1.Relationship of cholesterol ester transfer protein TaqIB gene polymorphism with type 2 diabetes mellitus in Chinese Han nationality patients

Objective To explore the relationship between plasma cholesterol ester transfer protein(CETP) and TaqIB gene polymorphism and type 2 diabetes mellitus(DM).Methods The genotypes of the first intron of CETP TaqIB gene in 102 patients with type 2 DM and 103 healthy controls were analyzed by polymerase chain reaction-restricted fragment length polymorphism(PCR-RFLP),and plasma CETP were measured by enzyme-linked immunosorbent assay(ELISA).The association of gene polymorphism with plasma lipids,lipoproteins and apolipoproteins was also analyzed.Results No significant differences in both of the alleles frequencies and genotypes in type 2 DM were noted in comparison with controls.There were no relationships between CETP TaqIB gene polymorphism and gender,family history,smoking and body mass index(BMI).No significant differences of plasma lipid levels were observed in healthy controls with different genotypes.However,significant differences in level of HDL-C or apoAI among different CETP TaqIB genotype groups were observed,and significant correlation between low high-density lipoproteinemia and B1 allele in type 2 DM was observed.Conclusion CETP TaqIB gene polymorphisms associated with lipid metabolism,which may be an important genetic factor for abnormal lipid metabolism in type 2 DM.
2.Comparison of placement of peripherally inserted central catheters using vascular ultrasound guidance system and traditional method in 938 tumor patients

Objective To compare the placement of peripherally inserted central catheters(PICC)by using vascular ultrasound guidance system and traditional method.Methods Totally 938 patients undergoing PICC were divided into the traditional method group and the vascular ultrasound system and microintroducer techniques group according to the puncture time.The differences in the one-attempt success rate and the overall success rate of the placement and the incidences of complications were compared between the two groups.Results The one-attempt success rate was 93.01% in the traditional method group and 98.76% in the vascular ultrasound system and microintroducer techniques group(P =0.005).The successful rate of PICC placement was 100%.The incidence of complication was 11.29% in the traditional method group and 2.47% in the vascular ultrasound system and microintroducer technique group(P =0.000).Conclusion The PICC placement using vascular ultrasound guidance system and microintroducer techniques can increase one-attempt success rate and decrease complications.
3.IL-1β promotes glial scar formation after spinal cord injury in rats by JAK2-STAT3

Objective To investigate the mechanism of IL-1β in promoting glial scar formation after spinal cord injury.Methods The experimental model of SCI was created by extradural compression of the spinal cord using an aneurysm clip.Rats were randomly divided into model group, sham operation group, IL-1β inhibitor IL-1RA group, IL-1β group and IL-1β+JAK2-STAT3 inhibitor AG490 group, according to different interventions, then were given normal saline, IL-1RA, IL-1β and IL-1β+AG490 every 10 μL respectively, sham group received only laminectomy.The motion function of the hindlimbs of rats was measured by Basso Beattie Bresnahan(BBB) scores and the expression of GFAP, vimentin and p-STAT3 were detected by Western blot technique, immunofluorescence assay and immunohistochemistry technique at corresponding time points(at the 8th, 12th hour, 1st, 3rd, 7th and 14th day after SCI).Results The expression trend of p-STAT3(at the 8th and 12th hour after SCI),GFAP and vimentin(at the 7th and 14th day after SCI)was: the expressions of p-STAT3, GFAP and vimentin in the model group were significantly higher compared with the sham group(P<0.01), the expression of p-STAT3,GFAP andvimentin in the IL-1RA group were significantly lower compared with the model group(P<0.05) whereas significantly higher compared with the sham group(P<0.05);the expressions of p-STAT3, GFAP and vimentin in the IL-1β+AG490 group were significantly lower compared with the model group(P<0.05)whereas significantly higher compared with the sham group(P<0.05), the expressions of p-STAT3, GFAP and vimentin in the IL-1β group were significantly higher compared with the model group(P<0.05).Conclusions IL-1β can improve glial scar formation via JAK2-STAT3 signal.Inhibition of IL-1β or JAK2-STAT3 can reduce glial scar formation and promote functional recovery of spinal nerve.
4.Anterior Compression Index for Evaluating Atlantoaxial Dislocation and Restoration

