1.The study on early diagnosis of invasive aspergillosis by the use of ELISA assay and nest PCR

Objective To develop a new specie-specific nest PCR to diagnose the invasive aspergillosis and to estimate the detection of circulating galactomannan by sandwich ELISA in immunocompromised patients. Methods (1) To develop an animal model of IA and collect the whole blood and tissues from the animal model. To extract the DNA from the whole blood and perform nest PCR of the DNA ;To perform the nest PCR of the clinical blood samples from immunocompromised patients (2)To detect the circulating galactomannan from the immunocompromised patients samples by use of sandwich ELISA. Results (1) In animal model, the positive rate of nest PCR and blood culture were 77.8% and 16.7%;The sensitivity and specificity of nest PCR in animal model with invasive aspergillosis are 77.8% and 100%;(2) The sensitivity and specificity of nest PCR in immunocompromised patient were 100% and 83.3%;The sensitivity and specificity of sandwich ELISA in immunocompromised patient were 55.6% and 88.2%. Conclusions The positive rate of the nest PCR was higher than that of the blood culture in animal model. The nest PCR method has good sensitivity and specificity in both animal model and patients. It will be a promising method in diagnosis of invasive aspergillosis. The ELISA method will be helpful in diagnosis of invasive aspergillosis.

6.Inorganic Arsenics Have Different Actions in the Induction of DNA Strand Breaks in Primary Cultured Human Skin Fibroblasts

Objective To investigate the action of inorganic arsenic in the induction of DNA strand breaks in primary cultured human skin fibroblasts. Methods Sodium arsenate and sodium arsenite were used as test inorganic arsenics. DNA strand breaks were assessed by single cell gel electrophoresis assay (SCGE). Results Arsenate at 1~10 ?mol/L dose_dependently induced DNA strand breaks in cells. Arsenate at lower concentrations induced mainly degree I DNA strand breaks, while the proportion of cells with degree Ⅱ DNA strand breaks increased to 50% when treated with 10?mol/L arsenate,but no cells with degree ⅢDNA strand breaks were observed.Cells treated with 1?mol/L arsenite showed no significant increase in DNA strand breaks. At the concentration of 10?mol/L, however, arsenite induced DNA strand breaks with different degrees, and the apoptotic type DNA strand breaks were the major type. Conclusion Sodium arsenate mainly induced general type DNA strand breaks and sodium arsenite induced apoptotic type,beside general type of DNA strand breaks in primary cultured human skin fibroblasts. This could be explained by their different reaction modes with DNA, and further studies were needed.
7.Identificantion of Common Pathogenic Candida Species by PCR-RFLP Analysis

Objective To study the molecular identification method of Candida species and lay the foundation of molecular diagnosis for deep candidiasis. Methods PCR was used to amplify the internal transcribed spacer region of ribosomal DNA from 11 type strains of 9 pathogenic Candida spp. and 39 clinical Candida isolates, and the products were digested with three types of restriction endonucleases (MspⅠ,DdeⅠand HaeⅢ). Results Five patterns of products with different molecular weights were obtained after amplification from 9 Candida spp., and the products were digested with MspⅠ, DdeⅠand HaeⅢ, producing 8, 5 and 4 unique band patterns, respectively. Conclusion PCR-RFLP is reliable, specific and useful for the identification of common pathogenic Candida species.

9.Diagnosis and treatment of blunt liver injury ( a report of 135 cases)

Objective To study the effective method of improving the diagnosis and treatment of blunt liver injury.Methods The clinical data of 135 cases with blunt liver injury hospitalized in Daxing District People' s Hospital from January 2000 to October 2010 were reviewed retrospectively.Results By standard grading of American Society of Trauma Surgery (AAST) in 1994,36 cases were grade Ⅰ,42 cases gradeⅡ,33 cases gradeⅢ,14 cases grade Ⅳ,10 cases gradeⅤ.Fifty-one cases were treated by non-operative therapy,among which 42 cases were crued,9 cases underwent operative surgery.Ninety-three cases were treated by operative therapy,seven cases died.The morbidity related to blunt liver injury was 5.19%.Conclusions Non-operative therapy may be used firstly in patients with stable hemodynamics,but close monitoring should be done for changes of the life signs.Timely and reasonable choice of time and the way of surgery is important to avoid complications and reduce the mortality rate.
10.Three-dimmentional conformal radiotherapy for mediastinal metastasis from esophageal carcinoma after surgery

Objective To evaluate the effect of 3-dimmentional conformal radiotherapy (3-DCRT) for mediastinal metastasis from esophageal carcinoma after surgery.Methods Sixty-three such patients were treated by 3-DCRT. Tumor diameter was: cm 20 patients. Positive supraclavicular lymph node was found in 9 patients. The number of tumor at the upper right, upper left and upper median mediastinum was 27, 27 and 20. All patients received 3-DCRT to the dose of 40?Gy (2?Gy/f, 5f/w), after resetting of portals, the dose was boost to 64~70?Gy by 3?Gy/f, 5f/w. Dose of the patients who had been irradiated was boosted to 50?Gy without further positioning. Results Complete response rate and partial response rate were 43% and 48% one month after the treatment was concluded,with no progression of disease. The 1-, 2- and 3-year survival rates were 75%, 43% and 15%. Conclusions Three-dimmentional conformal radiotherapy, with little side-effects, can markedly improve the local control rate and suvival rate of patients with mediastinal metastasis from esophageal carcinoma after surgery.
