1.Investigation and analysis of psychological consulting requirement for the hospital staff

Objective To learn the psychological needs of the hospital staff in terms of their life,medical care and psychological health.Methods 2910 hospital staff were interviewed with questionnaires and the outcomes analyzed with x2 test and descriptive analysis.Results 96.9%of the surveyed found themselves in need of psychological counseling; considerable consulting needs of the staff; most of them turn to friends to complain instead of their leaders.Conclusion The psychological counseling should be enhanced to build effective communication channels and ease stress of the staff.
2.Experimental study on denitrification using coated electrode of immobilized denitrifying bacteria.

OBJECTIVETo develop a coated electrode of immobilized denitrificants and to evaluate the performance of a bioelectrochemical reactor to enhance and control denitrification.
METHODSDenitrifying bacteria were developed by batch incubation and immobilized with polyvinyl alcohol (PVA) on the surface of activated carbon fiber (ACF) to make a coated electrode. Then the coated electrode (cathode) and graphite electrode (anode) were transferred to the reactor to reduce nitrate.
RESULTSAfter acclimated to the mixtrophic and autotrophic denitrification stages, the denitrifying bacteria could use hydrogen as an electron donor to reduce nitrate. When the initial nitrate concentration was 30.2 mg NO3- -N / L, the denitrification efficiency was 57.3% at an applied electric current of 15 mA and a hydraulic retention time (HRT) of 12 hours. Correspondingly, the current density was 0.083 mA/cm2. The nitrate removal rate of the reactor was 34.4 g NO3- -N/m3 x d, and the surface area loading was 1.34 g NO3- -N / m2 x d.
CONCLUSIONThe coated electrode may keep high quantity of biomass, thus achieving a high denitrification rate. Denitrification efficiencies are related to HRT, current density, oxidation reduction potential (ORP), dissolved oxygen (DO), pH value, and temperature.

3.Study on the application of benchmark dose and biological monitoring indexes of fluorides in drinking water.

OBJECTIVEBy using Benchmark Dose (BMD) approach to explore the relations among drinking water fluoride, urine fluoride, serum fluoride and dental fluorosis; and to evaluate the significance of urine fluoride and serum fluoride in control and prevention of endemic fluorosis.
METHODS512 children (290 in Xinhuai Village, 222 in Wamiao Village) aged 8-13 years were recruited in the study. Epidemiological methods were used to investigate the prevalence of dental fluorosis, and the levels of urine fluoride, serum fluoride, and drinking water fluoride in superficial well. The children were divided into six subgroups by the concentration of fluoride in drinking water: < 0.5 mg/L, 0.5-mg/L, 1.0-mg/L, 2.0-mg/L, 3.0-mg/L and > or = 4.0 mg/L.
RESULTSThere was significant dose-response relationship between the drinking water fluoride and the prevalence of dental fluorosis or the prevalence of defect dental fluorosis. The BMDLs (Benchmark Dose Lower Bound) were 1.01 and 1.30 mg/L, respectively. Urine fluoride and serum fluoride also had significant dose-response relationship to the prevalence of dental fluorosis or defect dental fluorosis. The correlation coefficient between drinking water fluoride and urine fluoride was 0.717, and it was 0.855 between drinking water fluoride and serum fluoride, and 0.617 between urine fluoride and serum fluoride.
CONCLUSIONSThe currently national standard of fluoride in drinking water in China is safe and reasonable. As a biological monitoring index, the levels of fluoride in serum may be more useful than that in urine in the control and prevention of endemic fluorosis.

4.A randomized trial of Chinese medicine Lirukang Granule combined with psychological intervention for cyclomastopathy and menoxenia.

OBJECTIVETo observe the influence of therapy with Chinese medicine Lirukang Granule (, LRKG) combined with psychological intervention on anxiety states and sex hormones in patients with cyclomastopathy and menoxenia.
METHODSA total of 470 subjects were randomly assigned to three groups by the net-central randomization system, the treatment group (161 patients, treated with LRKG and psychological intervention), the Chinese medicine group (157 patients, treated with LRKG), and the psychological intervention group (152 patients, treated with psychological intervention). The dose of LRKG was 12 g three times per day; psychological intervention included establishing relations, cognitive intervention and psychological persuasion, 30-40 min per session, once a week. The therapy duration for all groups was three months. The efficacy was compared and anxiety state/State-Trait Anxiety Invertory (STAI) scoring was measured before and after treatment. The serum estradiol (E2), progesterone (P), prolactin (PRL) and follicle stimulating hormone (FSH) levels of 60 patients selected randomly from each group during the luteal phase were measured before and after treatment, and a group of 20 healthy women were evaluated for comparison. A follow-up was arranged for one year after treatment.
RESULTSThirty subjects were lost to follow-up. (1) Comparison of efficacy: the markedly effective rate and the total effective rate of the treatment group were 86.67% (131/150) and 98.00% (147/150), respectively; of the Chinese medicine group, 64.58% (93/144) and 90.27% (130/144), respectively; and of the psychological intervention group, 0% (0/146) and 3.42% (5/146), respectively. The markedly effective rate and the total effective rate in the treatment group were significantly higher than those in the Chinese medicine and psychological intervention groups (P < 0.05). (2) Comparison of STAI scoring: STAI scoring was decreased dramatically in the treatment group after treatment compared with that of the Chinese medicine group (P < 0.01), but there was no significant difference compared with the psychological intervention group. (3) Comparison of levels of sex hormones: E2, P, PRL and FSH of the three patient groups were disordered before treatment, and significantly different from healthy women (P < 0.01). After treatment, the levels of P and FSH of the treatment group were significantly increased (P < 0.01), E2 and PRL were significantly reduced, which were also significantly decreased compared with the psychological intervention groups (P < 0.01). (4) FOLLOW-UP: the markedly effective rate and the total effective rate of the treatment group remained higher than those of the other two groups after one year of treatment (P < 0.05). (5) Adverse reactions: no obvious adverse reactions were found among the three groups.
CONCLUSIONSTherapy with Chinese medicine combined with psychological intervention was effective for short-term and long-term treatment of cyclomastopathy and menoxenia. The mechanism might be related to the regulation of sex hormones.

