1.Adjustment of immunosuppression and prevention of pulmonary infection in recipients of renal transplantation

Objective To investigate the adjustment of immunosuppression and prevention of infections in lung after renal transplantation.Methods 253 cases of pulmonary infections after renal transplantation between January 1999 to March 2004 were analyzed. After 2003, postinduction period, the blood concentrations of CsA and FK506 were maintained at 0.125~0.166 ?mol/L and 5 ?g/L respectively. The oral dose of MMF was 1 g every day. Results The creatinine levels were stable 253 patients after treatment. Among 253 cases, 229 cases were cured (90.51 %) and 24 died (9.49 %). After adjustment of immunosuppressants, common pulmonary infections and severe pulmonary infections were decreased.Conclusions Infection in lung was a severe complication after renal transplantation. Timely treatment, nursing care and adequate adjustment of immunosuppression may achieve satisfactory effects in preventing lung infection.
2.Renal angiomyolipoma with spontaneous rupture(report of 23 cases)

Objective To investigate the clinical features,diagnosis and treatment of spontaneous rupture of renal angiomyolipoma (RAML). Methods A total of 23 cases of spontaneous rupture of renal angiomyolipoma were studied retrospectively.The main clinical manifestations were summarized as follows.Sudden and unexpected lumbar and abdominal pain occurred in 21 cases and lumbar swollen pain in 2.Of them 3 cases concomitantly had hematuria.Nausea and vomiting occurred in 2 cases,shock in 3 and fever in 1.All cases underwent B-ultrasonic examination and 21 had CT examination. Results Diagnosis of RAML was established preoperatively in 16 cases,but 4 cases were misdiagnosed for carcinoma of kidney,1 case for renal infectious disease.Two cases were diagnosed by frozen-section sample during operation.Nephrectomy was performed in 10 cases and nephron sparing nephrectomy in 12,and digital subtraction angiography (DSA) embolization in 1. Conclusions Spontaneously ruptured RAML often changes its clinical symptoms and imaging features,which may result in a high misdiagnosis rate and subsequent nephrectomy.Comprehensive analysis of clinical manifestations and image findings can improve preopreative diagnosis rate,and frozen slice sample examination during operation can avoid unnecessary nephrectomy.
3.Expression of immune effector molecules in renal allografts and its significance

Objective To define the relationship between genes expression of 5 immune effector molecules and acute rejection of renal allografts. Methods The magnitude of intragraft Fas, Fas ligand (FasL), granzyme B(GB) , perforin(P) and T cell intracellular antigen-1 (TIA-1) mRNA were quantified by competitive PCR in 42 samples. Results were expressed as the ratio of picograms of target gene to picograms of ?-actin, and were compared with the histological diagnosis based upon Banff criteria. Results Of all 42 tissues, the transcripts were detectable in 41 for Fas, 11 for FasL, 19 for GB, 33 for P and 27 for TIA-1. Intragraft expression of FasL, GB, P and TIA-1 but not Fas was significantly elevated in acute rejection group as compared with chronic rejection group ( P
4.Observation of the Therapeutic Effect of Pagosid on Prostatodynia

OBJECTIVE:To evaluate the therapeutic effect of pagosid on prostatodynia.METHODS:30patients with prostatodynia were evaluated;pagosid was taken orally1tablet t.i.d and NIH-CPSI was scored before treatment and q.w.4weeks after treatment.The results were evaluated.RESULTS:Effective rates from the first to the fourth week were16.7%,33.3%,40.0%,76.7%respectively.No obvious adverse reactions were found in the period of treatment.CONCLUSION:Pagosid has obvious effect on prostatodynia with pain as chief complaint,and few adverse reactions will develop.However it is slower to take effect,and therefore nonsteroid analgesic is recommended in the early stage of treatment.
5.Clinical study of early use of neuromuscular blocking agents in patients with severe sepsis and acute respiratory distress syndrome