Objective The study was to investigate the value of deduction and application of anterior compression index in evaluation of atlantoaxial dislocation and restoration. Method Twenty-eight cases of the control group and 28 cases of atlantoaxial dislocation treated with posterior restoration and screw-rod internal fixation technique before and af?ter surgery were recruited in this study and their data was retrospectively analyzed. All of the people underwent sagittal computerized tomography scan films. The anterior compression index was measured in all cases. Results The mean value of anterior compression index of the control group was 87.86±24.98. The mean value of anterior compression index of the preoperative patients was 230.44 ± 97.60 and the mean value of the postoperative patients was 106.27 ± 73.53. There was significant difference in those two parameters between the preoperative patients and the controls(t=-7.50,P<0.0001). There was no significant difference between the postoperative patients and the control group (t=-1.26, P=0.2171). Anteri?or compression index were significantly lower after surgical operation (t=10.35, compared with before, P<0.0001). Con?clusion Anterior compression index can be an excellent measurement tool for the assessment of relationship of atlas and axis in atlantoaxial dislocation patients before and after posterior restoration operation.
6.Optimization of Extraction Process of Flavonoids from Toosendan Fructus by Response Surface Methodology

Objective To optimize extraction process of flavonoids from Toosendan Fructus.Methods Three parameters that significantly affect the extraction rate of flavonoids, ratio of material to water, extraction time and ethanol concentration, were taken into consideration for extraction ratio of flavonoids from Toosendan Fructus. Based on the results of single-factor experiment, the extraction parameters of flavonoids from Toosendan Fructus were optimized by Box-Behnken of response surface methodology (RSM).Results The optimum extraction conditions were as follow: material to water ratio was 1:35; extraction time should be 3 h; ethanol concentration was 70%. The extraction ratio was up to 0.587 3% in the optimum extraction conditions.Conclusion The actual value is highly matching the theoretically value of RSM model. The results indicated that this method can rationally optimize extraction process of flavonoids from Toosendan Fructus.
7.Application value of serum pancreatic isled autoantibodies and biochemical indicators in classification diagnosis of diabetes mellitus

Objective To investigate the application value of serum pancreatic isled autoantibodies and biochemical indicators in classification diagnosis of type 1 diabetes mellitus (T1DM ) and type 2 diabetes mellitus (T2DM ) .Methods The clinical data and laboratory detection results in 99 cases of T1DM and 577 cases of T2DM were retrospectively analyzed .The levels of pancreatic isled autoantibodies and biochemical indicators were compared between the two groups and their characteristics were analyzed .Re‐sults The positive rates of single detection and combine detection of glutamic acid decarboxylase autoantibodies (GADA) ,insulino‐ma‐associated antigen‐2 autoantibodies (IA‐2A ) ,islet cell autoantibodies (ICA ) and ZnT8 autoantibodies (ZnT8A ) in the T1DM group were higher than those in the T2DM group ,the differences were statistically significant (P<0 .05) .The onset age ,fasting and postprandial 2 h CP ,fasting and postprandial 2 h insulin(2 h INS) ,triglyceride(TG) and body mass index (BMI) in the T1DM group were lower than those in the T2DM group ,while the levels of fasting and postprandial 2h blood glucose ,glycosylated hemo‐globin(HbA1c) and high‐density lipoprotein cholesterol(HDL‐C) were higher than those in the T2DM group ,the differences were statistically significant (P<0 .05);however total cholesterol and low‐density lipoprotein cholesterol levels had no statistical differ‐ence between the T1DM group and T2DM group (P>0 .05) .Moreover ,the fasting and postprandial 2 h CP levels in the T1DM group showed decreasing trend as the T1DM course extending ,and the difference had statistical difference among different disease courses ;but the fasting and postprandial 2 h CP levels in the T2DM group had no obvious decreasing trend .The areas under the receiver operating characteristic(ROC) curve of fasting and postprandial 2 h CP for differential diagnosis of T1DM and T2DM in the patients with the disease course < 2 year were 0 .902(95% CI:0 .850-0 .954) and 0 .905(95% CI:0 .852-0 .958) respective‐ly .The suitable threshold value of fasting CP was 0 .283 nmol/L ,its sensitivity and specificity were 82 .6% and 89 .2% ,respective‐ly ,which of postprandial 2 h CP was 0 .421 nmol/L ,its sensitivity and specificity were 84 .8% and 89 .2% respectively . Conclusion T1DM and T2DM are different in onset age ,BMI value ,serum GADA ,IA‐2A ,ICA ,ZnT8A ,insulin ,CP ,glucose , HbA1c ,TG and HDL‐C levels ,which may assist clinic in their classification diagnosis .
8.Primary Schwann cell growth on electrospun polymethyl methacrylate nanofiber scaffolds

