1.Application of SM22α promoter and enhancer against angiostenosis following coronary artery bypass grafting

BACKGROUND:Great saphenous vein is the most common vascular material in coronary artery bypass grafting, however, the graft neointimal hyperplasia and subsequent atherosclerosis result in angiostegnosis, which affect the long-term efficacy. OBJECTIVE: To analyze the pathophysiology and mechanism of the angiostenosis, additionally, to construct smooth muscle cell specific SM22a promoter and SMHC enhancer. METHODS: The database of PubMed, HighWire and CNKI were retrieved with key words of "SM22α promoter, SMHC enhancer, stenosis, Pi3k and RNAi/RNA interference". The language was confined to English and Chinese. The literatures correlated with angiostenosis, VSMC-specific SM22α promoter and enhancer were selected. The reviews and overlapped researches were excluded. In vascular smooth muscle cell proliferation and apoptosis of graft neointimal hyperplasia, as well as the extent of graft thrombosis were considered as evaluation index. RESULTS AND CONCLUSION: More than 8 730 literatures were searched by the computer. According to inclusion and exclusion criteria, the SM22α promoter and angiostenosis were analyzed. The neointimal hyperplasia and thrombosis had a strong impact on prospective efficacy after coronary artery bypass grafting. The PI3K-Akt-mTOR pathway was a crucial signal passage which regulated cell proliferation and migration. RNA interference targeting Pik3cb can efficiently suppress the neointimal hyperplasia through down regulating the PI3K-Akt-mTOR pathway. The shRNA eukaryon expression plasmid vectors targeting rat Pik3cb were constructed using VSMC-specific SM22α promoter. On the one hand, it can suppress the neointimal hyperplasia, and on the other, it can prevent the occurrence of thrombosis.SM22a is a specific gene in vascular smooth muscle cell. The studies that applied VSMC-specific SM22α promoter/enhancer to vascular grafting provide a new strategy to prevent angiostenosis and thrombosis.
2.Moxibustion in the Wanan Fang and its clinical applications.

A summary and review on the Japanese ancient medical literature Wanan Fang is performed. Through the analysis and textual research on the Wanan Fang, it is found moxibustion is highly valued in this book, most of which is derived from the Chinese ancient medical literature before Yuan Dynasty, thus preserving valuable documents regarding moxibustion in the Chinese medical literature. The moxibustion indications recorded in the Wanan Fang are extensive, including disorders of internal medicine, external medicine and pediatrics; the moxibustion contraindications and post-moxibustion sore are also involved in the book. In addition, this book is characterized with more use of moxibustion and less use of acupuncture, and the integrated treatment of moxibustion and medicine is frequently described. Some of the moxibustion indications and approaches recorded in this book are still in accord with the modern clinical practice of TCM, which are worthy of further researching.

3.Effect of pre injection of penehyclidine hydrochloride on the airway gland secretion of children with laparoscopic inguinal hernia surgery under general anesthesia

Objective To investigate the effect of pre injection of penehyclidine hydrochloride on the airway gland secretion of children with laparoscopic inguinal hernia surgery under general anesthesia.Methods 72 cases of children undergoing laparoscopic inguinal hernia surgery under general anesthesia in pediatric surgery department in our hospital were selected and divided into two groups,grouped by random number table method,control group of 36 cases were treated with preoperative injection of atropine,the experiment group of 36 cases were treated with preoperative injection of penehyclidine hydrochloride.Heart rate on pre injection(T0),before induction of anesthesia(T1),five min after induction(T2),ten min after induction(T3)and extubation(T4),the airway gland secretion,respiratory function related indexes,and the incidence of adverse reactions were compared.Results Compared with the control group,re fixed rate of tracheal tube during operation and the incidence of choking tube at T4 time were lower,the levels of PIP、Raw were lower and the level of CL was higher on five min after intubation,the HR at T1,T2,T3,T4 time was lower,with statistical significance(P<0.05);the incidence of adverse reactions in the control group 30.56%was higher than the experiment group 11.11%,with statistical significance(P<0.05).Conclusion Pre injection of penehyclidine hydrochloride can inhibit airway gland secretion in children with laparoscopic inguinal hernia surgery under general anesthesia,protect the respiratory function,and does not increase heart rate,the safety was higher.
4.Pharmacotherapy of poststroke aphasia

Poststroke aphasia is an acquired language disorder caused by stroke.It seriously affects the social ability in patients and reduces their quality of life.It might be an unbearable burden on the family and on society.However,the traditional treatment of aphasia has no positive effect.Therefore,pharmacotherapy of aphasia has received much attention.This article reviews the recent progress in researches on pharmacotherapy of poststroke aphasia.
5.Determination of Icariin in Chuanlong Guci Pian by HPLC

Objective To establish the method for detemining the content of Icariin in Chuanlong Guci Pian by HPLC. Methods Diamonsil ODS1 C18 Column was used with acetonitrile-water (27∶73) as the mobile phase, detection wavelength as 270 nm, and flow rate was 1.0 mL/min. Results The calibration curve was linear at the range of 0.125~1.00 ?g for Icariin and linear equation was Y=81 601X+10 356, r 2=0.999 7. The average recovery was 99.12% and RSD was 0.65% (n=6). Conclusion This method was simple, accurate and proper, and the reduplication of the result was good, which provides scientific quantitative analysis of Icariin in Chuanlong Guci Pian.
6.Comparison of placement of peripherally inserted central catheters using vascular ultrasound guidance system and traditional method in 938 tumor patients

