1.Effects of vitamin A deficiency during pregnancy on the lung vitamin A status,its receptor and pulmonary surfactant of fetal rats

Objective:To observe the effects of vitamin A deficiency during pregnancy on the lung vitamin A status,its receptor and pulmonary surfactant of fetal rats.Methods:Prenatal vitamin A normal(VAN)and marginal vitamin A deficiency(MVAD)of Sprague-Dawley rat models were employed.There were 6 in each group.Then they were randomly divided into dexamethasone(Dex)group and control group respectively.I.e VAN-Dex group,VAN-NS group,MVAD-Dex group and MVAD-NS group.From the gestational days 16 to 18,all the rats were given Dex or Saline by intramuscular injection respectively.On the gestational day 19,fetal rats were harvested by cesarean section.The vitamin A concentrations of lung were detected by high-performance liquid chromatography(HPLC),and the levels of pulmonary surfactant(PS)and retinoic acid receptor(RAR)mRNAs were detected by semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR);then the values of PS and RAR mRNAs were compared to glyceraldehyde-3-phosphate dehydrogenase(GAPDH)mRNA.Results:(1)The vitamin A concentrations of lungs in VAN-NS group were higher than those of MVAD-NS group,higher in VAN-Dex group than in MVAD-Dex group,higher in MVAD-Dex group than in MVAD-NS group,and higher in VAN-Dex group than in VAN-NS group.There were significant differences among the four groups(P0.05).(3)With regard to the levels of RAR mRNA,the levels of RAR-? and RAR-? mRNAs in VAN-Dex group were higher than those in VAN-NS group,higher in MVAD-Dex group than in MVAD-NS group,lower in VAN-Dex group than in MVAD-Dex group,lower in VAN-NS group than in MVAD-NS group.The levels of RAR-? mRNA in the four groups were similar to those of the RAR-? and RAR-? mRNAs.And There were significant differences(P
2.A randomised study of parenteral nutritional suport in the post-palliative operations of advanced pancreatic carcinoma patients receiving chemotherapy

Objective The incidence of pancreatic cancer is increasing worldwide, and also in china. The characteristics of the cancer are: short course, rapid progress and high mortality, with median survival time of about 6 months. Unfortunately, the early symptoms of patients are not peculiarity. There are not simple, direct and effective methods of early diagnosis. When the patients have jaundice and abdominal pain, probably they are the advanced disease (stage Ⅲ-Ⅳ) ,with unresectable tumours,and have to perform biliary bypass operation for alleviative treatment. Furthermore, the most of all patients exhibit advanced disease, which is often associated with a reduced performance status, malnourished condition, immune function deficiency, all of which severely limit patients tolerance for toxic chemotherapy regimens. These may result in physiological problems that can alter the metabolism and distribution of cytotoxic drugs and therefor lead to an increase in treatment-related toxicity. Purpose To explore the effect of TPN in the post-palliative operations, Roux-en-Ycholedceho-jejunostomy, of advaneed pancreatic carcinoma patients receiving chemotherapy. Methods From Jan. 1996 to Jan. 1999, 40 cases had receiving combination chemotherapy after palliative operations for relief of jaundice by biliary hypass, and surgical biopsy for histologically cemented. The patients were randomly divided inter 2 groups: (1) treatment group (chemotherapy + TPN), 12 men and 8 women had a median age of 57.8 years old. (2) Control group (chemotherapy + routine treatment), 13 men and 7 women had a median age of 58.2 years old. All the patients received combined chemotherapy, the regimen was 5-Fu+ CF + MMC + DDP/EPI (5-fluorouracil + Calcium folniate + Mitomycin C + Cisplatin or Epirubicin ) or IFN-γ (interferon-γ). Dosages of drugs were modified for bone marrow toxicity, stomatits and declining performance status. After 28 days, the regimen performed once again. The regimen of TPN was: total caloric value (NPC) 20 Kcal/kg·d, nitrogen 0.2kg/kg·d, N/Q = 1g: 125Kcal, Glucose/Fat=6:4. The nutritional & immunological parameters, quality of life, effects of treatment wrer monitored. Results Treatemnt group is superior to control group. After TPN, serum pre. albumin, transferrin concentration, retinol-binding protein, total lymphocyte count, natural killer cells activity, IL-2R+ cells (CD25) percentage of peripheral blood, grades of quality of life and survival time increased significantly (P<0.05, P<0.01 respectively). However, no significant difference was observed in IFN-γ application group (P>0.05). Conelusions The nutritional support can improve nutritional and immune situation and quality of life, and prolong the survival time of the patients, but no relapse-free survival.
3.Evaluation of reliability and validity of SF-36 Health Survey in old people in community

Objectives To evaluate the reliability and validity of SF-36 in old people in community. Methods 378 old people in community were investigated by SF-36 by face-to-face interview, and the score of items was analyzed.Reliability and validity of SF-36 were evaluated by item-internal consistency,item discriminant vadility,Cronbach's alpha coefficient, et al. Results The clustering and ordering of the item means of SF-36 were the same as that hypothesized, except for items PF2,PF10,RP3,GH4. The correlation between an item and its hypothesized scale was 0.4 or above for all items. The Cronbach's alpha coefficient were above the standard of 0.7 for all except SF scale. Conclusions SF-36 Health Survey Questionnaire is available for the elderly people. The present evaluation proposes some minor word and item changes which is more suitable for old people.
4.Diagnostic value of gram-stained sputum smear in elderly patients with ventilator-associated lower respiratory infection