Objective To evaluate the value of electrospun polymethyl methacrylate (PMMA) nanofibers with different topological structures as scaffolds for growth of Schwann cells (SCs).Methods Electrospun PMMA nanofibers with random or aligned topological structures were fabricated and measured with biocompatibility.Lentivirus-transfected green fluorescent protein was used as the reporting gene to monitor form and growth manner of SCs on different substrates and dependency of cell body and process with fiber structure,with PMMA thin films served as the control.Results Electrospun PMMA nanofibers revealed good biocompatibility and could exert contact guidance to the growth of SCs.Topological structures of the electrospun nanofibers influenced cell morphology.SCs were aligned with the orientation of substrate fibers and form longer cell process when growing on aligned nanofibers (P <0.01).Primary SCs preferred to follow the cue of aligned nanofibers compared to random fibers.Conclusion Aligned electrospun PMMA nanofibers have the potentiality as transplantable scaffolds for loading SCs after neural injury.
9.Anterior cervical discectomy with fusion and posterior cervical expansive open-door laminoplasty for multilevel cervical spondylotic myelopathy: its postoperative stability

BACKGROUND: The treatment of multilevel cervical spondylotic myelopathy (MCSM) aims at sustaining the spinal cord compression and restoring the stability of the cervical vertebrae at most.OBJECTIVE: To analyze the clinical characters of anterior cervical discectomy with fusion and expansive open-door laminoplasty for MCSM.METHODS: Sixty-seven patients with MCSM were divided into two groups, and treated with anterior cervical discectomy with fusion (group A) or posterior cervical expansive open-door laminoplasty (group B). All the patients were followed up for 12 months, and the range of motion of cervical vertebrae, cervical curvature index were observed,as well as the Visual Analogue Scale and Japanese Orthopaedic Association scores were condueted. Moreover, the operation time, blood loss and adverse reactions were recorded.RESULTS AND CONCLUSION: (1) The loss of range of motion of the cervical vertebrae in the group B was significantly less than that in the group A (P < 0.05). (2) The cervical curvature index in the group A was significantly improved (P < 0.05), but the index had no significant change in the group B. (3) The axial systems were significantly improved in both groups, especially in the group A (P < 0.05). (4) The neurological function was significantly improved in both groups (P < 0.05), which showed no significant difference between two groups (P > 0.05). (5) The intraoperative blood loss in the group B was significantly more than that in the group A (P < 0.05). (6) The incidence of hoarseness and dysphagia in the group A was 19%. The incidence of wound infection, cerebrospinal fluid leakage and C5 nerve root palsy in the group B was 9%. (7) These results suggest that during choosing an appropriate method for MCSM,surgeons' skills and patients' situation should be considered.
10.Neural stem cell transplantation for central nervous system diseases via the cerebrospinal fluid

BACKGROUND:Currently, neural stem celltransplantation can be performed through three main approaches:local lesions, blood circulation, and cerebrospinal fluid.
OBJECTIVE:To review the transplantation of neural stem cells or neural precursor cells via the cerebrospinal fluid in the treatment of central nervous system diseases.
METHODS:A computer-based search of PubMed and CHKD databases was performed to retrieve articles concerning transplantation of neural stem cells via the cerebrospinal fluid, and its application and therapeutic mechanism in the treatment of central nervous system diseases in both animal experiment and clinic study published from 2000 to 2009.
RESULTS AND CONCLUSION:It is suitable for neural stem cellsurvival, proliferation, and differentiation in the cerebrospinal fluid. Transplantation of neural stem cells via the cerebrospinal fluid is effective and feasible to treat central nervous system diseases. However, some problems have not been solved, such as the source of neural stem cells, the optimal time window and celldose, the safety and the long-term effect. Further studies are needed to pave the way for the intrathecal injection of neural stem cells in the treatment of central nervous system diseases.