Objective To observe the clinical effects of early use of neuromuscular blocking agents (NMBA) in patients with severe sepsis and acute respiratory distress syndrome (ARDS).Methods A prospective study was conducted.96 patients with severe sepsis and ARDS admitted from July 2012 to September 2013 to intensive care unit (ICU) of Liuzhou People's Hospital in Guangxi Zhuang Autonomous Region were enrolled and divided into severe ARDS group (n=48) and moderate ARDS group (n=48) according to the Berlin definition of ARDS.Then patients in each group were randomly divided into treatment group (n =24) and control group (n=24).All patients with diagnosis in accordance with the 2008 international septic shock and severe sepsis treatment guidelines were provided with comprehensive treatment and mechanical ventilation on the basis of analgesia and sedation.The patients in treatment group were given a loading dose of vecuronium during mechanical ventilation,started with 0.l mg/kg up to 0.05 mg ·kg 1 ·h 1 for continuous intravenous infusion for 24-48 hours.The acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score,sequential organ failure assessment (SOFA),arterial oxygenation index (PaOfFiO2),central venous oxygen saturation (ScvO2),arterial blood lactate (Lac),C-reactive protein (CRP) levels of two groups were compared before treatment and 48 hours after treatment,and 21-day mortality rate was finally compared.Results In moderate or severe ARDS group,there were no statistically significant difference in APACHE Ⅱ score,SOFA score,PaO2/FiO2,ScvO2,Lac and CRP before treatment between two groups.APACHE Ⅱ score,SOFA score,PaO2/FiO2,ScvO2,and Lac 48 hours after treatment were significantly improved in severe ARDS group compared with control group [APACHE Ⅱ score:16.58 ± 2.41 vs.19.79 ± 3.52,t=3.679,P=0.010; SOFA score:12.04 ± 2.17 vs.14.75 ±3.26,t=3.385,P=0.010; PaO2/FiO2 (mmHg,1 mmHg=0.133 kPa):159.31 ±22.57 vs.131.81 ± 34.93,t=3.239,P=0.020; ScyO2:0.673 ± 0.068 vs.0.572 ± 0.142,t=3.137,P=0.030; Lac (mmol/L):3.10 ± 1.01 vs.4.39 ± 1.72,t=3.161,P=0.030],while the value of CRP (mg/L) showed no significant difference (180.91 ±37.14 vs.174.66 ± 38.46,t=0.572,P=0.570).21-day mortality in treatment group was significantly lower than that in control group [20.8% (5/24) vs.50.0% (12/24),x2=4.463,P=0.035].In moderate ARDS group,each of the above clinical parameters were improved in both groups expect for CRP at 48 hours after treatment,but the indexes showed no statistically significant difference between two groups (all P>0.05).21-day mortality rate in the treatment group was slightly lower than that in the control group which showed no statistically significant difference [16.7% (4/24) vs.25.0%(6/24),x2=0.505,P=0.477].Conclusion The early use of NMBA treatment of patients with severe sepsis and severe ARDS cannot only improve the severity but also reduce 21-day mortality.
6.A single-center 10-year report containing 94 kidney transplantation cases with different living-related donors: Is there a relationship among donor, type matching and transplantation effect?

OBJECTIVE: Some studies have documented that living donor kidney transplantation has a better clinical results than cadaveric donors, however, it is poorly understood whether different living-related donors has different effects on kidney transplantation. The aim of this study is to discuss the clinical results of different living-related donors on kidney transplantation, in addition, to analyze the related problems including the donor selection, and relation of type matching with transplantation results. METHODS: Donors comprise 36 males and 58 females, aged (42.6±8.8) years. There were 72 males and 22 females received kidney transplantation, aged (33.9±8.7) years. Except for 6 cases being donated by spouse, 35 cases were donated by parents and 53 cases were donated by brothers and sisters. As to human leucocyte antigen (HLA) matching, 18 cases were full matched, 2 cases shared one-haplotype mismatched, 5 cases with 2-haplotype mismatched, 2 cases with 3-haplotype mismatched, 2 cases with 4-haplotype mismatched, and 1 case with 5-haplotype mismatched, 10 cases with full mismatched. Totally 85 cases had identical blood type, 9 cases had blood type compatibility. All recipients were negative for panel reaction antibody with smaller than 10% of lymphocytotoxicity tests prior to transplantation. RESULTS: All donors recovered well. Acute rejection episodes occurred in 8 patients, 4 cases donated by parents, 3 cases donated by siblings and 1 case donated by spouse. One recipient donated by spouse suffered accelerated rejection and delayed graft function recovery occurred. The renal function recovered in 5 weeks post-operation. All cases were reversed successfully by high intravenous dose of methyl-prednisolone or polyclonal anti-T-cell antibodies. A 6 month to 10 years follow-up showed all the donors kept normal kidney function without impairment of life quality, and 92 recipients with 92 grafts survived yet. Two recipients were dead: one due to serious pulmonary infection, and the other due to heart failure. 2 recipients underwent chronic allograft nephropathy. CONCLUSION: Living-related donor kidney transplantation has optimal HLA matches, low incidence of acute rejection and long-term surviving ratio and blood relative, which is better for renal transplantation.
7.Growth characteristics of basic fibroblast growth factor gene-transfected bone marrow mesenchymal stem cells seeded on coral skeleton in vitro

