Background and purpose:Tumour cell is recognized and rejected by immunocytes.The local infiltration of immunocytes and the clinical curative effect after the percutaneous microwave coagulation therapy(PMCT) is worthy of further study.We investigated the local immune response after PMCT under ultrasound guidance and evaluated if the local immune response had any influence on the prognosis of patients.Methods:89 cases of patients with hepatocellular carcinoma were treated by PMCT.At the time of the fourth week after PMCT and before PMCT,the patients were given percutaneous biopsy under ultrasound guidance.And the samples were stained by immunohistochemical methods.Then the infiltration of CD4+、CD57+ and CD68+ of local treatment areas were compared before and after the treatment of PMCT.Meanwhile,the patients underwent follow-up to survey the possibility of recrudescence.Moreover,the authors analyzed the relation between the infiltration of immunocytes and recrudescence.Results:A few CD4、CD57 and CD68 positive cells were observed in the cancer stoma and tissues surroundings the liver before PMCT.After PMCT,the immunocytes of CD4、CD57 and CD68 positive cells were significantly increased both in the cancer and the adjacent liver tissue.These immunocytes were enlarged in size.The infiltration of CD4+、CD57+ and CD68+ increased significantly in post-PMCT as compared with those of pre-PMCT(P

Objective To study the macrophage immune response after the percutaneous microwave coagulation therapy (PMCT) under the ultrasound guide. Methods Eighty-nine cases of primary hepatic carcinoma patients were treated by PMCT. At the time of the fourth week after PMCT and before PMCT, the patients were given percutaneous biopsy under ultrasound guide. And the samples were stained by immunohistochemical methods. Then the infiltration of macrophage on local treatment areas were compared before and after the treatment. Moreover the function of immunocytes were observed by electronic microscope. Results The infiltration of macrophage increased significantly in post-PMCT as compared with that in pre-PMCT. And there were more protuberance from the macrophage and much more lysosome and endoplasmic reticulum and mitochondrium observed by electronic microscope. Conclusion The local macrophage cellular immunity can be more improved after the primary hepatic carcinoma treated by PMCT.

4.Questionnaire Survey for Researchers of Medicine Clinical Trial in Guang’anmen Hospital of China Academy of Chinese Medical Sciences

ObjectiveTo understand the status and requirement of the clinical researchers in Guang’anmen Hospital of China Academy of Chinese Medical Sciences (hereinafter referred to as Guang’anmen Hospital).MethodsSelf-filling questionnaire was used. All medical care personnel in the clinical specialty with the GCP qualification were set as respondents. The three aspects of recognition degree of clinical trials quality, initiative of participation and knowledge demand for GCP were investigated. Data were analyzed through SPSS11.5.Results 95.6% of clinical researchers considered that the course of clinical trial was legal;89.9% of clinical researchers believed that the quality of clinical trial was good. Most clinical researchers participated in the clinical trials actively (86.9%) and they considered that the trials were helpful for them to enhance research ability and to issue paper.Conclusion Clinical researchers are familiar with GCP knowledge and can participate in the clinical trials actively. They still expect to learn more.
5.Detection and Plasmid Profile Analysis of ESBLs-producing Klebsiella pneumoniae in Burn Patients

OBJECTIVE To investigate the biological and molecular biological characteristics of Klebsiella pneumoniae in burn patients in order to give the first hand information for preventing and controlling of hospital acquired infections.METHODS The identification was done by Bio-Merieux ATB expression.Antimicrobial susceptibility test was performed with K-B method.The plasmid DNA was extracted by Alkaline Lysis,and separated by electrophoresis on the gel.The ESBLs detection was based on NCCLS.RESULTS The K.pneumoniae from the burn patients and the environment were sensitive to CIP,FOX and IPM,but showed resistance to the rest 12 antibiotics.The plasmid DNA profile analysis showed 3 types,and the relative molecular mass was approximately 4.7?106,3.6?106 and 2.0?106.The molecular biological characteristics showed these pathogens were ESBLs-producing K.pneumoniae,which was different from the control bacteria.At the same time,the pathogens caused the original infection were detected,and they were accordingly Staphylococcus aureus,Streptococcus pyogenes,and Pseudomonas aeruginosa.CONCLUSIONS The outbreak in burn patients is caused by ESBLs-producing K.pneumoniae,which has the same antibiotic resistance spectrum and plasmid DNA profile.This ESBLs-producing K.pneumoniae has the same origin.The pathogen might be transmitted by the case history clips and the door knobs.It was suggested that something must be done to enhance the antisepsis administration in order to prevent the hospital acquired infection.
6.Diagnostic Value of Fine Needle Aspiration Biopsy for Children with Thyroid Nodules

Objective To investigate the diagnostic value of fine needle aspiration biopsy for children with thyroid nodules.Methods Eleven children with thyroid nodule were examined by fine needle aspiration biopsy.The cytologic features of thyoid nodules were observed by the light microscope.The pathological sections of thyroid nodules were acquired and compared with fine needle aspiration biopsy.Results Eleven cases were diagnosed by fine needle aspiration biopsy including nodule goiter 4 cases,papillary thyroid carcinoma 2 cases,follicular adenoma 2 cases,medullary carcinima 1 cases,Hashimoto′s thyroiditis 1 case and cyst 1 case.Compared with pathological section,10 cases were diagnosed correctly and one was misdiagnosis.Conclusion Fine needle aspiration biopsy is a valuable methods to the diagnosis of children with thyroid nodules.
7.Effects of penehyclidine hydrochloride on the diaphragm of rats with omethoate poisoning

