1.Predictors of perioperative myocardial ischemia in patients undergoing noncardiac surgery

Objective To determine the incidence of pre- and postoperative myocardial ischemia (MI) in patients undergoing noncardiac surgery and to identify the predictors of perioperative MI. Methods One hundred and fifty patients (99 male, 51 female) aged 42-89 yr, weighing 35-87 kg undergoing elective noncardiac surgery were enrolled in this study. Patients with left or right bundle branch block, left ventricular hypertrophy, low voltage on ECG using limb leads, being treated with digoxin or on artificial pacemaker were excluded from the study. The patients were premedicated with intramuscular atropine 0.5 mg and phenobarbital sodium 0.1 g, and were monitored continuously with dynamic ECG (DMS Holler 5.0 USA) for at least 12 h before surgery and 48 h after surgery. The incidence of MI, the number of ischemic episodes, the duration of MI and the area under the ST curve per hour were computed pre- and postoperatively. Factors that could affect MI including age, sex, weight, ASA class, history of cardiac disease, cardiac risk factors, cardiac medication, anesthesia, surgery, laboratory and other physiological data were also recorded. The potential predictors of perioperative MI were identified by univariate model and were then entered into multivariate logistic models. Results The incidence of preoperative MI was 4.7% and postoperative MI was 22.7 % . The incidence of MI and number of ischemia episodes achieved peak levels at 12-24 h after operation, while the duration of MI and the area under the ST curve peaked at 0-12 h after operation. Predictors of postoperative MI were : preoperative MI identified by ECG, age ≥ 65 yr, ASA class ≥ Ⅲ , history of angina, or hypertension or diabetes and high VAS score. Conclusion Postoperative MI usually develops on the first day after surgery. The patients at high risk for developing postoperative MI can be identified by predictors.
2.The preventative effects of protein tyrosine kinase on the inflammation and airway remondeling in lung of guinea pigs with bronchial asthma.

AIMTo investigate the effects of protein tyrosine kinase on the inflammation and airway remodeling in lung of guinea pigs with bronchial asthma.
METHODS30 adult male guinea pigs were randomly divided into 3 groups (n=3): control group (C group), asthmatic group(A group)and genistein group (B group). Asthmatic model was established by ovalbumin intraperitoneal injection and ovalbumin inhalation. The total cell and the proportion of inflammatory cell in bronchial alveolar lavage fluid(BALF), inflammatory cell infiltration and index of remodeling of bronchiole were measured, respectively. The expression of p-tyrosine in lung tissue was examined by immunohistochemistry.
RESULTSThe total cell and proportion of eosinophil in BALF of A group were significantly higher than that of C group (P < 0.01), but compared with A group, the total cell and proportion of eosinophil in BALF of B group were much lower (P < 0.01). The number of eosinophile and lymphocyte of bronchiole in A group were significantly higher than that of C group (P < 0.01), but compared with A group, the number of eosinophile and lymphocyte in bronchiole of B group were much lower (P < 0.01). Compared with A group, the remodeling of bronchiole of B group was significantly relieved (P <0.01), there was no difference between B and C group (P > 0.05). Immunohistochemistry indicated that in A group the p-tyrosine was more positively expressed at the bronchial smooth muscle, bronchial epithelium, smooth muscle of vessel and inflammatory cell, especially at smooth muscle of bronchi and vessel and inflammatory cell than that of C group (P <0.01), there was no difference between B group and C group (P > 0.05).
CONCLUSIONPTK played a key role in inflammation and bronchial remodeling in lung of guinea pigs with bronchial asthma. The Protein tyrosine kinase inhibitor genistein could prevent and inhibit the inflammation and bronchial remodeling in lung of guinea pigs with bronchial asthma.

3.Zoledronic acid combined with single or multiple fraction radiotherapy for metastatic bone pain

0.05).ConclusionZoledronic acid combined with single or multiple fraction radiotherapy has similar clinical response.Zoledronic acid combined with single fraction is an efficient and cost-effective alternative to traditional multiple fraction radiotherapy for metastatic bone pain.
4.Advance of indocyanine-green in fluorescent staining tumor in surgical operation

Surgery is so far the most widely used and effective treatment of neoplastic diseases.However,residual tumour cells during surgery remain a major trigger of cancer recurrence and matastasis.Although intraoperative rapid pathological R0 resection can be achieved based on preoperative imageological examination,but for small satellite lesions and the naked eye can not find the error quickly and so often cause pathological presence of residual tumour cells.Thus,quick and accurate identification of residual cancer cells is crucial for prognosis of cancer patients.Indocyanine green (ICG) is a new type of fluorochrome that can stain tumours under the near-infrared fluorescence during surgery,the paper will be reviewed latest developments in the reagent for fluorescence in tumours.
5.Establishment of early warning control charts based on the syndromic surveillance data of outpatient diarrhea in Beijing

Objective:To establish the control charts for early warning of diarrhea based on the syn-dromic surveillance data from enteric clinic in Beijing .Methods:The outpatient data from enteric clinic of a Grade Three General hospital in Haidian district , Beijing from April 1 to Oct.31, 2009 and from May 1 to Nov.10, 2010 were collected, according to the moving average method , the baseline calcula-ted, the value of probability αand μα, the early warning value based on the formula “w=Xj +μαSj”calculated and the early warning control charts drew at last .Results:According to the harmfulness , the severity and controllability of diarrheal diseases , the value of probability αwas determined as 0.01, thenμα( unilateral) as 2, based on the early warning value , the control charts of diarrheal diseases , bacillary dysentery and other infectious diarrhea were established .Conclusion:The enteric clinic requires to fur-ther collect baseline data to evaluate and continuously adjust the established control charts for the best early warning model in accordance with the enteric clinic .
6.The functional status of the paraspinal muscles among patients with degenerative lumbar kyphosis before and after surgery

Objective To study any changes in the surface electromyography of the paraspinal muscles before and after surgery in patients with degenerative lumbar kyphosis.Methods Thirty patients with lumbar degenerative kyphosis (LDK) were enrolled and underwent pedicle subtraction osteotomy.Surface electromyograms were recorded before the operation and 3 months afterward to observe the myoelectric activity of the paraspinal muscles.The electromyograms were also compared with those of healthy counterparts.Results The preoperative average EMG (AEMG) values in the LDK group in static standing were significantly higher than those of the control group.After the operation the values had decreased significantly,but were still significantly higher than in the control group.The preoperative AEMG values of the LDK group from the L1-2 segment during torso stretching were significantly lower than those of the control group.They increased significantly after the operation and were no longer significantly different from the control groups' readings.The pre-and post-operative AEMG values from the L4-5 segment were not significantly different and remained significantly lower than in the control group.During lifting and holding the mean power frequency slopes of the LDK group were significantly lower than those of the controls both before and after the operation,though the postoperative slopes were significantly higher than the preoperative ones.Conclusion Patients with lumbar degenerative kyphosis have obvious paraspinal muscle weakness and severe fatigue.Early pedicle subtraction osteotomy can significantly improve the functional status of the paravertebral muscles in the short term.