6.Bigelovii A induced autophagy in MCF7 cells by inhibiting mTOR signal pathway

Aim To investigate the effects of Bigelovii A on autophagy and its mechanism.Methods Fluorescence microscope,flow cytometry and Western blot were employed to analyze autophagy.Western blot was used to detect the protein expressions of mTOR pathway.MTT colorimetry was used to assay cell viability after treatment with 3-MA and Bigelovii A or Bigelovii A alone.Results Bigelovii A-treated MCF7 cells displayed a dramatic increase in the number of MDC-labeled vesicles and the expressions of LC3-Ⅱ,indicating cell autophagy.Ⅰt was proved that in MCF7 cells,Bigelovii A inhibited mTOR signaling by decreasing Akt and p-ERK.Consistently,Bigelovii A decreased phosphorylation levels of mTOR,p70S6K (Ser371,Thr389) and 4EBP1 proteins.Inhibiting Bigelovii Ainduced autophagy with the autophagy inhibitor 3-methyladenine significantly decreased cell viability,which suggested that Bigelovii A-induced autophagy played a pro-survival role.Conclusion Bigelovii A is likely to induce autophagy through inhibiting mTOR pathway.
7.Silencing of vacuolar ATPase c subunit ATP6V0C inhibits invasion of prostate cancer cells

Objective:Vacuolar ATPase (V-ATPase) was found within the membranes and internal organelles of a vast array of eukaryotic cells,and was related to various kinds of highly metastatic tumors.LASS2/TMSG1 gene was a novel tumor metastasis suppressor gene cloned from human prostate cancer cell line PC-3M in 1999 by our laboratory.It was found out that protein encoded by LASS2/TMSG1 could interact with the c subunit of V-ATPase (ATP6V0C).In this study,To use RNA interference to suppress the expression of ATP6V0C and try to further investigate the molecular mechanism of ATP6V0C in tumor metastasis and its relationship with LASS2/TMSG1 gene.Methods and Results:The expression level of ATP6V0C mRNA and protein in high metastatic potential prostate cancer cell lines (PC-3M-1E8 and PC-3M) was significantly higher than that in low metastatic potential prostate cancer cell lines (PC-3M-2B4 and PC-3),the expression level in PC-3M-1E8 being the highest.Follow-up tests selected PC-3M-1E8 cells for gene silencing.The expression and secretion of MMP-2 and the expression of MMP-9 in ATP6V0C siRNA transfected PC-3M-1E8 cells displayed no obvious change,but the activity of secreted MMP-9 was abated noticeably compared with the controls (P < 0.05).Extracellular hydrogen ion concentration and V-ATPase activity in interference group were both reduced significantly compared with the controls (P < 0.05).The migration and invasion capacity of ATP6V0C siRNA interfered cells in vitro were diminished significantly compared with the controls (P < 0.05).Furthermore,a dramatic reduction of LASS2/TMSG1 mRNA and protein level after transfection of siRNA in PC-3M-1 E8 cells was discovered (P < 0.05).Confocal immunofluorescence showed a vast co-localization of ATP6V0C protein and LASS2/TMSG1 protein in plasma and membrane.The co-localization signals of control group were much stronger than those of interference group.Conclusion:Specific siRNA silencing of ATP6V0C gene inhibits the invasion of human prostate cancer cells in vitro by mechanism of inhibiting V-ATPase activity and then reducing the extracellular hydrogen ion concentration,inhibiting MMP-9 activation and affecting ECM degradation and reconstruction.Meanwhile,ATP6V0C and LASS2/TMSG1 have interaction and it is likely that ATP6V0C functions as a feedback regulator of LASS2/TMSG1.
8.Advanced and Recurrent Malignant Lymphoma Were Treated by BEAC Regimen Supported with Autologous Hemotopoietic Stem Cells Transplantation

