1.Clinical observation of different needle retention times for acute lumbar sprain treated with float needle.

OBJECTIVETo observe the difference of the clinical effects of different needle retention times for acute lumbar sprain treated with float needle.
METHODSOne hundred and twenty patients with acute lumbar sprain were randomly divided into a 6 h group, a 12 h group, a 24 h group and a western medication group, 30 cases in each one. In the three float needle groups, plastic hose was detained near the pressure pain point on the lumbar after subcutaneous scattering with float needle. The needle retention time was 6 h, 12 h and 24 h respectively. In the western medication group, 75 mg voltaren tablet was adopted orally every day and the treatment was given for four days. Visual analogue scale (VAS) and Oswestry dysfunction index (ODI) about low back pain were observed before and after treatment. Also, the comprehensive effect was assessed after treatment.
RESULTSAfter treatment, the VAS scores and the ODI scores were improved obviously than those before treatment in all groups (all P< 0. 05). In the three float needle groups, the VAS scores and the ODI scores were superior to those in the western medication group (all P<0. 05). The differences in VAS scores and ODI scores among the three float needle groups were not statistically significant after treatment (all P>0. 05). The effective rate in the 6 h, 12 h and 24 h group was 93. 3% (28/30), 90. 0% (27/30) and 93. 3% (28/30) respectively, which were better than 73. 3% (22/30) in the western medication group (all P<0. 05).
CONCLUSIONThe effect of float needle for acute lumbar sprain is better than voltaren, and there is no apparent difference in retention times.

2.Observation on Clinical Effects of Acupotomy plus Cupping for Knee Osteoarthritis

Objective: To observe the clinical effect of acupotomy plus cupping for knee osteoarthritis (KOA). <br> Methods: Sixty cases with KOA were randomly divided into a treatment group and a control group, 30 cases in each group. The patients in the treatment group were given acupotomy and cupping, while the patients in the control group were given injection of Sodium Hyaluronate. The index of severity for osteoarthritis (ISOA), the change of the effusion of knee joint and clinical effects were observed after treatment. <br> Results: The total effective rate was respectively 96.7% in the treatment group and 66.7% in the control group. The difference in the clinical effects between the two groups was statistically significant (P<0.05). After treatment, ISOA scores and scores of knee effusion in the two groups were remarkably reduced than those before treatment (P<0.05). The changes of the scores of the two indexes were statistically significant (both P<0.05). <br> Conclusion: Acupotomy plus cupping is better than injection of Sodium Hyaluronate in treatment of KOA.
3.Treatment effect of Tongxinluo combined peripheral blood derived mesenchymal stem cells transplantation on angiogenesis of HIF-1/VEGF pathway and miR-210 in diabetic foot rats

Aim To investigate the treatment effect of Tongxinluo(TXL)combined peripheral blood derived mesenchymal stem cells(PB-MSCs)transplantation on angiogenesis of HIF-1 /VEGF pathway and miR-21 0 in diabetic foot(DF)rats.Methods Seventy male Spra-gue-Dawley rats were fed with high-fat diet and given
combined streptozotocin injection to build diabetic rat model.The femoral artery and vein of right lower ex-tremity of modeled rats were ligatured to induce DF model.The normal rats in the same batch copied is-chemia model served as control group.The modeled rats were divided into 6 groups :model group ,TXL
group,Cilostazol group,PB-MSCs group,TXL com-bined PB-MSCs(T-MSCs)group and Cilostazol com-bined PB-MSCs(C-MSCs)group.After the treatment for 28 days,animal gastrocnemius muscle and serum were taken for detection,in which serum was used to detect VEGF-A and HIF-1 α by ELISA,and muscle was used to detect VEGF-A and VEGF-R2 by Western blot,and VEGF-A by RT-PCR.Results Compared with the control group,the expressions of VEGF-A, HIF-1 α,VEGF-R2 and miR-21 0 in the model group were significantly reduced(P <0.01 ).Compared with the model group,the expressions of VEGF-A,VEGF-
R2 and miR-21 0 in each treatment group were signifi-cantly increased(P <0.01 ),where the expressions of VEGF-A,HIF-1 α,VEGF-R2 and miR-21 0 in the T-MSCs were higher than these in the TXL,cilostazol and PB-MSCs group(P <0.05,P <0.01 ).Conclusion TXL combined with PB-MSCs transplantation may be regulated by HIF-1 /VEGF pathway and miR-21 0,pro-moting endothelial cell proliferation,differentiation, and promoting the formation of new blood vessels.
4.Relationship between annexin 1 and endogenous protective mechanism during intestinal epithelial cell injury induced by endotoxin

Objective To evaluate the relationship between annexin 1 (ANXA1) and the endogenous protective mechanism during intestinal epithelial cell injury induced by endotoxiu.Methods The intestinal epithelial cells at the logarithmic growth phase were seeded in culture palates and randomly divided into 4 groups (n =36 each) using a random number table:control group (group C),cell injury group (group I),ANXA1 overexpression group (group OE),and ANXA1 silencing group (group S).Lentivirus with ANXA1 overexpression and silencing was transfected into intestinal epithelial cells to construct a stable cell line.In I,OE and S groups,endotoxin was added with the final concentration of 100 μg/ml,and the cells were then incubated for 24 h to establish the cell injury model.The culture medium was changed,and the cells were then incubated for 24 h in group C.The cell apoptosis was detected by flow cytometry,the cell permeability was determined by Transwell assay,and the cell viability was evaluated by methyl thiazolyl tetrazolium assay.The apoptosis rate was calculated.Results Compared with group C,the apoptosis rate was significantly increased,and the cell permeability and viability were significantly decreased in I,OE and S groups (P<0.05).Compared with group Ⅰ,the apoptosis rate was significantly decreased,the cell permeability and viability were significantly increased in group OE,and the apoptosis rate was significantly increased,and the cell permeability and viability were significantly decreased in group S (P<0.05).Conclusion ANXA1 is involved in the endogenous protective mechanism during intestinal epithelial cell injury induced by endotoxin.
5.Effect of Acupoint Injection at Quyuan (SI13) on the Vibrating Perception Threshold and Its Efficacy in Treating Cervical Spondylosis of Nerve Root Type