Objective To explore the diagnostic value of gram-stained sputum smears in elderly patients with ventilator-associated lower respiratory infection (VALRI).Methods 368 elderly patients undergoing mechanical ventilation over 48 hours in ICU were retrospectiely analyzed from Jun 2009 to Jun 2011.42 patients fulfilled the inclusion criteria (with good-quality sputa and presence of the same microorganism in sputum and blood cultures).58 gram-stained smears and sputum cultures were obtained and analyzed.Results The diagnostic sensitivity and specificity of Gram-positive bacteria and Gram-negative bacteria by gram-stained smears were 82.35% and 78.05%,80.49% and 82.35%,respectively.The positive and negative predictive value of Gram-positive bacteria and Gramnegative bacteria were 63.64% and 91.43%,91.67% and 60.87%,respectively.The coincidence rates in diagnosing Gram-positive bacteria and Gram-negative bacteria were 81.03% and 79.31%,respectively.Conclusions Gram-stained sputum smear is reliable in diagnosing ventilator-associated lower respiratory infection in elderly patients,and has a certain clinical significance in guiding to the selection of antibiotics.
6.Scientific Optimizing of Flow of Drug Purchasing in Hospital

OBJECTIVE:To construct a high-efficiency, high-performance, low-cost flow of drug purchasing. METHODS: The defects involved in the current drug purchasing flowsheet prevalent in many hospitals were analyzed and the optimized flowsheet was put into practice by combining the actuality of our hospital. RESULTS & CONCLUSIONS: The traditional drug purchasing modality can not meet the requirement of current hospitals in that it involves large work load and high probability of error, and it is tedious and time-consuming. The establishment of drug catalog database and the online drug purchase by means of instant communication software can help simplify drug purchase flow, lessen work load, enhance work efficiency, and effectuate a high-efficiency, high-performance, low-cost flow of drug purchasing.
7.Relationship and nursing between stress rise of blood glucose and the PEEL prognostic index of pa-tients with acute myocardial infarction

Objective To study the relationship between stress rise of blood glucose and the PEEL prognostic index of the patients with acute myocardial infarction, and provide the basis for a quick judgement and evaluation of prognosis to nurses. Methods Taking the 6.8,7.8 and 8.8 mmol/L as demarcation point to study the PEEL prognostic index in 59 patients with acute myocardial infarction, and compare the PEEL score value on two sides of each demarcation point. Results No significant difference in PEEL evaluation score when taking 6.8 mmol/L as the demarcation point, there was significant difference when taking 7.8 mmol/L as the demarcation point, PEEL score in blood glucose > 7.8 mmol/L group was higher than that of ≤7.8mmol/L group, so was when taking 8.8 mmol/L as the demarcation point. Conclusions There was instructive signifi-cance for nurses to observe a stress rise of blood glucose timely in evaluating the prognosis of patients with a-cute myocardial infarction. Nurses should strengthen disease observation and preventive nursing of admission patients whose fasting blood glucose are above 7.8 mmol/L as well their blood glucose monitoring.

Objective To study to take the oxidized Mannan?modified 786?0 in renal clear cell carcinom as tumor cells antigen to sensitize Dendrit?ic cells(DC)and to observe the its killing effect on renal clear cell carcinoma of CTLs induced. Methods Getting the peripheral blood mononucle?ar cells from the volunteers,and then to be stimulated to turn to be maturation by GM?CSF and IL?4 in vitro. Taking the clear renal carcinoma cell as the tumor antigen,and then making it to be modified by oxidized Mannan to acquire the tumor cell vaccine.Experimental groups include:blank group:DC?PBS group,control group:control?DC?786?0,experimental group:DC?Ox?Mannose?786?0 group. Taking the flow cytometry to detect the changes of DC phenotype,then taking the ELISA to detect the sencretion levels of supernatant of IL?12 of DC,then taking the CCK to detecte the cytotoxicity of lymphocytes(CTL)induced by DC of these experiment groups. Results Results by flow cytometry:the mature phenotype of DCs sensitized by Ox?Mannose?786?0 group included CD80,CD83,CD86 and HLA?DR expressed significantly higher than the other groups. As well as the secretion levels of IL?12. Meanwhile the cytotoxicity activity of lymphocytes(CTLs)induced by DCs which are sensitized by Ox?Mannose?786?0 increased more significantly than the other groups. Conclusion Glycosylated Antigens can be more effective in sensitizing antigen?presenting cells DC,and stimulating them to be maturation,while the killing effect to tumor cells also have noticeably improved.
10.Serum cystatin C concentration as an independent marker for hypertensive left ventricular hypertrophy

Background Serum cystatin C levels can be used to predict morbidity and mortality in patients with cardiovascular disease. However, the clinical relevance of serum cystatin C levels in patients with hypertensive left ventricular hypertrophy (LVH) has rarely been investigated. We designed the present study to investigate whether serum cystatin C levels are associated with cardiac structural and functional alterations in hypertensive patients. Methods We enrolled 823 hypertensive patients and classified them into two groups:those with LVH (n=287) and those without LVH (n=536). All patients underwent echocardiography and serum cystatin C testing. We analyzed the relationship be-tween serum cystatin C levels and LVH. Results Serum cystatin C levels were higher in hypertensive patients with LVH than in those without LVH (P<0.05). Using linear correlation analysis, we found a positive correlation between serum cystatin C levels and interven-tricular septal thickness (r=0.247, P<0.01), posterior wall thickness (r=0.216, P<0.01), and left ventricular weight index (r=0.347, P<0.01). When analyzed by multiple linear regression, the positive correlations remained between serum cystatin C and interventricular septal thickness (β=0.167, P<0.05), posterior wall thickness (β=0.187, P<0.05), and left ventricular weight index (β=0.245, P<0.01). Con-clusion Serum cystatin C concentration is an independent marker for hypertensive LVH.
