higher than 40 was 91.2% in PE group and 100% in control group,showing no significant difference between these two groups(P>0.05).Conclusions Plasma exchange treatment can decrease the serum TBIL level, INR and MELD score of patients with fulminant hepatitis and improve liver function.Compared with the control group,plasma exchange can significantly decrease the mortality of patients in PE group with MELD score from 30 to 40,but no effect on patients with MELD score higher than 40.
8.The effect of window setting technique on measuring colon disease in CT virtual colonoscopy

Objective To study the value of lung window and soft tissue window in measuring colon diseases with experimental CTVC examination.Methods Twenty-four artificial polypoid diseases were placed with glue in a thoroughly cleaned porcine colon(overall length,150 era)that was distented with air and submerged in a water phantom and scanned 10 times on a 64-MSCT(GE Light speed)with identical scanning parameters.The data were reconstructed every 0.625 mm and reviewed using 2D images, navigation,MPR and 3D volume-rendered images on a GE AW 4.2 workstation.The maximum diameters of the artificial polypoids were measured by two radiologists with more than 10 years working experience in lung window(W = 1000,L =-700)and soft tissue window(W =400,L =40),respectively.The average value of maximum diameters of the artificial polypoids were measured for 10 times and were compared with the actual diameter.Results Twenty-four artificial polypoid diseases and their shape,size,loeation and relation of diseases with bowel wall were all showed excellently.The maximum diameter measured was close to the actual diameter.The average value was mostly less than actual diameter and a few of them more than the actual diameter.The maximum diameter measured in lung window was close to the actual diameter,and there was no difference between the two data sets(t =0.431,P =0.669).There was difference between the maximum diameter measured in soft tissue window and the virtual diameter(t = 2.691,P = 0.010). Conclusion In screening the colon diseases,CTVC is a good method of choice with no insult or less insult, and higher repeatability.The lung window should be chosen in measuring colon diseases in CTVC examination.
9.Preparation and measurement of magnetic adriamycin-verapamil-albumin nanoparticles

Objective To prepare magnetic adriamycin-verapamil-albumin nanoparticles(MAVANs) and measure the physical property of the MAVANs,such as magnetism,drug loading(DL)and size of nanoparticles in order to direct its further experimental study on animals and its application in clinical practice.Methods Commercial cottonseed oil was refined,then mixed with adriamycin,verapamil,human albumin and Fe3O4 magnetic powder according to appropriate proportion,and MAVANs was prepared by using a heat denaturation technique.The size and shape of nanoparticles were determined by electron transmission micro-photography.The magnetic response and structure of the MAVANs were analyzed by upside-down microscope.The standard curve was plotted.The drug loading of verapamil in the MAVANs was quantified by high performance liguid chromatography.The drug loading of adriamycin was quantified by a standard spectrophotonuorometry.Results The nanoparticles featured homogenous size,regular and spherical shape,and good magnetic response.The mean diameter of nanoparticles was 0.5?m.The loading amount of verapamil was 0.8%.The drug loading of adriamycin was 1.25%.Conclusions The heat denaturation technique is feasible to prepare the nanoparticles which can load two kinds of drug with good physical property.The process is simple and the testing condition is easy to control.