Objective To compare the vibrating perception threshold (VPT) between normal subjects and subjects with cervical spondylosis of nerve root type, and to observe the clinical efficacy of acupoint injection at Quyuan (SI13) in releasing pain in cervical spondylosis of nerve root type.Method Sixty-three patients with cervical spondylosis of nerve root type were recruited and randomized into a treatment group of 35 cases and a control group of 28 cases. The treatment group received acupoint injection at Quyuan, and the control group received acupoint injection at Jiaji (EX-B2) points. The short-form McGill Pain Questionnaire (MPQ) was adopted. From the questionnaire, the Pain Rating Index (PRI), sensory (S), affective (A), and total (T) pain rating indexes, Visual Analogue Scale (VAS), and Present Pain Index (PPI) were used to comprehensively evaluate the pain improvement and to quantify the therapeutic efficacy, and the VPT was also considered. The Clinical Assessment Scale for Cervical Spondylosis (CASCS) by West China Rehabilitation Center was also used to analyze the result in the treatment group.Result After the intervention, the MPQ and VAS scores in the treatment group were significantly lower than those in the control group (P<0.05). The total effective rate of the treatment group was significantly higher than that of the control group (P<0.05). Meanwhile, the VPT was improved after the treatment.Conclusion Acupoint injection at Quyuan can produce a more significant efficacy than at Jiaji (EX-B2) in treating cervical spondylosis of nerve root type.
6.Tongxinluo protects vascular endothelial cells against ox-LDL-induced injury

Ob jectiev Oxidized low-density lipoprotein ( ox-LDL) induces vascular endothelial cell injury , which is one of the factors initiating atherosclerosis .This study aimed to investigate the protective effect of Tongxinluo ( TXL ) on vascular endothelial cells with ox-LDL-induced injury . Methods Human umbilical vein endothelial cells ( HUVEC ) were cultured in vitro and divided into five groups:normal control, oxidative stress injury (OSI) model, and high, medium and low dose TXL.The HUVECs were incubated with ox-LDL at the concentration of 30 mg/L for 24 hours to induce oxidative stress injury and then treated with TXL at 50, 100 and 150 mg/L for 4 hours, followed by 24 hour incubation with 30 mg/L ox-LDL added to the culture medium .The viability of the cells was detected by MTS assay, the nitric oxide (NO) content, superoxide dismutase (SOD) activity and mitochondrial membrane poten-tial ( MMP) in the cell culture supernatant were measured with respective kits , and the expressions of iNOS , MMP9, and NF-κBp65 proteins were determined by Western blot . Results The HUVECs of the OSI model group showed a significant decrease in cell via-bility compared with the normal control , ([73 .89 ±0.67] vs [100.00 ±2.23]%, P<0.01) but a remarkably increase after treated with medium and high dose TXL ([92.15 ±0.76]%and [ 97.19 ±1.45]%, P<0.01).The MMP, NO content, and SOD activity were markedly reduced in the model group (P<0.01) but elevated in the low, medium, and high dose TXL groups (P<0 .01).The expressions of the iNOS, MMP9, and NF-κBp65proteins were significantly up -regulated in the model group (P<0.01) but down reg-ulated in the low, medium, and high dose TXL groups (P<0.05).C on clusion TXL has the effects of anti-oxidation and anti-in-flammation and can protect vascular endothelial cells against ox-LDL-induced injury .
7.Effect of molecular subtype on chemosensitivity and survival in patients with breast cancer following neoadjuvant chemotherapy

Objective Pathologic complete response (pCR) has been suggested as a surrogate prognostic indicator in breast cancer patients treated with neoadjuvant chemotherapy.We assessed whether the likelihood of pCR and survival is associated with the immunohistochemistry-based molecular subtypes.Methods We retrospectively analyzed the records of l01 patients with breast cancer who received neoadjuvant chemotherapy between January 2007 and January 2010.Patients were dassified into four molecular subtypes based on the immunohistochemistry prnfiles of estrogen receptor,progesterone receptor,and HER2.Logistic regression was used to analyze variables associated with pCR.Results The pCR was achieved in 19 patients (18.8%).The triple negative subtype was an independent predictive factor for pCR (odds ratio,3.35,95% confidence interval,1.25-9.79,P =0.012),and the Her-2 subtype showed a trend for higher pCR rates (odds ratio,3.11;95% confidence interval,1.09-10.89,P =0.021) compared with the luminal A subtype.The pCR was significantly associated with prolonged disease-free survival (P =0.002).The triple negative subgroup had shorter disease-free survival (P =0.0006) and overall survival (P =0.008) than the other subgroups.Conclusions We demonstrated that the triple negative and Her-2 subtypes are more likely to obtain pCR when neoadjuvant chemotherapy is given,compared to the luminal A subtype.Despite the high pCR rate,the triple negative subtype showed worse survival outcomes,paradoxically,primarily due to patients who had residual disease.
