1.Road traffic injuries in urban Beijing: a single hospital investigation

Objective To explore regional characteristics of road traffic injury in urban Beijing through analysis of the investigation by a single hospital. Methods Using Emergent Road Traffic Injury Questionnaire we designed, we performed a survey for 2 weeks on all the patients with road traffic injury who had sought medical care in the emergency department of our hospital. A comparison was made between them and other patients at the emergency department at the same period. Results From 8:00 on 25 April 2006 to 8:00 on 8 May 2006, 18 emergent cases of road traffic injury were treated. They accounted for 0.54% of all the emergency patients and 4.5% of all the trauma patients. They were 7 males and 11 females, with an average age of 48.3±14. 3 years. The average time to hospital was (26.8±14.4) rain. Seventeen cases of injury happened in the street, 1 in the highway;15 in Xicheng District, 1 in Haidian District, and 2 in the suburbs. They included 8 pedestrians, 9 riders, and 1 passenger. They were injured by 12 light vehicles, 2 heavy vehicles, 2 special vehicles, and 2 bicycles. Twelve patients had injury at extremity, 3 at spine, and 3 at multiple sites, They received treatment only in Emergency Department. No one was dead or hospitalized. Conclusions Contrary to common belief, road traffic injury patients seem to account for a small part of trauma patients and the injury tends to be moderate. Females and extremities may be more likely to get injured.
2.Mutation analysis in RECQL4 gene in a patient with Rothmund-Thomson syndrome

Objective To detect the mutations in RECQL4 gene in a Chinese patient with Rothmund- Thomson syndrome (RTS). Methods Blood samples were collected from a sporadic patient with RTS, his unaffected parents and 30 unrelated population-matched controls. DNA was extracted, and all the coding sequences of RECQL4 gene were amplified by PCR. Direct sequencing was performed with the amplicons to detect the possible mutations in these subjects. Results Two mutations, i.e., IVS11-1G > A and 3401 A >T, which resulted in a premature termination codon at amino acid 560, were found in the RECQL4 gene of the patient. His father was heterozygous for IVS11-1G > A, and his mother for 3401 A>T. Meanwhile, neither of the two mutations were observed in 30 unrelated normal control individuals. Conclusion Two mutations, including IVS11-1G>A and 3401 A>T are present in the RECQL4 gene of the sporadic patient with RTS.
3.Use of trimetazidine in ischemic heart disease

Ischemic heart disease (IHD) is characterized pathologically by the atheromatous plaque which may induce stenosis or obstruction of the coronary arterial lumen leading to myocardial ischemia, experienced by the patients as chest pain. Plaque rupture provides a focus for platelet deposition and thrombosis, and results in unstable angina or myocardial infarction. Stable angina pectoris is treated conventionally by using beta blockers, nitrates and calcium channel blockers (CCB), all of which act by either increasing the supply or decreasing the demand of oxygen in myocardium. In recent years a group of drugs known as metabolic modulators have been particularly shown to be beneficial in protecting the myocardium. Trimetazidine has been a special target in this group. It has been found that in ischemic myocardium there is high rate of fatty acid (FA) oxidation and subsequent inhibition of glucose oxidation. This drug is thought to have direct cytoprotective action in ischemic myocardium through inhibition of the oxidation of FA. Numerous studies are still being carried out to further clarify its roles in various other conditions related directly or indirectly with IHD. The purpose of the review was to assimilate the results of various trials done all over the world in the recent years to find out the role in IHD and the exact mechanism of action, in order to know its true worth in myocardial protection.
4.Investigation of Rehabilitation Demands of People with Disabilities in Wuxi

MethodsRehabilitation demands of 74387 people with disabilities in Wuxi were surveyed.ResultsThere were significant differences in rehabilitation needs of the disabled from age and different types of disabilities.ConclusionDisability community-based rehabilitation system should be improved according to different demands of disability.
5.Comparison of postoperative parameters between Chinese-made Z-type stainless steel stent and Ni-Ti alloy mesh stent in treating 79 patients with advanced esophageal cancer

0.05). However, incidences of VRS paining grade, nausea and vomiting in Z-type stainless steel stent group were significantly higher than Ni-Ti alloy mesh stent group (P 0.05). All patients in both groups did not have general toxicity and hypersensitivity reaction, and they had normal cell component, number and form of hematopoieticsystem. CONCLUSION: Both Z-type stainless steel stent and Ni-Ti alloy mesh stent are suitable for alleviative treatment of advanced esophageal cancer; however, side effects induced by Ni-Ti alloy mesh stent is less than by Z-type stainless steel stent in an early postoperative stage.

