6.Effect and potential mechanism underlying different anesthetic depths on postoperative delirium in the elderly rats
The Journal of Practical Medicine 2017;33(1):63-66
Objective To investigate the potential mechanism of POD in the elder rat model in different depths of anesthesia. Method 120 elderly rats were randomly divided into the A1,A2,A3,B1,B2 and B3 groups. The incidence of POD in the elderly rats was assessed in different depths of anesthesia ,and then the death of neurons, and the expressions of Bid,Bim,Puma and Caspase?3 were detected by Annexin/PI ,real?time PCR and Western blot assay ,respectively. Results Compared with the A1 and A3 group ,the incidence of POD in the elderly rats was decreased and the death of neurons ,and the expressions of Bid ,Bim ,Puma and Caspase?3 were decreased in Group A2(P<0.05). Compared with the preoperative condition, the incidence of POD in the elder?ly rats was increased and the death of neurons, and the expressions of Bid,Bim,Puma and Caspase?3 were in?creased in all groups (P < 0.05). Additionally, similar results were found in the group of inhalational anesthesia. Conclusion The depth of BIS 60 ~ 75/BIS 30 ~ 45 in the elderly rats lead to the increase in the incidence of POD,and that might result from the apoptosis of neurons and the increases of Bid,Bim,Puma and Caspase?3.
7.Study on Serum Levels of Pepsinogen and Gastrin-17 and their Relationships with Helicobacter pylori Infection in Uygur and Han Population with Gastric Cancer in Xinjiang Area
Chinese Journal of Gastroenterology 2016;21(6):348-352
Background:Serum levels of pepsinogen( PG),gastrin-17( G-17)and Helicobacter pylori( Hp)infection are closely associated with gastric cancer. Studies on relationship of serum levels of PG and G-17 with Hp infection in Uygur and Han population with gastric cancer in Xinjiang area are rare. Aims:To investigate serum levels of PG and G-17 and their relationships with Hp infection in Uygur and Han population with gastric cancer in Xinjiang area. Methods:A total of 198 Uygur and 50 Han gastric cancer patients were enrolled, and healthy people with the same race were served as normal controls. Serum levels of PGⅠ,PGⅡ,G-17 were determined by ELISA,PGⅠ/ PGⅡ ratio(PGR)was calculated. Urea breath test and stool Hp antigen test were used to detect Hp infection. Results:Serum PGⅠ level and PGR in Uygur and Han patients with gastric cancer were significantly decreased than those in corresponding control groups(P < 0. 05),and serum levels of PGⅡ and G-17 were significantly increased(P < 0. 05). Serum PGⅠ level and PGR in Uygur gastric cancer patients were significantly decreased than those in Han gastric cancer patients(P < 0. 05),and serum G-17 level was significantly increased(P < 0. 05). Hp infection rate in Uygur and Han patients with gastric cancer were significantly increased than those in corresponding control groups( P < 0. 05). Serum level of PG Ⅰ and PGR were significantly decreased in Han gastric cancer patients with Hp-positive than in Hp-negative patients, and serum levels of PGⅡ and G-17 were significantly increased(P < 0. 05). Serum levels of PGⅠ,PGⅡ and G-17 were significantly increased in Uygur gastric cancer patients with Hp-positive than in Hp-negative patients,PGR was significantly decreased(P < 0. 05). Serum levels of PGⅠ,PGⅡ and PGR were significantly increased in Uygur gastric antral cancer patients than in gastric corpus cancer and proximal gastric cancer patients(P < 0. 05). Conclusions:Decreased serum PGⅠ level,PGR and increased serum level of G-17 can be used as biomarkers to screen gastric cancer in Uygur and Han population in Xinjiang area. Hp infection in Uygur and Han patients with gastric cancer is related to the changes of serum level of PGⅠ,PGR and G-17.
8.Short-term application of noisy pressure support ventilation in patients of AECOPD with respiratory failure
Tianjin Medical Journal 2017;45(3):278-281
Objective To investigate the effects of noisy pressure support ventilation (Noisy PSV) in patients of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with respiratory failure, and to provide basic support for the new weaning mode for AECOPD. Methods A single-center, prospective, randomized, crossover clinical research was conducted. A total of forty-six AECOPD combined with respiratory failure patients who mechanically ventilated more than 24 hours in intensive care unit (ICU) of Tianjin Haihe Hospital from November 2014 to October 2015 were enrolled in this study. When the respiratory failure improved through conventional treatment, patients were mechanically ventilated with conventional PSV and Noisy PSV for two hours respectively in random sequence during spontaneous breathing recovery phase. The pressure support (PS) level was titrated to reach tidal volumes (Vt) about 8 mL/kg in both modes. The level of positive end-expiratory pressure (PEEP) and fraction of inspired oxygen were kept unchanged in both modes. The coefficient of variation of PS during Noisy PSV was set at 30%. Data of gas exchange, hemodynamics, lung functional parameters, breathing patterns and patient-ventilator synchrony were analyzed. Results All of the patients were well tolerated with the two modes, and no adverse reactions were found. The changes of gas exchange, hemodynamics, respiratory mechanics in PSV and Noisy PSV showed a clinical acceptable range. The variability of Vt was significantly increased in Noisy PSV mode than that in conventional PSV mode. While the number of asynchrony events was decreased in Noisy PSV mode than that in conventional PSV mode. Conclusion The Noisy PSV is safe and feasible for patients of AECOPD with respiratory failure, which may be a new weaning mode for AECOPD.
9.Relationship between executive function and depressive symptoms during the tower of London test in patients with major depressive disorder
Chinese Journal of Behavioral Medicine and Brain Science 2017;26(3):247-250
Objective To investigate the relationship between executive function and depressive symptoms in patients with major depressive disorder( MDD) under the Tower of London Test ( TOL) . Meth-ods Thirty depression patients and thirty age?, gender?, education?matched normal controls participated in the study. All subjects received the Tower of London Test. The severity of depressive and anxiety symptoms were assessed by Hamilton Rating Scale for Depression ( HAMD) ,Hamilton Anxiety Rating Scale( HAMA) and Beck Depression Rating Scale( BDI) . Results ( 1) The numbers of correct response and total response of the TOL in MDD(9.1±5.1,12.1±5.3) were significantly lower than those of the control group(13.8±5.0, 17.3±3.9)(P<0.05).The response time of the TOL test in patients with MDD((10.4±2.8)s) was signifi?cantly longer than that of the control group((8.5±2.2)s)(P<0.05). (2)The scores of HAMD ,BDI and HAMA were negatively correlated with numbers of total response( r=-0.403,-0.544,-0.495,) in patients with MDD ( all P<0.05) . Conclusion The executive function of patients with MDD is impaired and nega?tively correlated with depressive and anxiety symptoms under the Tower of London Test.