Objectives: High dose chemotherapy supported therapeutic outcome by AHSCT has developed dramatically in recent years and become the most effective approach to improve for the chemo-sensitive lymphoma. The purpose of this study was to evaluate the efficacy of mobilization regimen, effectiveness and tolerance of BEAC regimen in Chinese patients with advanced and recurrent lymphoma,and hemotopoietic reconstitution. Methods: After confirmed complete or partial remission from conventional chemotherapy, 20 patients with advanced or recurrent lymphoma, 1 recurrent HD and 19 NHL;14 male and 6 female with median age 28(range,13-48)years old, were enrolled into this study and treated with BEAC regimen(CTX 3600-4000 mg/m2, VP-16 1200 mg/m2. BCNU 300 mg/m2 and Ara-C 1500-2000 mg/m2). Three patients were supported by ABMT and 17 by APBSCT. Mobilization regimen for APBSCT was CTX 3500 mg/m2+ G-CSF 3.5-5 μ g/kg+ Dexamethasone 10 mg. Autologous hemotopoietic stem cells was re-infused 24-48 hours after completion of high dose chemotherapy. Results: MNC 1.3(range,1.0~1.7)× 108/kg and,MNC 1.8(range,1.0-4.4)× 109, CFU-GM 5.1(range,1.9-9.6)× 105/kg and CD34+ cells 2.9 (range,1.9~8.7)× 106/kg were re-infused in the ABMT group and APBSCT group respectively. All patients obtained prompt and sustained hemotopoietic reconstitution. ANC ≥ 0.5× 109/L and Pt ≥ 2.0× 109/L were at day 9 (range,6~17) and day 10 (range,0~31) respectively. Fourteen patients were alive with median 18(range,1~67)months follow-up till end of April,2000. The 1,2,and 3 years survival rate were 61.2% , 53.4% and 53.4% ,respectively. Non-hemotologic toxicity was mild and tolerable. Conclusion: High dose chemotherapy supported by AHSCT in the treatment of poor-prognostic and recurrent lymphoma is a safe and effective modality. However further investigation is warranted.
9.Research on the status and reform direction of community nursing personnel education in Zhejiang province

Objective To analyze the status of community nursing personnel training and explore its reform direction.Methods Totals of 160 community health service institutions in 11 cities in Zhejiang province were investigated by convenience sampling.The content validity of the modified questionnaire was 0.9,and the reliability Cronbach's α coefficient was 0.87.Results One hundred and sixty questionnaires were issued and 150 questionnaires were responsive with an effective response rate of 93.8%,including 119 community health service centers and 31 community health service station.Most community organizations (57.1%) thought that current personnel training program of nursing could not satisfy the requirements of practical community nursing.The shortcomings were as follows:1) insufficient health and recovery knowledge (54.7%) ; 2) insufficient humanity knowledge,such as psychology and law (45.2%) ; 3) insufficient knowledge about epidemic disease and preventive medicine (42.6%) ; 4) insufficient class hour on community nursing (40.0%) ; 5) insufficient knowledge on nursing research (18.7%).Most community organizations thought it was necessary for the college to set up community nursing class (54.8%).The courses they suggested were:preventive medicine,recovery medicine,health education,psychology,epidemic medicine,medicine and society.The core skills of the community nurses should include the skill to communicate with patients,the ability of disease systematic nursing,independent analysis and problem solving.Conclnsions Current college education can not fully meet the requirements of community nursing personnel training.College should set up special class on community nursing,revise the courses oriented towards community nursing,and emphasize the core skills of community nursing.
10.Clinical Features and Prognostic Analysis of Newly Diagnosed Diffuse Large B-cell Lymphoma Combined with Hemophagocytic Syndrome

Objective:To compare the clinical features and prognosis between newly diagnosed diffuse large B-cell lymphoma(DLBCL)patients with and without hemophagocytic syndrome(HPS).Methods:The clinical data of 45 DLBCL patients in Gansu Provincial Hospital from January 2012 to December 2021 were retrospectively analyzed.The patients were divided into HPS group(15 cases)and non-HPS group(30 cases).The clinical features and prognosis of the two groups were compared,and survival analysis was performed using Kaplan-Meier method.Results:Patients with HSP were mostly characterized by fever,cytopenia and splenomegaly.The levels of ferritin and soluble CD25 increased in all patients.The level of fibrinogen decreased in 66.67%patients,while triglyceride increased in 53.33%patients,and bone marrow hemophagocytosis occurred in 80.00%patients.Compared with non-HSP group,the proportions of patients with advanced stage(Ann Arbor stage Ⅲ/Ⅳ)and lactate dehydrogenase(LDH)≥ 240 U/L were higher in HSP group(both P<0.05).The median survival time of HSP group was 8.0 months,which was significantly shorter than 45.5 months of non-HSP group(P<0.001).Conclusion:The DLBCL patients with HPS have later Ann Arbor stage,higher LDH and shorter overall survival time compared with patients without HPS.