7.Effect of Modified Liujunzi Decoction on Syndrome of Spleen Deficiency and Phlegm Stagnation in Patients with Type 2 Diabetes Mellitus

Objective To observe the effects of modified Liujunzi Decoction on improving isletβ-cell function of patients with the syndrome of spleen-deficiency and phlegm-stagnation of type2 diabetes mellitus.Methods Totally 72 patients were selected and divided into treatment group and control group randomly, with 36 cases in each group. Both groups were under diet adjustment and appropriate exercise and were given metformin hydrochloricle tablets, 0.5 g each time (1.0 g each time after 2 weeks), twice a day; acarbose, 50 mg each time, three times a day. The treatment group was given modified Liujunzi Decoction additionally, 200 mL each time, three times a day, orally. The course of treatment of the two groups was three months. Blood glucose, C peptide, INS, HbA1c, HOMA-IR, HOMA-β andΔI30/ΔG30 after 30 minutes of glucagon loading was recorded.Results Compared with before treatment, FBG, 30 min PBG, 2h PBG, HbA1c, FINS, fasting C-peptide, and HOMA-IR decreased in both groups, and the 30 min INS, 30 min C peptide, andΔI30/ΔG30 increased in the treatment group, with statistical significance (P<0.05). After treatment, there was statistical significance in FBG, 30 min PBG, 2 h PBG, HbA1c, fasting C-peptide, FINS, 30 min INS, 30 min C peptide, HOMA-IR, andΔI30/ΔG30 between the two groups (P<0.05).Conclusion Modified Liujunzi Decoction combined with Western medicine can better improve isletβ-cell function of patients with the syndrome of spleen-deficiency and phlegm-stagnation of type2 diabetes mellitus than Western medicine merely.
8.The impact of combined laparoscopic and choledochoscopic treatment on biliary leak in hepatic hydatid disease in a high attitude region

Objective To explore the impact of combined laparoscopic and choledochoscopic treatment on bile leak in hepatic hydatid disease at a high attitude region.Methods The clinical data of 50 patients who underwent combined laparoscopic and choledochoscopic treatment were compared with 50 patients who underwent laparoscopic surgery alone for hepatic hydatid internal capsule excision surgery.Results In the combined laparoscopic and choledochoscopic group,the incidence of biliary fistula after surgery was 15%,the incision infection rate was 13%,the length of hospitalization was (11.6 ± 3.8) days and the postoperative time to take off drain was (14.3 ± 7.9) days.These were significantly better than those in the laparoscopic alone group.The operation time in the former group was (108.2 ± 28.4)min,which was slightly longer than the laparoscopy group.Conclusion The combined laparoscopic and choledochoscopic treatment is a safer therapeutic option in treating hepatic hydatid disease with less trauma,faster recovery and shorter hospital stay than laparoscopic surgery alone in a high attitude region.
9.Meta-analysis of Fasudil Hydrochloride Injection in the Treatment of Aneurysm Postoperative Cerebral Vasospasm

Objective:To evaluate the curative effect and safety of fasudil hydrochloride injection in the prevention and treatment of aneurysm postoperative cerebral vasospasm by meta-analysis. Methods: The randomized controlled trials were retrieved from PubMed, EMBASE, Cochrane Library, VIP, Wangfang, CNKI and so on. Meta-analysis was conducted using RevMan 5. 0 software. Results:Totally 418 reference studies were screened, from which 11 ones were chosen including 786 patients in all. In the treatment of cerebral vasospasm (CVS), there was no significant difference between the groups (OR=1. 56, 95%CI:0. 95-2. 58, P>0. 05). While in the prevention of CVS, the incidence rate of CVS in fasudil group was significantly lower than that in nimodipine group ( OR=0. 43, 95%CI:0. 23-0. 81, P=0. 008). However, the incidence rate of ADR in fasudile group was higher than that in nimodipine group (OR=0. 43, 95%CI:0. 25-0. 75,P=0. 003). Conclusion:In the prevention of CVS, fasudil may be better than nimodipine, while the incidence of ADR is higher.
10.Surgical therapy of radial head fractures combined with ulnar collateral ligament injuries

ObjectiveTo analyze treatment methods for radial head fractures combined with ulnar collateral ligament injuries and discuss the related effects.MethodsA total of 38 patients undergone operations for radial head fractures combined with ulnar collateral ligament injuries from August 2004to January 2011 at People' s Hospital of Peking University were followed up and 24 patients had complete follow-up data.According to the modified Mason typing,there were six patients with Mason-Johnston type Ⅱ fractures,15 with type Ⅲ and three with type Ⅳ. Conventional screws,Herbert screws,hollow screws,absorbable screws and mini-type plates were used for fracture fixation respectively.Severe communicated fractures were treated with radial head replacement and the injured collateral ligament complex with direct suture or ligament reconstruction.Results The duration of follow-up was 10-84 months (average 37.1 months).According to the Mayo Elbow-Performance Score,the results were excellent in 18 patients,good in four and fair in two,with excellence rate of 92%.ConclusionsFor the radial head fractures combined with ulnar collateral ligament injuries (severer than Mason type Ⅱ ),the first choice should be radial head reconstruction.If the fractures cannot be reconstructed,the radial head replacement is the best choice.If there still exists the elbow valgus instability after radial head reconstruction,the ulnar collateral ligament should be repaired or constructed.Primary repair and reconstruction of the radial and ulnar collateral ligaments is necessary when the radial head replacement is performed.
