1.An Approach to the Technique of Double Spiral CT Cerebral Angiography

Objective To explore the technique of double spiral CT cerebral angiography in order to improve the quality of CTA.Methods All 62 cases underwent cerebral CTA,including vascular lesions in 30,normal in 20 and others in 12.All were scanned with GE Hispeed NX/I CT,three-dimensional models were used in images reconstruction.Results The branches of 1~4 grades of cerebral arteriae could be displayed well in all cases,some cases could also be visible up to 5th grades.All cerebral vascular lesions could be showed by CTA and the same as DSA.Cerebral tumor could be demonstrated with tumor stain.The image quality would be better when the pitch was smaller with the same layer.Conclusion The factors that influence on the quality of CTA images are a lot of,but mostly are concerned with the pitch,contrast volume,injection rate and delayed time.
2.Imaging Analysis for Giant Hypertrophy of the Gastric Mucosa

Objective To improve the understanding of giant hypertrophy of the gastric mucosa.Methods Giant hypertrophy of the gastric mucosa in 6 cases proved by operation or gastroscopy biopsy pathological examination were reviewed.Barium meal examination was performed in all cases,CT in 3 cases.B-type ultrasound in 2 cases.Results It was ease to be misdiagnosed giant hypertrophy of the gastric mucosa on the basis of the conventional imaging appearances.Laboratory data and gastroscopy biospsy played an important role in diagnosis and differential diagnosis.Conclusion Giant hypertrophy of the gastric mucosa is uneasily to correct diagnosed,comprehensive evaluation is necessary for it.
3.Determination of taursodeoxycholic acid and taurchenodeoxycholic acid in Longze Xiongdan capsules by HPLC-ELSD

Objective: To establish a method for determining the content of bear bile powder in Longze Xiongdan capsules with taursodeoxycholic acid(TUDCA) and taurchenodeoxycholic acid(TCDCA) as indexes.
Methods: HPLC series evaporation photodetector was adopted on Chrom Core AQ C18 column(4.6 mm×250 mm, 5 μm), with the mobile phase comprising of acetonitrile ( A ) and 5 mmol·L-1 ammonium acetate solution (B) in a gradient elution (0-40 min, 25%A; 40-50 min, 25%A→29%A; 50-80 min, 29%A; 80-100 min, 29%A→40%A) at the flow rate of 1.0 mL·min-1. The column temperature was 30 ℃. The ELSD was used, of which the drift tube temperature was 110 ℃ and the flow rate of carrier gas(N2) was 2.5 L·min-1.
Results: In the ranges of 1.069-9.57 μg and 0.740 46-7.404 64 μg, logarithms of the injected amount of TUDCA and TCDCA presented good linear relationships with logarithms of the peak area, respectively. The RSDs of precision, repeatability and stability tests were all lower than 2.0%. At three concentration levels the recoveries of TUDCA and TCDCA were 95.2%-97.7% and 91.9%-95.9%, respectively. Samples of 42 batches showed that the contents of TUDCA and TCDCA were 0.18-0.43 and 0.10-0.44 mg·granule-1, respectively.
Conclusion: This method can be used for the quality control of bear bile powder in Longze Xiongdan capsules, thus provides a scientific basis for improving its quality standard.
4.CT and MR findings of TolosaGHunt syndrome

Objective To analyze CT and MRI features of Tolosa-Hunt syndrome (THS),to improve the diagnostic accuracy of this lesion.Methods CT and MRI findings of 1 6 patients with THS confirmed clinically were studied retrospectively.Results Eleven out of 1 6 cases’images were abnormal,including unilateral sinus cavernous enlarged asymmetrically,irregular shapes,the outer edges being smooth and straight or slightly bulging,and the inner edges without clear boundary.The maximum diameters of the lesions varied from 1.2 to 3.8 cm,with an average of (2.3 ± 0.9)cm.Ten cases of CT examination showed uniform densities with significant enhancement after enhanced scan.Six patients underwent MR scan and showed equal signals on T1 WI with equal or low signals on T2 WI,equal signals on DWI in 3 patients and obvious enhancement after contrast agent was inj ected.Among 6 cases with MR examination,5 were found to spread along the ipsilateral optic nerve to the orbital apex,4 temporal dura mater were thickened locally and obvious enhancement, 2 super petrosal veins were thickened,and 1 middle cerebral vein was thickened.Conclusion The diagnosis of THS should be considered when the unilateral cavernous sinus is increased asymmetrically,and other diseases are excluded.The helpful imaging feature includes relatively uniform density or signal of the lesion,significant enhancement,the lesion spreading along the optic nerve to the tip of the sac,and the thickening of temporal dura mater and super petrosal vein in the same lateral.The display of THS by MR seems to be superior to CT.
5.Genetic analysis of a case of B-acute lymphoblastic leukaemia with double Philadelphia chromosomes and double derivative chromosome 9s.