Objective To compare the placement of peripherally inserted central catheters(PICC)by using vascular ultrasound guidance system and traditional method.Methods Totally 938 patients undergoing PICC were divided into the traditional method group and the vascular ultrasound system and microintroducer techniques group according to the puncture time.The differences in the one-attempt success rate and the overall success rate of the placement and the incidences of complications were compared between the two groups.Results The one-attempt success rate was 93.01% in the traditional method group and 98.76% in the vascular ultrasound system and microintroducer techniques group(P =0.005).The successful rate of PICC placement was 100%.The incidence of complication was 11.29% in the traditional method group and 2.47% in the vascular ultrasound system and microintroducer technique group(P =0.000).Conclusion The PICC placement using vascular ultrasound guidance system and microintroducer techniques can increase one-attempt success rate and decrease complications.
7.Cloning and Sequencing of the Gene Encoding Variant-Specific Surface Antigen from Giardia lamblia

Objective To clone and sequence variant-specific surface antigen gene from Giardia lamblia isolate SUCH/89/BTMRI/2(C2) derived from human in China. Methods Total genomic DNA of G.lamblia was extracted and a full-length variant-specific surface antigen gene fragment was amplified by pelymerase chain reaction (PCR). The PCR product was cloned into pMD19-T simple-vector, transformed into an Escherichia coli JM109 strain and then sequenced. The sequence analysis for cloned fragment was finished by Vector NTI 9.0 software for the homology of Giardia variantspecific surface antigen gene to that of sequences publishend in GenBank. Results The full-length variant-specific surface antigen gone fragment from G. lamblia was found to be 2 142 bp, encoding a 713 amino acid polypeptide and contained a single open reading frame (ORF). The deduced polypeptide sequence was rich in cysteine (11.8 mol%), most of which occurred with in 29 copies of the 4-amino acid CXXC motif, one GGCY-tetrapeptide motifs and three NXS consensus N-linked glycosylation sites. This polypeptide was also rich in threonine (10.2 mol%), glycine (12.1 mol%) and alanine (10.1 mol%). Like other previously identified VSPs, it contained a highly conserved hydropbebic Cterminal region. The homology of G. lamblia SUCH/89/BTMRI/2(C2) variant-specific surface antigen gene to that of sequence (TSA417) published in GenBank was 99% both at the nueleotide and the amino acid levels. Conclusion The full length variant-specific surface antigen gene from the isolate of G. lamblia has the common characteristics with other previously identified VSPs.
8.Ultrasound measurement of detrusor wall thickness in equivocal bladder outlet obstruction

Objective To estimate the application of ultrasound measurement of detrusor wall thickness (DWT) in the assessment of curative effect after operation. Methods Detrusor thickness was measured by linear ultrasound (7. 5 MHz) either at a filling volume of 50% of cystometric capacity or at 250 ml filling in 86 patients, who were diagnosed equivocal BOO, during a pressure-flow study. All patients accepted transurethral resection of the prostate. At 3 months post-surgery, the patients were divided into two groups according to curative effect after operation. The volume of the prostate, age and DWT were compared between the two groups. Results There was no difference in either age or volume of the prostate between the two groups. DWT was significantly higher (P<0.01) in the more curative effect group (37 cases, DWT 2. 5±0.3 mm) compared to the less curative effect group (49 cases, 2.2±0. 3 mm). As a predictor of curative effect, DWT of 2. 8 mm or greater had a positive predictive value of 100%, a negative predictive value of 62%, specificity of 100% and sensitivity of 19%. Receiver operating characteristic analysis (ROC) revealed that DWT had a high predictive value for curative effect post-surgery with an AUC of 0. 84±0. 04. Conclusions In patients with equivocal BOO, ultrasonographically assessed detrusor thickness may have a predictive value for curative effect post-surgery. However, this cutoff value needs to be validated in a larger study population.
9.Complex of growth differentiation factor-5 and fibrin gel for repair of lumbar disc injury in rabbits

Objective To investigate the result of local injection of growth differentiation factor-5 (GDF-5) and fibrin gel for treatment of lumbar disc injury in rabbits.Methods Lumbar puncture with a 20-gauge needle was performed at L3/4,L4/5,and L5/6 discs of 40 New Zealand white rabbits.After needle puncture,L3/4 discs were injected with GDF-5 and fibrin gel (compound group) ; L4/5 discs fibrin gel (fibrin gel group) ; L5/6 discs nothing (blank control group).Two weeks later,intervertebral disc degeneration in each group was observed via radiography,MRI and nucleus proteoglycan content detection and histological examination.Results At postoperative 2-,4-,8-,and 12-weeks,X-ray films revealed a gradual decrease in disc height index (DHI) among the three groups,but the decreasing velocity was lower in compound group than in other two groups (P < 0.05).On MRI,the signal of intervebral discs among the three groups diminished progressively with time,but a relatively lower decreasing was observed in compound group (P < 0.05).At postoperative 4-week,proteoglycan content of the nucleus pulposus was (6.3-± 0.4) in compound group,higher than (5.9-0.4) in blank control group and (5.8-± 0.3)in fibrin gel group (P <0.05).At postoperative 2-week,histological evaluation showed (5.28 ±0.41)points in compound group,lower than (7.54 ± 0.53) points in blank control group and (7.21 ± 0.44)points in fibrin gel group (P < 0.05).Conclusion Local injection of GDF-5 and fibrin gel facilitates the restoration of the injured discs and delays further disc degeneration.