Objective To study the morphological changes of diaphragm of rats with omethoate poisoning and the protection of penehyclidine hydrochloride.Methods The experimental model of Wistar rats was made by eliac injection ofomethoate,96 rats was divided randomly into four groups in average:the hrinematched control group (Group NO);the omethoate intoxication matched control group (Group PO);atropine and pralidoxime chloride cure group (Group AC); penehyclidine hydrochlofide and pralidoximc chloride cure group (Group PC).The whole blood cholinesterase (ChE) and creatinekinase (CK) activtitise were measured 2 h after poisoning.To observe the morphological changes of diaphragmat different time from 1 to 7 days.Results All the poisoned rats showed that the diaphragmatic histologic damage of the penehyclidine hydrochloride and pralidoxime chloride cure group was slighter than that of atropine cure group.Conclusion A possible reasons of the respiratory muscle paralysis conduced by AOPP was the putrescence of diaphragm muscle fiber, and penehyclidine hydrochloride could definitely protect the diaphragm of rats with omethoate poisoning,and we could deduce that it prevented from intermediate myasthenia syndrome.
8.Effect of oral mucosa transplantation in the treatment of severe contracted conjunctival sac after ocular prosthesis implantation

AlM: To observe the clinic effect of oral mucosa transplantation in the treatment of severe contracted conjunctival sac after ocular prosthesis implantation .
METHODS: Thirty-three cases ( 33 eyes ) with globe disorders and severe contracted conjunctval sac were operated ocular prosthesis implantation firstly, and conjunctival sac plasty using oral mucosa after 6mo.
RESULTS: Thirty - one cases were successful, no complications appeared. One case had primary ptosis and 1 case had recurrent conjunctival sac contracture.
CONCLUSlON:lt is recognised that the methods of oral mucosa transplantation in severe contracted conjunctival sac after ocular prosthesis implantation are effective on those cases.
9.Investigation on Degree of Familiarity of GCP in Guang’anmen Hospital of China Academy of Chinese Medical Sciences

Objective To investigate the mastery degree of GCP (Good Clinical Practice) in Guang’anmen Hospital of China Academy of Chinese Medical Sciences (hereinafter referred to as“Guang’anmen Hospital”). Methods Totally 338 clinical researchers, covering 18 clinical professional sections and medical clinical trial institutions, in Guang’anmen Hospital with both qualification of clinical trials and GCP certificate were investigated in 23th-27th June 2014. The investigation was in the written answer sheet form, including four noun explanations and 36 multiple choice questions, which were related to GCP. Results 338 researchers could accurately explain the concepts of 4 nouns, and 57 (16.86%) researchers could do all multiple choice questions correctly. 141 (41.72%) researchers chose 1 wrong answer and 89 (26.33%) researchers chose 2 wrong answers. 39 (11.54%) researchers failed to answer 3-4 questions. Only 12 (3.55%) researchers failed to answer 5 or more questions. Conclusion The mastery degree of clinical researchers with qualification of clinical trials and GCP certificate in Guang’anmen Hospital is satisfied. In the future, pertinence and diverse training should be strengthened, in order to improve the overall level of the mastery degree of GCP.
10.Risk factor analysis of idiopathic epiretinal membranes in a routine health check-up group

BackgroundIdiopathic epiretinal membranes(ERMs) is a common eye disease condition that leads to progressive decline of visual acuity. Studying the risk factors relating to this disease will shed light on its pathogenesis and allow opthalmologists to screen the affected individuals among the high-risk population and prepare for prevention and management strategies. ObjectiveThis survey was to investigate the risk factors of idiopathic ERMs in the population undergoing routine health check-ups. MethodsThe clinical data of idiopathic ERMs was obtained from the population of routine health check-ups in Peking Union Medical College Hospital from November 2009 to October 2010. The examination outcomes were compared between the individuals with and without idiopathic ERMs. The demographic and clinical factors associated with idiopathic ERMs were analyzed and assessed using univariate and multivariate logistic regression analyses. Result A total of 27 400 people were included in the survey and idiopathic ERMs were diagnosed in 76 cases. No obvious eye complaint was obtained from the idiopathic ERMs. The number of people affected with idiopathic ERMs was 12 ( 12/11 659 ) in the below 40 years group, 21 (21/4595) in the 51-60 years group and 32 (32/2544) in the over 60 years group. Hypertension, diabetes, diedyslipidemia, renal function insufficiency ,and cataract were found in 42% ,5% ,66% ,6% and 8% of the patients, respectively. The univariate logistic regression analyses revealed that significant correlations were found between age,hypertension,hyperlipidemia and history of cataract( P<0. 01 ). Multivariate regression models showed that the risk of idiopathic ERMs increased in age of 51-60( OR=2. 5,95% CI:1. 2-5.4,P=0.02) and over 60 years( OR =7.3,95% CI:3.4-15.6 ,P<0.01 ) and patients suffering from hyperlipidemia ( OR--2. 1,95% CI:1. 3-3.5, P<0. 01 ). ConclusionsOver the age of 50 years and hyperlipidemia are primary risk factors of idiopathic ERM.