OBJECTIVE:
To explore the genetic basis for a rare case of acute B-lymphocytic leukemia (B-ALL) with double Philadelphia chromosomes (Ph) and double derivative chromosome 9s [der(9)].
METHODS:
A patient with double Ph and double der(9) B-ALL who presented at Shanghai Zhaxin Intergrated Traditional Chinese and Western Medicine Hospital in June 2020 was selected as the subject. Bone marrow morphology, flow cytometry, G-banding karyotyping, fluorescence in situ hybridization (FISH), genetic testing and chromosomal microarray analysis (CMA) were used to analyze bone marrow samples from the patient at various stages.
RESULTS:
At initial diagnosis, the patient's bone marrow morphology and flow immunotyping have both supported the diagnosis of B-ALL. G-banded karyotyping of the patient indicated double Ph, in addition with hyperdiploid chromosomes involving translocations between chromosomes 9 and 22. BCR-ABL1 fusion gene was positive. Genetic testing at the time of recurrence revealed presence of a heterozyous c.944C>T variant in the kinase region of the ABL1 gene. FISH showed a signal for ABL1-BCR fusion on both chromosome 9s. CMA showed that the mosaicism homozygosity ratio of chromosome 9 was about 40%, and the mosaicism duplication ratio of chromosome 22 was about 43%.
CONCLUSION
Since both der(9) homologs were seen in 40% of cells, the possible mechanism for the double der(9) in this patient may be similar to that of double Ph, which might have resulted from non-disjunction during mitosis in the Ph chromosome-positive cell clone.

6.Clinical analysis of 39 patients with parapneumonic pleural effusion caused by streptococcus anginosus group

Objective:To explore the clinical characteristics, chest imaging manifestations, RAPID score and therapeutic situation in patients with parapneumonic pleural effusion (PPE) caused by streptococcus anginosus group (SAG), in order to provide help for the early diagnosis and treatment in clinical practices. Methods:The clinical data of 39 patients with PPE caused by SAG from January 2015 to May 2020 in Affiliated Hospital of Jining Medical University and Jining First People′s Hospital were retrospectively analyzed. The patients were classified by RAPID score.Results:Among 39 cases, males was in 31 cases (79.5%), females in 8 cases (20.5%), and aged 46 to 89 (65.31±10.53) years old. Fever was in 27 cases (69.2%), chest pain in 19 cases (48.7%), and dyspnea in 18 cases (46.2%). The chest CT findings showed consolidation shadows was in 30 cases (76.9%), encapsulated pleural effusion in 21 cases (53.8%), ground glass shadow in 18 cases (46.2%), nodules in 12 cases (30.8%), atelectasis in 8 cases (20.5%), and pneumothorax in 5 cases (12.8%). The complexity PPE was in 23 cases (59.0%), and empyema in 16 cases (41.0%). The microbiological culture results showed that streptococcus constellatus was detected in 25 cases (64.1%), streptococcus anginosus in 13 cases (33.3%), and streptococcus intermadius in 1 case (2.6%). After comprehensive treatment, 36 cases (92.3%) were improved, 3 cases (7.7%) died. According to the RAPID score, low-risk was in 13 cases (33.3%), intermediate-risk in 16 cases (41.0%), and high-risk in 10 cases (25.7%). The RAPID score in patients with low-risk, intermediate-risk and high-risk was (1.85 ± 0.38), (3.43 ± 0.51) and (5.30 ± 0.67) scores, and there was statistical difference ( F = 124.88, P<0.05). the length of stay in patients with low-risk, intermediate-risk and high-risk of RAPID score was (16.84 ± 5.57), (16.56 ± 7.05) and (28.20 ± 17.97) d, and there was statistical difference ( F = 4.41, P<0.05); the length of stay in patients with high-risk was significantly longer than that in patients with low-risk and intermediate-risk, and there was statistical difference ( P<0.05), there was no statistical difference between intermediate-risk patients and low-risk patients ( P>0.05). Conclusions:SAG, as important pathogens for the PPE, tends to induce CPPE and even pyopneumothorax. Clinical manifestations and imaging are not specific, which should be payed attention in clinical work. The patients with high-risk of RAPID score have more serious condition and worse prognosis.
7.Efficiency and safety of belimumab in the treatment of lupus nephritis in Chinese adults:a meta-analysis

OBJECTIVE To systematically evaluate the real-world effectiveness and safety of belimumab in the treatment of lupus nephritis (LN) in Chinese adult patients. METHODS Retrieved from PubMed, Embase, Web of Science, Cochrane Library, Wanfang data, CNKI, VIP and CBM, real-world studies on belimumab in the treatment of LN in Chinese adult patients were collected from the inception to July 7th, 2023. Two reviewers independently screened the literature, extracted data, and assessed the quality of the included studies. Meta-analysis was then performed using RevMan 5.3 software. RESULTS A total of 10 real- world studies were included, involving 253 Chinese adult patients with LN. The results of the meta-analysis demonstrated that the complete renal response rate, partial renal response rate, and the incidence of adverse reaction rate in Chinese adult patients with LN treated with belimumab were 61% (95%CI was 46%-76%, P<0.000 01), 23%(95%CI was 2%-44%, P=0.03), and 30% (95%CI was 16%-43%, P<0.000 01), respectively. Belimumab could reduce the 24-hour urinary protein (MD=-1.71, 95%CI was -3.02--0.40, P=0.01), urine protein-creatinine ratio (MD=-1.76,95%CI was -2.06--1.46,P<0.000 01), the systemic lupus erythematosus disease activity index (MD=-8.63, 95%CI was -12.12--5.13, P<0.000 01), and glucocorticoids dosage (MD=-18.65, 95%CI was -31.82--5.48, P=0.006). In addition, it could elevate the levels of complement C3 (MD=0.19, 95%CI was 0.08-0.30, P=0.000 6) and complement C4 (MD=0.06, 95%CI was 0.02-0.09, P=0.001). However, belimumab could not improve the levels of serum creatinine and estimated glomerular filtration rate (P>0.05). CONCLUSIONS Belimumab has good efficacy and safety in Chinese adult patients with LN.