BACKGROUND: Jaw defects are common clinically. It is desirable to find ideal seed cells combined with scaffolds to construct tissue engineered jaws for curing these diseases. OBJECTIVE: To investigate the growth characteristics of bone marrow mesenchymal stem cells (BMSCs) transfected with basic fibroblast growth factor (bFGF) gene after seeded on coral scaffold in vitro. DESIGN, TIME AND SETTING: An experimental study of bone tissue engineering was performed in the Research Institute of Stomatology, Sun Yat-sen University between March 2006 and June 2008. MATERIALS: Natural coral from China Hainan bench was made into pieces of 8 mm×8 mm×2 mm. METHODS: BMSCs were isolated from New England rabbits by density gradient centrifugation and then purified by adherent separation. bFGF-pcDNA3 gene was transfected into BMSCs using Lipofectamine TM 2000. bFGF gene-transfected (transfected group) or untransfected (untransfected group)BMSCs were seeded on different coral scaffolds. In addition, bFGF gene-transfected BMSCs were simply cultured but not on the coral scaffold for control (simple culture group). MAIN OUTCOME MEASURES: BMSC proliferation was detected by methyl thiazolyl tetrazolium (MTT) assay and BMSC growth on coral scaffold was observed under the scanning electron microscope. RESULTS: MTT assay showed that the BMSC proliferation rate was significantly higher in the transfected group than in the untransfected group (P < 0.05) and that there was no significant difference in BMSC proliferation between the transfected and simple culture groups (P > 0.05). Scanning electron microscope results displayed that BMSCs adhered to and spread over the coral scaffold, exhibiting various appearances, with some cells had grown into scaffold micropores or spanned micropore surface, and some extracellular matrix secreted by BMSCs were found. CONCLUSION: The transfected group exhibited better growth of BMSCs transfected by bFGF gene than the untransfected group. These findings indicate that coral skeleton does not influence BMSC proliferation and can be used as a scaffold of BMSCs to construct tissue-engineered bone.
8.Transobturator tension-free vaginal tape technique procedure for the treatment of female stress urinary incontinence

Objective To evaluate effectiveness and safety of the transobturator tension-free vaginal tape technique (TVT-O) for the treatment of female stress urinary incontinence(SUI). Methods Twenty-six cases of SUI patients with average (52.6±6.3) years old were included. Mean course of disease was (9.2± 2.3) years. Two cases were type Ⅱ ,23 cases were type Ⅱ/Ⅲ,and 1 case was type ID. One case had history of pelvic operation procedure,2 cases accompanied by urgent urinary incontinence. All cases were undergone TVT-0 procedure. The methods of this procedure and the outcomes and complications were closely analyzed. Results The average operating duration was (18.0 ±2.3) min (15-20 min) and average blood loss was (20.0±3.4)ml (10-40 ml). Twenty-four cases (92.3%) were cured, 2 cases (7.7%) were improved, 2 cases (7.7% ) had dysuria, 1 case improved after prolonged indwelling of catheter, 1 case was recovery with urethral dilations. Four cases (15.4% ) suffered pain of thigh skin around the wound, and relieved by analgesia. One case (3.8%) complained of frequency of micturition, and improved by medicine. Twenty-six cases were followed up of 8-16 months with no recurrence. Conclusion TVT-0 for the treatment of SUI is a simple optimal and reliable method with less complications and eusemia.
9.Effectiveness Evaluation for Auricular Syndrome Differentiation and Yongquan Magnetic Point Application on Patients with Mild to Moderate Hypertension

Objective To observe the efficacy of lowering blood pressures and TCM syndrome on the mild to moderate hypertension via the treatment of auricular syndrome differentiation combined with magnetic point application;To provide reference for clinical treatment of hypertension by non-pharmaceutical therapy. Methods Totally 260 patients with mild to moderate hypertension were randomly divided into control group and treatment group, 130 cases for each. The control group was given Norvasc by conventional oral dosage, and the treatment group was given auricular syndrome differentiation and Yongquan (KI1) magnetic point application additionally. SBP, DBP and TCM syndrome scores of the two groups were observed before and after treatment to evaluate the antihypertensive efficacy and TCM syndrome efficacy. The side effects were also observed. Results After treatment, SBP of the two groups decreased significantly (P<0.05), with no significant difference (P>0.05). DBP of the treatment group decreased significantly (P<0.05), with significant difference compared with the control group (P<0.05). There was no significant difference in antihypertensive efficacy between the two groups (P>0.05). TCM syndrome of treatment group improved significantly compared with the control group (P<0.05). The total effective rate of TCM syndromes in treatment group was better than the control group (P<0.05), without side effects. Conclusion Auricular syndrome differentiation combined with magnetic point application can not only significantly improve the TCM syndrome of patients with mild to moderate hypertension, but also has synergistic hypotensive efficacy and high safety.
10.Protective effects of hyperpolarized cardiac preservation solution on isolated rat heart

Objective To evaluate the protective effects of a novel hyperpolarized cardiac preservation solution on isolated rat heart. Methods Male Sprague Dawley rats (250~300 g, n =24) were randomly divided into three groups ( n =8 in each group) and their hearts were preserved with K H, UW, HCS solution respectively for 24 h. The isolated hearts were perfused in the Langendorff rat mode for functional evaluation before and after preservation. Lactate dehydrogenase and creatine phosphokinase enzyme leakages, contents of adenosine triphosphate and wet weights in myocardium were measured after reperfusion. Results The impairment of the left ventricle function in K H group was serious, while that in HCS and UW groups was mild. The recovery of coronary flow in HCS group was better than K H and UW groups. There were no significant differences in releases of LDH and CPK and wet weight of myocardium between UW group and HCS group. HCS group offered a less variety of pH and had the highest ATP content of myocardium among the three groups. The time, from the reperfusion to beating of heart, showed no difference between UW group and HCS group. The ultrastructural changes were slight in HCS group. Conclusions HCS solution can protect rat heart for 24 h effectively. The recovery of heart function by HCS is similar to that by UW solution. HCS solution can improve the coronary flow and acidosis and protect ATP more effectively than UW solution.
